RESUMEN

Congenital hepatic fibrosis is a developmental abnormality that may appear either sporadically or in a familial form. It is an inherited disease defined pathologically by bands of fibrous tissue within the liver, and is occasionally associated with cystic kidney disease. A 21-year-old woman was admitted to our hospital for evaluation of pancytopenia. She showed esophageal varices, hepatomegaly and splenomegaly, but had normal results on her liver function test. Peripheral stigmata of chronic liver disease such as palmar erythema or spider angioma was not found. Hepatosplenomegaly, polycystic kidney and psoas muscle cyst were detected through an abdominal CT and MRI. The patient is diagnosis was confirmed as congenital hepatic fibrosis using laparoscopic liver biopsy. The first case of congenital hepatic fibrosis associated with polycystic kidney disease in Korea is herein reported.

RESUMEN

We studied a group of spinal cord injured patients, using two different mattresses, to analyze statistically the pressure measured over sacral area and skin changes developed on the dorsal skin surface of patients for pat ients for specific duration of time. Local pressure measured sacral area and skin change score were lower on a Bazooka system than common hospital mattress. And the weight. BMI(Body Mass Index) and % IBW(Ideal Body Weight) of patients significantly correlated with the skin changes developed on the dorsal skin surface after lying on common hospital mattress for 2 hours. But skin changes developed after lying on a Bazooka system for 8 hours didn't correlated with these variables. Therefore a Bazooka system may be effective in the prevention of pressure ulcers for spinal cord injured patients.

RESUMEN

The medical records of two hundred and sixteen(216) cases of renal cell carcinoma during the period. January 1980 to June 1991, were investigated and analyzed with particular attentions on incidental discovery, stage and prognosis ; and value of ultrasonographic examination for early detection. Of 216 cases, 65 cases(30.1%) were diagnosed incidentally ; 10.4% in 1980-1985. 21% in 1986-1989, 65% in 1990 and 68.4% in 1991. Of 65 cases of incidental discovery, 44 cases(67.8%) were initially detected by ultrasonography. Stages were lower in incidental cases(64.6 %. stage I and II) than in sysmptomatic cases(39.7 %, stages I and II). Ultrasonography is very valuable in early detection and screening of renal cell carcinoma but its cost-effectiveness should be furtherevaluated.
