RESUMEN

Objective:To investigate the effect of ultrasonic debridement mediated by 0.9% sodium chloride solution and 0.5% iodophor volt combined with eddy current washing and high pressure pulse washing on the removal of colonized bacteria on the wound surface of diabetic foot and wound healing.Methods:From March to November 2020, a total of 60 patients using ultrasonic therapy for debridement were divided into control group, experimental group 1, experimental group 2 and experimental group 3 by random digit table in the fourth People′s Hospital of Dalian. The final effective data collected was 15 cases in each group. The control group was given ultrasonic debridement mediated by 0.9% sodium chloride solution and eddy current washing.Experimental group 1 was given ultrasonic debridement mediated by 0.9% sodium chloride solution and high pressure pulse washing. Experimental group 2 received 0.5% iodophor mediated ultrasonic debridement and eddy current washing. Experimental group 3 0.5% iodophor mediated ultrasonic debridement and high pressure pulse washing. The size of the wound was measured, sampled and bacterial cultured before and after the first, fifth and 10th intervention. The wound bacterial clearance rate and wound area reduction rate were calculated and compared.Results:Before and after 3 interventions, the bacterial clearance rate and the total reduction of wound surface in 4 groups were increased ( P<0.01), the total bacterial clearance rate of experimental group 3 was the highest, which was (93.85 ± 9.87)%.The total reduction rate of wound in experimental group 2 was the highest, which was (20.831 4 ± 9.379 8)%. Conclusions:0.5% iodophor mediated ultrasonic debridement combined with high pressure pulse washing is the most effective way in the removal of diabetic foot wounds, and 0.5% iodophor solution mediated ultrasonic debridement combined with eddy current washing is the most effective in reducing diabetic foot wounds.

OBJECTIVE@#To explore the genetic basis for a pedigree affected with familial adenomatous polyposis (FAP).@*METHODS@#The proband, with recurrence of blood in the stool, was diagnosed with FAP by endoscopy, pathological examination and a family history. She was subjected to next generation sequencing to detect genetic variant. Suspected variant was verified by Sanger sequencing of members from her pedigree.@*RESULTS@#The proband, her mother and brother were found to carry a heterozygous c.532-1G>A variant of the APC gene, which may lead to aberrant splicing of mRNA resulting in a truncated protein, which may lose its normal function and promote the tumorigenesis. Based on the American College of Medical Genetics and Genomics standards and guidelines, c.532-1G>A variant of APC gene was predicted to be pathogenic(PVS1+PP1+PP4+PP5).@*CONCLUSION@#The c.532-1G>A variant of the APC gene probably underlay the pathogenesis of FAP in this pedigree.

RESUMEN

In recent years, the incidence of hepatocellular carcinoma (HCC) has risen year by year, leading to a high mortality rate. At present, surgical treatment is the major cure for HCC, and in general, HCC is diagnosed at late stages. Due to the heterogeneity of HCC and different sensitivities to drugs, the treatment efficacy of advanced HCC is poor. In this paper, we retrospectively analyzed the prog-ress of HCC treatments and reviewed important progression, which provides new view for the clinical improvement of the total surviv-al of patients with HCC.

RESUMEN

Objective To explore the relationship between autism spectrum disorder (ASD) and gastrointestinal problems. Methods 172 ASD children were investigated with self-made questionnaire from July, 2014 to Augest, 2015. The frequencies of motor behaviors, vocal behaviors and behavioral problems, and the intensity of the behavioral problems in the period with or without gastrointestinal symptoms were compared. Results The frequencies of motor behaviors (F=67.4, P<0.001), vocal behaviors (F=43.6, P<0.001) and behavioral prob-lems (F=79.4, P<0.001), and the intensity of the behavioral problems (F=65.7, P<0.001) were all more in the period with gastrointestinal symptoms than without gastrointestinal symptoms. Conclusion There are more behavioral problems in the ASD children with the presence of gastrointestinal symptoms;while the behavioral symptoms may be a indicator of gastrointestinal problems in children with ASD.

An expanded-granular sludge bed (EGSB) reactor was set-up with artificial water by seeding a 60 d stored ANAMMOX sludge. The nitrogen removal efficiency of ANAMMOX enrichment culture in the reactor was determined. In addition, the main microbial populations and the relative abundance of ANAMMOX bacteria were investigated by molecular approaches. Results show that the maximum nitrogen removal rate was 3.0 kg-N·m(-3)·d(-1) after 185 d, and the ammonium and nitrite removal efficiencies were all over 85%. Analysis of 16S rRNA gene-cloning indicates that the main microbial population in the ANAMMOX enrichment culture was changed from Candidatus Brocadiafulgid and Candidatus Brocadia brasiliensis (0 day) to Candidatus Jettenia asiatica (185 day). Fluorescence in situ hybridization analysis shows that the relative abundance of ANAMMOX bacteria was increased from (57.69 ± 4.79)% to (83.32 ± 4.40)%. The results of qPCR further indicate that the gene copies of ANAMMOX bacteria in the granules were increased from 1.14 x 10(11) copies/g wet weight to 3.69 x 10(11) copies/g wet weight.

<p><b>OBJECTIVE</b>To establish an HPLC-DAD/ESI-MS method for quickly identifying chemical constituents in diterpene lactone effective fraction of Andrographis panniculata and to study its pharmacodynamics.</p><p><b>METHOD</b>The separation was performed on an Agilent SB-C18 column (2.1 mm x 150 mm, 5 μm) with a mobile phase of acetonitrile (A) and water (B). The flow rate was maintained at 0.4 mL x min(-1) and detection wavelength was set at 205 nm. The samples were analyzed in positive ion mode, and mass scan range was m/z 50-1 000. Using two kinds of tumor cell lines made living animal models, and studied preliminary pharmacodynamics on anti-tumor aspect.</p><p><b>RESULT</b>Five diterpene lactones in the diterpene lactone effective fraction of A. panniculata could be separated in one run. Pharmacodynamic experiments showed that the effectve fraction had an inhibitory effect on the growth of tumor.</p><p><b>CONCLUSION</b>A rapid and efficient HPLC-ESI-MS method to determine the chemical constituents in diterpene lactone effective fraction of A. panniculata has been established, and the preliminary pharmacodynamics research has been done, which could be used for the quality control and further studies of diterpene lactone effective fraction of A. panniculata in vivo.</p>

RESUMEN

<p><b>OBJECTIVE</b>To investigate the clinical effect of atlantoaxial pedicle screw internal fixation for treatment of atlantoaxial dislocation.</p><p><b>METHODS</b>Sixteen patients with atlantoaxial dislocation were treated from Dec. 2005 to June 2007, included 10 males and 6 females, aged from 38 to 45 years old (means 40.5 years). Among them 12 patients combined with nerve injury, according to ASIA grade: there were 3 cases in grade B, 5 cases in grade C, 4 cases in grade D. All patients received preoperative CT, radiograph and skull traction. Intraoperative posterior approach general spine pedicle screw-rod orthopaedics fixation system used and iliac bone block were implanted in space of posterior atlantal arch and axial vertebral plate. The outcome and complications were observed in the near future.</p><p><b>RESULTS</b>There was no vascular or neural injury found. The patients were followed up for 12 to 24 months (means 18 months). All head pain, acid storm symptoms were improved after operation. According to the Odom's clinical efficacy evaluation standard, 12 cases were excellent, 4 were good. Eleven cases of 12 with nerve injury recovered significantly. By ASIA classification: 1 cases was in grade B, 2 cases were in grade C, 5 cases were in grade D, 4 cases were in grade E. No looseness or breakage of screw occurred. Bony fusion was achieved in all cases.</p><p><b>CONCLUSION</b>Posterior approach atlantoaxial pedicle screw internal fixation have the advantages of direct screw placement, short-segment fusion, intraoperative reduction, fixation reliable, high fusion rate, and it can restablish the upper cervical vertebrae stability and help to recover the spinal cord and nerve function.</p>

RESUMEN

Purpose:To observe the effect of thalidomide combined with chemotherapy in the treatment of newly diagnosed multiple myeloma. Methods:34 patients were divided into two groups at random. 14 cases in the treated group were initially administered thalidomide at doses ranging from 100mg~200 mg/d,and the dosage was escalated by 100mg~200 mg every two weeks until patients could not tolerate the treatment.The maximal dose did not exceed 600mg/d.MP or M 2 protocol was delivered to those patients in the treated group concomitantly.20 cases in the control group received chemotherapy consisting of MP or M 2 protocol only.Evaluation of 3-months effective(partial remission plus progress)rate,one-year event free survival and two-years overall survival was done in both groups. Results:3-months effective rate is 85.7% in the treated group and 60% in control group.One-year EFS is 71.4% in the treated group while that in the control group is 35%.Two-years OS are 57.1% and 25% respectively.The side effects of thalidomide are tolerable,and constipation is the most common. Conclusions:The combination of thalidomide and chemotherapy is an effective treatment for newly diagnosed MM,and it demonstrates an improvement in effective rate,one-year EFS and two-years OS.
