RESUMEN

Objective To investigate the potential role of miR-34a on breast cancer recurrence and prognosis.Methods In this study,88 breast cancer patients underwent mastectomy with detailed clinical follow-up information.Extracting RNA from the formalin-fixed paraffin embedded samples,miR-34a levels were quantified by quantitative real-time polymerase chain reaction (qRT-PCR).miR-34a levels among clinico-pathological variables were accessed by Mann Whitney-U test.RFS and OS survival curves were derived from Kaplan-Meier estimates and the curves were compared by Log-rank tests.All statistical tests were two-sided.Results Significantly lower miR-34a level was found in tumor tissue compared to paired normal tissue (P =0.000).A potential relationship between miR-34a levels and existing clinico-pathological parameters of breast cancer,such as menstrual status,tumor size,nodal involvement,stage of disease,hormone receptor status,HER2 status,or tumor subtype was investigated.No statistically significant difference were identified for these features (P > 0.05).miR-34a level was significant lower among G3 group than G1 + 2 group (P =0.024).Down-regulated miR-34a level was observed in breast cancer with later relapse compared to patients without relapse (P =0.008).When considering 2-△Ct =0.117 (median level)as cut-off value,patients with miR-34a up-regulation showed a positive association towards a longer survival,either RFS(P=0.026,Log-rank test) or OS(P =0.019,Log-rank test).Conclusions miR-34a,as a tumor suppressor,promotes differentiation and contributes to relapse when down-regulated.miR-34a has the potential as prognostic factor for breast cancer.

RESUMEN

Objective To analyze the clinical pathologic features and prognosis of triple negative breast cancer. Method A total of 570 cases of breast cancer between January 2005 and June 2007 were analyzed,113 cases were negative for estrogen receptor(ER), progesterone receptor(PR) and HER-2 as the triple negative group, 48 cases were negative for ER, PR and positive for HER-2 as the control group. The clinical pathologic features, recurrence and survival of the patients were compared. Results The median age was 46 years old, 15 cases (13.3%) had a breast cancer family history or oophoroma family history,86.7%(98/113) of pathological types were invasive ductal carcinoma,lymph node metastases were detected in 31.0%(35/113 ) in the triple negative group. No significant difference was found for p53 staining between the triple negative group[58.4%(66/113)positive] and the control group [41.7%(20/48) positive] (P>0.05).The median time of follow-up was 24 months,the loss was 5 cases, 10 cases (9.3%, 10/108) had local recurrence and metastasis,and 8 cases (7.4%,8/108) died from breast cancer metastases,2-year survival rate was 96.2% in the triple negative group.There was no significant difference in local recurrence and survival time between the triple negative group and the control group (P>0.05). Conclusions The incidence of triple negative breast cancer is higher than other data reported. Triple negative breast cancer exhibits the same clinical pathological features compared with ER-/PR-/HER-2 + subtypes,and the prognosis is similar to the controls.

RESUMEN

Objective To investigate the risk factors of complications induced by intraperitoneal chemotherapy after radical resection for colorectal cancer,and to summarize the prevention and treatment methods.Methods The clinical data of 234 colorectal cancer patients who received intraperitoneal chemotherapy after radical resection at the Union Hospital of Fujian Medical University from January 1997 to March 2007 were retrospectively analyzed.Risk factors influencing the incidence of complications were determined by analyzing 12 relevant factors with one-way analysis of variance(ANOVA)and Logistic multivariate regression analysis.Results The incidence of complications after intraperitoneal chemotherapy for colorectal cancer was 37.2%(87/234),incidence of catheter-associated complications was 20.5%(48/234),incidence of adhesive ileus was 12.4%(29/234),and the incidence of chemical peritonitis was 19.2%(45/234).The results of one-way ANOVA and Logistic multivariate regression analysis showed that abdominal surgery history,surgical modality,postoperative complications,courses and frequencies of chemotherapy and chemotherapeutics were the risk factors influencing the incidence of complications after intraperitoneal chemotherapy.Conclusions One or two courses of intraperitoneal chemotherapy after radical resection for colorectal cancer is safe with few complications.Chemical peritonitis is the main cause for adhesive ileus after intraperitoneal chemotherapy,maintaining intestinal tract unobstracted during intraperitoneal chemotherapy is effective in preventing adhesive ileus.
