Abstract Background: ST-elevation myocardial infarction (STEMI) systems of care have reduced inter-hospital transfer times and facilitated timely reperfusion goals. Helicopters may be an option when land transportation is not feasible; however, the safety of air transport in patients with acute coronary syndrome (ACS) is a factor to consider. Objetives: The aim of this study was to evaluate the safety of helicopter transport for patients with ACS. Methods: Prospective, observational, and descriptive study including patients diagnosed with ACS within the STEMI network of a metropolitan city transferred by helicopter to a large cardiovascular center to undergo percutaneous coronary intervention. The primary outcome of the study was the incidence of air-travel-related complications defined as IV dislodgement, hypoxia, arrhythmia, angina, anxiety, bleeding, and hypothermia. Secondary outcomes included the individual components of the primary outcome. Results: A total of 106 patients were included in the study; the mean age was 54 years and 84.9% were male. The most frequent diagnosis was STEMI after successful fibrinolysis (51.8%), followed by STEMI with failed fibrinolysis (23.7%) and non-reperfused STEMI (9.4%). Five patients (4.7%) developed at least one complication: IV dislodgement (1.8%) and hypoxemia (1.8%) in two patients and an episode of angina during flight (0.9%). A flight altitude of > 10,000 ft was not associated with complications. Conclusions: The results of this study suggest that helicopter transportation is safe in patients undergoing acute coronary syndrome, despite the altitude of a metropolitan area.


Resumen Antecedentes: Los sistemas de atención de IAMCEST han reducido los tiempos de transferencia interhospitalaria y han facilitado las metas de reperfusión oportuna. Los helicópteros pueden ser una opción cuando el transporte terrestre no es factible; sin embargo, la seguridad del transporte aéreo en pacientes con síndrome coronario agudo (SICA) es un factor a considerar. Objetivos: Evaluar la seguridad del transporte en helicóptero para pacientes con SICA. Métodos: Estudio prospectivo, observacional, descriptivo. Se incluyeron pacientes con diagnóstico de SICA dentro de la red IAMCEST en metrópolis extensa, trasladados en helicóptero a un centro cardiovascular. El resultado primario del estudio fue la incidencia de complicaciones relacionadas con los viajes aéreos definidas cómo desalojo de catéter intravenoso, hipoxia, arritmia, angina, ansiedad, sangrado e hipotermia. Resultados: Total de 106 pacientes; la edad media fue de 54 años y 84,9% eran hombres. La altitud media de vuelo fue de 10,100 pies y la distancia media de vuelo fue de 50,0 km. El diagnóstico más frecuente fue IAMCEST tras fibrinolisis exitosa (51,8%), seguido de IAMCEST con fibrinolisis fallida (23,7%). Cinco pacientes (4,7%) desarrollaron una complicación: desalojo IV (1,8%) e hipoxemia (1,8%) en dos pacientes y un episodio de angina durante el vuelo (0,9%). Una altitud de vuelo mayor de 10,000 pies no se asoció a complicaciones. Conclusiones: Los resultados de este estudio sugieren que el transporte en helicóptero es seguro en pacientes con SICA, incluso en altitudes > 10,000 pies.

OBJECTIVE: To discuss the applicability of 3 risk assessment methods to evaluate the occupational health risk of benzene and its analogues in the workplace of an aircraft maintenance enterprise. METHODS: The workshops of compound material,structural and electromechanical involved in exposure of benzene and its analogues in an aircraft maintenance enterprise were chosen as study subjects. The optimal indicator system of risk assessment on occupational chemical hazards( hereinafter referred to as“optimal indicator system”),the Singapore semi-quantitative risk assessment model and risk index method were used to classify the benzene hazards of 5 operating posts in 3 workshops. The risk stratification results of the 3 assessment methods were compared. RESULTS: The results of optimal indicator system for benzene risk rating was mild risk in all workplaces. The risk classifications for toluene and xylene were light risk. The risk classifications of Singapore semi-quantitative risk assessment model for benzene,toluene and xylene were all low risk in all workplaces. The risk classifications of risk index method for benzene were all moderate risk in all workplaces. The risk classifications for toluene were harmless in all workplaces. The risk classifications for xylene were mild risk in all workplaces. CONCLUSION: There are differences among the evaluation results of the 3 risk assessment methods. The optimal indicator system could be more suitable for occupational health assessment of benzene and its analogues in aircraft maintenance industries.

RESUMEN

The objective of this study was to describe lesions in four birds, victims of collisions with aircraft, coming from the International Airport of Recife/Guararapes Gilberto Freyre, Pernambuco, Brazil. Contused wounds were predominant in birds in this study, characterized by the presence of fractures, bruises and hematomas and justified by the blunt action promoted by the type of vulnerant agent involved (aircraft). The analysis under medico-legal veterinary aspect of lesions in bird strike is essential, because in addition to the attempt to establish a standard for differentiating the aircraft lesions, it can guide the bird strike hazard management programs at airports, since the necropsy allows the identification of the species involved or at least facilitates the obtaining biological material for identification.(AU)

BACKGROUND: This paper presents the findings of a pilot research survey which assessed the degree of balance between safety and productivity, and its relationship with awareness and communication of human factors and safety rules in the aircraft manufacturing environment. METHODS: The study was carried out at two Australian aircraft manufacturing facilities where a Likert-scale questionnaire was administered to a representative sample. The research instrument included topics relevant to the safety and human factors training provided to the target workforce. The answers were processed in overall, and against demographic characteristics of the sample population. RESULTS: The workers were sufficiently aware of how human factors and safety rules influence their performance and acknowledged that supervisors had adequately communicated such topics. Safety and productivity seemed equally balanced across the sample. A preference for the former over the latter was associated with a higher awareness about human factors and safety rules, but not linked with safety communication. The size of the facility and the length and type of employment were occasionally correlated with responses to some communication and human factors topics and the equilibrium between productivity and safety. CONCLUSION: Although human factors training had been provided and sufficient bidirectional communication was present across the sample, it seems that quality and complexity factors might have influenced the effects of those safety related practices on the safety–productivity balance for specific parts of the population studied. Customization of safety training and communication to specific characteristics of employees may be necessary to achieve the desired outcomes.

An introduction was first given to the advantages and application status of multi-rotor unmanned aerial vehicle (UAV) in the field of medical rescue. Then, the latest worldwide application and research status of multi-rotor UAV in this field were analyzed according to available research. On this basis, the application prospects of multi-rotor UAV in the scenarios of medical rescue field were assumed. Finally, it was pointed out that multi-rotor UAV would show more functions in medical rescue field owing to various technological development and breakthrough, signifying the bright deepening application prospect of multi-rotor UAV.

The objective s of the research are t o identify the noise sources and noise level exposure inside C130H aircraft ’s hangar ; to produce nois e mapp ing ; to identify hearing status of C 130H aircraft maintenance crews (MC) ; and suggesting the noise control measure that can be applied. A field survey on noise level by using a digital s ound level m eter and producin g noise mapping . A h earing test ing among 63 MC was carried out in Institute of Aviation Me dicine , Royal Malaysian Air Force ( RMAF ) . Reviewing the literature and a nalyzing some control measure s to be taken. Hearing test result shown 41.2% of the MC are having hearing impairment. The highest noise level at the central of the hanga r is 92.2 dB A ( day - time) and 94.2 dBA (night - time) when there is a C130H aircraft starting its engine at 50 meter s from the centr al of the hanga r, 95.3 dBA (day - time) and 97.3 dBA (night - time) when there is a C130H a ircraft Engine Ground Run at 150 meter s fr om the centra l of the hanga r. Besides, Auxiliary Power Unit is producing the highest nois e level which is 125.7 dBA (day - time) and 127.7 dBA (night - time) . The application of Personal Protective Equipment (PPE) is the very likely control measure to be taken while engin eering control is ve ry costly but can be considered. Since noise is recognized as a hazard, hearing protection as PPE will not be the ultimate solution as a control measure. Hence, the Engineering Control must be identified and studied to be implemented as an ultimate solution to control the noise hazard in long term duration.

ABSTRACT: Despite all advances obtained with the pesticide use worldwide and in Brazilian agriculture, there is still a lack of technologies for agricultural aircrafts. The objective of this study was to evaluate the drift produced by the use of conical nozzles and electrostatic system in aircraft application. The experimental design was completely randomized with a factorial arrangement and four replications. Treatments included two spraying equipment: electrostatic system with 5L ha-1 of spray volume and conical nozzle with 15 and 20L ha-1. In all treatments, azoxystrobin fungicide was used at a dose of 100g a.i. ha-1 and paraffinic mineral oil at a dose of 0.5% v.v. Volume median diameter (VMD) and droplet density were estimated using water sensitive paper. Evaluations of drift distance were performed with glass collectors distributed horizontally on the soil surface, 50m before the application band, at the application site and 12,5, 25, 50, 75, 100, 150, 200 and 300m after the application site, along the wind direction. Azoxystrobin concentration in glasses were quantified by UHPLC-MS/MS. Results indicated that the drift distance can reach more than 300 meters away from the application site, with variable concentrations according to the equipment used. Electrostatic system reduced drift when compared to hydraulic conical nozzles.


RESUMO: Apesar dos avanços obtidos no uso de agrotóxicos na agricultura mundial e brasileira, ainda existe carência em tecnologias voltadas para aviação agrícola. O objetivo deste estudo foi avaliar a deriva ocorrida com a utilização de bicos hidráulicos com pontas cônicas e sistema eletrostático em aplicação aérea. O delineamento experimental adotado foi inteiramente casualizado com quatro repetições. Os tratamentos foram compostos por dois equipamentos de pulverização: sistema eletrostático com volume de aplicação de 5L ha-1 e bicos hidráulicos com pontas cônicas, utilizando volumes de aplicação de 15 e 20L ha-1. Em todos os tratamentos, foi utilizado o fungicida azoxistrobina na dose de 100g i.a. ha-1 e óleo mineral parafínico na dose de 0,5% v.v. As avaliações de diâmetro mediano volumétrico e densidade de gotas foram realizadas através de papéis hidrossensíveis na faixa de aplicação. As avaliações de distância de deriva foram realizadas com coletores de vidro distribuídos horizontalmente sobre a superfície do solo, 50m antes da faixa, na faixa de aplicação e aos 12,5, 25, 50, 75, 100, 150, 200 e 300m após a faixa de aplicação, no sentido da direção de vento. Azoxistrobina foi quantificada por UHPLC-MS/MS. Os resultados indicam que a distância de deriva pode alcançar mais de 300 metros do local de aplicação, com concentrações variáveis de acordo com o equipamento utilizado. O sistema eletrostático reduz a deriva, quando comparado aos bicos hidráulicos com pontas cônicas.

The aim of this study was to investigate the efficiency of domestic physician-staffed helicopter emergency medical service (HEMS) for the transport of patients with severe trauma to a hospital. The study included patients with blunt trauma who were transported to our hospital by physician-staffed HEMS (Group P; n = 100) or nonphysician-staffed HEMS (Group NP; n = 80). Basic patient characteristics, transport time, treatment procedures, and medical treatment outcomes assessed using the Trauma and Injury Severity Score (TRISS) were compared between groups. We also assessed patients who were transported to the hospital within 3 h of injury in Groups P (Group P3; n = 50) and NP (Group NP3; n = 74). The severity of injury was higher, transport time was longer, and time from hospital arrival to operation room transfer was shorter for Group P than for Group NP (P < 0.001). Although Group P patients exhibited better medical treatment outcomes compared with Group NP, the difference was not statistically significant (P = 0.134 vs. 0.730). However, the difference in outcomes was statistically significant between Groups P3 and NP3 (P = 0.035 vs. 0.546). Under the current domestic trauma patient transport system in South Korea, physician-staffed HEMS are expected to increase the survival of patients with severe trauma. In particular, better treatment outcomes are expected if dedicated trauma resuscitation teams actively intervene in the medical treatment process from the transport stage and if patients are transported to a hospital to receive definitive care within 3 hours of injury.

BACKGROUND: Aircraft noise is a major environmental noise problem. This study was conducted in order to investigate the relationship between sleep disturbance and exposure to aircraft noise on the residents who are living near an airport. METHODS: There were 3308 residents (1403 in the high exposure group, 1428 in the low exposure group, and 477 in the non-exposure group) selected as the subjects for this study. The Insomnia severity Index (ISI) and Epworth Sleepiness Scale (ESS) questionnaires were used to evaluate sleep disturbance. RESULTS: The mean ISI and ESS scores were 6.9 ± 6.4 and 5.5 ± 3.7, respectively, and the average scores were significantly greater in the aircraft noise exposure group, as compared to the non-exposure group. The percentage of the abnormal subjects, which were classified according to the results of the ISI and ESS, was also significantly greater in the noise exposure group, as compared to the control group. The odd ratios for insomnia and daytime hypersomnia were approximately 3 times higher in the noise exposure group, as compared to the control group. CONCLUSIONS: The prevalence of insomnia and daytime hypersomnia was higher in the aircraft noise exposure group, as compared to the control group. Further study is deemed necessary in order to clarify the causal relationship.

OBJECTIVE: To determine the relationship between whole body vibration (WBV) induced helicopter flights and degenerative changes of the cervical and lumbar spine. METHODS: We examined 186 helicopter pilots who were exposed to WBV and 94 military clerical workers at a military hospital. Questionnaires and interviews were completed for 164 of the 186 pilots (response rate, 88.2%) and 88 of the 94 clerical workers (response rate, 93.6%). Radiographic examinations of the cervical and the lumbar spines were performed after obtaining informed consent in both groups. Degenerative changes of the cervical and lumbar spines were determined using four radiographs per subject, and diagnosed by two independent, blinded radiologists. RESULTS: There was no significant difference in general and work-related characteristics except for flight hours and frequency between helicopter pilots and clerical workers. Degenerative changes in the cervical spine were significantly more prevalent in the helicopter pilots compared with control group. In the cervical spine multivariate model, accumulated flight hours (per 100 hours) was associated with degenerative changes. And in the lumbar spine multivariate model, accumulated flight hours (per 100 hours) and age were associated with degenerative changes. CONCLUSION: Accumulated flight hours were associated with degenerative changes of the cervical and lumbar spines in helicopter pilots.

PURPOSE: The Ministry of Health and Welfare of korea started operating the Helicopter Emergency Medical Service (HEMS) System in 2011 to improve emergency medical services given by physicians aboard helicopters. The purpose of this study was to evaluate the HEMS education program for its effectiveness and satisfaction from trainees. METHODS: A survey of trainees was conducted at the very end of training and education. Trainees for a HEMS crew surveyed included doctors (8), nurses (4), emergency medical technicians (12), pilots (10), flight dispatchers (4) and an administrator (1). The study period was from August 8, 2011 to August 12, 2011. The amount of HEMS education was 16 hours, including 8 hours of theoretical education, 5 hours of simulation and 3 hours of table-top simulation. RESULTS: In an analysis of programs according to trainee satisfaction, the theoretical training program received an average of 4.2+/-0.7 points and; the practical and simulation training program received; 4.4+/-0.8 points. The simulated practical training program thus received a higher score than theoretical education, but the difference was insignificant (p=0.834). CONCLUSION: For the HEMS training programs, the practical and simulation training satisfaction score were higher than the theoretical program. A focus on simulation programs for HEMS could therefore serve as a goal of the HEMS education program.

Background: To investigate the association between aircraft noise and blood pressure. Methods: A nested case-control study was conducted on Indonesian Air Force pilots doing annual medical check-ups at the Saryanto Institute for Aviation and Aerospace Health (LAKESPRA) from 2003 – 2008. The data extracted from medical records were age, total flight hours, type of aircraft, fasting blood glucose and cholesterol levels, waist circumference, height and weight (Body Mass Index), and blood pressure. Results: There were 549 pilots, 49 were found to be hypertensive, with SBP ≥ 140 mmHg and/or DBP ≥ 90 mmHg. Helicopters pilots were at an almost 2 fold risk of hypertension compared to pilots of the fixed wing aircrafts. Pilots with more than 1400 hours of flight had more than 2 fold risk of being hypertensive compared to those with 1400 flight hours or less. Conclusion: The type of aircraft, which is related to the noise generated, may be a risk factor for developing hypertension in pilots. Increased total flight hours also increased the risk of hypertension.

OBJECTIVES: To determine the level of aircraft noise exposure and how it relates to hearing loss, and to investigate the association between noise exposure and anxiety and depression in subjects residing adjacent to a military airbase. METHODS: The study was conducted upon 898 inhabitants between the ages of 30-79, living near the military airbase in Jeonra-do. The subjects were divided into three noise-exposure groups: high-exposure, low-exposure, and a control group. The cut-values were 80 and 60 on the Weighted Equivalent Continuous Perceived Noise Level scale. A self-administered questionnaire including the Beck Anxiety Inventory (BAI) and the Beck Depression Inventory (BDI) were completed. Pure tone audiometry tests were performed. RESULTS: Hearing loss prevalence was defined by a >40 dB loss in bilateral ears or in one ear; the difference of or =22 was 317 (35.3%), with BDIs > or =21 was 347 (38.6%). The pure tone average, BAI, and BDI scores were higher in the noise-exposure groups compared to the control. The BAI/BDI abnormal subjects showed a higher hearing threshold shift level compared to the normal scored subjects. The odd ratios for anxiety was significantly high in both noise exposure groups and the hearing loss, for depression was significantly high in high-exposure group and hearing loss. CONCLUSIONS: Anxiety and depression were higher when exposure to high-level noise and further complicated by hearing loss. Further investigation is needed to determine the cause-effect relationship.

Aircraft accident investigation is divided between human and machine factors. Human factors include intoxication, cardiovascular pathology, CO Poisoning, hypoxia, vertigo, operational errors. Machine factors are focused on impact based on speed, direction of travel, angle of impact and altitude. Autopsy data from individual aviation crashes have long been used in aviation safety research. The present study examines the injury pattern in occupants of a light executive jet aircraft crashed in bad monsoon weather, while on a flight, killing all eight persons on board including the two pilots. The bodies of the victims were brought to All India Institute of Medical Sciences, New Delhi. All bodies were sufficiently intact and were easily identifiable. Detailed autopsies were performed on all the eight occupants, which yielded findings relevant to the cause of the accident. Our findings emphasize the need for autopsy on all aviation accident victims, especially pilots, as it is the only reliable method to find out the various factors contributing to an accident.

The Korea Centers for Disease Control and Prevention confirmed two patients, who had taken the same plane from Los Angeles to Seoul, with novel influenza A (H1N1). Through contact tracing, we concluded that the second patient was infected during the flight.

OBJECTIVES: This study was conducted to investigate the effects of aircraft noise exposure on children's mental health problems such as depression and personality. METHODS: We obtained data from the health examinations of school children in the vicinity of Gunsan airport. This survey was conducted in the rural area, Gunsan during 2009. The study population included 195 children attending 4th to 6th grade from 6 elementary schools near the airport. Based on 60 Weighted Equivalent Continuous Perceived Noise Level (WECPNL) measures of aircraft noise, participants were divided into 2 groups; the exposure group consisted of 106 children from 4 schools, and the reference group included 89 children from the 2 schools in Gunsan. All children completed questions on general characteristics, the Korean Children's Depression Inventory (KCDI) for depression, and the Korea Human Nature Inspection (KHNI) for personality. RESULTS: Children in the exposure group were at higher risk of depression (5.62, 95% CI 1.12~28.25), and personality changes (2.08, 95% CI 1.07~4.05) than those of the reference group. In particular, children's diligence, cooperation, and concentration, subgroups of personality measure, were affected significantly and negatively, by aircraft noise level. CONCLUSIONS: Our results suggest that exposure to aircraft noise at school is associated with mental health problems in children. Requirements should be implemented to manage aircraft noise of its affect on children's health.

RESUMEN

Aim: To analyze the effects of aircraft noise, resting pulse rate, and other factors on the risk of high diastolic blood pressure (DBP) in Indonesian Air Force pilots. Methods: A nested case-control study was conducted using data extracted from annual medical check-ups indoctrination aerophysiologic training records at the Saryanto Aviation and Aerospace Health Institute (LAKESPRA) in Jakarta from January 2003 – September 2008. For analysis of DBP: the case group with DBP ≥ 90 mmHg were compared with contral group with DBP < 79 mmHG. One case matched to 12 controls. Results: Out of 567 pilots, 544 (95.9%) had complete medical records. For this analysis there were 40 cases of high DBP and 480 controls for DBP. Pilots exposed to aircraft noise 90-95 dB rather than 70-80 dB had a 2.7-fold increase for high DBP [adjusted odds ratio (ORa) = 2.70; 95% confi dence interval (CI ) = 1.05-6.97]. Pilots with resting pulse rates of ≥ 81/minute rather than ≤ 80/minute had a 2.7-fold increase for high DBP (ORa = 2.66; 95% CI = 1.26-5.61). In terms of total fl ight hours, pilots who had 1401-11125 hours rather than 147-1400 hours had a 3.2-fold increase for high DBP (ORa = 3.18; 95% CI = 1.01-10.03). Conclusion: High interior aircraft noise, high total flight hours, and high resting pulse rate, increased risk for high DBP. Self assessment of resting pulse rate can be used to control the risk of high DBP.

RESUMEN

Objective: To investigate the relationship between aircraft noise and the results of audiological evaluation on tinnitus and hearing loss among residents near United States military air-bases in Pyeongtaek city. METHODS: Residents (n=492) exposed to aircraft noise were selected from eight villages near U.S. military air-bases (K-55 and K-6) in Pyeongtaek city. Residents (n=200) from five villages located at least 10 km away from the air-base were selected for the control group. All participants completed a questionnaire on tinnitus and audiological evaluations included pure tone audiometry (PTA) and distortion product otoacoustic emissions (DPOAE). Statistical analyses involved a general linear model and multiple logistic regression stratified by gender and data was adjusted by age, educational level, stress response index and noise type (i.e. military, agricultural and occupational). RESULTS: The equivalent noise levels (Leq) in the vicinity of the air-field were 73.4~81.5 dB (A). Tinnitus prevalence was 47.3% and odds ratio (OR) was 2.06 (95% confidence interval (CI): 1.09~3.88) among noise-exposed males. Tinnitus prevalence was 50.8% and OR was 1.97(95% CI: 1.17~3.30) among noise-exposed females. Tinnitus handicap scores among exposed group were significantly higher in functional, emotional, and catastrophic subclasses (p<0.001). PTA revealed hearing thresholds among the exposed group of 0.5 and 1 kHz (male right ear), 0.5 and 1 kHz (female left ear) and 0.5, 1, 2, 3, 4 and 6 kHz on female right ear that were significantly higher than those of the unexposed group. No significant difference in DPOAE was observed among noise-exposed males. However, significant differences were evident for noise-exposed females at 3175 and 4007 Hz. ORs of right female ears 3175 and 4007 Hz were 1.73(95% CI: 1.01~2.99) and 1.78(95% CI: 1.01~3.15). ORs of left female ears at 3175 Hz and 4007 Hz were 1.92(95% CI: 1.10~3.36) and 2.71(95% CI: 1.49~4.91) CONCLUSIONS: Aircraft noise may adversely affect hearing function and tinnitus.
