RESUMEN

Objective:To explore the clinical efficacy of modified Zhizhutang in treating senile primary constipation due to Qi-Yin deficiency and its influences on gastrointestinal neurotransmitters and gastrointestinal motility. Method:One hundred and twenty-one eligible patients were randomly divided into a control group (<italic>n</italic>=60) and an observation group (<italic>n</italic>=61). On the basis of lifestyle adjustments, patients in the control group further received oral Qihuang Tongmi soft capsule,three capsules per time,three times per day, whereas those in the observation group were further treated with modified Zhizhutang,one bag per day, for eight successive weeks. The number of complete spontaneous bowel movements (CSBM, normal defecation was considered if CSBM ≥ three times per week) before and after treatment was recorded. The main symptoms of constipation,the patient assessment of constipation quality of life (PAC-QOL), and the traditional Chinese medicine (TCM) syndrome before and after treatment were scored, followed by the anorectal pressure measurement and colonic transit test before and after treatment. The levels of somatostatin (SS),motilin (MTL),substance P (SP) and vasoactive intestinal peptide (VIP) before and after treatment were detected. Result:The weekly average number of CSBM in the observation group was greater than that of the control group (<italic>P</italic><0.01), but the TCM syndrome score was lower than that of the control group (<italic>P</italic><0.01). The normal defecation rate in the observation group was 57.38% (35/61),which was higher than 35% (21/60) of the control group (<italic>χ</italic><sup>2</sup>=6.092,<italic>P</italic><0.05). The scores of the main symptoms of constipation in the observation group were significantly lower than those in the control group (<italic>P</italic><0.01). The PAC-QOL subscale scores and the total score of the observation group significantly declined as compared with those of the control group (<italic>P</italic><0.01). The anal resting pressure and anal maximum systolic pressure of the observation group were higher than those of the control group (<italic>P</italic><0.01),while the first sensation volume, urge sensation volume and maximum tolerable volume were lower (<italic>P</italic><0.01). The excretion rates of markers in the observation group at 24 h,48 h and 72 h were significantly elevated in comparison with those in the control group (<italic>P</italic><0.01). Compared with the control group, the observation group also exhibited increased serum MTL and SP (<italic>P</italic><0.01) and decreased SS and VIP (<italic>P</italic><0.01). The recurrence rate in the observation group was 34.29% (12/35),obviously lower than 61.90% (13/21) in the control group (<italic>χ</italic><sup>2</sup>=5.051,<italic>P</italic><0.05). The total effective rates in the observation group and the control group were 95.08% (58/61) and 81.67% (49/60) , implying a significant difference (<italic>χ</italic><sup>2</sup>=5.321,<italic>P</italic><0.01). Conclusion:For the elderly with primary constipation due to Qi-Yin deficiency, oral administration of modified Zhizhutang helps to regulate gastrointestinal neurotransmitters,improve gastrointestinal motility and gastrointestinal emptying rate, alleviate the symptoms of constipation, restore spontaneous bowel movements, improve their quality of life, and lower the recurrence rate. Its clinical effects were better than those of Qihuang Tongmi soft capsule.

Objective:To analyze the unqualified rate of the observation indicaors before and after the implementation of “Sunshine Kitchen Project” in order to provide reference for improving the management of catering industry. Methods:This study included 415 catering establishments with implementation of the “Sunshine Kitchen Project” as intervention group and 210 catering establishments as control group without the implementation. By combining cohort study with case-control study, the unqualified rate of each observation indicator in intervention group and control group was analyzed. Results:After the intervention of “Sunshine Kitchen Project”, the unqualified rate of seven observation indicators related to operational behavior was significantly reduced, with an overall decrease of 12.44%. There were significant differences among the unqualified rate of large and medium-sized catering units and canteens in the intervention group (P < 0.001). There was no statistically significant change in the unqualified rate of seven indicators in the control group (P > 0.1). Compared with the control group before and after the intervention, the reduction of unqualified rate in the intervention group was significantly larger than that in the control group. The overall unqualified rate of the intervention group was significantly lower than that of the control group (P < 0.001). Conclusion:The project of “Sunshine Kitchen” plays an active role in promoting the standardized operation of catering industry.

Objective:To observe clinical effect of addition and subtraction therapy of Xiaoyaosan combined with Simotang to irritable bowel syndrome with predominant constipation (IBS-C) and syndrome of stagnation of liver Qi and to study influence to gut-brain axis. Method:One hundred and forty-four patients were randomly divided into control group (72 cases) and observation group (72 cases) by random number table. The 66 patients in control group completed the therapy (2 patients were falling off or missing visit, 5 patients were eliminate), 65 patients in observation group completed the therapy (4 patients were falling off or missing visit, 2 patients were eliminate). Patients in control group got Simotang, 20 mL/time, 3 times/day. In control group, patients in observation group added addition and subtraction therapy of Xiaoyaosan combined with Simotang, 1 dose/day. And courses of treatment in two groups were 4 weeks. Before and after treatment, degree of abdominal pain, irritable bowel syndrome (IBS) symptom severity scale (IBS-SSS), quality of life in irritable bowel syndrome (IBS-QOL), syndrome of stagnation of liver Qi, scores of Hamilton anxiety scale-14(HAMA-14), Hamilton depression scale-17(HAMD-17), complete spontaneous bowel movement (CSBM), response rate of abdominal pain and defecation, remission rate of IBS-SSS were all recorded. And levels of vasoactive intestinal peptide (VIP), substance P (SP), neuropeptide Y (NPY), 5-hydroxytryptamine (5-HT), somatostatin (SS) and calcitonin related gene peptide were detected, and safety was evaluated. Result:Degree of abdominal pain, scores of IBS-SSS, syndrome of stagnation of liver Qi , HAMA-14, HAMD-17 and levels of VIP, NPY, 5-HT, SS and CGRP in observation group were all lower than those in control group (P<0.01). And times of CSBM, score of IBS-QOL and level of SP were all higher than those in control group (P<0.01). Besides, response rate of abdominal pain and defecation and remission rate of IBS-SSS in observation group 95.38%(62/65), 93.85%(61/65) and 90.77%(59/65)were higher than 83.33%(55/66),78.79%(52/66)and 75.76%(50/66) in control group (P<0.05). And curative effect of traditional Chinese medicine (TCM) syndromes in was better than that in control group (Z=2.1034, P<0.05). No serious adverse events happened and no adverse reaction caused by TCM. Conclusion:Addition and subtraction therapy of Xiaoyaosan combined with Simotang can significantly ameliorate IBS-C symptoms, reduce bad mood, improve patients' quality of life, regulate a variety of brain gut peptide factors, and improve brain gut axis disorder. It has good clinical efficacy and safety.

BACKGROUND@#Epithelial to mesenchymal transition (EMT) is strongly linked with tumor invasion and metastasis, which performs a vital role in carcinogenesis and cancer progression. Emerging evidence suggests that microRNAs (miRNAs) expression are closely associated to EMT by regulating targeted genes. MiR542 has been found to be involved in the EMT program and bound up with various cancers. However, the functions of miR542 and its underlying mechanism in glioblastoma multiforme (GBM) remain largely unknown. In the current study, we investigated the effect of astrocyte elevated gene-1 (AEG-1) on U251 cells aggressiveness, proliferation, apoptosis, and cell cycle.@*METHODS@#The screening of targeted miRNAs was performed, as well as the functional roles and mechanisms of miR542 were explored.@*RESULTS@#MiR542 was selected as the target because of the most significantly differential expression and this high level of expression negatively correlated with cell migration and proliferation, which suggested that miR542 could be a novel tumor suppressor. Moreover, we confirmed that AEG-1 was a direct targeted gene of miR542 by luciferase activity assay, reverse transcription-polymerase chain reaction, and immunoblotting analysis. Furthermore, miR542 suppressed the expression of AEG-1, which upgraded the level of E-cadherin and degraded Vimentin expression contributing to retraining EMT.@*CONCLUSION@#The in vitro findings demonstrated that miR542 inhibited the migration and proliferation of U251 cells and suppressed EMT through targeting AEG-1, indicating that miR542 may be a potential anti-cancer target for GBM.

Objective Studies are rarely reported on the effect of short peptides of the pigment epithelium derived factor (PEDF) on the proliferation of human cutaneous squamous (SCL-1) cells. The purpose of this study is to investigate segmented cloning and expression of the PEDF protein and observe its effect on the proliferation of human SCL-1 cells. Methods The target genes of PEDF1, PEDF2 and PEDF3 were amplified by PCR and the recovered fragments subjected to double digestion of NheⅠ and Hind Ⅲ and inserted into the pET28a(+) plasmid. The product was transformed into human E coli BL21 and induced to express, followed by isolation and purification of the fusion protein. CCK-8 assay was used to detect the proliferation of the SCL-1 cells with PEDF1, PEDF2 and PEDF3 at 100, 400, 800 and 1000 nmol/L at 24, 48 and 72 hours. Results The prokaryotic expression vectors of PEDF1, PEDF2 and PEDF3 were successfully constructed, and their fusion proteins prepared, with the molecular weight of 18 000, 17 000 and 13 000, respectively. The proliferation of the SCL-1 cells was significantly decreased in the 800 and 1000 nmol/L PEDF3 groups compared with that in the 0 nmol/L PEDF3 group at 24 hours (0.16 ± 0.03 and 0.78 ± 0.07 vs 1.00 ± 0.00, P < 0.05), inhibited in a concentration-dependent manner in the 400, 800 and 1000 nmol/L PEDF3 groups at 48 hours (P < 0.05), markedly lower in the 800 and 1000 nmol/L PEDF3 groups at 72 hours (0.53 ± 0.05 and 0.51 ± 0.05) than in the in the 400 and 0 nmol/L PEDF3 groups (0.60 ± 0.05 and 1.00 ± 0.00) (P < 0.05). Conclusion The PEDF fusion proteins were successfully segmentally cloned and expressed and PEDF3 inhibited the proliferation of SCL-1 cells, which has paved the ground for further screening of active functional short peptides of PEDF.

Objectives:To systemically review the safety and efficacy of bioresorbable vascular scaffold (BVS) versus everolimus eluting stent (EES) for percutaneous coronary intervention (PCI). Methods:The database searched includes PubMed, Medline, MEDILINE, EMBASE, Cochrane library, CNKI and Wanfang. Database retrieval time was between database establishment time to October 2017. During the same time, authors accessed the conference summary and related websites to collect published randomized controlled trials of published data. To evaluate the quality of the literature according to the modified Jadad scale and extracted the data. Meta-analysis was performed using Review Manager 5.3 software. Results:Nine trials were included; 6 721 patients were randomized to receive BVS (n=3 670) or EES (n=3 051). Time of follow-up was ranged from 6 to 36 months. Compared with metallic EES, risk of target lesion failure (RR=1.31, 95%CI:1.08-1.58; P=0.005) and in-stent thrombosis (RR=2.89, 95%CI:1.85-4.53; P<0.0001), ischemia-driven target lesion revascularization (RR=1.44,95%CI:1.12-1.86, P=0.005)、target-vessel myocardial infarction (RR=1.74, 95%CI:1.33-2.27, P<0.0001) and all myocardial infarction (RR=1.49, 95%CI:1.16-1.91, P=0.002) were all significantly higher in BVS group than in EES group. There were no significant differences in all-cause death (RR=0.87, 95 % CI:0.57-1.33, P=0.520), cardiovascular mortality (RR=0.78, 95%CI:0.54-1.11, P=0.160) and composite endpoints (RR=1.10, 95%CI:0.95-1.27, P=0.210) between the two groups. Conclusions:Compared with metallic EES, the BVS appears to be associated with both lower efficacy and higher thrombotic risk during the observation period.

The type of basic media and the contents of plant growth substances were investigated by orthogonal design experiment,and also the effects of different culture conditions on the growth of suspension cells and the accumulation of total flavonoids in Eucommia ulmoides were studied.The results showed that B5 medium supplemented with 0.5mg/L NAA,0.6mg/L 6-BA and 30g/L sucrose,at initial pH 5.0~5.5,20g(FW)/L inoculation quantity and 110 r/min of rotation speed was a preferable culture conditions for E.ulmoides suspension cells growth and flavonoids synthesis.The results of metabolic kinetics analysis for E.ulmoides cell suspension culture showed that the logistic and Luedeking-Piret equations can be used for describing the kinetics of cell growth,sucrose consumption and flavonoids production during the process.The maximum specific growth rate(?m),the actual growth yield based on sucrose(YG) and maintenance coefficient(m) were 0.417/d,0.619g/g and 0.0206g/(g?d-1) respectively.All these outcomes could give a basis for establishing the suspension cell culture of E.ulmoides and production of the natural active components in large-scale.
