Value of serum IgG4 level in differential diagnosis of IgG4-related pancreatic and hepatobiliary diseases and non-IgG4-related pancreatic and hepatobiliary diseases / 临床肝胆病杂志

Objective To investigate the value of serum IgG4 level in the differential diagnosis of IgG4-related pancreatic and hepatobiliary disease (IgG4-PHD) and non-IgG4-related disease (non-IgG4-RD). Methods Clinical data were collected from 491 patients who were hospitalized and 50 individuals who underwent physical examination in Huaian No. 1 People's Hospital Affiliated to Nanjing Medical University, Subei People's Hospital, and The First Affiliated Hospital of Xuzhou Medical University from August 2014 to April 2021. The 491 patients were divided into IgG4-PHD group with 20 patients, non-IgG4-RD autoimmune disease group with 431 patients (104 patients with systemic lupus erythematosus, 79 with rheumatoid arthritis, 174 with Sjogren's syndrome, 16 with ankylosing spondylitis, 11 with scleroderma, 4 with adult-onset Still's disease, 30 with myositis, 3 with psoriasis, and 10 with primary sclerosing cholangitis), and malignant pancreatic/hepatobiliary tumor group with 40 patients, and the 50 individuals undergoing physical examination were enrolled as healthy control group. Scattering immunoturbidimetric assay was used to measure serum IgG4 concentration. The two-sample Mann-Whitney U test was used for comparison of normally distributed continuous data between groups, and the Fisher's exact test was used for comparison of categorical data between groups. The receiver operating characteristic (ROC) curve was plotted to determine the optimal cut-off value of serum IgG4 in the diagnosis of IgG4-PHD. Results The IgG4-PHD group had a significantly higher serum IgG4 level than the non-IgG4-RD autoimmune disease groups, the malignant pancreatic/hepatobiliary tumor group, and the healthy control group (all P < 0.05), and the Sjogren's syndrome group had a significantly lower serum IgG4 level than the healthy control group ( Z =2.958, P < 0.05). With serum IgG4 ≥1.35 g/L and IgG4 ≥2.01 g/L as the cut-off values, the IgG4-PHD group had a significantly higher positive rate than the non-IgG4-RD autoimmune disease group and the healthy control group (all P < 0.05). The ROC curve analysis showed that IgG4 had an area under the ROC curve of 0.980 in the differential diagnosis of IgG4-PHD and non-IgG4-RD autoimmune diseases, with a sensitivity of 100.00% and a specificity of 94.00% at the optimal cut-off value of 2.21 g/L. Conclusion Serum IgG4 level may also increase in non-IgG4-RD autoimmune diseases, while the cut-off value of 2.21 g/L can improve the differential diagnosis of IgG4-PHD and non-IgG4-RD autoimmune diseases, which requires further verification in clinical practice.

Imaging features of the initial chest high resolution CT scan in juvenile patient with coronavirus disease 2019 / 中华全科医师杂志

The coronavirus disease 2019 (COVID-19) was confirmed by nucleic acid test in 15 juvenile patients aged 4-17 years in Hangzhou Xixi Hospital from January 24 to February 10, 2020. The clinical data and the initial chest high resolution CT (HRCT) findings were retrospectively analyzed.Among 15 cases, 8 patients with a mean age of (6.5±2.3) years (4-11 year) had normal HRCT manifestations (mild disease), while 7 patients with a mean age of (11.9±3.0) years (7-17 years) showed abnormal manifestations (moderate disease). In the 7 moderate cases the CT findings included local thickening of bronchial wall in 2 cases, single lesion in 1 case, bilateral pulmonary lesions in 4 cases. There were total 11 lesions, 6 of which were ground glass and slightly high-density nodules, 5 of which were speckled ground glass and 2 with a little consolidation. There were 10 subpleural lesions, including 7 in the lower lobe of the two lungs, and 1 non-subpleural lesion; the thickened local bronchioles were observed in 3 cases. None of them had hilum and mediastinal lymph node enlargement or pleural effusion.

Analysis on the individual-response behavior and the influence factors to violent terrorist attacks among undergraduates in Guangzhou / 中华流行病学杂志

Objective To analyze related behaviors of individual preparedness and influencing factors on violent terrorist attacks among undergraduates.Methods A total of 1 800 undergraduates from 5 colleges or universities in Guangzhou were selected,using the stratified cluster method.A questionnaire involving the response to violent terrorist attack behavior was used to assess the individual preparedness behaviors among undergraduates.A self-made questionnaire was applied to collect information on demographic factors,cognitive and preparedness behaviors.Results The mean score of individual preparedness behavior among undergraduates was 13.49 ± 5.02 while information on seeking behavior was 4.27 ± 1.64,avoidance behavior was 5.97± 2.16 and violent terrorist attack response behaviors was 23.73 ± 7.21,with 30.0 percent of undergraduates behaved properly.Significant differences were found in the scores of behaviors on the response to violent terrorist attack with different gender,major they pursue or religious belief (P＜0.05),among undergraduates involved in this study.Results from the logistic regression analysis revealed that persons being girls (OR=1.46,95％ CI:1.06-2.01),with bigger perceived probability (OR=1.60,95％ CI:1.12-2.30),with higher alertness (OR=3.77,95％ CI:2.15-6.61),with stronger coping confidence (OR=0.34,95％ CI:0.24-0.48) and bigger affective response (OR1=3.42,95％ CI:2.40-4.86; OR2 =0.23,95 ％ CI:0.1 3-0.41),would present more prominent behavior responses when facing the violent terrorist attack.Conclusion Individual response behaviors to violent terrorist attacks among undergraduates were relatively ideal.Perceived probability,alertness,coping confidence and affective response appeared to be independent influencing factors related to response behaviors against violent terrorist attack.In colleges and universities,awareness on violent terrorist attacks should be strengthened among undergraduates.Focus should target on psychological education dealing with disaster,knowledge and skills needed for emergency response,so as to respond to the emergency on campus.

Cloning and functional characterization of pathogenesis-related PR10-1 gene in Panax notoginseng / 药学学报

With homology cloning approaches coupling with RACE (rapid-amplification of cDNA ends) techniques, the full-length coding sequence of pathogenesis-related protein PR10-1 with differential expression was cloned from the total RNA of the root of Panax notoginseng, and its function was explored furtherly. As a result, the longest 465 bp ORF (named as PnPR10-1 with the Accession No. KJ741402 in GenBank) was detected from the cloned sequence with full-length of cDNA of 863 bp. The corresponding peptide encoded consisted of 155 amino acids, contained some domains such as Bet-v-I, and showed high similarity with that from Panax ginseng by analysis of phylogenetic trees created from the alignments. Real-time quantitative PCR showed that the expression of PnPR10-1 gene was constitutive in different tissues of 1-3 year old plant, suggesting that it might be involved in growth, development, and secondary metabolism; yet it was up-regulated significantly with the infection of Fusarium oxysporum in root, suggesting that it might be involved in defense against many diseases including root rot in P. notoginseng.

Quality of life and related factors among people living with HIV/AIDS in Guangdong province / 中华流行病学杂志

<p><b>OBJECTIVE</b>To explore the quality of life and related factors among people living with HIV/AIDS in Guangdong province.</p><p><b>METHODS</b>A total of 443 subjects older than age 18 who had been diagnosed as HIV/AIDS were recruited from AIDS Outpatient Clinic Departments in Guangzhou and Shenzhen. HIV/AIDS-targeted quality of life(HAT-QoL)scale was used to measure the quality of life on the patients. Nonparametric test method and multiple linear regression analysis were performed to explore the related factors.</p><p><b>RESULTS</b>The average scores of the following nine domains of HIV/AIDS-targeted quality of life scales were:overall function as 59.89 ± 23.35, concerns on medication as 62.91 ± 24.01, worries on factors as being disclosed, on health, on financial situation were 19.97 ± 23.04, 54.05 ± 12.24 and 35.94 ± 26.45, respectively, trust on providers as 68.72 ± 26.17, rate of understanding HIV as 61.46 ± 25.72, on life satisfaction as 56.90 ± 25.30 and on sexual function as 50.82 ± 23.86. Factors associated with quality of life would include sex, age, residential area, education, employment, marital status, complications, HAART-related prescriptions, route of infection and duration of HIV/AIDS etc.</p><p><b>CONCLUSION</b>The quality of life among HIV/AIDS patients had declined, especially on worries related to disclosure and financial situation. More attention should be paid on those with poor education, low income and lack of family support in order to improve their quality of life and reducing mortality.</p>

Studies on quality standard of Fushengkang Capsule / 中草药

Object To establish the method for quality control of Fushengkang Capsule. Methods Semen Livistonae Chinensis, Fructus Camptothecae Acuminatae and Rhizoma Curcumae in this capsule were identified by TLC. The content of camptothecine was determined by HPLC. Condition of HPLC was: Dikma Diamonsil C 18 column (250 mm? 4 6 mm, 5 ?m) the mobile phase was a mixture of methanol water (55∶45), the flow rate was 0 8 mL/min, and detection wavelength at 254 nm. Results The spots of TLC were clear and concentrative. There was a good linear relationship for camptothecine within the range of 0 050 6 - 0 253 0 ?g. The average recovery was 96 90%, RSD=1 74% (n=6). Conclusion This method is sensitive, accurate, reproducible and exclusive. It can be used for quality control of Fushengkang Capsule
