RESUMEN

Objective:To investigate the positive and negative affect of patients with systemic lupus erythematosus (SLE), explore the effect of social support, self-efficacy and psychological resilience on positive and negative affect and relationships among these factors, and provide reference for improving the emotional health of SLE patients.Methods:This was a cross-sectional survey. From February to May 2021, 142 SLE patients attending the outpatient clinic of the Department of Rheumatology and Immunology of West China Hospital, Sichuan University were selected by convenience sampling. Participants were investigated by the general information questionnaire, Perceived Social Support Scale, General Self-efficacy Scale, Connor-Davidson Resilience Scale, and Positive and Negative Affect Schedule. And path analysis was performed to analyze the effect of social support, self-efficacy and psychological resilience on positive and negative affect.Results:A total of 127 valid questionnaires were collected, with an effective response rate of 89.44%. The positive and negative affect scores of 127 SLE patients were (28.09±7.31) and (25.83±8.10) points. And the positive rate (ratio of positive to negative affect) was 1.09; positive affect was positively correlated with social support, self-efficacy and psychological resilience ( r=0.412, 0.448, 0.533, all P<0.01), and negative affect was negatively associated with social support, self-efficacy and psychological resilience ( r=-0.313, -0.239, -0.382, all P<0.01); the results of the path analysis showed that the standardized total effects of social support, self-efficacy and psychological resilience on positive and negative affect in SLE patients were 0.341, 0.313, 0.632 and -0.237, -0.218, -0.439, respectively. There were significant mediating effects between social support and positive and negative affect (95% CI 0.035 -0.111, -0.087- -0.026) mediated by self-efficacy and psychological resilience. Social support could affect positive and negative affect through psychological resilience ( β=0.104, -0.080, both P<0.01) and through self-efficacy and psychological resilience ( β=0.065, -0.050, both P<0.01). Conclusions:Social support, self-efficacy and psychological resilience are the primary factors influencing positive and negative affect in SLE patients. Increasing social support to encourage patients to take social participation more actively and improving the self-efficacy as well as psychological resilience are ought to do for promoting their emotional health.

The reflective ability of nursing staff has been paid more and more attention, nurses' reflection can promote professional competence development, which also can improve clinical practice ability, knowledge expansion ability and innovation ability. Therefore this article reviews the current situation on nurses′ reflection, the assessment tools, the modes, forms, methods and types, future prospects of clinical nurses reflective practice, in order to provide reference and basis for the relevant research on the reflection practice of clinical nursing staff in China.

Excess activation of cardiac fibroblasts inevitably induces cardiac fibrosis. Emodin has been used as a natural medicine against several chronic diseases. The objective of this study is to determine the effects of emodin on cardiac fibrosis and the underlying molecular mechanisms. Intragastric administration of emodin markedly decreased left ventricular wall thickness in a mouse model of pathological cardiac hypertrophy with excess fibrosis induced by transaortic constriction (TAC) and suppressed activation of cardiac fibroblasts induced by angiotensin II (AngII). Emodin upregulated expression of metastasis associated protein 3 (MTA3) and restored the MTA3 expression in the setting of cardiac fibrosis. Moreover, overexpression of MTA3 promoted cardiac fibrosis; in contrast, silence of MTA3 abrogated the inhibitory effect of emodin on fibroblast activation. Our findings unraveled the potential of emodin to alleviate cardiac fibrosis upregulating MTA3 and highlight the regulatory role of MTA3 in the development of cardiac fibrosis.

Objective To explore the effect of two different urethral stent flush methods for postoperative complications of hypospadias repair in children patients. Methods A total of 182 cases of children patients who underwent urethroplasty with pedicled double skin flaps from March 2014 to March 2016 were divided into intervention group (n=89) and the control group (n=93) by using random number method. The control group was treated with gentamicin irrigation urethral stent;the intervention group was treated with gentamicin irrigation urethral stent, and the patients were trained to urinate twice with the stent from the fourth day after surgery. The incidence of urinary fistula and urethral stricture within one year after surgery, and the anxiety level of patients' caregivers were assessed. Results The incidence of urinary fistula in the intervention group and the control group were 5.7%(5/87) vs. 15.2%(14/92), and the difference was statistically significant(χ2=4.227, P<0.05). In the intervention group, the cases of non anxiety, mild anxiety, moderate anxiety and severe anxiety respectively were 33(37.9%), 42(48.3%), 8 (9.2%) and 4(4.6%). However, in the control group the cases were 21(22.8%), 46(50.0%), 12(13.0%), 13 (14.1%) respectively. The anxiety degree of patients' caregivers was significantly different between intervention group and control group (Z=2.757, P<0.01). Conclusions The training of early micturition with stent after hypospadias surgery can significantly reduce the incidence of urinary fistula and the anxiety level of caregivers.

Objective To discusses the reason and effective prevention and control nursing measures in severe craniocerebral injured patients complicated with secondary pulmonary aspergillosis.Methods The reason of 21 severe craniocerebral injury patients infected by pulmonary aspergillosis were analyzed,and comprehensive nursing care was given to these patients,including strict disinfection and isolation,strengthening management of hospital environment,early close observation of disease,applying effective antifungal drugs in time,strengthening the medication nursing and nutrition support,standard antibiotic application,attention to the respiratory tract management,basic and college nursing.Results 13 cases were recovered,4 cases were better,4 cases died,the mortality rate was 19%.Conclusions Reasonable treatment and nursing care to the severe craniocerebral injury patients complicated with secondary pulmonary aspergillosis can effectively prevent and control the spread of pulmonary aspergillosis in the hospital,improve the cure rate,reduce the complications and the mortality rate,and ensure the safety of patients.

OBJECTIVE To investigate the expression of PTEN and VEGF gene in laryngeal squamous cell carcinoma and their clinical significance. METHODS The expression of PTEN and VEGF gene were examined by using immunohistochemical S-P staining method in 10 cases of normal tissues, 20 cases of para-tumor tissues and 60 cases of LSCC. RESULTS The positive rates of PTEN in normal tissues, para-tumor tissues and LSCC were 100.0 %, 95.0 %, and 70.0 % respectively (P<0.05),and VEGF protein showed positive expression of 20.0 %,50.0 %,73.3% in normal, para-tumor and LSCC tissues with statistical significance(P<0.05), In LSCC, the expression of PTEN and VEGF was positively correlated with TNM stage, differentiation of tumor, cervical and distant metastasis of lymph node, 3-year survive rate of patients(P<0.05),No significant association was observed in expression of PTEN and VEGF with tumor location,patient's age and sex(P>0.05). Spearman correlation analysis revealed that PTEN was negatively correlated with VEGF expression(?=-0.3948, P=0.0018). CONCLUSION The aberrant expression of PTEN and VEGF may play a role in occurrence, progression and metastasis of LSCC.
