RESUMEN

OBJECTIVE@#To explore the effects of serum free light chain (sFLC) and monoclonal protein (MP) on the efficacy and prognosis of patients with multiple myeloma relapse, and investigate the clinical value of light chain escape (LCE).@*METHODS@#The relationship between sFLC/MP levels and clinical features and outcomes in 71 patients with multiple myeloma after relapse were retrospective analyzed. The patients were divided into MO group, MLC group and LCE group according to different levels of sFLC/MP after relapse. Then the clinical indicators, efficacy, survival after relapse (SAR) and overall survival (OS) of the patients in each group were compared. Meanwhile a paired sample t test was used to analyze the relevant indicators of the patients before and after relapse in LCE group.@*RESULTS@#There were significant differences in ISS stage, the levels of Hb, PLT, ALB, SFLC/MP and the proportion of myeloplasma cells afte relapse (P<0.05). The initial treatment effect of the patients in MO group was better than those in the other groups, and the LCE group was the worst (P<0.05). Comparison of relevant indicators between the patients before and after relapse in LCE group showed that the levels of MP, Hb and PLT decreased significantly, while sFLC, LDH and Cr increased significantly (P<0.05). Multivariate analysis showed that MO was the independent risk factor affecting SAR, while MO and LCE were the independent risk factors affecting OS (P<0.05) of the patients.The average SAR of the patients in MO, MLC and LCE group was 41, 28.6 and 23.5 months (P=0.002), and the average OS was 79.6, 57.9 and 41 months (P<0.001), respectively. The patients in MO group showed longer SAR and OS, while the LCE group was the shortest.@*CONCLUSION@#After relapse, patients only with elevated MP levels have a better curative effect and prognosis, while only with elevated sFLC levels have poor curative effects and prognosis, which suggesting that sFLC/MP levels can be used as a good indicator for predicting the prognosis of multiple myeloma patients.The appearance of LCE indicates disease progression, poor prognosis and early relapse.

OBJECTIVE@#To study the value of lactate dehydrogenase (LDH) in predicting refractory Mycoplasma pneumoniae pneumonia (RMPP) in children.@*METHODS@#Propensity score matching was used to select 73 children with RMPP (refractory group) and 146 children with non-refractory Mycoplasma pneumoniae pneumonia (common group). The logistic regression analysis, restricted cubic spline model, and decision curve analysis were used to analyze the clinical value of LDH in predicting RMPP.@*RESULTS@#There were significant differences in the incidence of high fever, white blood cell count, platelet count, percentage of neutrophils, and serum levels of C-reactive protein, procalcitonin, hemoglobin, albumin, glutamic-pyruvic transaminase, aspartate aminotransferase and LDH (P<0.05). There were also significant differences between the two groups in the Mycoplasma pneumoniae-DNA load in nasopharyngeal aspirates and the incidences of pleural effusion, pulmonary consolidation, atelectasis, shortness of breath and skin lesions (P<0.05). The multivariate logistic regression analysis showed that high fever, hemoglobin level, LDH level, and pulmonary consolidation were independent predictive factors for RMPP (OR=10.097, 0.956, 1.006, and 3.756; P<0.05). The results of the restricted cubic spline analysis showed a non-linear dose-response relationship between the continuous changes of LDH and the development of RMPP (P<0.01). The decision curve analysis showed that LDH had an important clinical value in predicting RMPP.@*CONCLUSIONS@#LDH is an independent predictive factor for the development of RMPP and its intensity of association with the development of RMPP exhibits a non-linear dose-response relationship.

OBJECTIVE@#To investigate the value of red blood cell distribution width (RDW) and fibrinogen (Fib) level for the evaluation of therapeutic efficacy and prognosis in patients with diffuse large B-cell lymphoma (DLBCL).@*METHODS@#The relationship between RDW/Fib at initial diagnosis and efficacy and the clinical outcome was retro-spectively analyzed based on the study of 105 patients with DLBCL. The patients were divided into two groups: low RDW group (≤15%) and high RDW group (>15%), low Fib group (Fib≤4 g/L) and high Fib group (Fib>4 g/L) according to the normal values of RDW and Fib. Therapeutic efficacy, overall survival (OS) time and progression free survival (PFS) time were compared between two groups. The correlation between each factors and efficacy, prognosis was analyzed by univariate and multivariate regression.@*RESULTS@#The therapeutic efficacy (P<0.001), OS time(P=0.004), and PFS time(P=0.007) were poorer in the high RDW group as compared with the low RDW group. The efficacy (P=0.015) and PFS time(P=0.04) were poorer in the high Fib group as compared with the low Fib group. Multivariate analysis showed that high RDW was the independent risk factor for efficacy of DLBCL patients (OR=3.394, 95% CI 1.093-10.539, P=0.035).@*CONCLUSION@#High RDW and high Fib associate with poor efficacy in DLBCL patients.

Aim: To establish an ideal anti-tuberculosis drug induced liver injury model and provide a suitable animal model for its pharmacodynamic evaluation. Methods To explore the contribution rate and interaction of isoniazid (INH) and rifampicin (RIF) on liver injury, mice received intragastric administration of RIF 100 nig · kg-1, 300 mg · kg-1 once, or RIF 300 mg · kg-1, INH 150 mg · kg-1, and RIF 300 mg · kg-1 + INH 150 mg · kg-1 for 1 -3 weeks. Then the biochemical and pathological indexes were determined and analyzed by factorial design ANOVA. Results After single intragastric administration of rifampicin, the serum bilirubin levels gradually increased, but no other indicators were affected in mice. Continuous intragastric administration of RIF 300 mg · kg-1, INH 150 mg · kg-1 or RIF 300 mg · kg-1 +INH 150 mg · kg-1 for 1 ∼3 weeks could significantly increase the liver index of mice. RIF alone or combined with isoniazid could significantly increase the level of ALT, AST and TBIL, resulting in vacuolar lesions in liver tissues in mice. The analysis of variance demonstrated that the combined use of RIF and INH for two weeks or three weeks showed significant antagonism in liver index and the level of ALT, and marked antagonism in TBIL at three weeks. The pathological results were basically consistent with the biochemical indicators. Conclusions RIF is the leading cause of liver injury in mice, and its hepatotoxicity is related to cholestasis. INH has a significant antagonistic effect on liver toxicity of RIF when they are combined, and the deep action mechanisms remains to be further explored.

OBJECTIVE@#To investigate the prognostic significance of immune changes in patients with newly diagnosed multiple myeloma(MM) after chemothrapy.@*METHODS@#The clinical data of 99 patients with multiple myeloma received treatment in Department of Hematology, Lanzhou University Second Hospital from April 2011 to December 2017 were collected and retrospectively analyzed. The change of immune status was defined by changes of lymphocyte/monocyte ratio(LMR) level. The prognosis value of age, sex, typing, hemoglobin (Hb), β2-microglobulin (β2-MG), lactate dehydrogenase (LDH), albumin (albumin, ALB) and LMR changes were investigated in patients with newly diagnosed MM, and the relationship between above inentioned factors and changes of LMR was also explored. Overall survival rate between different subgroups was compared by using Kaplan-Meier curves and detected by Log-rank tests. Univariate and multivariate analysis of prognosis was performed by using the COX proportional hazards regression model. Paired samples Wilcoxon test were used to compare changes in ALC, AMC and LMR before and after chemotherapy, and logistic regression was used to investigate the clinical factors that affect the changes of LMR.@*RESULTS@#The median value of ALC increased from 1.25 (0.84-1.81)×10/L to 1.39 (1.02-1.9)×10/L (P=0.029) after treated for 1 month; the median value of AMC decreased from 0.37 (0.23-0.47) ×10/L to 0.29 (0.2-0.44)×10/L (P=0.026), and the median value of LMR increased from 3.552 (2.405-5.208) to 5.138 (3.22-6.471) (P=0.002). Multivariate survival analysis showed that increasing of LMR (HR 0.459, 95% CI 0.241-0.875, P=0.018) and LDH (HR 2.368, 95% CI 1.123-4.995, P=0.024) were considered to be the independent factors affecting the prognosis of MM patients.@*CONCLUSION@#The increasing of LMR level after treatment indicates a longer survival time of newly prognostic MM patients. Combination with LMR can not only reflect the effect of treatment on the immune status, but also predict the prognosis of MM patients much better.

Purpose To explore the value of echocardiography and CT angiography (CTA) in diagnosing persistent fifth aortic arch (PFAA). Materials and Methods Eight children clearly diagnosed as PFAA in Children's Hospital of Chongqing Medical University from Jan. 2012 to Dec. 2016 were retrospectively analyzed. Clinical data were collected, and the features of echocardiography and CTA images were compared and summarized, which were then compared with the surgical results. Results ①Eight cases received CTA, CTA could clearly reveal anatomical spatial structure of PFAA such as its origin and courses. ②Four cases were fully recovered and discharged from hospital after receiving extracorporeal circulation PFAA sternotomy under general anesthesia.③According to Weinberg classification, 2 cases were classified as type A, 5 cases type B, and 1 case type C. ④5 cases of PFAA were firstly diagnosed via echocardiography (1 case of type A and 4 cases of type B), the diagnose accordance rate with the results of CTA and/or surgery was Conclusion Echocardiography and CTA can effectively diagnose PFAA, help early detection and improve prognosis.

AIM:To explore the protective effect of morinda officinalis oligosaccharides monomer HexB on hy -poxia/reoxygenation(H/R)-induced injury in human umbilical vein endothelia cells(HUVECs).METHODS:HUVECs were treated with HexB,4-phenylbutyric acid(4-PBA)and thapsigargin(TG),respectively.The cells were divided into control group,HexB group,H/R group,HexB+H/R group,4-PBA+H/R group,TG group and HexB+TG group.The cell viability was measured by CCK-8 assay.The apoptotic rate was detected by flow cytometry.Western blot was used to determine the protein levels of endoplasmic reticulum stress(ERS)related molecules chaperone protein glucose-regulated protein 78(GRP78),C/EBP homologous protein(CHOP),apoptosis-related protein caspase-12 and phosphorylated c-Jun NH2-terminal kinase(p-JNK).RESULTS: The viability of HUVECs was reduced in H/R group and TG group(P<0.05),increased in HexB+H/R,4-PBA+H/R and HexB+TG group(P<0.05).The apoptotic rate,the protein levels of GRP78,CHOP,caspase-12 and p-JNK were increased in H/R group and TG group(P<0.05),weakened in the HexB+H/R group(P<0.05),4-PBA+H/R group and HexB+TG group(P<0.05).No significant change in the apoptotic rate,cell viability,protein levels of GRP78, CHOP, caspase-12, p-JNK between HexB +H/R group and 4-PBA+H/R group was observed.CONCLUSION:HexB attenuates HUVECs injury caused by H/R through suppressing ERS and ap-optosis.The possible mechanism may be involved in the apoptotic pathways related to GRP 78,CHOP,caspase-12 and p-JNK.

Objective To investigate the effects of bradykinin (BK) on the proliferation of rabbit corneal endothehal ceils (RCECs) and the expression of tight junction-related proteins zonula occludens-1 (ZO-1) and zonula occludens-1-associated nucleic-acid-binding protein (ZONAB),and to explore the underlying mechanisms of BK on cell proliferation in corneal endothelium.Methods RCECs at logarithmic growth phase were treated with different concentrations of BK (0.01,0.1,1,10 μmol · L-1) BK group,with the controls left untreated.Morphological changes of cells in each group were examined under phase-contrast microscope,and MTT assays were used to detect cell proliferation at 24 h,48 h,72 h,96 h after BK treatment.And,at 72 h,the expression levels of ZO-1 and ZONAB protein were determined by Western blot.Results After 72 h of treatment,the cells in each group were fused into pieces and closely linked into a monolayer;but after 96 h,the growth of the cells was restricted,with the intercellular space become larger and the cells exfoliated.Compared with the control group,BK induced a significant increase of absorbance value and cell viability,and the differences were statistically significant (all P < 0.001),and the promoting effects showed a concentration-dependent manner,and 1 μmol · L-1 BK demonstrated the strongest regulative effect (P < 0.001).Western blot results showed that BK upregulated the expression of ZO-1 and ZONAB protein in a concentration-dependent manner.Conclusion BK can stimulate the proliferation of RCECs in a time-and concentration-dependent manner,and the mechanisms are probably associated with ZO-1/ZONAB-mediated signaling pathway.

Objective:To observe and research the diagnostic value of enhanced MRI of pituitary for pituitary stalk interruption syndrome (PSIS) and explore the correlation between enhanced MRI and pituitary target gland function damage.Methods: 80 patients with PSIS were divided into part of pituitary stalk interruption group (38cases) and complete pituitary stalk interruption group (42cases). And 80 healthy people were divided into control group. The relative hormonal level of three groups were detected, and the correlation between appearance of MRI and insufficient hormonal type, amount and severity degree were analyzed.Results: In part of pituitary stalk interruption group, the heights of anterior pituitary gland were less than 3mm in 10 cases, and they were between 3-5mm in 23 cases and they were more than 5mm in 5 cases. And in them, there were 4 cases were posterior pituitary deficiency, 15 cases were heterotopia, 11 cases were volume diminished. In complete pituitary stalk interruption group, the heights of anterior pituitary gland were less than 3mm in 23 cases, and they were between 3-5mm in 16 cases and they were more than 5mm in 3 cases. And in them, there were 8 cases were posterior pituitary deficiency, 32 cases were heterotopia, 1 cases were volume diminished. All of the hormonal levels included growth hormone, insulin-like growth factor, free thyroxine, serum thyroid stimulating hormone (TSH), cortisol and adrenocorticotrophic hormone (ACTH) in complete pituitary stalk interruption group were significantly lower than that in part of pituitary stalk interruption group and control group (F=224.92,F=2571.80, F=369.50,F=67.73,F=2677.94,F=24.17,P<0.05), respectively. The differences of growth hormone, TSH, cortisol and ACTH, excepted insulin-like growth factor and free thyroxine, between part of pituitary stalk interruption group and control group were not significant. And pituitary-gonadal hormones level of part of pituitary stalk interruption group was significantly lower than that of control group (F=354.94,F=247.00, F=247.83,F=655.05,F=48.10,P<0.05). Besides, the differences of follicle-stimulating hormone, luteinizing hormone, estradiol, testosterone and prolactin levels between part of pituitary stalk interruption group and complete pituitary stalk interruption group were no significant.Conclusion: Enhanced MRI of pituitary can effectively diagnose pituitary stalk interruption syndrome and damage degree of pituitary target gland function.

Objective To observe the clinical efficacy of the combined therapy of super-laser radiation and plum acupuncture on alopecia areata.Methods Altogether 178 alopecia areata patients were randomly divided into 3 groups:65 patients in the treatment group being given super-laser radiation and plum acupuncture,55 in the control group A solely with super-laser radiation and 58 in the control group B solely with plum acupuncture.The therapeutic effects of the treatment group and the control groups were compared by rank sum test.Results At the end of the 1st treating period,the cured cases,markedly effective cases,ordinary effective cases and invalid cases in control group A were 1,4,10 and 40;those numbers of control group B were 1,3,11 and 43;those numbers of the treatment group were 3,12,14 and 36.At the end of the 2nd treating period,those numbers of control group A were 8,8,16 and 23;those numbers of control group B were 7,8,18 and 25;those numbers of the treatment group were 11,20,19 and 15.At the end of the 3rd treating period,those numbers of A control group were 21,11,12 and 11;those numbers of B control group were 20,14,11 and 13;those numbers of the treatment group were 33,17,10 and 5.Comparing the performance of each groups at the end of the 3 periods,we found that effects were always better in the treatment group than the other two control groups.The difference between the treatment group and the other two control groups was statistically significant (P<0.05).Conclusions It is effective and safe to use super laser radiation combined plum acupuncture on alopecia areata.The clinical efficacy of this joint method is better than either of the two separate ways.

<p><b>OBJECTIVE</b>To investigate the clinical value of combination of human epididymis protein 4 (HE4), CA125 and the Risk of Ovarian Malignancy Algorithm (ROMA) in diagnosis of ovarian carcinoma.</p><p><b>METHODS</b>To detect the serum concentration of HE4 using ELISA and CA125 using ECL in patients of ovarian carcinoma group (n = 119), borderline ovarian tumor group (n = 36), benign ovarian neoplasm group (n = 96) and female healthy control group (n = 53). The ROMA based on the serum level of CA125, HE4 and a woman's menopausal status was used to calculate the predicted probability (PP) and diagnostic results of ovarian cancers.</p><p><b>RESULTS</b>The receiver operating characteristic (ROC) analysis showed the cut-off value was 67.3 pmol/L (the AUC was 0.906, the sensitivity was 80.7% and specificity was 94.6%). The serum levels of HE4 and CA125 in the ovarian carcinoma group were significantly higher than that in the borderline ovarian tumor group, benign ovarian neoplasm group and female healthy control group (P < 0.01). The serum levels of CA125 and HE4 showed statistically no significant difference between the borderline ovarian tumor group and benign ovarian neoplasm group (P > 0.05). The levels of HE4 and CA125 were reduced significantly in ovarian patients after surgery therapy (P < 0.01). The sensitivity and specificity of HE4 + CA125 combination was 92.7% and 72.5%. The ROMA that can classify patients into high and low risk groups was established as 9.3% in premenopausal and 27.3% in postmenopausal women.</p><p><b>CONCLUSIONS</b>HE4 is a helpful biomarker for ovarian carcinoma diagnosis. Biomarker combination of HE4 and CA125, and applying of the ROMA are helpful to improve the accuracy in diagnosis of ovarian cancers.</p>

<p><b>OBJECTIVE</b>To investigate the value of combined detection of Epstein-Barr virus (EBV) VCA/IgA, EA/IgA, Rta/IgG and EBNA1/IgA in serodiagnosis of nasopharyngeal carcinoma (NPC).</p><p><b>METHODS</b>Serum samples obtained from 211 untreated patients with NPC and 203 non-NPC ENT patients were examined for the presence of VCA/IgA and EA/IgA by immunoenzymatic assay and for Rta/IgG and EBNA1/IgA by enzyme-linked immunosorbent assay (ELISA). The receiver operating characteristic (ROC) curve was generated to confirm the cutoff values of different antibodies. The evaluation indexes of combined detection of multiple antibodies used for serodiagnosis of NPC were calculated with compounded positive judgment method.</p><p><b>RESULTS</b>Compared to a single antibody, combined detection achieved a higher sensitivity and specificity. The sensitivity of VCA/IgA + Rta/IgG + EBNA1/IgA (98.1%) was higher than the other 3 combinations with a specificity, accuracy, Youden index and positive predictive value (PPV) of 88.7%, 93.5%, 0.868 and 90.0%, respectively. The combination of EA/IgA+Rta/IgG+EBNA1/IgA had the highest specificity (95.1%), accuracy (94.9%), Youden index (0.899) and PPV (95.2%), with a sensitivity of 94.8%, suggesting its higher accuracy in the serodiagnosis of NPC. Combined detection of the 4 antibodies had the highest sensitivity (98.6%) with a specificity, accuracy, Youden index and PPV of 88.2%, 93.5%, 0.868 and 89.7%, respectively.</p><p><b>CONCLUSIONS</b>Combined detection of Rta/IgG against immediate early antigens, EA/IgA against early antigens, VCA/IgA against late antigens, and EBNA1/IgA against latent antigens provides better understanding of the expression profiles of EBV lytic and latent antigens with excellent complementarity, and may serve as an optimal combination for NPC serodiagnosis.</p>

<p><b>BACKGROUND</b>Multi-detector computed tomography (MDCT) has already been the first line investigation method for diagnosis of pulmonary embolism (PE). Reducing the amount of contrast medium used during CT scanning could decrease the incidental rate of adverse reactions. Our study amied to evaluate the image quality of pulmonary arteries using 64 slice multi-detector CT with small volumes of contrast media injection.</p><p><b>METHODS</b>Forty nonconsecutive patients without PE or other lung diseases were randomly assigned to two groups. Group A underwent CT scanning with 16 x 1.25 mm collimation and a 70 ml contrast injection, while group B had CT with 64 x 0.625 mm collimation and 20 ml of contrast injection. Two readers independently depicted the segmental and subsegmental pulmonary arteries. Reasons we could not analyze the pulmonary artery or that led to misdiagnosis of pulmonary embolism were evaluated, including the degree of contrast enhancement of the main pulmonary artery, and factors that caused misdiagnosis of PE (flow-related artifacts, partial volume artifact, beam-hardening artifacts and enhancement of pulmonary vein). The independent samples t-test, Mann-Whitney U test and Pearson chi-square test were applied.</p><p><b>RESULTS</b>There were no significant differences in image quality of segmental and subsegmental arteries between the two groups. No significant difference was found for factors that made pulmonary arteries non-analyzable or in the misdiagnosis of PE, except the degree of contrast enhancement.</p><p><b>CONCLUSION</b>64 x 0.625 mm collimation with 20 ml contrast injection could depict the pulmonary arteries well.</p>

RESUMEN

<p><b>OBJECTIVE</b>To evaluate the clinical effect and side-effect of peg-interferon alfa-2a (PEG-IFN alfa-2a) and highly active antiretroviral therapy (HAART) for patients infected with hepatitis C virus (HCV) and co-infected with human immunodeficiency virus (HIV).</p><p><b>METHODS</b>Twenty-two patients with HCV/HIV co-infection received highly active antiretroviral therapy initially; after their CD4 lymphocyte counts rose to over 0.20x10(9)/L, they were separated into two groups: one group with CD4 lymphocytes over 0.35x10(9)/L (high group) and one group with CD4 lymphocytes below 0.35x10(9)/L (low group). Both groups were given 180 microg of PEG-IFN alfa-2a every week intramuscularly. HCV RNA and HIV RNA loads, blood cell and CD4 lymphocyte counts, and liver functions were routinely examined.</p><p><b>RESULTS</b>After 12, 24 and 48 weeks of PEG-IFN alfa-2a therapy, mean HCV RNA loads reduced 2.0650 log10 copies/ml (t=3.8733), 2.9146 log10 copies/ml (t=7.6741) and 2.4315 log10 copies/ml (t=5.8202) from the baseline at week 0 in the 13 patients in the high group, and reduced 1.1522 log10 copies/ml (t = 2.8937), 1.4189 log10 copies/ml (t=2.4422) and 1.1167 log10 (t=1.1261) in the 8 patients of the low group. However, there was no significant difference between the early viral response rate (EVR) and the end of treatment viral response rate (ETVR) of the two groups. In the high group, the white blood cell count was lower at 24 weeks than the base line (t=2.4700), and the blood platelet count was lower both at 24 and 48 weeks than the base line (t=2.3273 and t=3.6149).</p><p><b>CONCLUSIONS</b>PEG-IFN alfa-2a can effectively reduce HCV RNA loads in patients with HCV-/HIV co-infection, and the inhibition rate in patients with higher CD4 lymphocyte counts is better. The EVR and ETVR of the two groups of patients show similar results after the treatment. PEG-IFN alfa-2a can reduce the white blood cell counts and the blood platelet counts in the peripheral blood.</p>

RESUMEN

<p><b>OBJECTIVE</b>To examine the effect of multiple micronutrients supplementation on anti-oxidative activity and decreasing oxidized DNA damage of lymphocytes in Chinese children.</p><p><b>METHODS</b>82 healthy children in a rural areas, aged 9-11 years, were selected and randomized allocated into group receiving supplements and control group with each of them 41. 24-hour dietary recall was used to collect data on daily nutrient intakes of the research subjects. The subjects in the supplement group were given vitamin A (VA) 600 microg, beta-carotene (beta-C) 1.0 mg, vitamin E (VE) 100 mg, vitamin C (VC) 300 mg and Na2SeO3(Se) 200 microg in a tablet on daily base while those in the control group took a same-sized color placebo tablet. The trial lasted 8 weeks. 5 ml blood samples from each subject were taken during 7 to 9 o'clock in the morning. DNA damage of lymphocytes and levels of plasma VA, VE, VC, beta-C, Se, malondialdehyde (MDA), activities of superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) were then analyzed twice before and after the 8-week of trial.</p><p><b>RESULTS</b>The low intakes of VA, VC and Se only accounted for 50.6%, 65.6% and 67.3% of their recommended nutrient intake (RNI) respectively. After the trial, levels of plasma beta-C, VA, VE, VC and Se in the supplemented group increased by 13.4%, 32.8%, 11.5%, 46.9% and 24.6% respectively, compared with the control group, indicating that nutritional status regarding antioxidant nutrients had largely been improved. GSH-Px activity had a significant increase in the supplement group than before the supplement and in the control group (P < 0.01). GSH-Px before the trial (the 100.4 U/ml) also showed significant increase after the trial (161.7 U/ml) (P < 0.01). However, the values of SOD and MDA significantly decreased after the trial. Analysis of DNA damage indicated that there was no significant difference in the intrinsic damage of DNA (P > 0.05). Significant decreases of oxidized DNA damage induced by 5 micromol/L, 10 micromol/L and 25 micromol/L H2O2 were found more in peripheral lymphocytes of the supplemented group, than in pre-supplement and the control group after the trial (P < 0.01).</p><p><b>CONCLUSION</b>Supplementation of multiple micronutrients could effectively increase the levels of beta-C, VA, VE, VC and Se in plasma, and GSH-Px activity. In the meantime, MDA and oxidized DNA damage induced by a low level H2O2 decreased significantly after the trial. The reason accounted for the decrease of SOD activity after the trial needs to be further studied.</p>

<p><b>OBJECTIVE</b>To study the clinical significance of serum CEA, SCC and Cyfra21-1 test in the diagnosis, prediction of prognosis and postoperative monitor of recurrence in esophageal cancer.</p><p><b>METHODS</b>The concentration of serum CEA and Cyfra21-1 was measured by electrochemiluminescence immunoassay (ECLIA) using Elecsys 2010, CEA kit and Cyfra21-1 kit. Serum SCC was measured by microparticle enzyme immunoassay (MEIA) using IMx System and SCC kit. Serum of 206 patients with esophageal cancer (203 squamous cell carcinoma, 2 small cell carcinoma and 1 adenosquamous carcinoma) was measured preoperatively, 71 of whom also measued 8 to 12 days after resection.</p><p><b>RESULTS</b>The cut-off value of CEA and Cyfra21-1 was < or = 3.25 ng/ml and < or = 2.61 ng/ml, which were determined by the data of 45 healthy Chinese measured during the same period. The positive ratios of serum CEA and Cyfra21-1 in 206 cases were 29.1% and 45.1%. The combined positive ratio of CEA and Cyfra21-1 was 57.3%. The CEA positive ratios, according to the pathological stage of 165 resectable patients, were 16.6% (stage I), 26.8% (II) and 30.8% (III). For Cyfra21-1, they were 27.8%, 37.5% and 50.5%. For CEA combined with Cyfra21-1, they were 38.9%, 50.0% and 63.7%. The mean value of CEA, SCC and Cyfra21-1 (especially SCC and Cyfra21-1) was found to be well correlated with the tumor volume, TNM stage and depth of tumor invasion. Patient with bulky tumor or advanced tumor (T4) usually had much higher mean value than those with early stage tumors. One week after radical resection, the level of the three tumor markers fell to normal level in 92.9% of 71 patients. The level of serum CEA and Cyfra21-1 varied greatly in a small part of the patients. Extremely elevated serum CEA and Cyfra21-1 usually indicated advanced lesion or tumor metastasis.</p><p><b>CONCLUSION</b>Preoperative and postoperative measurement of serum CEA, SCC and Cyfra21-1 (especially Cyfra21-1) is helpful in the diagnosis, prediction of prognosis and monitor of postoperative recurrence in patients with esophageal cancer.</p>

RESUMEN

Objective To investigate the effect of MRI in the evaluation of extra-capsular extension (ECE)of prostate cancer.Methods Preoperative MR imaging findings were correlated with histologic findings in 29 patients underwent radical prostatectomy regarding the ECE,the diagnostic accuracy was evaluated retrospectively.Results Histopathological results showed capsular penetration in 9 of 29 patients (31.0%).The sensitivity,specificity,positive predictive value,and negative predictive value of MR imaging were 3/9,90%(18/20),3/5 and 75%(18/24),respectively.The overall accuracy was 72.4% (21/29).Conclusion MR imaging has relatively low sensitivity and high specificity for diagnosis of ECE of prostate cancer,the accuracy is somewhat limited.

Objective To study the relationship between renal morphology and renal function,and to assess the value of CT as a criterion to grade renal function.Methods Enhancement CT were performed in 89 patients with no local renal disease whose split renal glomerular filtration rates(GFR)were measured by renal dynamic imaging with ~(99)Tc~m-DTPA.The 178 kidneys were divided into normal renal function,mild and severe renal impairment groups according to renal function.Differences between three groups respect to the mean thickness of renal cortex and parenchyma were assessed by ANOVA.Using Pearson's correlation test,the correlation between the renal cortex,parenchyma thicknesses and renal GFR were examined.The value of CT in predicting renal function was assessed by using ROC analysis.Results The renal cortex thicknesses of normal renal function,mild and severe renal impairment groups were(5.9?1.1),(4.6? 1.1),and(3.3?1.0)mm respectively,and the renal parenchyma thicknesses were(26.3?4.2), (21.3?4.6),(16.2?4.6)mm.There were significant differences of renal cortex,parenchyma thicknesses between 3 groups(cortex F=54.78,P

RESUMEN

Objective To detect the change of cerebral perfusion in pediatric patients with Leigh's syndrome (LS)by using MR perfusion technique.Methods Twelve patients with Leigh's syndrome and thirteen normal children were scanned with the sequence of flow-sensitive alternating inversion recovery exempting separate T_1 measurement (FAIREST).Their relative cerebral blood flow(rCBF)values were obtained in regions of bilateral basilar nuclei and bilateral thalami.Student t-test was used to compare them between the two groups and receiver operating characteristic(ROC)curve analysis was carried out.Results Statistical analysis revealed significant difference between two groups in the regions of bilateral basilar nuclei and right thalamus(t =3.26,P =0.002;t =2.25 ,P =0.018 ;t =2.88 ,P =0.004,respectively).The rCBF values for LS group and control group were 0.432?0.158 and 0.619?0.125 for right basilar nuclear, 0.478?0.186 and 0.621?0.123 for left basilar nuclear,0.630?0.189 and 0.833?0.160 for right thalamus,respectively.The areas under the ROC curves were 0.833 and 0.756 for the rCBF of right and left basilar nuclear,respectively.Conclusion Relative CBF maps may reveal changes of cerebral blood flow in some specific brain regions in patients with Leigh's syndrome.It can provide additional information to the clinicians in the evaluation of the disease.
