RESUMEN

Squamous cell carcinoma of the pancreas is a rare, uncommon tumor that is characterized by squamous metaplasia of the ductal columnar cells. We report the image findings of a rare case of the pancreatic squamous cell carcinoma associated with chronic pancreatitis.

RESUMEN

Synovial sarcoma is rare soft tissue tumor mesenchymal origin. Osseous involvement of synovial sarco-ma is rare. A 24-year-old man presented with pain and swelling of the lower extremity. MRI of the lower extremity demonstrated a large mass encircling tibia with osseous involvement. Surgical excision of the mass was done and the mass was diagnosed as biphasic synovial sarcoma with bone marrow involvement. We also discuss the other imaging findings of synovial sarcoma on MRI.

Primary undifferentiated cardiac sarcomas are extremely rare, and typically undergo aggressive local spread. The prognosis of undifferentiated sarcoma is reported to be very poor even if surgical resection, with or without adjuvant chemotherapy and radiotherapy, is performed. Here we present our case of primary undifferentiated sarcoma of the left atrium. A 33-year-old woman presented with a month history of progressive breathlessness and exertional chest pain. Transthoracic echocardiography showed a large mobile mass obstructing much of the left atrium. At surgery a superseptal approach to the atria enabled removal of all the left heart tumor which was attached to the left upper pulmonary vein. Immunochemistry staining revealed the undifferentiated atrial sarcoma. She received three cycles of darcabazine and adriamycin and after three months there was evidence of tumor recurrence. Eleven months later from operation, she died with congestive heart failure.

PURPOSE: Diabetes mellitus is one of the most common causes of erectile dysfunction (ED). The pathogenesis of ED in diabetic patients is not clear, although vasculogenic and neurogenic factors are involved. This study was designed to further characterize the pathophysiologic manifestations of ED in diabetic patients. MATERIALS AND METHODS: Fifty-seven important patients aged 20 to 71 (mean 45) years participated in this study. On the basis of their medical history, physical examination, and multidisciplinary impotence work-ups, patients were divided into diabetic (n=25) and non-diabetic (psychogenic; n=32) groups. To evaluate vasculogenic manifestations, a pharmacological erection test and penile duplex ultrasonography were performed. To evaluate neurologic manifestations, nicotinamide adenine dinucleotide phosphate (NADPH) diaphorase staining was performed on cavernous tissue samples obtained by percutaneous biopsy. Staining was assessed by counting the number of nitric oxide synthase (NOS)-containing nerve fibers present in four random fields (power 400x). In the diabetic group, we additionally assessed the duration of diabetes, the duration of treatment, and the latency between the onst of ED and the time diabetes was diagnosed. RESULTS: The pathophysiologic causes for ED in the diabetics proved to be neurogenic in 44%, vasculogenic in 20%, and mixed (combined neurogenic and vasculogenic) in 36%. Vascular assessment in the diabetics showed that penile rigidity was decreased and end-diastolic velocity was increased compared with the nondiabetics. Latency to the onset of ED from the diagnosis of diabetes was 0 to 15 (average 5.3) years, and it was closely correlated with the status of NOS-containing nerves (p<0.05). The status of NOS-containing nerves also correlated well with the degree of diabetic control but not with the control method. CONCLUSIONS: Diabetes causes ED by a variety pathophysiologic mechanisms, including neurogenic, vasculogenic, or both. Early and appropriate control of diabetes is required to prevent ED.

PURPOSE: Recent studies have shown that the expression of mucin is organ- and cell-type specific and it is increasingly possible that its change could result from oncogene activation.To evaluate histogenesis and prognostic factors for gastric carcinoma, we studied the oncoprotein expression in gastric cancer cells classified by mucin phenotye. MATERIALS AND METHODS: Mucin histochemistry and immunohistochemistry for ras, c-erbB2, and p53 oncoprotein expression were performed in 101 surgically resected gastric carcinoma specimens. PAS-Con A, GOS, and HID-AB staining techniques were employed in identifying mucosubstances, RESULTS: Of the 101 specimens studied, 73(72.3%) revealed as having mixture of various mucin-secreting cancer cells. Overall, ras immunoreactivity was observed in 72(71.3%), c-erbB2 in 7(6.9%), and p53 in 47(46.5%). Of the 73 mucus-secreting carcinomas, the surface mucous cell type were shown in 65 (89.0%), the pyloric gland cell type in 48(65,8%), the sialomucin type in 47(64.4%), and the sulfomucin type in 54(74.0%). There was significant association between mucin secretion and ras expression, but not c-erbB2 and p53 expression. There was no significant association between mucin secreting cell types and Lauren classification. Ras expression was correlated with serosal invasion, lymph node metastasis and poor prognosis. CONCLUSION: The phenotypic expression by mucin histochemistry may be not more important for studying of histogenesis in gastric carcinoma than Lauren classification. Ras expression is a poor prognostic indicator and may be correlated with phenotypic expression of surface mucous cell and intestinal cell type in gastric carcinoma.

Hashimotos thyroiditis and Graves disease have been thought to be the autoirnmune diseases having their distinct and separate clinical and pathologic features. Because of the partial and complete simi1arity in histologic feature, it has been hypothesized that Hashimoto's thyroiditis and Oraves disease may be interrelated. Several patients who had been diagnosed as Graves disease showed features of hypothyroidism, and were proven to have histologie features of Hashimotos thyroiditis in their thyroid tissue. Two meehanisms had been suggested to explain tbis phenomenon. One is that the combined Hashimotos thyroiditis in Graves' disease may become predominant with time, The other is that the amount of TSH receptor blocking antibody may increase in the course of Graves disease. Early recognition of these patients would be impartant to estabilish therapeutic plan. Futhermcee, extensive study of these patients would give more understanding of the mechanism of these diseases. Here we report 5 cases of clinically diagnosed Graves disease with pathologic features of Hashimotos thyroiditis or focal lymphocytic thyroiditis.

RESUMEN

This article is the case report of the peripheral T-cell lymphoma which involove the stomach, small bowel, and colon. A 40-year-old male was admitted to Inha University hospital because of epigastric pain, diarrhea, and weight loss. He was taken by gastrofiberscopy with biopsy, upper G-I series, colonscopy with biopsy, lower G-I series, ultrasonogram, and abdominal computerized tomogram. Endoscopic picture demonstrated the malignant ulcer in the posterior wall of angle. Sigmoidoscopic picture demonstrated the polipoid hyperemic mucosal edema. In the X-ray & endoscopic findings, the lesion was suspected as lymphoma. Histologically, the lesion was diagnosed as peripheral T-cell lymphoma. Immunohistochemical staining using UCHL-1(pan-T-cell maker) demonstrated the lymphoma as T-cell origin. The patient was treated with the multiagent chemotherapy(Bleomycin + Vincristin + Cyclophosphamide + PDS).
