Objective To explore the safety and efficiency of patients with impacted upper ureter calculus combined with renal intrarenal infection treated by ureteroscopic pneumatic lithotripsy (URL ) and minimally invasive percutaneous nephrolithotomy (MPCNL) .Methods 126 cases of impacted upper ureteral calculi combined with renal intrarenal infection were treated in this hos-pita1 from July 2007 to July 2011 ,including 58 cases of URL ,68 cases of MPCNL .The success rate of primary lithotripsy ,stone-free rate ,postoperative adjuvant therapy ,operative time ,hospital stay ,incidence of postoperative complications and other data were analyzed .Results The success rate of group URL was 82 .76% (48/58) ,the success rate of group MPCNL was 100 .00% .The stone-free rate seven day after operation :URL was 62 .07% (36/58) ,MPCNL was 98 .53% (67/68)(P0 .05) .Conclusion It has higher successful rate and stone-free rate in patients with upper ureter calculus combined with renal intrarenal infection by MPCNL than those treated by URL .The safety and efficency of the former is better that of the latter .MPCNL can be the first choice for the upper ureter calculus combined with renal intrarenal infection .
RESUMEN
Objective To evaluate the role and safety of continuous vano-venous hemofiltration(CVVH) in treating critical patients. Methods We summarized 109 cases of critical patients treated by CVVH in our ICU from 2002 to 2006. Results The therapy time ranged from 18 to 72 hours. Of all patients, there were 13 cases of hypotension, 5 cases of filter occlusion,2 cases of bleeding and 3 cases of local or blood infection. Conclusion CVVH can be appli-Gated in critical patients with good safety and efficiency. High-quality management plays an important role in the carry--out of CVVH.

AIM:To study the effects of domestic glimepiride(Gli) on blood sugar,glucose tolerance and release of insulin in rabbits and rats METHODS:Taken the changes of blood sugar level and glucose tolerance before and after giving drug as parameters,the hypoglycemic effect of Gli was observed in rabbits and rats RESULTS:Gli dose-dependly decreased the levels of blood glucose in rats with a decrease of 23% in high-dose group and seemed to be similar to glibenclamide in potency of lowering blood sugar Gli increased release of insulin by 40%～70% in rats and rabbits and was twice as potent as glibenclamide;Glucose tolerance test showed that Gli could restrain the rising of blood glucose at 30 min and 60 min after glucose was administrated ig in rabbits Time-response relation displayed that blood glucose began to drop 30 min after Gli 1 0mg/kg administered and the levels of blood glucose dropped by 40% 3 h later CONCLUSION:The domestic Gli has similar effects to glibenclamide on lowering blood sugar However,its effect on lowering blood sugar seems to be quicker,shorter and more moderate than that of glibenclamide

Objective To evaluate the clinical applications of noninvasive mechanical ventilation in the treatment of acute cardiogenic pulmonary edema. Methods From Jan.2002 to Apr.2004 60 patients in Dalan Municipal Central Hospital were randomly assigned to receire conventional oxygen therapy or NMV supplied by a standard ventilator through a face mask,with adjustment of pressure support at 8-15 cm H_2O in addition to a positive end-expiratory pressure of 5-8 cmH_2O.Blood gas and vital signs were obtained at 1 h,2 h,3 h,4 h,and 12h.Results Endotracheal intubation was required in 5(16.7%)of 30 patients assigned to receire NMV and in 12(41.4%)of 29 assigned to receive conventional oxygen therapy(P=0037). Objective To observe the clinical efficacy and safety of propafenone on paroxysmal supraventricular tachycardia (PSVT).Methods 37 patients with PSVT were injected with 70 mg propafenone intravenously in 5 minutes.The unrecovered patients in 20 minutes were injected with 70 mg propafenone intravenously again,who were given 70 mg propafenone intravenously once again if not controlled in late 30 minuted.Blood pressure,heart beat,12 lead electrocardiogram were recorded before and after use of drug.Results The significant effective rate was 77.5% and effective rate was 21.6% with overall effective rate of 97.3% and effective time of 1～55 (7.1?2.8) minutes.The average accumulated dose of propafenone was 105 mg.Some side effects were observed in part of patients,including slight reduction of blood pressure,P R interval.QRS wave and Q T interval prolonged,temporary type sino atrial block,vertigo and nausea,etc whice could vanish gradually if not treated.Conclusion Propafenone by intravenous injection can quickly contro PSVT with saftey and effectiveness.

AIM: To study the effects of norepinephrine preconditioning(NE-P) and ischemic preconditioning (IP)on apoptosis and Bcl-2, Bax expression in rat myocardial cells in myocardial ischemic reperfusion (I/R). METHODS: The model of rat ischemic-reperfusion was used to conduct NE-preconditioning. Apoptotic myocytes were detected with TUNEL. Bcl-2, Bax expression were detected with immunohistochemistry. RESULTS: The rate of apoptosis cells in I/R group was higher, the rate of apoptosis cells in NE-P group and IP was lower significantly than that in I/R group( P0.05). CONCLUSION: NE-P reduced myocyte apoptosis by I/R in rats; The expression of Bcl-2 ,Bax genes played an important role in myocardial apoptosis.
