RESUMEN

Objective:To investigate the relationship between carotid atherosclerosis(CAS)and subclinical left ventricular(LV)dysfunction in type 2 diabetes mellitus patients with preserved LV ejection fraction(LVEF).Methods:A total of 120 patients with type 2 diabetes mellitus who had LVEF≥50% were selected in the Department of Endocrinology, the First Affiliated Hospital of Air Force Medical University from June 2021 to October 2021. The global longitudinal strain(GLS)was obtained by two-dimensional speckle tracking echocardiography(STE)to assess subclinical LV systolic function. The mitral ratio of peak early to late diastolic filling velocity(E/A), and mitral velocity to early diastolic velocity of the mitral annulus(E/E′)ratio were obtained by pulsed tissue Doppler echocardiography to assess LV diastolic function. Acrroding to bilateral carotid ultrasound examination, the subjects were divided into normal carotid arteries group( n=46) and CAS group( n=74). Demographics and biochemical parameters were compared between two groups. Binary logistic regression and Pearson correlation analysis were used to evaluate the relationship between CAS and subclinical LV dysfunction. Results:The CAS group had a higher proportion of men, older age, and a longer duration of diabetes than the normal carotid arteries group(all P<0.05). There was no difference in LVEF and GLS between the two groups [normal carotid arteries group vs CAS group, LVEF: (60.72±4.73)% vs(60.07±4.28)%; GLS: (18.24±3.72)% vs(17.81±3.47)%, respectively; both P>0.05]. However, compared with normal carotid arteries group, E/A ratio was decreased and E/E′ ratio was significantly increased in CAS group(both P<0.01). Pearson correlation analysis showed that GLS was not correlated with carotid plaque thickness and carotid intima-media thickness(CIMT; both P>0.05). By contrast, E/E′ ratio was positively correlated with carotid plaque thickness and CIMT(both P<0.05). Binary logistic regression analysis showed that GLS and E/E′ ratio were not associated with CAS( both P>0.05). However, decreased E/A ratio was significantly associated with the existence of CAS( OR=0.09, 95% CI 0.01-0.67, P=0.018). Conclusions:In type 2 diabetes mellitus patients without overt heart failure and with preserved LVEF, the occurrence of CAS is not associated with subclinical LV systolic impairment assessed by GLS, but is significantly associated with LV diastolic dysfunction, and is independent of traditional cardiovascular risk factors.

Objective To observe the characteristics of bone development and body development in 3-7 years old children with cerebral palsy. Methods 50 girls and 50 boys aged 3-7 years with cerebral palsy were included. Their height, weight, length of right and left upper ex-tremities, and both hands bone age were measured by trained professionals. Results Their bone ages of both hands were 1 year younger than actual age (P0.05). Their height growth was in line with the normal children, while the weight growth was a little different. Both sides of upper limbs growed synchronously, and slowed down with the time. There was no significant difference in both sides (P>0.05). Conclusion The bone development of children with cerebral palsy lags behind the life age. Their height growth is close to normal children. The development shows no no significant lateral dominance.

@#Objective To observe the effect of Motomed Gracile on spastic cerebral palsy. Methods 48 cerebral palsy children were divided into two groups: intervention group (n=24) and control group (n=24). The children in the control group accepted comprehensive rehabilitation, while the children in the intervention group were trained with Motomed Gracile in addition. They were evaluated with Ashworth Assessment and manual muscle testing (MMT) 6 months after treatment. Results The scores of intervention group improved more significantly than those in control group (P<0.05). Conclusion Motomed Gracile can facilitate the recovery of children with spastic cerebral palsy.

@# Objective To investigate the requirment of family rehabilitation training and provide instructions and references to guide parents practising family rehabilitation training for cerebral palsy (CP) children in Shanghai.Methods300 CP children were surveyed with the Requirement Questionnaire of Family Guidance Training Program of Rehabilitation for Cerebral Palsy Children. Then 100 children out of them were selected randomly and a visiting training and guidance of rehabilitation in family was afforded.ResultsAfter providing training and guidance of rehabilitation in family, the requirement of family rehabilitation increased from 41.5% to 92.7% ( P<0.01) and the duration and the times of training were also increased ( P<0.01).ConclusionThe families having CP children in Shanghai don't understand fully about the family rehabilitation. The visiting training and guidance program will be helpful for CP children having complete rehabilitation and coming back to society.

RESUMEN

With Chinas accession to the WTO, implementation of the intellectual property agreement, exemptions and reductions of tariff and cancellation of non tariff barriers, and execution of the general agreement on services and trades will exert a widespread impact on the domestic medical market. Reform of the medical and health system will become extremely urgent; competition in the market of medical services will grow increasingly fierce; management of human resources will find itself in a dilemma; and supervision and control of the medical market will turn more difficult. A thorough study of the opportunities and challenges confronting the medical industry after Chinas WTO entry will, therefore, contribute to a better understanding of the reality and adequate preparation for the challenges. It is imperative to establish the idea of lifelong learning, to always remember that "development is a golden principle"; to set up the commanding elevation of talents in the 21 st century; to speed up innovations in medical technology; and to construct a modern hospital management system.

RESUMEN

In light of the emergence and development of the idea of people oriented management, the paper discusses the cultural pathways for people oriented management. People oriented management is a must for the progress of advanced culture; hospital culture is in essence people oriented culture; hospital culture and people oriented management are intrinsically unified; and cultural strength constitutes the hospital's perpetual competitiveness. For this reason, it is imperative to pinpoint the intrinsic demands of hospital culture and people oriented management so as to enhance the hospital's credibility and competitiveness. The following steps can be taken: ①identifying common objectives; ②fostering and upholding the hospital spirit; ③cultivating the idea of people oriented services; ④developing a "learning type" hospital; and ⑤making full use of the human resources on the basis of the "four respects".
