RESUMEN

OBJECTIVE To optimize the extraction process of polysaccharides from Dendrobium officinale, and preliminarily study its effect on acute lung injury (ALI) in mice. METHODS Using D. officinale as raw material, the polysaccharides were extracted from D. officinale by ultrasonic-assisted hot water immersion. Using the extraction rate of D. officinale polysaccharides as response value, the single-factor experiments and Box-Behnken response surface method were used to optimize the ratio of material to liquid, extraction time and extraction temperature. ALI mice were induced by lipopolysaccharide. Using prednisone acetate (5 mg/kg) as the positive control, the effects on the mass ratio of wet and dry lung and pathological changes of lung tissue (HE staining and Masson staining) of low-dose, medium-dose and high-dose D. officinale polysaccharides (50,100,200 mg/kg) were investigated. RESULTS The optimal extraction technology of D. officinale polysaccharides was as follows: the ratio of material to liquid was 1∶25 (g/mL), the extracting time was 1 h, and the extracting temperature was 58 ℃ . Under these conditions, the average extraction rate of D. officinale polysaccharides was 37.75% (RSD=1.12%,n=3), the relative error of which with predicted value (38.63%) was 2.28%. Compared with the model group, the ratios of wet and dry lung in the positive control group and D. officinale polysaccharides groups were all decreased significantly (P<0.05 or P<0.01), and the pathological changes in lung tissue (severe destruction of alveolar structure, significant widening of alveolar septa, extensive infiltration of inflammatory cells and proliferation of fibroblasts) were alleviated to varying degrees. CONCLUSIONS The optimal extraction process of D. officinale polysaccharides is feasible; the obtained polysaccharide extract has a certain improvement effect on ALI in mice.

Objective:To compare the changes of biliary microbiota after enteral extended biliary stents (EEBS) implantation with that of conventional plastic stents in animal experiment, and to preliminarily investigate its possible mechanism in preventing stents occlusion.Methods:A total of 12 healthy Bama minipigs were randomly assigned to the conventional plastic stent group ( n=6) and the EEBS group ( n=6) using simple random method. The bile samples of all pigs were collected before stents implantation and 4 weeks after stents placement. The biliary microbiota composition and diversity before and after different stents implantation were analyzed by 16S rRNA gene sequencing and compared. Results:No complications including acute cholangitis, perforation, bleeding, or death occurred in 12 pigs. Eight days after stents implantation, stents were out of bile duct in all pigs under endoscopy, while the bile samples were collected again for analysis. The main composition of biliary microbiota at the phylum level were Proteobacteria, Firmicutes and Bacteroidota. Alpha-diversities revealed the Shannon ( P=0.004) and Simpson index ( P=0.008) significantly decreased in the conventional stent group after stents placement, and Bata diversity analysis also showed a significant difference in microbial composition (Anosim: R=0.514 8, P=0.011). There was no significant difference in Observed species index ( P=0.095), Chao1 index ( P=0.136), Shannon index ( P=0.353), Simpson index ( P=0.227) or Bata diversity (Anosim: R=0.059 3, P=0.187) in the EEBS group before and after stents placement. LEfSe algorithm indicated Bacteroides_ fragilis and Proteobacteria- Gammaproteobacteria- Enterobacterales- Enterobacteriaceae- scherichia_ Shigella- Escherichia_ coli significantly increased in the conventional stent group, and Desulfobacterota- Desulfovibrionia- Desulfovibrionales- Desulfovibrionaceae- Bilophila significantly increased in the EEBS group after stents placement. Conclusion:The biliary microbiota change slightly after EEBS implantation in the short-term, and EEBS may prevent duodenobiliary reflux by prolonging the reflux path.

BACKGROUND@#Familial hypercholesterolemia (FH) is a common autosomal dominant hereditary disease. Its early diagnosis and intervention significantly improve the patient's quality of life. However, there are few types of research on the FH pathogenic genes in China.@*METHODS@#In this study, we recruited a family diagnosed with FH and used whole exome sequencing (WES) to analyze the proband variants. Intracellular cholesterol level, reactive oxygen species (ROS) level, and the expression of pyroptosis-related genes were detected after overexpression of wild-type or variant LDLR in L02 cells.@*RESULTS@#A heterozygous missense variant predicted to be deleterious to LDLR (c.1879G > A, p.Ala627Thr) was identified in the proband. Mechanistically, intracellular cholesterol level, ROS level, and the expression of pyroptosis-related genes, nucleotide-binding oligomerization domain-like receptor family protein 3 (NLRP3) inflammasome and components (caspase 1, apoptosis-associated speck-like protein containing a caspase recruitment domain (ASC) and NLRP3), gasdermin D (GSDMD), interleukin (IL) -18, IL-1β was elevated in the variant LDLR group, which was attenuated by inhibition of ROS.@*CONCLUSIONS@#FH is associated with a variant (c.1879G>A, p.Ala627Thr) in the LDLR gene. Regarding the mechanism, the ROS/NLRP3-mediated pyroptosis in hepatic cells may contribute to the pathogenesis of the LDLR variant.

OBJECTIVE@#To investigate the clinical efficacy of the combined application of blistering cupping with thunder-fire moxibustion in treating bronchial asthma of cold-wheezing syndrome, and its influences on airway remodeling, inflammatory factors, lung function, and quality of life on the base of conventional western medicine treatment.@*METHODS@#A total of 76 patients with bronchial asthma of cold-wheezing syndrome were randomly divided into an observation group and a control group, 38 cases in each group. In the control group, the basic treatment was used, i.e. budesonide formoterol powder inhalation. In the observation group, on the basis of the treatment as the control group, blistering cupping combined with thunder-fire moxibustion was supplemented, Dazhui (GV 14), Danzhong (CV 17) and bilateral Feishu (BL 13), Gaohuang (BL 43), and Zhongfu (LU 1) were selected; blistering cupping was administered once a day and thunder-fire moxibustion was given twice a day. One course of treatment was composed of 7 days in both groups, and 2 courses of treatment were required. Before and after treatment, the airway remodeling indexes (matrix metalloproteinase-9 [MMP-9], tissue inhibitor of matrix metalloproteinase-1 [TIMP-1], and transforming growth factor-β1 [TGF-β1]) and inflammatory indexes (interleukin [IL] -1β、IL-25) were detected by using radioimmunoassay in the patients of the two groups. The lung function, traditional Chinese medicine symptom score, and asthma quality of life questionnaire (AQLQ) score were observed in the patients of the two groups.@*RESULTS@#After treatment, the serum levels of MMP-9, TIMP-1, TGF-β1, IL-1β, IL-25, peak expiratory flow (PEFR), traditional Chinese medicine symptom scores, and AQLQ scores were decreased compared with those before treatment in the patients of the two groups (P<0.05), and the results in the observation group were lower than those in the control group (P<0.05). After treatment, the first second forced expiratory volume (FEV1) and peak expiratory flow rate (PEF) were increased compared with those before treatment in the two groups (P<0.05), and the results in the observation group were higher than those in the control group (P<0.05).@*CONCLUSION@#On the basis of the conventional western medicine treatment, the combination of blistering cupping with thunder-fire moxibustion can effectively ameliorate the clinical symptoms of patients, reduce inflammatory levels, inhibit airway remodeling and improve the lung function and quality of life in the patients with bronchial asthma.

ObjectiveTo explore the influencing factors of willingness to quit smoking in smokers, and to provide reference for further smoking cessation intervention. MethodsThrough purposive sampling, smokers were recruited from 4 residential communities and 3 companies in Shanghai for an online questionnaire survey. The survey included general demographic characteristics, willingness to quit smoking, smoking condition, tobacco addiction, awareness of influence of smoking, smoking abstinence self-efficacy, and smoking rationalization belief. T-test, χ2 test and multiple logistic regression were used to analyze the data. ResultsA total of 336 valid questionnaires were collected. The average age of the smokers was (37.1±10.2) years, and most of their education level was college or above (58.3%). The daily number of cigarette use was (11.46±7.22) and most of smokers were classified as low nicotine dependence (58.3%). Today 248 smokers planned to quit smoking in the next six months. Compared with smokers with low smoking abstinence self-efficacy, smokers with high self-efficacy were more likely to quit smoking (OR=2.406,95%CI: 1.357‒4.267); Smokers with low smoking rationalization belief were more likely to quit smoking than those with high rationalization belief (OR=2.112, 95%CI: 1.167‒3.821). ConclusionSmoking abstinence self-efficacy has a positive impact on willingness to quit smoking, while smoking rationalization belief has a negative impact on the willingness to quit smoking. Efforts to improve smoking abstinence self-efficacy and dispel smoking rationalization beliefs of smokers should be made to promote the intention to quit among smokers.

Objective To investigate the expression of CK2α' in hepatocellular carcinoma (HCC) tissues and its prognosis value. Methods Tissue microarray was made from tumor and adjacent normal tissues of 83 patients with HCC. Immunohistochemical was used to stain the microarray for semi-quantitative analysis, and the expression of CK2α' in HCC tissues and adjacent normal tissues was compared. The associations between CK2α' expression and clinicopathologic features of HCC patients were analyzed with χ2 text. The prognostic value of CK2α' was estimated using Kaplan-Meier approach. Results The average positive expression rate of CK2α' was 47.8% in HCC and 65.4% in adjacent normal tissues. The positive expression rate of CK2α' in HCC tissues was lower than that in adjacent normal tissues (P < 0.01). The expression level of CK2α' in the cancer tissues of 74.7% cases was significantly lower than that in the adjacent tissues. HCC patients with low CK2α' expression had higher risk of recurrence and shorter overall survival time (P < 0.05). The expression of CK2α' was significantly correlated with CK19 (P < 0.05). Conclusion CK2α' is down-regulated in HCC tissues and correlated with the prognosis of patients. It could be a potential prognostic marker for HCC.

Objective To investigate the expression level of calcitonin gene-related peptide-receptor component protein (CRCP) in hepatocellular carcinoma (HCC) tissue and adjacent tissue and its association with the clinicopathological features and prognosis of patients. Methods HCC and adjacent tissue samples were collected from 79 HCC patients who underwent surgical resection in Eastern Hepatobiliary Surgery Hospital, Navy Medical University, from June 2003 to September 2009. Tissue microarray was prepared, and immunohistochemistry was used for quantitative analysis. Related proteins were extracted and measured by Western blot, and the expression of CRCP was compared between HCC tissue and adjacent tissue. The chi-square test was used for comparison of categorical data between groups. The receiver operating characteristic (ROC) curve analysis was performed to obtain the area under the ROC curve (AUC), and goodness of fit was evaluated. Youden index was used to determine the optimal cut-off value. and the Kaplan-Meier survival analysis was used to analyze the association of CRCP expression with the recurrence and prognosis of HCC, and the log rank test was used for comparison between the two groups. Results Among the 79 HCC patients, there were 67 male patients and 12 female patients, with an age of 10-72 years, and 20 patients had portal vein tumor thrombus. As for pathological grade, 1 had grade 4 HCC, 61 had grade 3 HCC, and 17 had grade 2 HCC; as for BCLC stage, 5 had BCLC stage 0 HCC, 55 had BCLC stage A HCC, 11 had BCLC stage B HCC, and 8 had BCLC stage C HCC. Western blot showed that the expression level of CRCP in HCC tissue was lower than that in adjacent tissue in 4 patients, and immunohistochemistry showed that the expression level of CRCP in HCC tissue was significantly lower than that in adjacent tissue in 75.9% of the patients. Low CRCP expression was associated with CK19 positivity, incomplete tumor capsule, presence of portal vein tumor thrombus, and high pathological grade ( χ 2 =6.410, 4.829, 9.319, and 9.083, all P < 0.05). Compared with the low CRCP expression group, the high CRCP expression group had a significantly longer overall survival time and a significantly lower recurrence rate ( P < 0.001 and P =0.009). Conclusion Patients with low CRCP expression in HCC tissue tend to have a poorer prognosis than those with high CRCP expression, and CRCP may participate in the development, progression, and metastasis of HCC, suggesting that this molecule can be used as a potential biomarker to predict the prognosis of HCC patients.

Objective To identify the osteogenesis genes whose expression is altered in hypertrophic chondrocytes treated with HO. Methods Murine chondrogenitor cells (ATDC5) were differentiated into hypertrophic chondrocytes by Insulin-Transferrin-Selenium (ITS) treatment, and then treated with HO. Suitable conditions (concentration, time) were determined by using the MTT assay. After total RNA isolation and cDNA synthesis, the levels of 84 genes were determined using the PCR array, whereas quantitative RT-PCR was carried out to validate the PCR array data. Result We identified 9 up-regulated genes and 12 down-regulated genes, encoding proteins with various functions, such as collagen proteins, transcription factors, proteins involved in skeletal development and bone mineral metabolism, as well as cell adhesion molecules. Quantitative RT-PCR confirmed the altered expression of 5 down-regulated genes (Smad2, Smad4, transforming growth factor $\beta$ receptor 1, transforming growth factor $\beta$ receptor 3, and matrix metalloproteinase 10). Conclusions HO significantly changed the expression of several genes involved in a variety of biological functions. Because of the link between oxidative damage and Kashin-Beck disease, these genes may also be involved in the deep-zone necrosis of the cartilage observed in Kashin-Beck disease.

Objective:To investigate the clinical effect and mechanisms of ceftriaxone combined with ranitidine on the acute pancreatitis.Methods:92 cases of patients with acute pancreatitis were selected and randomly divided into the control group (n=46) and experimental group (n=46),the control group was treated with ceftriaxone,and the experimental group was treated with ranitidine based on the control group,the serum levels of intedeukin-6 (IL-6),c-reactive protein(CRP),platelet activating factor (PAF),superoxide dismutase (SOD),propylene glycol (MDA),gastric secrete element,stomach,heart rate (HR),mean arterial pressure (MAP),and the relief time of clinical manifestation and the clinical efficacy were observed and compared between the two groups.Results:After treatment,the serum levels ofIL-6,CRP,PAF,MDA,gastric secrete element and HR of experimental group were significantly lower than those of the control group (P<0.05).The serum levels of SOD,stomach motion element and MAP of experimental group were higher than those of the control group (P<0.05).The relief time of clinical manifestation and total efficiency of experimental group were better than those of the control group (P<0.05).Conclusions:Ceftriaxone combined with ranitidine could effectively enhance the clinical efficacy of acute pancreatitis,which might be related to the anti-oxidation and anti-inflammation.

OBJECTIVE To explore the protective effect of polysaccharides from Salvia Chinensis Benth. (PSSC) on lipopolysaccharide (LPS) and D- galactosamine (GalN)- provoked mouse acute liver failure (ALF) and the possible molecular mechanism. METHODS Kunming mice were randomly divided into four groups: normal control, model, model+PSSC 30 and 100 mg·kg- 1 groups. PSCC was given once a day and for a week. To establish an ALF model, mice of model and PSSC groups were ip injected with LPS 10 μg·kg-1 and GalN 700 mg·kg-1 at the end of PSSC treatment. The microscopic structure of the liver was detected by HE staining. Serum and hepatic biochemical parameters of glutamic-oxalacetic transaminase (GOT), glutamic- pyruvic transaminase (GPT), catalase (CAT), superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), malondialdehyde (MDA), and glutathione (GSH) were detected by colorimetric methods. The relative content of hepatic reactive oxygen species (ROS) was measured by DCFH-DA fluorescent probes. Levels of cytokines tumor necrosis factor-α (TNF-α), interleukin 1β(IL-1β) and IL- 6 in the serum and liver were detected by ELISA. Activity of caspase 3 in liver homogenates was detected by aspase 3 activity assay kit. RESULTS Compared with normal control group, in the liver of model group, hepatocytes were arrayed in disorder, cytoplasm of hepatocytes shrank, and boundaries between cells were fuzzy, the infiltration of a large number of inflammatory cells and tissue hemorrhage could be detected, pathological scores were elevated significantly (P<0.01), levels of MDA and ROS in the liver of ALF model mice were elevated to 2.2 and 4.3 times that of the normal control, respectively (P<0.01), the level of GSH decreased to 51% (P<0.01), and the activities of SOD, CAT and GSH- Px declined to 74%, 36% and 42%, respectively (P<0.01). Levels of TNF- α, IL- 1β and IL- 6 in the serum and liver of model group were increased (P<0.01), and caspase 3 activity was increased to 5.3 times that of the normal control (P<0.01). Compared with the model group, the number of surviving mice in PSSC groups increased, liver pathological scores declined (P<0.01), levels of MDA and ROS increased (P<0.01), levels of GSH, TNF-α, IL-1β and IL-6 in the liver and serum declined (P<0.01), and caspase 3 activity decreased (P<0.01). CONCLUSION PSSC is able to alleviate LPS and GalN-induced ALF in mice. Inhibition of hepatic oxidative stress, inflammatory response, and cell apoptosis is possibly implicated in the protective effect of PSSC.

Objective To analyze the rate of blood culture contamination,pollution path,the proportion of contaminated bacteria of First People′s Hospital of Kunming city from 2013 to 2015,and provide a basis for effective prevention and control of pollution.Methods A total of 34 713 cases of blood culture samples from 2013 to 2015,were retrospectively analyzed.Results A total of 2860 culture positive samples from 34 713 cases were found,from which 361 cases were polluted(1.04%).According to the classification of the year,the lowest rate of blood culture contamination was 2015 According to the classification in the quarter,there was no significant difference between the 4 seasons;According to the classification of internal and surgical systems,,the pollution rate of internal medicine system was 1.06%,while the surgical system was 0.96%.According to the classification of departments:the hemodialysis center has the highest pollution rate(2.71%),followed by ICU (2.23%).Galactophore Department has the highest pollution rate (2.30%) in the surgical system and followed by orthopedics(1.92%).According to the statistics of contaminated bacteria,Staphylococcus aureus was the highest,accounting for 32.41%,followed by Staphylococcus epidermidis,accounting for 31.30%.Conclusion Hospital has a high blood culture contamination rateand diversification pollution waywhich can not be ignored.The pollution of bacteria in the blood culture mainly for bottle of single positive coagulase negative Staphylococcus.

<p><b>OBJECTIVE</b>To explore the efficacy on knee osteoarthritis (KOA) treated with different acupuncture methods at different stages.</p><p><b>METHODS</b>One hundred and eighty cases of KOA were divided into stagnation stage, fascia cramp stage and tendon lesion stage according to the condition of disease, 60 cases in each stage. Each stage was randomized into a staging treatment group and an electroacupuncture (EA) group, 30 cases in each one. In the staging treatment group, acupuncture at the tendon points of meridians and electric thermal needling method were adopted for the cases at the stagnation stage; the small needle-knife therapy and bleeding method were used for the cases at the fascia cramp stage; and the electric thermal therapy with thick silver needles was applied for the cases at the tendon lesion stage. In the EA group, EA was applied for the cases of all the three stages at Liangqiu (ST 34), Xuehai (SP 10), Yanglingquan (GB 34), etc.</p><p><b>RESULTS</b>The total effective rate was 96.1% (87/90) in the staging treatment group and was 91.1% (82/90) in the EA group, without significant difference in comparison (P > 0.05). The controlled and remarkably effective rate (88.9%, 80/90) in the staging treatment group was higher apparently than (62.2%, 56/90) in the EA group (P < 0.001). And in the EA group, with the disease progression and the further disease stages, the controlled and remarkably effective rate was reduced. After treatment, the scores of symptoms and signs were decreased in both groups (both P < 0.001), the improvement in the staging treatment group was superior to that in the EA group (P < 0.001).</p><p><b>CONCLUSION</b>The different acupuncture methods at the three stages improve obviously the clinical effect and are highly targeted. The mechanism of the three stages on "meridian muscle region pathology" and the treatment based on the disease stages can be the effective approach to KOA.</p>

<p><b>OBJECTIVE</b>To discuss the mechanical virtual reality when acupuncture on Jianliao (TE 14) so as to get perform perceptual knowledge of acupuncture for beginners through studying 3D reconstruction of surrounding tissues of this acupoint.</p><p><b>METHODS</b>Related tissues were segmented and reconstructed by establishing mathematics model based on the operational platform of VOXEL-MAN (vision reappearance); and mechanical model was constructed and fused with related data of virtual human (touch reappearance); data communication between the force feedback apparatus and the operational platform of virtual human were set up by interchanging vision reappearance module and touch reappearance module.</p><p><b>RESULTS</b>In Linux system, the operator could experience the mechanical changes while needle-tip passing various tissues of Jianliao (TE 14) as well as lifting and thrusting evenly in needling process through the force feedback apparatus.</p><p><b>CONCLUSION</b>Based on the platform of VOXEL-MAN, the sensation of needling at jianliao (TE 14) could be reappeared idealy by strength feedback instrument, so that the beginners could imitate, learn and practice the virtual acupuncture.</p>

RESUMEN

This paper discusses the application of virtual reality technology in the 3-D visible human body and acupuncture research. Based on the 3-D visible human fused with the localization information and hierarchy of acupoints, the paper analyzes the force against the needle and haptic rendering during the needle manipulation according to the physical properties of different tissues. A haptic model is constructed to demonstrate the force behaviors during acupuncture, and the force will be produced and passed to the manipulator by a force feedback device. It enriches the contents of 3-D visible human project, provides a dynamic simulation instrument for acupuncture teaching, and supplies a platform for acupuncture research.

<p><b>OBJECTIVE</b>To discuss the problem difficult to be presented for nerves and blood vessels in the 3D reconstruction of acupoints through studying 3D reconstruction of Jianliao (TE 14).</p><p><b>METHODS</b>Muscles and other tissues were segmented by applying cointeractive segmentation method based on chromatic characteristic space; the nerve and blood vessels can be reconstructed by establishing mathematics model, and the needle-inserting animation of Jianliao (TE 14) could be obtained by running script file.</p><p><b>RESULTS</b>During the course of 3D needle-inserting animation of Jianliao (TE 14), the spatial location between all kinds of tissues including the nerve and blood vessels and the needle body could be systematically observed.</p><p><b>CONCLUSION</b>The difficulty of presenting the nerve and blood vessels in the study on 3D reconstruction of acupoints can be resolved by establishing mathematics model. The generation of 3D animation for acupoints with the nerve and blood vessels is benefit to acupuncture teaching and study on the essence of acupoints and channels.</p>
