RESUMEN

BACKGROUND@#This study aimed to develop a comprehensive instrument for evaluating and ranking clinical practice guidelines, named Scientific, Transparent and Applicable Rankings tool (STAR), and test its reliability, validity, and usability.@*METHODS@#This study set up a multidisciplinary working group including guideline methodologists, statisticians, journal editors, clinicians, and other experts. Scoping review, Delphi methods, and hierarchical analysis were used to develop the STAR tool. We evaluated the instrument's intrinsic and interrater reliability, content and criterion validity, and usability.@*RESULTS@#STAR contained 39 items grouped into 11 domains. The mean intrinsic reliability of the domains, indicated by Cronbach's α coefficient, was 0.588 (95% confidence interval [CI]: 0.414, 0.762). Interrater reliability as assessed with Cohen's kappa coefficient was 0.774 (95% CI: 0.740, 0.807) for methodological evaluators and 0.618 (95% CI: 0.587, 0.648) for clinical evaluators. The overall content validity index was 0.905. Pearson's r correlation for criterion validity was 0.885 (95% CI: 0.804, 0.932). The mean usability score of the items was 4.6 and the median time spent to evaluate each guideline was 20 min.@*CONCLUSION@#The instrument performed well in terms of reliability, validity, and efficiency, and can be used for comprehensively evaluating and ranking guidelines.

RESUMEN

Objective@#In view of the difficulty of the shortage of new coronavirus nucleal acid test in the early COVID-19 outbreak, to explore the application value of chest CT in screening COVID-19 patients.@*Methods@#Retrospective analysis was performed on the data of patients with fever who received chest CT and new coronavirus nucleal acid test during January 25, 2020 to February 2, 2020 in Zhongnan Hospital of Wuhan University. A total of 587 patients were enrolled, including 290 males and 297 females, aged from 11.0 to 96.0 (51.3±17.1) years old. Take the nucleic acid test results as the gold standard, the sensitivity, specificity and rate of missed diagnosis of CT screening COVID-19 were calculated.@*Results@#Among the 587 patients, there were 433 positive cases (73.8%, 433/587) and 154 negative cases (26.2%, 154/587) of novel coronavirus nucleic acid test. Using CT screening, 494 cases (84.2%, 494/587) were positive and 93 cases (15.8%, 93/587) were negative. The sensitivity of CT screening COVID-19 was 97.7% (423/433), specificity was 53.9% (83/154) and rate of missed diagnosis was 2.3% (10/433).@*Conclusions@#In the early COVID-19 outbreak, CT screening has the advantages of high sensitivity and low rate of missed diagnosis of COVID-19, which can compensate for the shortage of new coronavirus nucleal acid test and can be used as the basis for rapid screening for early prevention and control of COVID-19 outbreak.

RESUMEN

OBJECTIVE:To systematically evaluate the effectiveness and safety of escitalopram and duloxetine in the treatment of depression, and provide evidence-based reference for clinical treatment. METHODS:Retrieved from PubMed, Wanfang database,VIP,CNKI and CBM,randomized controlled trials (RCTs) about escitalopram (trial group) and duloxetine (control group) in the treatment of depression were collected. Meta-analysis was conducted by using Rev Man 5.3 software after data extraction and quality evaluation according to bias risk assessment tool recommended by system evaluator manual 5.3. RESULTS:Finally 25 RCTs were included, involving 2621 patients. The results of Meta-analysis showed that there was no statistical significance in total response rate between 2 groups after 1,2,4,6,8 weeks of treatment or cure rate between 2 groupsafter 4,6,8 weeks of treatment (P>0.05). There was no statistical significance in total response rate [RR=0.96,95%CI(0.88, 1.05),P=0.42] or cure rate [RR=0.91,95%CI(0.78,1.06),P=0.24] of female patients,as well as total response rate [RR=0.96, 95%CI(0.84,1.11),P=0.61] or cure rate [RR=0.90,95%CI(0.54,1.49),P=0.69] of elderly patients between 2 groups. The incidence of constipation [RR=0.59,95%CI (0.42,0.81),P=0.001],dry mouth [RR=0.65,95%CI(0.51,0.82),P=0.0004], nausea [RR=0.68,95%CI(0.56,0.83),P=0.0002] and decreased appetite [RR=0.74,95%CI(0.55,0.99),P=0.04] in trial group were significantly lower than control group, with statistical significance. CONCLUSIONS:The effectiveness of escitalopram is similar to duloxetine in the treatment of depression, but the safety of escitalopram is better than duloxetine.

RESUMEN

Objective To investigate the level of professional commitment of student nurses, and explore the impact of student nurses' role conflict and role ambiguity on their professional commitment. Methods From May to June 2014,totally 320 student nurses from a third-level hospital in Tianjin were recruited in this study. And Role Stress Scale and Nursing Professional Commitment Scale were used in the investigation. Results The total score of role stress of student nurses was (44.29±5.53) scores, and the total score of professional commitment of student nurses was (86.12±14.14) scores.Pearson correlation analysis and multiple linear regression analysis showed that student nurses' role conflict was positively correlated with professional commitment, but their role ambiguity was negatively correlated with professional commitment. Conclusion School and hospital managers should adopt effective measures to relieve student nurses' role ambiguity and guide the correct understanding of role conflict to maintain a stable nursing team.

RESUMEN

Objective The aim of this study was to evaluate reporting quality of researches about interviews and focus groups in Chinese nursing field. Methods All nursing specialization journals in China mainland were searched from the inception till December 2013.The search was performed on four Chinese databases:China National Knowledge Infrastructure Database,Chinese Biomedical Literature Database,Wanfang database and VIP database. The reporting quality was assessed according to COREQ standard. Results A total of 2 758 qualitative researches were identified, among which 789 articles accorded with the inclusion and exclusion criterion. These articles were published on 19 journals such as the Chinese Journal of Nursing and the Chinese Journal of Practical Nursing.Non of the articles accorded with the COREQ standard. The flaws of reporting quality were reflected in the inadequate description of personal characteristics, incomplete reporting of data collection and the deficiency of the coding tree. Conclusions From 2004 to 2013,qualitative literature in nursing field increased year-by-year.Till now, most of the qualitative studies on nursing showed flaw of different degrees, which need continuous improvement.

RESUMEN

Objective The article is prepared to evaluate the efficacy of Chinese acupressure and ventral massage on prevention and treatment of constipation. Methods Electronic databases such as CNKI,VIP,CBM,Wanfang,EBSCO,PubMed and the Cochrane Library were searched from their establishment to December 2013 to meet the inclusion criteria of randomized and quasi-randomized controlled trials.Meta-analysis was conducted using RevMan 5.2 and Stata 12.0 software. Results A total of 29 RCTs involving 3 084 participants were included. The Meta analysis showed that compared with the control group, the new option for prevention and treatment tended to be more effective for constipation.Compared with the control group, first defecation time, defecation difficulty rate, defecation time, dry stool rate and defecating unfinished feeling rate of the experimental group were lower.Laxative provided for the experimental group was less than that of the control group. Conclusions On the basis of available evidence, the traditional Chinese acupressure and ventral massage treatment are effective and safe in prevention and treatment of constipation.
