RESUMEN

Cytochrome P450 reductase (CPR) is essential for the electron transport chain of cytochrome P450s, playing an indispensable role in electron transfer in vivo. In this study, one cDNA encoding cytochrome P450 reductase (Ascpr1) was identified from the callus of Aquilaria sinensis. Ascpr1 contains an open reading frame of 2 124 bp. The deduced protein is composed of 707 amino acids, with a predicted molecular weight of 78.82 kD. Phylogenetic analysis revealed that AsCPR1 is a type Ⅱ CPR protein closely related to the CPR from Theobroma cacao. Transmembrane prediction using TMHMM 2.0 indicated that the amino acids 52-71 of AsCPR1 comprise a transmembrane region. After truncating of 67 amino acid residues from N-terminal, the truncated AsCPR1 was successfully expressed in E. coli Transetta (DE3). Further purification of the recombinant AsCPR1 by affinity chromatography and determination of the enzymatic activity allowed the reducing ability of AsCPR1 to cytochrome C in vitro. The results pave the way for further study on the synthesis of defensive chemicals involved in P450s and the functions of CPR in self-defense of A. sinensis.

RESUMEN

Chalcone isomerases (CHIs) play an essential role in the biosynthesis of flavonoids important in plant self-defense. Based on the transcriptome data of Aquilaria sinensis Calli, a full-length cDNA sequence of CHI1 (termed as AsCHI1) was cloned by reverse transcription PCR. AsCHI1 contains a complete open frame (ORF) of 654 bp. The deduced protein is composed of 217 amino acids, with a predicted molecular weight of 23.11 kDa. The sequence alignment and phylogenetic analysis revealed that AsCHI1 has conserved most of the active site residues in type I CHIs, indicating a close relationship with the CHI from Gossypium hirsutum. The recombinant AsCHI1 protein was obtained by heterologous expression of AsCHI1 in E. coli BL21(DE3). The purified AsCHI1 protein exhibited CHI activity by catalyzing the production of naringenin from naringenin chalcone. Remarkably, AsCHI1 expression in A. sinensis Calli treated with various abiotic stresses including salt, mannitol, cold, and heavy metals could be markedly increased, and plant hormones such as abscisic acid (ABA), gibberellin (GA3), and salicylic acid (SA) could also increase the expression of AsCHI1, suggesting that AsCHI1 might play an important role in plant self-defense. The results expand our understanding of the biosynthesis of flavonoids in A. sinensis and give further insight into the defensive responses of A. sinensis to abiotic and biotic stresses.

RESUMEN

Curcumin is exclusively isolated from Zingiberaceae plants with a broad spectrum of bioactivities. In the present study, we used the diketide-CoA synthase (DCS) and curcumin synthase (CURS) genes to construct a non-natural fusion gene encoding diketide-CoA synthase::curcumin synthase (DCS::CURS). This fusion protein, together with the acetyl coenzyme A carboxylase (ACC) and the 4-coumarate coenzyme A ligase (4CL), were introduced into Escherichia coli for the production of curcumin from ferulic acid. The process is divided into two stages, the growth stage using LB medium and the fermentation stage using the modified M9 medium. The yield of curcumin reached 386.8 mg/L by optimizing the induction of protein expression in the growth stage, and optimizing the inoculum volume, medium composition and fermentation time in the fermentation stage, as well as the addition of macroporous resin AB-8 into the second medium to attenuate the toxicity of the end product. The exploitation of the non-natural fusion protein DCS::CURS for the production of curcumin provides a new alternative to further promoting the production of curcumin and the related analogues.

OBJECTIVE@#To explore the influencing factors of perioperative blood transfusion in the treatment of elderly femoral intertrochanteric fractures with proximal femoral nail antirotation(PFNA).@*METHODS@#The clinical data of 109 elderly patients with intertrochanteric fractures who received PFNA treatment from July 2018 to January 2020 were retrospectively analyzed. Both pelvic hip X-rays and CT plain scans were performed before surgery. All patients were diagnosed by X-ray and CT plain scan of pelvis and hip before operation. Through the statistical analysis of the basic data of patients before and during operation, the risk factors of perioperative blood transfusion were explored.@*RESULTS@#Logistic regression analysis showed that age (@*CONCLUSION@#Age, fracture type, diabetes history, and preoperative hemoglobin are independent risk factors for perioperative blood transfusion in the treatment of elderly intertrochanteric fractures with PFNA. The older the patient, the history of diabetes, the more unstable the fracture, and the lower preoperative hemoglobin, the more likely it is to require a blood transfusion, which may provide a reference for clinical perioperative blood transfusion decisions.

Ginsenoside Ro decreased measures of inflammation, aging, oxidants and thrombus formation in a previous study. To measure ginsenoside Ro content in red ginseng from different years, an optimized extraction method was developed to determine ginsenoside Rg1, Re, Rb1 and Ro content by HPLC in 43 batches of red ginseng from different origins, growing years and manufacturers. The results indicate that the best extraction method was to ultrasonify a 1 g sample in 70% methanol for 50 min. The total running time of the optimized gradient was 50 min using a C18 core-shell column and was half the time described in the Chinese Pharmacopoeia, 2015 edition. The separation resolution of all of targeted compounds was greater than 1.6. The peak shape of ginsenoside Ro was optimal when the mobile phase consisted of acetonitrile and water with 0.1% phosphoric acid. The content of ginsenoside Ro was in the range of 0.11% to 0.43%, and the average content was 0.26%, which was higher than that of ginsenoside Rg1 and Re. The ratio of ginsenoside Ro and Re as a threshold could be used to discriminate red ginseng from different growing years; in addition, 100%, 94.4% and 46.6% of red ginseng from six, five and four years exceeded the threshold of 1.3. Our optimized analytical method for characterization of red ginseng is convenient and shortens the assay time.

This work describes the holistic fingerprinting method based on liquid chromatography coupled with charged aerosol detection(CAD) to profile non-saponin from water-soluble parts and determination of dencichine in Panax ginseng(PG), P. quinquefolium(PQ) and P. notoginseng(PNG). Sample extraction was carried out by water with ultra sonication for 30 min, which was eluted by Retain PEP for further analysis. The analysis was performed on a Hypercarb of porous graphitized carbon(3.0 mm×150 mm, 3 μm) column with acetonitrile and 0.1% perfluoropentanoic acid as mobile phase at a flow rate of 0.8 mL·min~(-1). Temperature of evaporator and nitrogen pressure for CAD were set at 50 ℃and 60.1 psi(1 psi≈6.895 kPa), respectively. As a result, dencichine and other polar components had a good performance on resolution and retention. The correlation coefficient(R~2) of dencichine was 0.998 2 in the concentration from 0.019 2 to 0.48 μg·mL~(-1). Limit of quantitation calculated by signal to noise of 10 was 7.4 ng·mL~(-1), and the recovery ranged from 95.52% to 102.7%. Chemical profile of the water-soluble part from PG, PQ and PNG was similar holistically, while the relative content for dencichine and other partial components varied significantly. The proposed method was used for characteristic of chemical profiling for non-saponin from water-soluble part, and determination of dencichine in PG, PQ and PNG.

Objective@#To evaluate the effect of vaccine and provide scientific evidence for prevention and control of influenza virus, this study aims to analyze the characteristics of genomic variation of influenza A (H1N1) pdm09 viruses in Inner Mongolia.@*Methods@#The 16 viral strains were selected randomly according to the influenza A (H1N1) pdm09 viruses isolated from network laboratories in Inner Mongolia, 2013-2017. The hemagglutinin(HA) and neuraminidase(NA) genomic sequences were obtained by using RT-PCR and sequencing, and genomic characteristics were analyzed via bioinformatics.@*Results@#Compared to the A/California/07/2009 vaccine strain, the relatively obvious variation of antigen of influenza A (H1N1) pdm09 viruses in Inner Mongolia since 2014, and the vaccine provided a poor protection to influenza A (H1N1) pdm09 virus infection, while the A/Michigan/45/2015 vaccine strain recommended by WHO recently has a satisfactory protective effects. Several viral isolates from Inner Mongolia increased the binding force of virus in human upper respiratory tract because of D222N and D222G substitution within HA. E119K and H275Y substitution within NA gene of viral strains, suggesting that the viruses were resistant to NA inhibitors.@*Conclusions@#The influenza A (H1N1) pdm09 viruses had gradual variations as time went on, and the WHO recommended vaccine was relatively lagging. Virulent strains and drug-resistant strains appeared in the population, and the genetic characteristics of influenza virus surveillance should be strengthened to find the new mutants of virus in time, which provide evidence for the prevention and control of influenza.

We have developed a new method using HPLC-CAD (charged aerosol detector) for the quantitative analysis of cyclovirobuxine D and related substances in the API of Huangyangning tablets. The related substances were further studied by HPLC-Q-Exactive coupled with hybrid quadrupole-orbitrap mass spectrometry. A HILIC column of XBridge Amide (4.6 mm×250 mm, 5 μm) was used, and the mobile phase was composed of acetonitrile and 100 mmol·L-1 ammonium formate (85∶15), which was adjusted to pH 2.8 with formic acid. Isocratic mode elution was adopted at a flow rate of 1.1 mL·min-1. The column temperature was set at 30 ℃. For CAD, the temperature of atomization and gas pressure were respectively set at 35 ℃ and 62.2 psi. This method detected and quantified five related substances to cyclovirobuxine D. The results showed that the LOD and LOQ of cyclovirobuxine D was 12.588 ng and 28.323 ng, respectively with an average recovery of 95.74% (RSD = 1.79%, n = 6). The content of cyclovirobuxine D in 12 batches of API samples provided by three manufacturers was from 79.94% to 88.49%, with an average value of 82.20%. The total content of the five related substances was from 15.99% to 22.15% with an average value of 20.10%, using an external standard method with cyclovirobuxine D as the reference and according to the CAD uniform response to non-volatile substances. The newly developed HPLC-CAD method has advantages in terms of the comprehensiveness of signals from Buxus alkaloids without UV absorption and with high sensitivity to its trace-related substances; the method yields good separation between the components and is compatible with mass spectrometry. It is applicable for the accurate quantitative analysis of main components and related substances in the API of Huangyangning tablets.

Objective To explore the actions of vagus nerve in electroacupuncture (EA) with unblocking and regulating needling therapy for the treatment of mammary gland hyperplasia(MGH)with liver stagnation type, and to provide experimental evidences for clinical usage of EA with unblocking and regulating needling therapy. Methods Sixty healthy SD rats were randomly divided into 5 groups,namely blank control group,model control group,EA group,EA with disconnection of vagus nerve group,and sham-operation EA group,12 rats in each group. Except for the blank control group, the rats in the remaining groups were modeled. After successful establishment of the model of mammary gland hyperplasia (MGH) with liver stagnation type, the rats in EA group with disconnection of vagus nerve were given disconnection of unilateral vagotomy operation, the rats in sham-operation EA group were given sham operation, and EA group had no treatment, and then all of the 3 treatment groups were given EA with unblocking and regulating needling therapy. At the end of the experiment, the diameter and height of rat nipple were measured,the serum contents of estradiol (E2)and progestin (P)in the rats were detected by enzyme-linked immunoassay(ELISA),and the contents of anti-estrogen receptor alpha (ERα)and anti-progesterone receptor(PR)in the rat breast tissue were detected by immunofluorescence double staining and Western blotting method. Results (1)EA treatment can obviously improve the diameter and height of MGH rat nipple, but the therapeutic effect of EA group with disconnection of vagus nerve was not obvious. (2)Compared with the model control group,the levels of serum E2 and mammary ERαwere markedly reduced(P<0.01),and serum P level and mammary PR content were obviously increased in EA group and sham-operation EA group (P < 0.01). However,the above indexes had no obvious changes in EA group with disconnection of vagus nerve (P>0.05). Conclusion The therapeutic mechanism of EA with unblocking and regulating needling therapy for liver-stagnation MGH may be closely related with the regulation of vagus nerve.

Objective To explore the elderly stroke patients into Maslow's hierarchy of needs theory to carry out the effect of nursing intervention on the Barthel and self efficacy, quality of diagnosis and treatment of this kind of special group that the accumulation of evidence-based care experience. Methods Eighty-two cases of elderly patients with stroke in our hospital from September 2015 to March 2017, with the random number table as the grouping criterion, will meet the inclusion criteria of the object is divided into study group and control group, 41 cases in each group. The control group received traditional nursing care, while the research group introduced the Maslow hierarchy of needs theory to intervene. The two groups of patients were evaluated before and after treatment, and the neurological function level, daily living ability, complication rate and nursing satisfaction were compared. Results After treatment, the score of basic knowledge, poor dietary harmful cognition, treatment knowledge, self-management knowledge and complication prevention knowledge were (2.3 ± 0.6), (2.4 ± 0.7), (2.3 ± 0.7), (2.5±0.7), (2.6±0.8) points respectively in study group, and (1.4 ± 0.4), (1.5 ± 0.4), (1.4 ± 0.4), (1.6 ± 0.5), (1.7 ± 0.5) points in control group. The difference was statistically significant (t=8.175-15.260, P<0.05).Neurological deficits after treatment in the study group were (7.8 ± 1.1) points lower than those in the control group (14.5±2.2) points. Barthel index was (61.5±5.4) higher than that in the control group (41.3± 4.0). The length of stay in hospital was (16.3±2.6) d, which was shorter than that in control group (25.3± 3.2) d, the difference was statistically significant (t=15.260, 12.352, 8.367, P<0.05). The complication rate was 9.8% (4/41) in the study group, which was lower than 29.3% (12/41) in the control group. The satisfaction rate in the study group was 97.6%(40/41), which was higher than 82.9%(34/41) in the control group), the difference was statistically significant (χ2=13.279, 13.267, P<0.05). Conclusions For elderly stroke patients, nursing intervention into Maslow's hierarchy of needs theory, help to improve their self-efficacy and nerve function level, and can improve the nursing satisfaction, is worthy of further promotion.

<p><b>OBJECTIVE</b>To evaluate the clinical effects of modified lamina osteotomy replantation versus traditional lamina osteotomy replantation in the treatment of lumbar disc herniation with lumbar instability.</p><p><b>METHODS</b>The clinical data of 146 patients with unilateral lumbar disc herniation with lumbar instability underwent surgical treatment from March 2008 to March 2013 were retrospectively analyzed. Patients were divided into two groups according to osteotomy replantation pattern. There were 77 patients in the traditional group (underwent traditional lamina osteotomy replantation), including 42 males and 35 females with an average age of (49.4±18.5) years;the lesions occurred on L₄,₅ in 46 cases, on L₅5S₁ in 31 cases. There were 69 patients in modified group (underwent modified lamina osteotomy replantation), including 37 males and 32 females with an average age of (49.8±17.9) years;the lesions occurred on L₄,₅ in 40 cases, on L₅S₁ in 29 cases. The operation time, intraoperative blood loss, complication rate during operation, lamina healing rate, recurrence rate of low back and leg pain were compared between two groups. Visual analogue scales (VAS) and Japanese Orthopadic Association (JOA) scores were used to evaluate the clinical effects.</p><p><b>RESULTS</b>The operation time and intraoperative blood loss were similar between two group (>0.05). There was significantly different in nerve injury rate(5.80% vs 16.9%) and dural injury rate(1.45% vs 9.09%) between modified group and traditional group(<0.05). The recurrent rate of low back pain of modified group was higher (91.30%, 63/69) than that of traditional group (76.62%, 59/77), and the intervertebral fusion rate of modified group was lower(8.70%, 6/69) than that of traditional group (29.9%, 23/77) at 3 years after operation. Postoperative VAS scores of all patients were significantly decreased at 6 months, 1, 2, 3 years, and JOA scores were obviously increased (<0.05). At 1, 2, 3 years after operation, VAS scores of modified group were significantly lower than that of traditional group(<0.05), and JOA scores of modified group were higher than that of traditional group(<0.05).</p><p><b>CONCLUSIONS</b>Modified lamina osteotomy replantation has better long-term efficacy(in the aspect of recurrent rate of low back pain, intervertebral fusion rate, VAS and JOA score at three years follow-up) in treating lumbar disc herniation with instability.</p>

BACKGROUND: The posterior anatomical locking plate has obtained excellent fixing performance in the treatment of complex fractures of the distal tibia; however, there are many drawbacks such as nail and plate broken, and the related rules are never reported. OBJECTIVE: To establish a finite element model of posterior anatomical locking plate for distal tibia and to provide references for its design and improvement. METHODS: The imaging data of a male patient aged 34 years old (body mass: 68 kg) with fracture on the left distal tibia were imported into Mimics16.01 software, and the proposed reconstruction parts were determined based on the default threshold of the software. The files were imported into the Ansys11.0 software, the distal tibial surface was fixed, and a force of 340 N was vertically loaded on the supine surface of the horizontal section of the large tibial shaft. RESULTS AND CONCLUSION: There was completely connected with the stepped surface of nut and screw thread portion in locking steel plate. The strain on the plate depended on the stress values. The posterior anatomical locking plate for distal tibia was more favorable for the fixation at the fracture site and could reduce the fixed plate and screws loosening, both of which could affect the fixed efficacy.

Objective To study the effect of over-expression of KLF 4 on the proliferation and migration ability of K562 cells.Methods The K562 cells with KLF4 over expression ( K562-KLF4 cells) were used as experimental group, and paralleled with the vector control ( K562-C1 cells) and blank K562 cell control.Real-time PCR was performed to detect the relative expression quantity of KLF4 mRNA of each group cells respectively;Western blot was performed to detect the level of KLF 4 protein of each group cells respectively .The cell proliferation was tested by MTS assay.The migration ability of K562 cells was detected by Transwell .Results Compared with K562-C1 cells and blank K562 cells, the relative expression of KLF4 mRNA of K562-KLF4 cells was significantly increased (P<0.05).The level of KLF4 protein of K562-KLF4 cells was also significantly increased, by 77.78%(P<0.05).The proliferation ability and migration ability of the K 562 cells with over-expressing KLF4 were inhibited sig-nificantly (P<0.05).Conclusions Over-expression of KLF4 could inhibit the proliferation and migration ability of K562 cells.

Objective To investigate Huaiqihuang particles in treatment of children with bron chialasthma and effect on immune function.Methods 100 cases of children with bronchial asthma were randomly divided into control group and treatment group,50cases in each group,all were given routine western medicine treatment,and the treatment group combined with Huaiqihuang particles.The levels of interleukin-4 (IL-4),IL-5 and interferon gamma (IFN-γ),diurnal and nocturnal symptom scores before and after treatment and clinical efficacy and recurrence rate in followed up for 3 months in two groups were compared.Results After treatment,serum IL-4,IL-5 levels of the two groups were significantly decreased,IFN-γ levels were significantly increased in the same group before and after treatment,the difference was statistically significant (P < 0.05).,serum IL-4 and IL-5 levels of treatment group were significantly lower than the control group,IFN-γ levels were significantly higher than the control group,there was statistically significant difference between groups (P < 0.05).After treatment,daytime and nighttime symptom scores of two groups were significantly reduced than before treatment (P < 0.05).And the treatment group was significantly lower than the control group (P < 0.05).The total effective rate of the treatment group (96%) was significantly better than that of the control group (82%) (P < 0.05).The treatment group recurred in 3 cases (6%),and the control group relapsed in 12 cases (24%).The recurrence rate of the treatment group was lower than that of the control group (P < 0.05).Conclusion Huaiqihuang particles can effectively improve the immune function of patients with bron chialasthrna,enhance efficacy and reduce recurrence.

Objective Determination of extractable components in medium/low borosilicate glass controlled injection bottles under 5 different extraction conditions,assess the risk of drug safety by the amount of migration.Methods Determination of 24 elements in extraction solution by ICP-MS.Result Establish the standard curves of 24 elements,and the detection limit and quantitative limit were determined.The determination of the elements in the 5 extraction solutions was completed and their safety was evaluated.Conclusion Medium/low borosilicate glass injection bottle for packing freeze-dried products of Tianjin Tasly Pride Pharmaceutical Co.,Ltd.,the daily intake of Li、V、Cr、Fe、Co、Ni、Cu、Zn、As、Cd、Sb、Ba、Hg、Pb was lower than permitted daily exposure,the amount of migration does not pose a risk to drugs,therefore,the injection quality is not affected.

Objective To construct a scenario database for high altitude military medical comprehensive exercises to enhance the training efficiency.Methods Related documents were used as guidelines for construction.Information technologies were used as construction support.The hierarchical model was used as a technical framework.JAVA,Linux,Oracle and Tomcat were used as development tools.Results First,the foundation of the system was elaborated regarding of guiding ideology,construction principles and standards.Second,the architectural design of the system was stated around the functional framework,technical framework and information framework.Third,the implementation of the system was outlined in terms of development tools and function modules.Conclusion The scenario database can provide standard data resources for related simulation training systems and perfect scenario support for high altitude health service training,which can contribute to the training reform and the improvement of training quality.

Objective:To study and design a disaster recovery backup system that not only can meet the continuous requirement of medical services but also can ensure data so as to achieve a data center plan that has maximum utilization ratio of resources. Methods: Face to the current situation of constructing data center of hospital, the cloud computing, virtualization technique, dual-active storage and database cluster were combined in the research, and through established calculation, storage, network, desktop and safety to finally achieve five private cloud included of producing cloud, testing cloud, disaster recovery cloud, desktop cloud and secure cloud. And then, based on above design to establish a plan about dual-active data center of private cloud included of recovery point objectives(RPO)=0 and recovery time objective(RTO)≈0.Results: This research has designed a dual-data center of private cloud with high availability, high performance, high expansion and high convenience management. It has realized simple and convenient management and operation for all data centers, and enhanced the utilization ratio of data resources.Conclusion: The dual-active data center of private cloud can farthest realize full use for resource and save investment and construction costs of IT infrastructure under the preconditions of meeting the continuous requirement of medical services.

RESUMEN

The novelty assessment of doctorial dissertations in Chinese Academy of Traditional Chinese Medicine was outlined, the selected topics and innovative characteristics of doctorial dissertations in Chinese Academy of Tra-ditional Chinese Medicine from 2012 to 2016 were summarized, the problems in their novelty assessment were ana-lyzed, and the following measures were put forward for improving the novelty assessment of doctorial dissertations, such as adjusting its timetable, finding its new model, carrying out its training for doctors, and attaching highly concern to it by the governments.

RESUMEN

Objective: To observe the effect of heat-sensitive moxibustion plus psychological intervention on serum contents of substance P (SP) and 5-hydroxytryptamine (5-HT), as well as the quality of life (QOL) in patients with irritable bowel syndrome (IBS). Methods: A total of 120 IBS patients were divided into three groups by random sampling method, 40 cases in each group. The heat-sensitive moxibustion group received heat-sensitive moxibustion, the psychological intervention group received psychological intervention, while the observation group received both methods. The therapeutic efficacy was evaluated after 3 courses of treatment. Results: After treatment, the therapeutic efficacy of the observation group was better than that of the heat-sensitive moxibustion group and the psychological intervention group (P<0.01), and the heat-sensitive moxibsution group was better than the psychological intervention group (P<0.05). In comparing the bowel symptom scale (BSS), the observation group was better than the other two groups (P<0.05), and there was a significant difference between the heat-sensitive moxibustion group and the psychological intervention group (P<0.05). In the comparison of QOL, the observation group was significantly superior to the other two groups (P<0.05), and there was a significant difference between the heat-sensitive moxibustion group and psychological intervention group (P<0.05). In comparing serum SP and 5-HT, the observation group was markedly better than the other two groups (P<0.05), and there were significant differences between the heat-sensitive moxibustion group and psychological intervention group (P<0.05). The self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were used to estimate the mental state of the three groups, revealing that the observation group was better than the heat-sensitive moxibustion group and the psychological intervention group (P<0.05), and there were significant differences between the latter two groups (P<0.05). Conclusion: Heat-sensitive moxibustion plus psychological intervention can regulate the levels of SP and 5-HT, improve BSS, SDS, SAS and QOL in treating IBS.
