RESUMEN

Objective:To investigate the effect of mechano-growth factor(MGF)on osteoclast activity and its mechanism. Methods:The RAW264.7 precursor osteoclast cell line was cultured with 25 ng/ml macrophage-colony stimulating factor(M-CSF)and 30 ng/ml receptor activator of NF-κB ligand(RANKL),and identified by tartrate resistant acid phosphatase(TRAP)staining after 7 days of culture. Western blot anslysis was used to determine the effect of 45 ng/ml MGF on the phosphoinositide-3-kinase/protein kinase B(PI3K/AKT)signaling pathway in separated osteoclasts, including levels of AKT,phosphorylation(p)-AKT,lactation mammalian target of rapamycin(mTOR),p-mTOR and TRAP at 0,4,8 and 12 hours. Real-time fluorescence quantitative PCR was used to expressions of TRAP in osteoclasts at 0,4,8 and 12 hours. The PI3K/Akt phosphorylation inhibitor LY294002(20 μmol/L)combined with MGF(45 ng/ml)was used to act on osteoclasts, and expression levels of Akt,p-Akt,mTOR,p-mTOR and TRAP were detected by Western blot at 0,4,8 and 12 hours. Results:After culturing RAW264.7 cells with M-CSF and RANKL for 7 days,a large number of osteoclasts with positive TRAP staining can be obtained. Western blot analysis showed expression levels of Akt and mTOR did not change significantly over time( P>0.05),expression levels of p-Akt and p-mTOR increased continuously from(2.18±0.34)pg/ml and(0.83±0.10)pg/ml at 0 hour to(3.86±0.36)pg/ml and(1.56±0.19)pg/ml at 12 hours( P<0.05),and expression level of TRAP decreased significantly over time,from(5.66±0.47)pg/ml at 0 hour to(3.76±0.38)pg/ml at 12 hours( P<0.05). Real-time fluorescence quantitative PCR analysis of expression of TRAP in osteoclasts showed that MGF inhibited the expression of TRAP in osteoclasts, which decreased from 1.02±0.06 at 0 hour to 0.53±0.11 at 12 hours( P<0.05). After acting LY294002 combined with MGF on osteoclasts,Western blot analysis showed expression levels of Akt and mTOR did not change significantly over time( P>0.05),expression levels of p-AKT and p-mTOR decreased significantly from(3.28±0.18)pg/ml and(3.29±0.22)pg/ml at 0 hour to(2.06±0.34)pg/ml and(2.04±0.20)pg/ml at 12 hours( P<0.05),and expression level of TRAP had no significant difference over time( P>0.05). Conclusions:MGF inhibits osteoclast activity by inhibiting the expression of TRAP in osteoclasts through PI3K/Akt signaling pathway. LY294002 inhibits the expression of PI3K/Akt signaling pathway in osteoclasts, further verifying the mechanism of MGF inhibiting osteoclast activity, and this finding puts forward new ideas for clinical prevention and treatment of osteoporosis.

The patients with intertrochanteric fracture complicated with pseudoaneurysm were selected as the research objects, and the duplicate cases were deleted; the age, gender, onset age, etiology, location, treatment and prognosis of the disease were retrospectively analyzed. Results:A total of 54 cases of femoral intertrochanteric fracture related pseudoaneurysms were selected from 45 literatures, including 25 males and 27 females, and 2 cases had no gender; aged from 43 to 94 years old, and 4 cases were younger than 60 years old, 6 between 61 and 70, 17 between 71 and 80, and 25 between 81 and 90. Forty-five cases occurred in deep femoral artery, 3 cases in superficial femoral artery, 1 case in superior gluteal artery, 1 case of medial circumflex artery, 1 case of lateral circumflex femoral artery, 1 case of femoral artery, and 2 cases of unknown location. The occurrence time of pseudoaneurysm: 17 cases within 1 week after fracture, 8 cases from 1 week to 3 weeks, 19 cases from 3 weeks to 3 months, 2 cases more than 3 months, and 8 cases unknown. The causes of pseudoaneurysm: among the 54 patients, 41 cases had definite causes, including 21 cases of vascular injury caused by fracture block; 20 cases of iatrogenic injury, of which the incidence of direct injury of blood vessels by screw or drill was the highest, accounting for 55% (11/20). Twenty-two cases were treated with vascular embolization, 10 cases with surgical sutures, 8 cases with surgical ligation, 4 cases with resection of pseudoaneurysm, 5 cases with covered stent, and 4 cases with thrombin injection. There were 42 cases who had follow-up data, and no recurrence and other complications were found during the follow-up period. Conclusion:The peak age of femoral intertrochanteric fracture complicated with pseudoaneurysm was 71-90 years old. The incidence of femoral intertrochanteric fracture complicated with pseudoaneurysm was more often within 1 week after fracture and 3 weeks to 3 months after fracture, due to fracture fragments and iatrogenic vascular injury caused by the location of the profundus femoris artery. Treatment options include vascular embolization, surgical ligation, suture or resection of pseudoaneurysm, covered stent and thrombin injection, and the prognosis is generally good.

RESUMEN

Objective:To explore the curative efficacy of pelvic reconstruction plate and T-plate elastic fixation in the treatment of acetabular fractures with comminuted posterior wall. Methods:A retrospective analysis was conducted of the 21 patients who had been treated at Department of Orthopaedics, Affiliated Hospital to the Second Mongolia Medical University for acetabular fractures with comminuted posterior wall from January 2017 to June 2019. They were 15 males and 6 females, aged from 29 to 55 years (average, 41.5 years). According to the Letournel-Judet classification, there were 15 cases of simple posterior wall fracture with dislocation of the femoral head, 4 cases of posterior wall and posterior column fracture, and 2 cases of double-column and posterior wall fracture. The time from injury to surgery averaged 3 days (from 2 to 5 days). The posterior acetabulum was treated by pelvic reconstruction plate combined with T-plate elastic fixation through the posterior acetabular Kocher-Langenbeck approach. Postoperative fracture reduction, fracture union time, function of the affected hip and complications at the last follow-up were evaluated. Results:This group of 21 patients were followed up for 6 to 24 months (average, 15 months). By the Matta imaging scoring, the postoperative reduction of the posterior wall fracture was evaluated as excellent in 18 cases and as good in 3, giving an excellent to good rate of 100%. The fracture union time averaged 10 weeks (from 8 to 12 weeks) for this group. By the improved Merle d'Aubigné & Postel evaluation at the last follow-up, the affected hips scored from 12 to 18 points (average, 16 points), yielding 18 excellent, 2 good and one poor cases, giving an excellent to good rate of 95.2%. There was no major hemorrhage, nerve injury or deep vein thrombosis intraoperatively. During the follow-up, mild ectopic ossification occurred in one case, and subluxation of the femoral head and traumatic arthritis were observed in another, but no patient had other complications like avascular necrosis of the femoral head. Conclusion:In the treatment of acetabular fracture with comminuted posterior wall, pelvic reconstruction plate and T-plate elastic fixation through the posterior acetabular Kocher-Langenbeck approach can lead to fine short-term outcomes.

RESUMEN

Pelvic fracture is a common serious injury in traumatic orthopedics. Internal fixation with percutaneous sacroiliac screws is an effective minimally invasive method widely applied in clinic which enables the patients to carry out early weight-bearing and functional exercise. Novel auxiliary placement techniques for sacroiliac screws have been introduced into the field of orthopaedics to improve placement accuracy and reduce operative risk, such as 3D navigation, 3D printing and orthopedic robots. These new techniques have advantages of precision and convenience, but their disadvantages and limitations cannot be ignored. This article reviews the research progress in the above 3 auxiliary techniques for placement of sacroiliac screws, and discuss their advantages, disadvantages and safety, hoping to provide a reference for clinicians who want to choose a new auxiliary placement technique in the treatment of pelvic fractures with sacroiliac screws.

RESUMEN

Objective:To analyze the biomechanical characteristics of bilateral lumbo-iliac fixation by finite element analysis after digital optimization of a safe screw pathway. Methods:Firstly, a healthy adult male volunteer was recruited for this study who underwent CT scanning of the lumbus and pelvis, with a slice thickness of 0.625 mm.The original data were imported into Mimics 16.0 software for 3D lumbar and pelvic reconstruction. At the same time, a geometric screw pathway and a free screw pathway were generated in the pedicles of L4 and L5 and ilium.Secondly, all 3D models were imported into Ansys 12 software to assemble a finite element model. The displacements and von Misses stress distribution were measured in the conditions of forward bending, backward extending, lateral bending to the left and right, and left and right rotations in the models of 2 kinds of pathway after gravity axial load of 500 N was applied. Results:The models of 2 different pathways were similar in comprehensive displacement and stress in the conditions of forward bending, lateral bending and backward extending, and in the maximum stress of the screw in the conditions of forward bending and lateral bending. However, the comprehensive displacement and stress in the condition of rotation and the stress of the screw in the conditions of backward extending and rotating in the model of geometric screw pathway were obviously smaller than those in the model of free screw pathway. The maximum pelvic stress was on the screw surface between 190 and 260 MPa, concentrated on the bending area of the 2 screws connecting the sacrum and vertebral body. The lumbar-iliac fixation model showed high com-pressive stiffness. The maximum von Misses stress appeared in the L4-L5 pedicle screw on the longitudinal rod.Under rotational loading, the upper longitudinal rod connecting the L4-L5 pedicle screw and the iliac bone screw bore the maximum stress. Conclusions:Our finite element analysis shows that double verte-bral bodies and long screw fixation are effective ways of iliolumbar fixation. The geometric screw path optimized by digital analysis exhibits better fatigue resistance, rotation resistance and overall stability.Internal lum-bo-iliac fixation can effectively share the stress of vertebral bodies.

RESUMEN

BACKGROUND:Mechano growth factor has the potential to activate muscle satelite cels and promote myogenic cel growth, and has dual roles in maintaining bone mass and repairing bone defects. OBJECTIVE: To explore the mechanism underlying osteogenic differentiation of rabbit bone marrow mesenchymal stem cels promoted by the mechano growth factor. METHODS:The best concentration and time of mechano growth factor to promote osteogenic differentiation of rabbit bone marrow mesenchymal stem cels were detected by MTT. The mRNA and protein expressions of alkaline phosphatase and osteocalcin were detected by qPCR and western blot, respectively. The phosphorylation level of AKT and mTOR were detected by western blot assay. RESULTS AND CONCLUSION:The best concentration and time of mechano growth factor was 45 μg/L and 5 days for promoting the osteogenic differentiation of rabbit bone marrow mesenchymal stem cels. The expressions of alkaline phosphatase and osteocalcin at mRNA and protein levels were highest after 4-hour intervention with 45 μg/L mechano growth factor, and meanwhile, the phosphorylation levels of mTOR and AKT were also highest. These findings indicate that the mechano growth factor can promote the differentiation of rabbit bone marrow mesenchymal stem cels into osteoblastsvia PI3K/AKT pathway, and its best concentration and time are 45 μg/L and 4 hours, respectively.

RESUMEN

Pituitary tumors are clinically common neoplasms involved in the central nervous system. The endoscopic endonasal approach is a safe and feasible technique for managing pituitary tumors. Endoscopic technology has been increasingly used in clinical practice because it results in limited nasal injury, decreased operating time and postoperative complications, and quick postoperative re-covery. This study aims to evaluate the advantages and effectiveness of the endoscopic endonasal transsphenoidal approach for the re-moval of pituitary tumors. This approach should be further modified and optimized in clinical trials and practices.

Objective To investigate the regulatory effect of CsA on the expression of NK cell inhibitory receptor ILT4 and cytotoxicity of NK cells. Methods NK cells treated with CsA ( 10 mg/L) or DMSO for 12,24 and 36 h were chosen as three experimental groups and control groups respectively.RTqPCR and flow cytometry were performed to detect the alteration of ILT4 at the mRNA and protein level respectively. The expression of HLA-G in human gastric cancer cell line BGC-823 and human placental choriocarcinoma cell line JEG-3 were measured at the same time, and then the cytolytic activity of the untreated NK cells and NK cells treated with CsA for 36 h against BGC-823 and JEG-3 cells was determined with MTT.One-way analysis of variance was employed to compare the different ILT4 expression at different time points after medication; Dunnett test was performed to carry out the pairwise comparison between each mean. The difference of HLA-G expression between JEG-3 cells and BGC-823 cells, and the difference of NK cell cytolytic activity against JEG-3 cells and BGC-823 cells were analyzed by student's t-test.Results RT-qPCR assay indicated that the relative levels of ILT4 mRNA in NK cells treated with CsA for 12,24 and 36 h in turn were 0.99 ± 0.27,1.79 ± 0.29,6.79 ± 0.64,and those of their contrast groups treated with DMSO were 0.86 ±0.11,0.94 ±0.12,1.06 ±0.17.The expression of ILT4 in NK cells treated with CsA for 24 h or 36 h was higher than that in NK cells of their contrast groups respectively ( t value of 4.69,14.99,P <0.05,respectively),but there was no significant difference between the two groups of NK cells treated for 12 h ( t =0.78,P >0.05 ).Through flow cytometry, the positive rates of ILT4 protein expression in NK cells treated with CsA for 12,24 and 36 h [(5.16 ± 0.42 ) %,( 6.23 ± 0.48 ) %,( 23.8 ± 1.5 ) %]were higher than those in NK cells after treatment with DMSO for 12,24 and 36 h respectively[(3.08 ±0.19)%,(3.35 ±0.12)%,(3.36 ±0.21 )% ;t value of 7.70,10.06,20.72,P<0.01,respectively].The expression of ILT4 in NK cells treated for 36 h was much higher than that in NK cells for 12 and 24 h at the mRNA and protein level (t value of 16.38,14.12 ;21.81,20.56,P < 0.01,respectively).Meanwhile the killing rates of NK cells treated with 10∶1 effector-target ratio CsA on BGC-823 cells (low HLA-G expression) were ( 8 1.96 ± 2.80 ) % ( before treatment) and ( 60.23 ± 1.57 ) % ( after treatment),which were higher than those on JEG-3 cells (HLA-G-overexpression) [(53.46 ±2.21 )% ( before treatment),(28.30 ± 1.85 ) % ( after treatment)].The changes of cytotoxicity of NK cells treated with CsA against target cells showed that CsA inhibited the killing activity of NK cells to BGC-823 and JEG-3 cells (t value of 11.74,15.16,P<0.01,respectively),and the inhibitory rates were (26.48 ±2.42)% and (47.10 ±1.59 ) % respectively.CsA had a higher killing rate inhibition on JEG-3 than on BGC-823 ( t =12.31,P <0.01 ).Conclusion CsA induces upregulation of ILT4 in NK cells, and the cytotoxicity of NK cells to tumor cells can be affected by interaction of ILT4 and HLA-G.

RESUMEN

Objective:To investigate the expression and relationship of differentiated embryo-chondrocyte expressed gene1(DEC1) and the signal tranducers and activator of transcription3 (STAT3) in gastric cancer tissues, and the significance of the two genes in gastric carcinogenesis. Methods:Immunohistochemistry method of SP was adopted to discover the expression of DEC1 and STAT3 in 59 gastric cancer tissues,19 normal tissues distant from tumor were regarded as control. Results:The positive expression rate of DEC1 in gastric cancer tissues was 71.2%(42/59),which was significantly higher than that in normal para-cancer tissues(26.3%)(P0.05). STAT3 expression was associated with TNM staging, invasion depth, lymph node involvement and tumor grade (P0.05). Conclusions:DEC1 expression was higher in gastric cancer tissues than that in normal para-cancer tissues distant from tumor, and was correlated with STAT3 in gastric cancer. DEC1 and STAT3 are both transcription factors, therefore the co-activation of DEC1 and STAT3 may play an important role in the carcinogenesis and development of gastric cancer.

RESUMEN

BACKGROUND: Bone defects forming after resection of bone tumor and tumor-like lesion are often packed with autogeneic bone. But sample amount cannot completely meet the clinical demand and different degrees of complications are often left in the bone graft donor region. All these problems remarkably limit the application of autogeneic bone. Allogenic bone is increasingly widely used in the clinical practice due to its structure and biological characteristics similar to autogeneic bone, rich sources, long-term preservation, easy to use and other advantages. OBJECTIVE: To investigate the biocompatibility and clinical application effects of allogeneic bone in filling and repairing benign bone tumor and tumor-like lesion after resection and curettage.DESIGN: A retrospective analysis. SETTING: Department of Pelvis Surgery, Second Affiliated Hospital of Inner Mongolia Medical College. PARTICIPANTS: Totally 230 patients (156 males and 74 females, with age of 5-56 years) were admitted to Department of Bone Tumor, Second Affiliated Hospital of Inner Mongolia Medical College from December 1999 to December 2005 to undergo curettage and hyperthermia inactivation due to benign bone tumor and tumor-like lesion and to receive the treatment by filling and repairing bone defects with lyophilized small-segment allogeneic bone, and were recruited for this study. Written informed consents of treatment were obtained from all the patients. The protocol was approved by the Hospital's Ethics Committee. METHODS: Allogeneic bone grafts (Shanxi Aorui Biomedical Co.,Ltd /Shanxi Provincial Medical Tissue Banking) were used to fill and repair bone defects. Patients who had benign bone tumor, bone cyst or osteofibrous dysplasia underwent cyst curretage. Allogeneic bones were used to pack empty cavity. Therapeutic effects were assessed according to the scoring criteria of allogeneic bone transplantation from Mankin et al, consisting of satisfactory and unsatisfactory two levels. X-ray plain films of surgery sites were taken at postoperative 3, 6 and 12 months. The patients were followed up for 38 months on average in order to observe the therapeutic effects. MAIN OUTCOME MEASURES: Histocompatibility of allogeneic bone in filling and repairing bone tumor defects. RESULTS: All 230 patients participated in the final analysis. ① Biocompatibility of allogeneic bone: Postoperatively, minority of patients had mild immunological rejections. Such allogeneic bone grafts had a good biocompatibility. They could directly fuse with bone tissue in the implantation location of patients, but not inhibit the normal activity of osteocytes on the allogeneic bone grafts or interfere the natural substitution of autogeneic osteocytes, i.e. there were no or less immunological rejections. Bone union was obtained in all the patients at postoperative 6-18 months (6.5 months on average). Thirty-four patients presented exudation of light yellow liquid from incision. Incisions healed in 30 (14.8%) patients at postoperative 2 weeks and in 4 (1.7%) in later time. Altogether 196 (85.2%) patients obtained satisfactory therapeutic effects, but 34 (14.8%) obtained unsatisfactory therapeutic effects. CONCLUSION: Small-segment allogeneic bones have good histocompatibility and osteogenesis, and they are good bone grafts in the bone transplantation.

RESUMEN

Objective: To investigate the apoptosis-inducing effect of arsenic trioxide (As2O3) on gastric carcinoma cell line AGS in vitro and to assess the influence of As2O3 on the expression of signal transducers and activators of transcription 3 (STAT3) and vascular endothelial growth factor (VEGF). Methods: AGS cells were treated with different concentrations of As2O3 (1, 5, and 10 ?mol/L) for 24,48, and 72 h. The cell proliferation was detected by MTT assay, cell apoptosis and cell cycle distribution were measured by flow cytometry and TUNEL, and the expression of STAT3 and VEGF was investigated by ELISA, immunohistochemistry and real-time PCR. Results: (1) As2O3 inhibited AGS cell proliferation in a time- and dose-dependent manner; (2) FCM results showed a typical sub-diploid peak before G0/ G1 phase and cell cycle analysis showed G2/M phase arrest; (3) TUNEL analysis revealed the DNA fragmentation; (4) During the As2O3-induced apoptosis of AGS cells, the expression of STAT3 and VEGF was down-regulated, especially when As2O3 was at 10 mol/L. Conclusion: As2O3 can inhibit the proliferation of AGS cells and induce AGS cell apoptosis, which might be related with cell cycle block and down-regulation of STAT3 and VEGF expression.
