RESUMEN

BACKGROUND:The uncemented prosthesis in primary total hip arthroplasty has obtained good outcomes, but its application in revision especialy in failed cemented hip arthroplasty deserves further investigations. OBJECTIVE: To observe the mid-term results of uncemented revision of failed cemented hip arthroplasty. METHODS: 41 patients (47 hips) with uncemented revision of failed cemented hip arthroplasty were folowed up from November 1997 to February 2013. In the 47 hips, 24 hips replaced the cemented stem component while 23 hips underwent the cemented acetabular revision. Prosthesis change was observed in radiographs. Harris functions were evaluated and prosthesis survival rate was analyzed. RESULTS AND CONCLUSION: A total of 33 patients (38 hips) were folowed up for 1.3-14.8 years. The mean preoperative Harris hip score increased from 34 (range, 11-66) points to 85 (range, 55-94) points at the time of final folow-up (P < 0.01). 36 hips (95%) had an excelent stability. Osteolysis and migration of the prosthesis were seen in 2 hips (1 in acetabulum and 1 in femur), which needed re-revision. Heterotopic ossification developed in 9 hips (24%). The mean polyethylene liner wear was 0.08 mm (range, 0-0.25 mm) per year. The wear rate only correlated with changes of abduction angle of the acetabulum (P < 0.01). Imaging verified that loosening served as a failure criterion. Kaplan-Meier survival analysis was used to calculate uncemented prosthesis for 7 years and showed that total survival rate was 89%. The survival rates of acetabular prosthesis and femoral prosthesis were respectively 93% and 92%. Results indicated that uncemented prosthesis could deal with the problem of bone deficiency and obtain satisfactory mid-term clinical results in revision of failed cemented hip arthroplasty.

BACKGROUND:The outcomes of SL-Plus? stem in primary total hip arthroplasty have been proved good, but whether SLR-Plus? revision stem can obtain a good outcome in revision hip arthroplasty needs more studies. OBJECTIVE:To observe the clinical results of revision hip arthroplasty with Zweymül er SLR-plus ? stem. METHODS:Revision hip arthroplasty of 41 hips in 39 patients was performed from November 1997 to May 2013 using SLR-plus ? stem. There were 26 hips (male) and 15 hips (female). They were at the age of 34-73 years old, 53 on average. In the 41 hips, 36 cemented and 5 uncemented femoral prostheses were used. Prosthetic changes on radiographs were observed. In accordance with Brooker classification, heterotopic ossification was classified to record the region and incidence of heterotopic ossification. Hip joint function was evaluated using Harris Hip Score, and survival rate of the prosthesis was analyzed. RESULTS AND CONCLUSION:A total of 31 patients (33 hips) were fol owed up for 1 to 16 years. The mean preoperative Harris hip score of 34 (range, 13-64) points improved to 85 (range, 55-94) points at the time of final fol ow-up. 32 hips (97%) had an excellent result. A 1 mm width radiolucent line was found in 1 femoral component without any symptom. Osteolysis and migration were seen in 1 hip, which needed re-revision. Heterotopic ossification developed in 9 hips, including 2 hips of Brooker grade 1, 4 hips of grade 2 and 3 hips of grade 3. No re-infection was found. Kaplan-Meier survivorship was 92%with radiographic loosening as the end point. Results demonstrated that the SLR-Plus? stem has sufficient immediate and long-term stability, which is reliable for patients undergoing hip revision surgery.

RESUMEN

Objective To explore the influencing factor of duration of untreated psychosis(DUP) and the relationship between duration of untreated psychosis and treatment outcome in epidemiological first-episode schizophrenia. Methods 912 medical records of the first episode schizophrenia patients were recruited in the study for epidemiological survey . The general medical data, clinical diagnosis, and treatment outcome were collected. DUP was determined refer to symptom onset for schizophrenia (SOS) scale. Patients were divided into short DUP and long DUP groups according to the median values of DUP values. Results Between short DUP and long DUP groups, the difference in marriage was significant ( P < 0. 05). The unmarried patients in the short DUP group (63.7% ) was higher than the long DUP group (52.6% ), the variance was significant (λ2 =5.990, P<0.05). The divorce in the short DUP group (0.6% ) was lower than the long DUP group (4.4% ) , the variance was significant (λ2 =5.079, P<0.01).The simple schizophrenia in the short DUP group (1.5%) was lower than the long DUP group (4. 1% ) , the variance was significant (λ2 =5.868, P<0.05). The treatment outcomes between the two groups had a significant variance ( λ2 =36.093, P < 0.01) , and the Ridit analysis of treatment outcomes between the two groups showed significant difference ( u = 5.183, P<0.01). The marriage,age of onset and clinical diagnosis were significantly correlated with DUP in multiple factor Logistic regression analysis. Conclusion The marriage,age of onset and clinical diagnosis are significantly correlated with DUP in the first episode schizophrenia patients,the longer of DUP and the worse of treatment outcome.

RESUMEN

Objective:To study the reliability and validity of schizophrenia quality of life scale (SQLS)in China.Method:Eighty-eight patients with schizophrenia were assessed with the SQLS.Results:The Cronbach's alpha were in 0.58-0.93.The split-half reliability coefficient were in 0.91-0.94.The test-retest reliability coefficient were in 0.57-0.76.The content validity and construct validity reached psychometrically acceptable level.Conclusion:The SQLS is a simple and reliable scale.

RESUMEN

Objective To discuss the CT value measurement of the soft tissue such as sludged blood,gel foam and residual nucleus in vertebral canal at the early stage after lumbar discectomy.Methods 40 patients of lumbar disc herniation were divided into gel foam group and sludged blood group randomly.There were 22 males,18 females,with the mean age 50.6 years(ranged from 25 to 75 years).All patients underwent lumbar CT scan before operation and CT values of herniated nucleus were measured(simulating residual nucleus after operation).Same surgeon group performed posterior approach discectomy through open window in the vertebral plate.A long column roll of gel foam was placed epidurally at the site where nucleus were take out to stop bleeding in gel foam group.While in sludged blood group just douched with saline.All patients underwent lumbar CT scan one week after operation and CT values of the soft tissue in vertebral canal measured.The CT values before and after operation and between groups were compared and analyzed.Results CT values of the herniated disc of 40 patients were 43.70-66.70 HU.CT values of the soft tissue in vertebral canal of 20 patients gel foam group were(23.00?3.36)HU.Paired-samples t test of the values of herniated nucleus before operation and the gel foam showed t=12.32,P=0.00,with statistic significance.CT values of the soft tissue in vertebral canal of sludged blood group were(25.60?6.75)HU.Paired-samples t test of the values of nucleus before operation and sludged blood showed t=13.84,P=0.00,with statistic significance.Independent-samples t test of values of gel foam group and sludged blood group showed t=-0.78,P=0.46.No significant differences existed.Conclusion CT value measurement is valuable to distinguish the nature of the soft tissue in vertebral canal in clinical use.
