RESUMEN

Objective To explore the rule of early Jun’ expression in craniocerebral gunshot injury and its significance. Methods After a direct shot of the dog′s head with a small calibre rifle, the expression of Jun ′ protein was assayed by immunohistochemistry method at different periods and in different regions, and the water contents and the ultrastructural changes in brain tissue were also observed. Results Nearly no Jun expression was found in cerebral tissues of the control group. RESUMEN

To improve the microsurgical curative effect of petroclival cholesteatoma (PCC), 52 patients harbouring PCC removed from 1993 to 2002 were analyzed retrospectively. Of all patients in this group, total tumor removal was achieved in 47 patients (90%), subtotal removal in 5 (10%). The new cranial nerve injuries occurred postoperatively in 4. Operative death occurred in 1 patient due to severe lung infection (1 9%). The key goal of surgical resection of the tumor was good. Suitable basicranial approaches and skillful microsurgical technique can increase the total removal rate, and decrease its mortality and morbidity.

RESUMEN

To probe the effect of transsphenoidal microsurgical removal on large pituitary adenomas (LPA), a total of 1 520 patients suffering from LPA were diagnosed by skull X ray plain films, CT, or MRI scanning. Operations were performed via a transsphenoidal approach under microscope. The results showed that gross total removal of an adenoma in 1 264 cases ( 83 2% ) and subtotal removal in 195 cases ( 12 8% ) were achieved; partial removal was carried out in the remaining 61 cases ( 4 0% ) of fibrous or dumbbell shaped adenomas. Death occurred in two patients (0 1%) after surgery. Long term follow up review( median: 3 5 years ) in 1 240 patients revealed that 1 219 cases( 98 3%) of those with preoperative diminished visual acuity had postoperative improvement; 21 cases (1 7%)had no change, and none deteriorated significantly. Among the 1 226 patients with preoperative visual field defects, postoperative improvement was good in 1 186 (96 7%), and poor in 40(3 3%).It is concluded that microsurgical removal of pituitary tumors by transsphenoidal approach is safe and very effective even for very large or giant adenomas except for fibrous and dumbbell shaped ones, since it allows rapid and adequate decompression of the optic nerves and chiasm.

RESUMEN

Tuberculum sellae meningioma comprises 5% to 10% of intracranial meningioma. Its surgery pays attention to operating procedures for it's special location. All the 29 cases underwent surgery via different approachs between May 1992 and September 2001. There were 6 men and 23 women, The mean age was 46 years (27~68 years). The mean size of the tumor was 3 4 cm (2 0~8 5cm). The primary symptoms were a progressive visual loss (72 4%), headache (24 1%) and seizure (3 4%).Tuberculum sellae meningiomas were approached through four cranial bone flaps: pterional approach in 13 cases, subfrontal approach in 11 cases, supraorbital approach in 3 cases and bilateral subfrontal approach in 2 cases. Complete tumor resection was done in 26 cases (89 7%)(Simpson's grade Ⅰ), subtotal surgical removal in 3 cases(10 3%)(Simpson's grade Ⅱ). No death occurred. Regardless of which surgical approach is selected, it is necessary to pay attention to the normal anatomical relationship and functions of the sellae region. The neurosurgeon must have skillful microneurosurgical techniques. On the other hand, identification of the normal structures and tumor capsule plays a role in protecting the important vessels, optic nerves and hypothalamus.

RESUMEN

To investigate the time dependent features and mechanisms of c fos expression in neurons after brain injury in mice. Using the mice model of severe traumatic brain injury, c fos expression in neurons was observed at different time points after trauma with the molecular hybridization and immunohistochemistry staining. A c fos expression process in neurons occurred after severe brain injury, peaking at 1 hour. Another expression peak appeared 24 hours later. Fos positive neurons were observed in the cerebral cortex of injured hemisphere, especially in Ⅱ Ⅳ layers. The c fos gene expression after brain injury in neuron has two peaks. c fos gene expression may be caused by Leao’s spreading depression, and may be associated with cell signal transduction and neuron apoptosis.

RESUMEN

SV40 large tumor antigen(Tag) expression was investigated by immunoprecipitation followed by silver staining and Western blot in 65 cases of human brain tumors and 8 cases of normal brain tissues, Tag P53 and Tag PRb complexes were screened respectively in 18 and 15 Tag positive tumor tissues. SV40 Tag was found in ependymomas, choroid plexus papillomas, pituitary adenomas, astrocytomas, meningiomas, glioblastomas multiforme and medulloblastomas, whereas 8 normal brain tissues were all negative for Tag. Tag P53 and Tag PRb complexs were detected respectively in Tag positive tumors. The results indicated that SV40 is associated with human brain tumori genesis, and the in activation of P53 and PRb due to the formation of Tag P53 and Tag PRb complexes is possibly an important mechanism in the etiopathogenesis of human brain tumors.
