RESUMEN

OBJECTIVE@#To explore the feasibility and effectiveness of a foldable pedicled latissimus dorsi myocutaneous flap to repair soft tissue defects in the shoulder and back.@*METHODS@#Between August 2018 and January 2023, the foldable pedicled latissimus dorsi myocutaneous flaps were used to repair soft tissue defects in the shoulder and back of 8 patients. There were 5 males and 3 females with the age ranged from 21 to 56 years (mean, 35.4 years). Wounds were located in the shoulder in 2 cases and in the shoulder and back in 6 cases. The causes of injury were chronic infection of skin and bone exposure in 2 cases, secondary wound after extensive resection of skin and soft tissue tumor in 4 cases, and wound formation caused by traffic accident in 2 cases. Skin defect areas ranged from 14 cm×13 cm to 20 cm×16 cm. The disease duration ranged from 12 days to 1 year (median, 6.6 months). A pedicled latissimus dorsi myocutaneous flap was designed and harvested. The flap was divided into A/B flap and then were folded to repair the wound, with the donor area of the flap being pulled and sutured in one stage.@*RESULTS@#All 7 flaps survived, with primary wound healing. One patient suffered from distal flap necrosis and delayed healing was achieved after dressing change. The incisions of all donor sites healed by first intention. All patients were followed up 6 months to 4 years (mean, 24.7 months). The skin flap has a good appearance with no swelling in the pedicle. At last follow-up, 6 patients had no significant difference in bilateral shoulder joint motion, and 2 patients had a slight decrease in abduction range of motion compared with the healthy side. The patients' daily life were not affected, and linear scar was left in the donor site.@*CONCLUSION@#The foldable pedicled latissimus dorsi myocutaneous flap is an ideal method to repair the soft tissue defect of shoulder and back with simple operation, less damage to the donor site, and quick recovery after operation.

OBJECTIVE@#To explore the feasibility and effectiveness of plantar medial thin skin flaps preserving plantar fascia with its superficial fascia tissue to repair skin defects in hands and feet.@*METHODS@#Between July 2017 and January 2023, 35 cases of hand and foot defects were repaired with plantar medial thin skin flaps preserving plantar fascia with its superficial fascia tissue (13 pedicled flaps and 22 free flaps). There were 18 males and 17 females, with an average age of 38.8 years (range, 8-56 years). Thirty cases of defects were caused by trauma, and the interval between injury and admission ranged from 2 to 6 hours (mean, 3.3 hours). Three cases were ulcer wounds with a course of 3.0, 3.8, and 7.0 months, respectively. Two cases were malignant melanoma. Eight cases of wounds located in the fingers, 13 cases in the palm, 12 cases in the heel, and 2 cases in the distal foot. The size of skin defects ranged from 4.0 cm×3.5 cm to 12.0 cm×10.0 cm, and the size of flap ranged from 5.0 cm×4.5 cm to 13.0 cm×11.0 cm. The donor sites were repaired with skin grafts.@*RESULTS@#All flaps were survived and the wounds healed by first intention after operation. The partial necrosis at the edge of the skin graft occurred in 1 case, which healed after dressing change; the other skin grafts survived successfully. All patients were followed up 6-24 months (mean, 18 months). The flaps exhibited similar color and thickness to the surrounding hand and foot skin. Two-point discrimination ranged from 7 to 10 mm in the flaps with an average of 8 mm. The donor sites had no painful scars or sensory abnormalities. Foot and ankle functions were good and gaits were normal.@*CONCLUSION@#Application of plantar medial thin skin flaps preserving plantar fascia with its superficial fascia tissue to repair skin defects in hands and feet had good flap shape, high survival rate of skin graft at the donor site, and no obvious complications.

RESUMEN

Chimeric antigen receptor (CAR)-T cell immunotherapy is a revolutionary advancement in cancer treatment, and is highly effective in treatment of B cell leukemia, lymphoma, and multiple myeloma. However, it has some deficiencies such as antigen escape, antigen heterogeneity, toxic and side effects, which also pose challenges for therapy of other hematological malignancies and solid tumors. Comprehensive and continuous laboratory monitoring before and after CAR-T cell immunotherapy plays an important role in early warning of toxic and side effects, as well as theraputic evaluation. In the future, new breakthroughs for tumor therapy. Should be focused on improving CAR-T cells design, enhancing their anti-tumor activity, and optimizing therapeutic regimen.

Objective:To investigate the effect of sodium arsenite (NaAsO 2) on transcriptional activity of nuclear factor E2-related factor 2 (Nrf2) signaling pathway in mouse lymph node vascular endothelial cell line (SVEC4-10). Methods:In vitro cell culture method was used to treat SVEC4-10 cells for 24 h with different doses of NaAsO 2 [0 (control), 2, 5, 10, 20, 50, 100, 150 μmol/L], and the cell viability was detected by tetrazole compound (MTS) method. The time-response relationship was studied with SVEC4-10 cells treated with 5 μmol/L NaAsO 2 for 0 (control), 2, 6 and 12 h; the dose-response relationship was studied with SVEC4-10 cells treated with 0 (control), 2, 5 and 10 μmol/L NaAsO 2 for 6 h; real-time fluorescence quantitative PCR (RT-qPCR) was used to detect the mRNA expression of Nrf2 and its downstream genes glutamate-cysteine ligase catalytic subunit (Gclc), glutamate-cysteine ligase modifier subunit (Gclm), NAD(P)H dehydrogenase quinone 1 (Nqo1) and metallothionein 1 (Mt1). Establishment of Nrf2 gene stably silenced (Nrf2-KD) cells using SVEC4-10 cells, the interference control (scramble, SCR) cells and Nrf2-KD cells were treated with 0(control), 10 and 20 μmol/L NaAsO 2 for 16 h, and apoptosis was detected by flow cytometry. Results:MTS test results showed that the cell viability of the control, 2, 5, 10, 20, 50, 100, 150 μmol/L NaAsO 2 treatment groups was (100.00 ± 19.53)%, (98.18 ± 9.85)%, (96.09 ± 30.04)%, (90.64 ± 8.74)%, (59.75 ± 12.09)%, (35.43 ± 8.58)%, (26.35 ± 5.89)% and (17.54 ± 4.48)%, respectivily. There was statistically significant difference in cell viability between different dose groups ( F = 18.30, P < 0.05); and the cell viability of the 20, 50, 100, 150 μmol/L NaAsO 2 treatment groups was significantly lower than that of the control group ( P < 0.05). The time-response relationship results showed that there were statistically significant differences in Nrf2, Gclc, Gclm, Nqo1 and Mt1 mRNA level between control, 2, 6 and 12 h treatment groups ( F = 56.69, 85.28, 90.82, 80.46, 758.60, P < 0.05); with extension of arsenic exposure time, the mRNA level of Nrf2, Gclc, Gclm and Mt1 first increased and then decreased, the mRNA level of Nqo1 increased continually; among them, the mRNA level of Nrf2 peaked at 2 h, the mRNA levels of Gclc, Gclm and Mt1 peaked at 6 h, and the mRNA level of Nqo1 peaked at 12 h. The dose-response relationship results showed that there were statistically significant differences in Nrf2, Gclc, Gclm, Nqo1 and Mt1 mRNA levels between control, 2, 5 and 10 μmol/L NaAsO 2 treatment groups ( F = 68.39, 72.26, 30.41, 397.00, 28.88, P < 0.05); with increasing of arsenic exposure dose, the mRNA levels of Nrf2, Gclc, Gclm, Nqo1 and Mt1 increased. The mRNA level of Nrf2 peaked at a dose of 5 μmol/L, and the mRNA levels of Gclc, Gclm, Nqo1 and Mt1 peaked at a dose of 10 μmol/L. Apoptosis test results showed that there were statistically significant differences in the apoptosis rates of SCR and Nrf2-KD cells between control, 10 and 20 μmol/L NaAsO 2 treatment groups ( F = 8.18, 9.66, P < 0.05); compared with the control group, the apoptosis rates of SCR and Nrf2-KD cells in the 20 μmol/L NaAsO 2 treatment group increased ( P < 0.05); and the apoptosis rate of Nrf2-KD cells in the 20 μmol/L NaAsO 2 treatment group was higher than that of SCR cells in the same dose group ( P < 0.05). Conclusions:NaAsO 2 exposure has caused the activation of Nrf2 signaling pathway in mouse lymph node vascular endothelial cell line SVEC4-10 cells, activated the adaptive antioxidant response, and altered transcriptional activity; while silence of Nrf2 has made SVEC4-10 cells more sensitive to NaAsO 2 toxicity.

Objective@#To explore the clinical effects of superior gluteal artery perforator island flap in repair of sacral pressure ulcer.@*Methods@#From May 2012 to May 2017, 20 patients with sacral pressure ulcers (14 males and 6 females, aged 27 to 67 years) were admitted to our department. According to the consensus staging system of National Pressure Ulcer Advisory Panel in 2016, 6 cases were in 3 stages, 14 cases were in 4 stages, with the area of pressure ulcers ranging from 5.0 cm×4.0 cm to 10.0 cm×8.0 cm. After debridement and vacuum sealing drainage, the superior gluteal artery perforator island flaps were used to repair the pressure wounds, with the area of flaps ranging from 6 cm×5 cm to 13 cm×8 cm. The donor sites were sutured directly. The survival of flaps after operation, the healing of wounds, and the follow-up of patients were observed.@*Results@#After surgery, flaps of 20 patients survived well without reoperation. The length of hospital stay of patients was 20 to 40 days, with an average of 25 days. Eighteen patients were followed up for 6 to 24 months, with an average of 12.2 months. The flaps were in good shape and elastic recovery. There were no complications such as seroma or hematoma in the donor sites. Both the patients and family members expressed satisfaction with the shape and texture of the flap and shape of hip.@*Conclusions@#The superior gluteal artery perforator island flap is reliable in blood supply and easy to rotate. The flap can carry a little muscle to increase the anti-infective ability. Moreover, the donor site can be directly sutured with slight damage. Thus, it is one of the good methods for repairing sacral pressure ulcers.

Pressure ulcer is localized damage to the skin and subcutaneous tissue usually over a bony prominence as a result of prolonged pressure, shear force, and friction. The effect of external force on soft tissue is affected by many factors, such as local microenvironment, tissue blood supply, nutritional status, and underlying diseases of patients. Although great efforts have been made by mankind to prevent and treat pressure ulcer in last decades, its prevalence is still high, and the curative effect is still not impressive. The treatment of pressure ulcer is a challenge today, and it is necessary to seek new treatment methods. However, the prerequisite for exploring new treatment methods is to find a proper animal model and further explore new therapies through animal experiments. The pathogenesis of pressure ulcer is complex, and the formation process is affected by a variety of factors. To date, there is no recognized standard animal model. We review the pathogenesis of pressure ulcer and the recently reported animal models of pressure ulcer, so as to provide basic experimental basis for further research on the occurrence, development, and prevention of pressure ulcer.

To investigate the effects of adipose-derived mesenchymal stem cells (AMSCs) from type 2 diabetes mellitus patients on wound healing of pressure ulcers in mice.Methods@#(1) In September 2016, the subcutaneous adipose tissue of a 60-year-old woman with type 2 diabetes mellitus was harvested, and then AMSCs were extracted by collagenase digestion and cultured. The third passage of cells were used for subsequent experiments. The morphology of cells was observed, and their osteogenic, chondrogenic, and adipogenic differentiation abilities were identified. The expressions of cell surface markers CD90, CD105, CD73, and CD34 were detected by flow cytometer (n=3). (2) Sixteen female C57BL/6 wild-type mice aged 6-8 weeks were selected, and one pressure ulcer wound was created on each side of the spine of each mouse by pressing the skin with two magnets. The two wounds of each mouse were paired and divided into diabetic AMSCs group and negative control group, injected with 100 μL phosphate buffer solution (PBS) containing green fluorescent protein-labeled AMSCs (1×106 cells) and 100 μL PBS, respectively. The wound healing status of the two groups within post injection day (PID) 21 was observed, and their wound healing rates on PID 5, 13, and 17 were calculated. Three mice were sacrificed on PID 11 and 21, respectively, and tissue of three wounds was harvested from each group. The skin structure was observed by hematoxylin-eosin staining, the collagen deposition was evaluated by Masson staining, and the positive expression of CD31, i. e., the number of new blood vessels was counted by immunohistochemistry. Wound tissue samples of two groups prepared on PID 21 as above-mentioned were harvested, and the positive cell rate of S100, representing the regeneration of Schwann cells, was detected by immunohistochemistry. Wound tissue samples of diabetic AMSCs group prepared on PID 11 as above-mentioned were harvested, and the colonization of AMSCs was observed by fluorescence tracer method. Data were processed with paired t test and Bonferroni correction.@*Results@#(1) The third passage of cells isolated and cultured from the subcutaneous adipose tissue of a type 2 diabetes mellitus patient grew adherently to the wall in a long spindle and vortex-like manner. After induction, the cells showed osteogenic, chondrogenic, and lipogenic differentiation abilities. The positive expression rates of CD90, CD105, and CD73 on the cell surface were higher than 90.00%, and the expression rate of CD34 was 0.46%. The cells were identified as AMSCs. (2) The mice wounds of diabetic AMSCs group healed quickly, and all the wounds healed completely on PID 17, while the mice wounds in negative control group were not completely closed at this time, and there was still scab on the surface. On PID 5, 13, and 17, the healing rates of mice wounds of diabetic AMSCs group were (35.6±6.5)%, (87.1±2.5)%, and 100.0%, respectively, significantly higher than (19.8±7.2)%, (66.2±5.2)%, and (86.9±5.3)% of negative control group (t=6.49, 14.31, 9.73, P<0.05). Compared with that of negative control group, the inflammatory cell infiltration was reduced in mice wounds tissue of diabetic AMSCs group on PID 11, and thicker epidermis and dermis as well as regenerated skin appendages were observed on PID 21. On PID 11 and 21, the collagen percentages of mice wounds tissue in diabetic AMSCs group was (48.3±1.3)% and (54.1±1.7)%, respectively, significantly higher than (41.4±1.7)% and (50.3±1.2)% of negative control group (t=6.98, 3.99, P<0.01). On PID 11 and 21, the numbers of new blood vessels in mice wounds tissue of diabetic AMSCs group were 17.2±1.3 and 18.0±2.1, respectively, significantly more than 8.0±1.4 and 14.0±1.5 of negative control group (t=10.69, 3.38, P<0.01). On PID 21, the S100 positive cell percentage in mice wounds tissue of diabetic AMSCs group was (1.76±0.12)%, significantly higher than (0.55±0.03)% of negative control group (t=21.68, P<0.001). On PID 11, the colonization of AMSCs in mice wounds tissue of diabetic AMSCs group was observed.@*Conclusions@#Transplantation of AMSCs from type 2 diabetic mellitus patients can accelerate wound healing of pressure ulcers in mice by promoting angiogenesis, collagen deposition, and Schwann cell regeneration.

Objective@#To investigate the feasibility of an accuracy evaluation method for 3D reconstructed bone model based on 3D reconstruction software Arigin3D Pro.@*Methods@#Pig femurs were used as solid models which were scanned by CT and MRI respectively. The scan data were imported into software Arigin3D Pro for 3D model reconstruction by 3 operators with different reconstruction experience (≤1 year, 2 to 3 years, and ≥4 years, respectively). Each operator reconstructed the femurs 3 times and in each reconstruction measured the diameter of the femoral head, the length of the femur and the width of the knee joint at the distal end of the femur 3 times respectively using software Geomagic Wrap. The above parameters of the solid models were measured using a vernier caliper. The parameter values of reconstructed models and solid models were compared and the differences were analyzed.@*Results@#The measurements by Geomagic Wrap showed deviations between the CT and MRI reconstruction models and the solid models, and the maximum deviation percentages were 1.47% and 1.08%, respectively. The percentages of intra-operater difference ranged from 0.29% to 1.53%; the 3D models reconstructed by operators with different reconstruction experience were not identical.@*Conclusions@#It is a feasible accuracy evaluation method to compare key parameters between the 3D bone model reconstructed by software Arigin3D Pro and the real animal bone. The deviations of 3D reconstructed bone model based on CT and MRI images are acceptable. The accuracy of 3D bone construction is related to the difference in operators.

RESUMEN

Klebsiella pneumoniae is one of the most common bacteria in nosocomial infections, and the second pathogen in bloodstream infections and urinary tract infections.In recent years, the emergence of Carbapenem-Resistant Klebsiella Pneumonia has become the focus in the field of anti-infection.At present, the treatment of Carbapenem-Resistant Klebsiella Pneumonia is mainly combined drug use, but which exists possible drawbacks including many side effects, higher cost, and more serious drug resistance and so on.With the increasing attention of the therapeutic effect, Chinese traditional medicine begins to come into researchers′sight as an alternative therapy or combination of traditional Chinese and Western medicine.Therefore, animal models used to evaluate the efficacy of drugs have been attracted wide attention.This article reviews animal models of Klebsiella Pneumonia infection and their applications in anti-infection of Chinese traditional medicine.

Objective@#To explore the effects of local transplantation of autologous adipose-derived stromal vascular fraction (SVF) on the hyperplastic scar (HS) formation in rabbit ears and the mechanism.@*Methods@#Twenty-four New Zealand white rabbits were used to reproduce HSs by making four full-thickness skin defect wounds with a diameter of 1 cm on the ventral surface of left ear of each rabbit. Wound epithelization and local-tissue proliferation were observed, and wound healing (complete epithelization) time and formation time of HS were recorded. The 24 rabbits were divided into SVF group, pure DMEM group, and pure HS group according to the random number table, with 8 rabbits and 32 wounds in each group. On post injury day (PID) 25 (after the complete epithelization of wounds), 0.2 mL of low glucose DMEM medium containing CM-Dil labeled autologous SVF was injected into HSs of rabbits in SVF group, while the same amount of low glucose DMEM medium was injected into HSs of rabbits in pure DMEM group. The frequency of injection was once every 5 days, totally for 3 times. HSs of rabbits in pure HS group did not receive any treatment. On PID 40, HSs of rabbits′ ears in each group were harvested, then the histological form was observed by hematoxylin and eosin staining, the arrangement of collagen in HS was observed by Van Gieson staining, the distribution of CM-Dil-labeled SVF in the HS was observed with fluorescence microscope, and the mRNA expression and the protein expression of transforming growth factor β1 (TGF-β1), Smad3, and Smad7 in HS were determined by real-time fluorescent quantitative reverse transcription-polymerase chain reaction and Western blotting, respectively. Data were processed with one-way analysis of variance and Tukey test.@*Results@#(1) Complete epithelization time of wounds of rabbits′ ears was (20.0±2.0) d post injury, and HSs were formed on PID 25. On PID 40, HSs of rabbits′ ears in pure DMEM group and pure HS group were still in hyperplasia, while those in SVF group became smaller, flat, soft, and light colored. (2) On PID 40, compared with those in pure DMEM group and pure HS group, the number of epithelium foot like structures was more and the amount of inflammatory cells was less. The collagen of HSs of rabbits′ ears in SVF group was arranged more regularly with broader gap between collagens. (3) On PID 40, CM-Dil-labeled SVF could still be observed in the HSs of rabbits′ ears in SVF group. (4) On PID 40, compared with those in pure DMEM group and pure HS group, the mRNA expressions of TGF-β1 and Smad3 in the HSs of rabbits′ ears in SVF group were significantly down-regulated (P<0.05), while the mRNA expression of Smad7 was significantly up-regulated (P<0.05). There were no significant differences in the mRNA expressions of TGF-β1, Smad3, and Smad7 in the HSs of rabbits′ ears between pure DMEM group and pure HS group (P>0.05). (5) On PID 40, compared with those in pure DMEM group (0.74±0.03, 0.73±0.10, 0.54±0.09) and pure HS group (0.72±0.08, 0.71±0.12, 0.53±0.06), the protein expressions of TGF-β1 and Smad3 in the HSs of rabbits′ ears in SVF group (0.57±0.06, 0.42±0.09) were significantly down-regulated (P<0.05), while the protein expression of Smad7 (0.71±0.05) was significantly up-regulated (P<0.05). The protein expressions of TGF-β1, Smad3, and Smad7 in the HSs of rabbits′ ears in pure DMEM group and pure HS group were close (P>0.05).@*Conclusions@#Autologous SVF transplantation can inhibit the formation of HS in the early stage of scar formation of rabbit, the mechanism may be related to the TGF-β1/Smad signaling pathway.

Objective To explore the status of adolescent's network usage and its effect on life satisfaction.MethodsTotally 691 teenagers were tested by self-regulation scale,the online identity experiment scale and teenager life satisfaction scale.Results①The scores of urban teenagers were significantly higher than those of rural teenagers in self regulation of network behavior(involved-emotional control:(31.40±0.31) vs (29.33±0.32),network congnition (24.37±0.25) vs (22.69±0.21),offline introspection:(15.89±0.30) vs (14.84±0.26),online self-control:(20.68±0.33) vs (19.25±0.28)) and life satisfaction (173.92±1.55) vs (164.00±1.48)(t=4.642,5.082,2.654,3.349,4.613;P<0.01),but lower in negative intrinsic character(5.37±0.13) vs (5.82±0.14)(t=-2.285,P<0.05).The scores of male adolescents were significantly higher than that of female adolescents in network identity and academic satisfaction(t=2.577,4.095,2.051,2.700,P<0.05,P<0.01),but lower in self regulation of network behavior (t=-3.966,-2.653,-5.059,P<0.01).②There was significantly positive correlation between youth life satisfaction and net-addiction cognition,involved-emotional control,network cognition,offline introspection,online self-control,self-control to network attachment,positive intrinsic character(r=0.164,0.307,0.252,0.386,0.430,0.222,0.404,P<0.01),and negatively correlated with positive extrinsic character and negative intrinsic character(r=-0.126,-0.156,P<0.01).③Positive intrinsic character,online self-control,involved-emotional control,offline introspection,self-control to network attachment could be used to predict life satisfaction,ans could explain 38.9% of total variance (F=87.26,P<0.01).ConclusionThere are significantly differences of self regulation of network behavior,network identity and life satisfaction in different gender,between urban and rural areas.The adolescents with higher self regulation of network behavior and network identity have higher life satisfaction.

RESUMEN

Objective To study the effects of chronic exposure to inorganic arsenic (iAs) in drinking water on bone mineral density (BMD) in mice and its underlying mechanisms.Methods Five-month-old female C57BL/6 mice were randomly divided into sham groups and ovarectomy (OVX) groups (n =19 mice each group),which were further randomly assigned into control group (distilled water) and iAs exposure groups [5 mg/L and 20 mg/L,inorganic arsenite (iAsⅢ):inorganic arsenate (iAsv) =1 ∶ 1].Following 3 months of exposure to iAs,BMD of the mice were determined by the dual energy X-ray detector.RAW 264.7 cell line and bone marrow hematopoietic stem cells (BMHSC) primarily isolated from C57BL/6 mice were used to study the in vitro effects of iAs on osteoclast differentiation and underlying mechanisms.During differentiation induced by receptor activator of nuclear factor-κ B ligand (RANKL,50 μg/L) and macrophage colony-stimulating factor (M-CSF,30 μg/L),RAW 264.7 cell line were treated with 0.00,0.25,0.50,0.75,1.00,1.50 μmol/L iAsⅢ,while BMHSC with 0.0,0.2,0.4,0.6,0.8,1.0 μmol/L iAsⅢ for 6 days.Based on the effect of iAsⅢ on the differentiation of RAW cells,RAW 264.7 cell line were treated by 0.6 μmol/L iAsⅢ combined with 0,5,10 mmol/L of N-acetyl-cysteine (NAC).Tartrate resistant acid phosphatase (TRAP)-positive red-colored cells with 3 or more nuclei were considered mature osteoclast.Results The femoral BMD of the mice [(80.04 ± 4.06) mg/cm2] that had been exposed to 20 mg/L of iAs for 3 months was substantially decreased compared to that of sham control mice [(84.44 ± 4.40) mg/cm2].As expected,the BMD of the OVX group [(76.36 ± 3.36) mg/cm2] was significant decreased compared to that of the sham control group (P < 0.05).However,the BMD among the OVX groups showed no significant difference [5 mg/L:(77.74 ± 4.91) mg/cm2;20 mg/L:(75.56 ± 3.71) mg/cm2,P > 0.05].In vitro studies,the iAsⅢ evidently affected the osteoclast differentiation in a concentration-dependent fashion.Low concentrations of iAs Ⅲ exposure significantly augmented osteoclast differentiation in the two cell models while high concentrations showed inhibitory effect.In RAW 264.7 cells,the number of osteoclasts in different groups was significantly different (F =1 522,P < 0.05),in the 0.50 μmol/L iAs Ⅲ group the number of osteoclasts reached the peak.In the BMHSC,the nmnber of osteoclasts in different groups was also significantly different (F =1 781,P < 0.05),in the 0.6 μmol/L iAsⅢ group the number of osteoclasts reached the peak.NAC pretreatment significantly abolished low-level iAsⅢ(0.6 μmol/L)-induced augmentation of osteoclast differentiation in a concentration-dependent fashion (0 mmol/L:109.33 ± 3.06;5 mmol/L:56.00 ± 2.65;10 mmol/L:22.67 ± 0.58,F =1 940,P < 0.05).Conclusions The inhibitory effect of iAs on bone metabolism is dependent on the availability of ovary function,suggesting that iAs may interfere with estrogen metabolism and/or function to disturb bone metabolism.Oxidative stress induced by iAs exposure stimulates osteoclast differentiation,and the increased osteoclast differentiation may be involved in the reduction of BMD caused by chronic iAs exposure.These preliminary findings suggest that antioxidant intervention may be an effective approach to prevent osteoporosis induced by chronic iAs exposure.

Objective To examine the bacteriostasis of baicalin against Escherichiacoli strain carrying NDM-1 gene.Methods Minimal inhibitory concentration(MIC)and minimal bactericidal concentration(MBC)were determined by broth dilution method.Synergy function was designed withcheckerboard method.Anti-infection effect in vivo of baicalin was observed in mice with bacteremia.Results In vitro antibacterial tests showed that the MIC and MBC baicalin were 8 mg/ml.Synergistic inhibitory effect was observed between baicalin and imipenem (FIC=0.125).In vivo inhibition experiments showed that ba-icalin had decreased the mortality of 25% of Escherichiacoli Strain carrying NDM-1 gene infection in mice.Conclusion Ba-icalin has antibacterial effect on NDM-1 E.coli.

Objective:To establish a method for the determination of oleanolic acid in She medicine Clematis florida var. plena. Methods:The HPLC method was carried on a Lanbo-Kromasil C18 chromatography column using acetonitrile-water ( 86∶ 14 ) as the mobile phase. The detection wavelength was 205 nm, the flow rate was 1. 0 ml·min-1 , and the column temperature was 25 ℃. Re-sults:The calibration curve of oleanolic acid was linear within the range of 0. 52-15. 60μg (r=0. 999 9). The average recovery of oleanolic acid was 92. 13%(RSD=3. 0%, n=9). Conclusion:The method is simple, rapid, accurate and reproducible, which can be used as an ideal method for the quality control of She medicine Clematis florida var. plena. .

Objective To investigate the phenotype and genotype in a carbapenem resistant L.adecarboxylata strain.Methods Microbial identification and drug susceptibility test was done with VITEK II Compact.Carbapenemases genes were detec-ted by PCR methods.Results The strain of carbapenem resistant L.adecarboxylata produced IMP-1 carbapenemase.Con-clusion L.adecarboxylata was found only rarely in the clinical isolates.This was the first Isolate of L.adecarboxylata pro-ducing IMP-1 metallo-beta-lactamase.

Objective To compare the effect of different concentrations of ropivacaine in epidural analgesia of childbirth.Methods According to the digital table,300 cases of our hospital childbirth puerpera were randomly divided into A,B,C three groups.A group was given 0.125% ropivacaine compound fentanyl analgesia,B group was given 0.1% ropivacaine compound fentanyl analgesia,group C was not required analgesia childbirth.The childbirth of three groups was observed.Results In group A,the labor time was (261.38 ± 19.87) min,postpartum 2h blood loss was (241.03 ± 34.57) mL.In group B,the labor time was (260.09 ± 19.69) mnin,postpartum 2h blood loss was (238.66 ± 35.01) mL.In group C,the labor time was (270.46 ± 20.86) min,postpartum 2h blood loss was (251.75 ± 36.79) mL.Statistical analysis showed that there was no significant difference (t =0.472,1.035 ; all P > 0.05).In group A,the fetal heart rate was (142.34 ±21.57)times/min,neonatal Apgar score was (9.77 ± 0.21),and umbilical artery blood pH value was (7.27 ± 0.06).In group B,fetal heart rate was (145.21 ± 21.49) times/min,neonatal Apgar score was (9.79 ± 0.20),and umbilical artery blood pH value was (7.26 ± 0.08) ; Fetal heart rate of group C was (143.78 ±22.01)times/min,neonatal Apgar score was (9.64 ±0.24),and umbilical artery blood pH value was (7.28 ± 0.07).The differences among three groups were not statistically significant (t =0.763,0.360,0.114,all P> 0.05).Analgesic effect time of group A was (12.13 ± 1.76) min,pain score was (1.03 ±0.46) points,analgesic harem duration was (22.39 ± 3.21) s,analgesic harem time interval was (3.26 ± 1.49) min,the cesareandelivery rate was 8.00%.In group B,the analgesic effect time was (12.04 ± 1.69mnin),pain score was (1.01 ± 0.52) points,analgesic harem duration was (21.04 ± 3.18) s,analgesic harem time interval was (3.4.9 ±1.51)min,the cesarean delivery rate was 9.00% ; Duration of the analgesic effect of group C was (16.77 ±16.77) min,pain score was (3.76 ± 1.23) points,analgesic harem duration was (26.98 ± 5.87) s,analgesic harem time interval was (2.65 ± 0.75) min,the cesarean delivery rate was 48.00%.The differences between groups were statistically significant (chi square or t =6.148,8.522,5.749,4.095,61.316 ;P < 0.05).Conclusion Application of 0.1% ropivacaine compound fentanyl anesthesia can effectively relieve patients'pain,shorten labor and reduce postpartum 2h blood loss,impact less on contractions at the same time,reduce the incidence of cesarean delivery,has no influence on the neonate,which is worth popularization and application in clinic.

Purpose To explore the value of spectral CT imaging in differentiating pancreatic carcinoma and mass-forming pancreatitis, and to improve the preoperative diagnosis accuracy. Materials and Methods The dual-phase spectral CT imaging data in patients with pathology-proven pancreatic carcinoma (n=62) and mass-forming pancreatitis (n=30) were retrospectively analyzed. At 40-140 keV (with 10 keV increment), the mean CT values, energy spectrum curves and iodine concentrations were measured and compared between pancreatic carcinoma and mass-forming pancreatitis in arterial phase and portal venous phase. Results The mean CT values of mass-forming pancreatitis at 40-140 keV were higher than pancreatic carcinoma in both arterial phase and portal venous phase with significant difference (t= -7.349- -4.810, P<0.05; t= -6.760- -5.496, P<0.05). The energy spectrum curves were significantly different between pancreatic carcinoma and mass-forming focal pancreatitis, especially at 40-70 keV. The iodine concentrations of pancreatic carcinoma were lower than mass-forming pancreatitis in arterial phase and portal venous phase with significant difference (t=-28.577, P<0.05; t= -28.451, P<0.05). Conclusion Pancreatic carcinoma and mass-forming pancreatitis can be differentiated by measuring the CT value at different energies, energy spectrum curves and iodine concentrations.

Objective To explore the effects of atorvastatin on inflammatory factors(hs-CRP,TNF-αand IL-6 levels )of coronary heart disease. Methods 160 patients with coronary artery disease were selected which were treated in Nanyang Municipal Central Hospital from January 2010 to July 2014.The 160 CHD patients were divided into two groups using the random number table method.On the basis of conventional treatment,patients of the experimental group were given atorvastatin,but the patients of control group were given only the other conventional treatment.Patients of two groups were tested about inflammatory factors before treatment and after two months treatment,and then analyzed and compared.Results It had no statistically significant difference on hs-CRP,TNF-αand IL-6 levels in patients of the two groups before treatment;it had statistically significant difference on hs-CRP,TNF-αand IL-6 levels in patients of the two groups of two months after treatment(P<0.05 );differences on hs-CRP,TNF-αand IL-6 levels of patients between pre-treatment and after treatment of two months in the experimental group were statistically significant (P<0.05 );however,it was opposite in the control group.Conclusion Atorvastatin could significantly reduce the levels of inflammatory factors(hs-CRP、TNF-αand IL-6)of patients with coronary artery disease.It has important clinical value.

Objective:To establish a method for the determination of rutin in Anoectochili roxburghii. Methods:The detection was carried out by HPLC. A Lanbo-Kromasil C18 column(250 mm × 4. 6 mm,5 μm) was used with methanol-0. 2% phosphoric acid solu-tion (40∶60) as the mobile phase;the detection wavelength was 257nm, the flow rate was 1. 0 ml·min-1 and the column temperature was 30℃. Conclusion:The method is simple, rapid, specific and reproducible.
