RESUMEN

PURPOSE: To compare the success rates and stabilities of postoperative alignment between adjustable and the non-adjustable surgeries in the treatment of sensory exotropia. METHODS: A retrospective analysis was performed on all patients with sensory exotropia who had undergone unilateral lateral rectus recession and medial rectus resection (R&R) between January 1998 and August 2005. Thirty-four patients underwent conventional R&R, and 20 patients underwent R&R with adjustable suture of the lateral rectus. The surgical results between the two groups were analyzed with regard to the preoperative and post-operative deviation angles and the postoperative drift. The postoperative deviation angle was measured on postoperative day 1 as well as at two weeks, three months, six months and the final visit after surgery. RESULTS: There were no statistically significant differences in the mean preoperative and postoperative deviation angles between the two groups. In 30 (88%) patients in the non-adjustable group and 15 (75%) patients in the adjustable group, postoperative deviation was less than 15 prism diopters (PD) at the three month follow-up. There was no significant difference in the mean postoperative drift between the two groups. CONCLUSIONS: Strabismus surgery with adjustable sutures did not show a significantly better result than surgery without adjustable sutures in the treatment of sensory exotropia. Considering the amount of postoperative exodrift in both groups, we postulate that the immediate ocular alignment after surgery for sensory exotropia should be orthophoric or 5-6 PD of esodeviation.

RESUMEN

PURPOSE: To describe a case of symptomatic rifabutin-related uveitis with hypopyon and vitreous opacity in apatient with acquired immunodeficiency syndrome infected with Mycobacterium tuberculosis. CASE SUMMARY: A 33-year-old male patient with acquired immunodeficiency syndrome was referred to our clinic for abruptly decreased vision in his right eye. Multi-drug therapy with rifabutin was administered for 5 weeks to treat tuberculosis enteritis and pulmonary tuberculosis. Visual acuity of the right eye was hand motion and hypopyon as well as vitreous opacity was found in ocular examinations. Other serologic tests, anterior chamber paracentesis and lumbar puncture test were normal. Rifabutin was immediately stopped and topical steroid and cycloplegics were administered, which resulted in resolution of the hypopyon, vitreous opacity and visual acuity. Four weeks after the initial episode, rifabutin was restarted to treat the pulmonary tuberculosis and rifabutin-related uveitis relapsed in the opposite eye. CONCLUSIONS: Rifabutin-related uveitis should be considered in cases of uveitis in immunosuppressive patients, especially in acquired immunodeficiency syndrome patients. Underlying disease and medication history should be carefully assessed.

RESUMEN

Total hip replacement is an available rnethod of treatment for various causes of disabled hips in adults, especially in painful stiff hips. In many cases, malposition has led to degenrative changes in the opposite hip, the lumbar spine or knee, often with severe loss of function due to pain. Between March, 1979 and February, 1983, twenty hips in sixteen patients with painful and deformed hips were operated at Kosin Medical College, and we obtained relif of pain and increase of range of motion of the hip joints. Furthermore we found relife of adjacent joint pain in several cases, and in addition correction of inequality of leg lenght.

RESUMEN

Overgrowth and hypertrophy of one or more extremities is an infrequentIy encountered congenital or acguired anomaly, It is referred to in the literature by different authors as local gigantism, hypertrophy etc. The etiology remains unexplained and the condition is only seldom found to be familiar. Hypertrophy of left lower extremity associated with neurofibromatosis of the sural nerve in the calf, in a 8 years old school boy, is presented with a review of the literature. Chief complaints were enlargement of left lower leg and limping gait since 5 years old. There was no change in sensory and motor function of the involved leg. X-ray films showed enlargement of tibia and fibula and diffuse hypertrophied soft tissue shadow in the left leg. One case of local excision of hypertrophied sural nerve and subcutaneous fat tissues was presented.

RESUMEN

The purpose of this study was to analyse th results of closed, and open reductions of displaced proximal humeral fractures, and to evaluate the technical aspects of thtreatment of the ese fractures. Forty six cases of these fractures, followed for from six months to five years, were presented. 1) The age of the patients ranged from thirty to forty years, and the distribution was almost equal. 2) The majority of these fractures were caused by traffic accidents. 3) The fractures were classified according to Neers method. The majority of the cases were 2-part fratsures, rating 52%, while 4-part fractures rated 2. 3%.4) The cases with 1-part and 2-part fractures were generally treated by conservative methods and open reductions were performed only in cases wilh 3-part or 4-part fractures and in a few unsuccessful closed reduction cases of 2-part fracture. 5) The results of these patients were rated by the Neers numerical system. Of forty six cases, thirty eight had exellent results, five, good results, and three, poor results.
