RESUMEN

PURPOSE: Genomic instability of microsatellite in patients with defects in the mismatch repair system of DNA resulting in replication error (RER) has a high risk of accumulating mutations in oncogene and tumor suppressor gene. In this study, we evaluated the incidence of microsatellite instability (MI) in renal cell carcinoma (RCC) by comparing polymerase chain reaction (PCR)-amplified sequences from frozen samples of normal and tumor tissue from affected patients. MATERIALS AND METHODS: Analysis of MI using the PCR based assay was performed in total 25 cases of RCC, including 21 clear cell types and 4 papillary types. MI of tumor DNA was observed as the occurrence of additional or absence of constitutional autoradiographic signals, or as at least a two to threefold increase or decrease of intensity of the autographic signals in comparison to the corresponding normal tissue DNA. Total 11 microsatellite loci were studied; 4 loci at 3p (D3S1274, D3S1296, D3S1300, D3S1313), 3 loci at 9p (IFNA, D9S1747, D9S171) and 4 loci at 17p (D17S513, D17S695, TP53, D17S261). RESULTS: Twenty-one of 25 RCC (84%) displayed MI in at least one informative locus. MI was found in 15 of 25 cases (60%) at 3p region and 7 of 25 cases (28%) at 9p region. All of them were clear cell type of RCC. MI was detected in 11 cases (44%) at 17p region, 7 of them were clear cell type. In all 4 cases of papillary type, MI was observed only at 17p region. CONCLUSIONS: These results suggest that alterations of 3p region might occur in the early stage of tumorigenesis in RCC. The DNA mismatch repair system may play an important role in tumorigenesis of RCC and MI analysis could be a useful method to detect early genetic alterations in RCC. Genetic alterations in tumorigenesis of RCC may have some differences according to the histologic characteristics.

RESUMEN

Wilms' tumor is one of the most common abdominal neoplasms in the pediatric age group. The incidence of bilateral involvement is generally estimated to be 5 to 10 per cent. In the past, synchronous bilateral Wilms' tumor was almost always atal but multidisciplinary treatments have resulted in clearly improved prognosis. When the clinical characteristics of patient with bilateral tumors were compared with those of patients with unilateral tumors, several features became evident. Herein the authors report a case of a 7 month old infant with synchronous bilateral Wilms' tumor with congenital anomalies.

RESUMEN

We studied 46 patients undergoing living donor nephrectomy for renal transplantation. After completion of tissue typing preoperative studies includedurinalysis and culture, creatinine clearance determination, IVP and renal arteriography. Patients characteristics, preoperative renal angiography, surgical approach, complications and follow-up were reviewed. The flank incision best provides exposure for removal of the kidney without traumatic hilar dissection. There was no mortality and morbidity was minimal. Donor nephrectomy is a worthwhile and safe procedure with an acceptably low incidence of postoperative complications.

RESUMEN

Rigid ureteroscopic treatment of ureteral calculi can be highly successful and result in lower morbidity. Manipulation of calculi can be done under direct vision using grasping forceps of stone baskets with increasing safety and efficacy. Between June 1986 and August 1987, 34 ureteroscopies were performed for the removal of ureteral stones in this urologic department. In 30 cases(88.2%), the stone was removed successfully. In lower ureteral stones, ureteroscopy has been used successfully to remove the stone in 28 of 31 cases(90.3%). Except for the ureteral perforation in one case, all complications have been minimal and resolved spontaneously.

RESUMEN

Malignant fibrous histiocytoma, which is interesting because of its pleomorphic nature, is a rare soft tissue sarcoma possibly arising from histiocyte. The tumors are usually situated in the skin or subcutaneous tissues, especially of the extremities, but they may also involve deeper structures, such as the retroperitoneum or genital organs. Herin we report a case of malignant fibrous histiocytoma in the retroperitoneal space and discuss the pathologic and clinical features.

RESUMEN

Documented lymphoma of the kidney is rare and its diagnosis is very difficult. In most cases the diagnosis is made after removal or biopsy of a kidney for suspected primary renal tumor or at autopsy. If a lymphoma of the kidney were primary, early diagnosis and aggressive treatment of the local area would be essential for a cure. Herein we report a case of primary renal lymphoma in 38-yr-old man who was treated with surgical intervention and combination chemotherapy.
