RESUMEN

SUMMARY: Frailty affects the functional autonomy (FA) of older adults and could manifest itself in muscle imbalances in the limbs, resulting in a disparity in size and strength between them. In Chile, information on the relationship between muscle strength (MS) levels and FA asymmetries in older women is limited. This study related the levels of MS, anthropometric parameters, and asymmetries of the lower and upper limbs, with the FA of a group of older Chilean women. The study included 39 women who participated, and their FA was evaluated using the GDLAM index (IG). Based on the score obtained in the IG, they were classified by percentiles as Group 1 with favorable FA (P ≤ 50) and Group 2 with low FA (P > 50). Anthropometric parameters were BMI, fat percentage, bone mass, circumferences (arm, thigh, calf), diameters (humerus, femur) and upper/lower limb strength was evaluated to determine asymmetries. The differences between the covariates of both groups were evaluated using the student's t test and the Mann-Whitney test for independent samples. G1 presented less asymmetry (p > 0.05) in the lower limbs and greater calf circumference than G2 (p < 0.05). G1 presented greater bilateral strength (dominant and non-dominant limb) compared to G2 (p < 0.05). The covariates of age, anthropometry, MS, and lower/upper limb asymmetries influence FA in older women.


La fragilidad afecta la autonomía funcional (AF) de las personas mayores y podría manifestarse en desequilibrios musculares en los miembros, dando lugar a una disparidad de tamaño y fuerza entre ellos. En Chile, la información que relaciona los niveles de fuerza muscular (FM) y las asimetrías con la AF en mujeres mayores es limitada. Este estudio relacionó los niveles de FM, parámetros antropométricos y asimetrías de los miembros inferiores y superiores, con la AF de un grupo de mujeres mayores chilenas. Participaron 39 mujeres, cuya AF se evaluó mediante el índice GDLAM (IG). En función de la puntuación obtenida en el IG, se clasificaron por percentiles en Grupo 1 con AF favorable (P ≤ 50) y Grupo 2 con AF baja (P > 50). Los parámetros antropométricos fueron IMC, porcentaje de grasa, masa ósea, circunferencias (brazo, muslo, pantorrilla), diámetros (húmero, fémur) y se evaluó la fuerza de los miembros superiores/ inferiores para determinar asimetrías. Las diferencias entre las covariables de ambos grupos se evaluaron mediante la prueba t de student y la prueba de Mann-Whitney para muestras independientes. G1 presentó menor asimetría (p > 0,05) en los miembros inferiores y mayor perímetro de pantorrilla que G2 (p < 0,05). G1 presentó mayor fuerza bilateral (miembro dominante y no dominante) en comparación con G2 (p < 0,05). Las covariables de antropometría, FM y asimetrías de extremidades inferiores/superiores influyen en la AF en mujeres mayores.

RESUMEN

Revisando el segundo volumen de actas del Primer Congreso Latinoamericano de Medicina, reunido en Santiago los nueve primeros días del siglo XX, encontramos una conferencia de Eloísa Díaz, primera mujer chilena y latinoamericana en graduarse de médico, pese a la discriminación sufrida, que la obligó incluso a asistir a clases acompañada por su madre. Se analiza tanto esta conferencia sobre la organización del servicio médico escolar, como la introducción a su tesis de grado, y de paso las presentaciones de otros médicos participantes, haciendo hincapié en la patología infecciosa discutida y en su lenguaje verboso y florido.


Analyzing the second volume of proceedings of the First Latin American Congress of Medicine held in Santiago on the first nine days of the twentieth century, we found a lecture by Eloísa Díaz, the first Chilean and Latin American woman to graduate as a doctor, despite the discrimination she suffered, which even forced her to attend classes accompanied by her mother. This conference on the school medical service is analyzed, as well as the introduction to her degree thesis, and incidentally the presentations of other participating doctors, emphasizing the infectious pathology discussed and its verbose and flowery language.

RESUMEN

This paper collected and sorted out the cross-cultural medical practice in the top 20 US hospitals in 2019-2020 through web surveys, and summarized the concepts, featured projects and models of cross-cultural medical services in American hospitals. Generally, hospitals in the US advocate the service concept of "patient-centered", provide full-process services before, during and after the treatment, involving appointments, accommodation, transportation, language translation, medical process management, etc., and attach great importance to the role of culture factors in medical treatment and cross-cultural medical education and training for employees. On this basis, it is proposed that Chinese medical institutions should focus on the following aspects in the development of foreign-related medical services: conform to international medical service concepts and standards, improve the cross-cultural medical competence of medical staff and other groups, explore cross-cultural medical service models with localized characteristics, and build the international image of the hospital with the construction of international environment.

Objective@#The American Heart Association released the Life s Essential 8 (LE 8) for the overall evaluation of cardiovascular health (CVH) on individual level. The present study aimed to describe the overall CVH in Chinese school aged children using LE 8 metrics.@*Methods@#Data of the present analysis came from a national representative multicentered cross sectional study conducted in 7 provinces of China in 2013. The original study used a multistage cluster sampling method. A total of 10 326 children aged 5 to 19 years with complete data of health behaviors and health outcomes were included in the study. Children s health behavior indicators included diet, physical activity, nicotine exposure and sleep health. Health outcome factors included body mass index, fast blood glucose, lipid profile and blood pressure.@*Results@#The median CVH score was 73.3 ( IQR =14.4) in boys and 73.4 ( IQR = 13.5) in girls. Compared to children aged ≤9 years, the health behavior scores were lowest in the 13-15 age group, with boys scoring 7.73 lower (95% CI =-8.35--7.12, P <0.01) and girls scoring 9.15 (95% CI =-9.83--8.48, P <0.01) lower. The ≥16 age group had the lowest health outcome scores, with boys scoring 7.85 (95% CI =-9.07--6.63, P <0.01) lower and girls scoring 6.11 (95% CI =-7.12--5.09, P <0.01) lower.@*Conclusions@#Chinese school aged children are generally at a moderate level of cardiovascular health. Specific LE 8 components vary substantially between groups and therefore require targeted intervention strategies.

RESUMEN

In November 2023, the American Heart Association and the American Academy of Pediatrics jointly released key updates to the neonatal resuscitation guidelines based on new clinical evidence. This update serves as an important supplement to the "Neonatal resuscitation: 2020 American Heart Association guidelines for cardiopulmonary resuscitation and emergency cardiovascular care". The aim of this paper is to outline the key updates and provide guidance on umbilical cord management and the selection of positive pressure ventilation equipment and its additional interfaces in neonatal resuscitation.

ABSTRACT Background: Chagas disease, a zoonosis transmitted mainly by hematophagous insects of the subfamily Triatominae, is caused by Trypanosoma cruzi, classified into six discrete typing units (DTUs: TcI-TcVI and Tcbat). Methods: Insect vectors were collected from 84 human dwellings in the municipality of Santo Domingo Tehuantepec, Oaxaca, Mexico; 4.76% were infested. DTUs were determined using conventional and nested PCR. Results: The infection rate was 43.6%. All insects were infected with TcI while one specimen showed mixed infection with TcII. Conclusions: This is the first report of T. cruzi mixed infection in Triatoma phyllosoma, its main vector in the study region.

Resumen Este texto discute sobre la psicología social latinoamericana -originariamente conectada con tradiciones críticas de acción social (educación popular, ciencias sociales militantes, filosofía de la liberación)- y la recepción en dicho espacio de paradigmas emergentes vinculados al giro decolonial, las epistemologías del Sur y otras tendencias autonomistas y subalternistas para pensar los procesos de transformación social. Se revisa como dichos movimientos estarían desordenando y revitalizando una disciplina que, en su expresión hegemónica, se encontraba adormecida y cooptada por la institucionalización burocrática y la producción teórica neoliberal para leer e intervenir en el campo social. Para ello, se sitúa el desarrollo histórico de la disciplina en el clima teórico de tres momentos relevantes en la configuración del campo social. Se concluye con algunas observaciones sobre las posibilidades que asoman para pensar lo social-comunitario a la luz de estas epistemes emergentes, pero también algunos problemas/limitaciones a tener en consideración.


Resumo Discute-se a psicologia social comunitária latino-americana - originalmente ligada às tradições críticas da ação social no continente (educação popular, ciências sociais militantes, filosofia da libertação) - e sua recepção nesse espaço de paradigmas emergentes vinculados à virada decolonial, às epistemologias do Sul e suas tendências autonomistas e subalternistas para pensar os processos de transformação social. Revisa como esses movimentos estariam desordenando e revitalizando uma disciplina que, em sua expressão hegemônica, foi cooptada pela institucionalização burocrática e pela produção teórica neoliberal para ler e intervir no campo social. Pelo exposto, o desenvolvimento histórico da disciplina situa-se no clima teórico de três momentos relevantes na configuração do campo social. Conclui com algumas observações sobre as possibilidades que surgem para pensar o social-comunidade à luz dessas epistemes emergentes, mas também sobre problemas/limitações a serem considerados.


Abstract We discuss Latin American Community-Social Psychology - originally connected with critical traditions of social action (popular education, militant social sciences, philosophy of liberation) - and its reception of emerging paradigms linked to the decolonial turn, the epistemologies of the South, and its autonomist and subalternist tendencies to think about the processes of social transformation. We review how these movements would disorder and revitalize a discipline that, in its hegemonic expression, laid dormant and co-opted by bureaucratic institutionalization and neoliberal theoretical production to read and intervene in the social field. For this, we place the historical development of the discipline in the theoretical climate of three relevant moments in the configuration of the social field. We conclude with some observations on the possibilities that seem to think about the social-community in light of these emerging epistemes but also some problems/limitations worthy of consideration.


Resumé Nous discutons de la psychologie sociale communautaire latino-américaine - à l'origine liée aux traditions critiques de l'action sociale (éducation populaire, sciences sociales militantes, philosophie de la libération) - et sa réception dans ledit espace de paradigmes émergents liés au tournant décolonial, aux épistémologies du Sud et ses tendances autonomistes et subalternistes à penser les processus de transformation sociale. Il examine comment ces mouvements perturberaient et revitaliseraient une discipline qui, dans son expression hégémonique, était cooptée par l'institutionnalisation bureaucratique et la production théorique néolibérale pour lire et intervenir dans le champ social. Considérant le précédent, le développement historique de la discipline se situe dans le climat théorique de trois moments pertinents dans la configuration du champ social. Nous concluons par quelques observations sur les possibilités qui apparaissent de penser la social-communauté à la lumière de ces épistémès émergentes, mais aussi sur quelques problèmes/limites à prendre en considération.

ABSTRACT The Pan American Health Organization (PAHO) and its Member States have been leading the efforts to eradicate wild poliovirus in the Region of Americas since smallpox's successful elimination in 1971. The region became the first to be certified free of wild poliovirus in 1994. However, in July 2022, an unvaccinated patient with no recent travel history was diagnosed with poliomyelitis in the United States of America. In response to the emergence of a circulating vaccine-derived poliovirus in the United States, PAHO established the Polio Incident Management Support Team. This team has been coordinating response efforts, focusing on: coordination, planning, and monitoring; risk communication and community engagement; surveillance and case investigation; vaccination; and rapid response. In this paper, we identified and documented best practices observed following establishment of the Incident Management Support Team (September 2022-2023) through a comprehensive review and analysis of various data sources and country-specific data from the polio surveillance dashboard. The aim was to share these best practices, highlighting technical support and implementation of polio measures by Member States. Despite several challenges, the Americas region remains polio-free. Polio risk is declining, with a July 2023 assessment showing fewer countries at medium, high, and very high risk. This progress reflects improved immunization coverage, surveillance, containment, health determinants, and outbreak preparedness and response. The PAHO Polio Incident Management Support Team has played a key role in supporting these efforts.


RESUMEN La Organización Panamericana de la Salud (OPS) y sus Estados Miembros han liderado los esfuerzos para erradicar el poliovirus salvaje en la Región de las Américas desde la eliminación exitosa de la viruela en 1971. En 1994, la Región fue la primera en obtener la certificación de libre del poliovirus salvaje. Sin embargo, en julio del 2022, se diagnosticó poliomielitis a un paciente de Estados Unidos de América no vacunado y sin antecedentes de viajes recientes. Para responder a la aparición de un poliovirus circulante derivado de la vacuna en ese país, la OPS creó el equipo de apoyo a la gestión de incidentes de poliomielitis. Este equipo ha asumido la coordinación de los esfuerzos de respuesta y se ha centrado en la coordinación, la planificación y seguimiento; la comunicación de riesgos y la participación de la comunidad; la vigilancia e investigación de casos; la vacunación; y la respuesta rápida. En este artículo, se determinan y documentan las mejores prácticas observadas después de la creación del equipo de apoyo a la gestión de incidentes (septiembre del 2022-2023) mediante una revisión y un análisis pormenorizados de datos procedentes de diversas fuentes y de datos específicos de los países del panel de vigilancia de la poliomielitis. El objetivo fue poner en común estas mejores prácticas y resaltar el apoyo técnico y la aplicación de medidas contra la poliomielitis por parte de los Estados Miembros. A pesar de los diversos desafíos, la Región de las Américas se mantiene libre de poliomielitis. El riesgo de esta enfermedad es cada vez menor, y la evaluación de julio del 2023 muestra una disminución del número de países con un riesgo medio, alto o muy alto. Este progreso refleja la mejora de la cobertura de inmunización, la vigilancia, la contención, los determinantes de la salud y la preparación y respuesta ante brotes. El equipo de apoyo a la gestión de incidentes relacionados con la poliomielitis de la OPS ha desempeñado un papel fundamental para brindar apoyo a estas iniciativas.


RESUMO Desde a eliminação bem-sucedida da varíola em 1971, a Organização Pan-Americana da Saúde (OPAS) e seus Estados Membros têm estado à frente de iniciativas para erradicar o poliovírus selvagem na Região das Américas. Em 1994, a região foi a primeira do mundo a ser certificada como livre do poliovírus selvagem. Entretanto, em julho de 2022, um paciente não vacinado e sem histórico de viagens recentes foi diagnosticado com poliomielite nos Estados Unidos da América. Em resposta ao surgimento de um poliovírus derivado de vacina circulante nos Estados Unidos, a OPAS criou a Equipe de Apoio à Gestão de Incidentes de Poliomielite. A equipe vem administrando os esforços de resposta, concentrando-se em: coordenação, planejamento e monitoramento; comunicação de risco e envolvimento da comunidade; vigilância e investigação de casos; vacinação; e resposta rápida. Neste documento, identificamos e documentamos as melhores práticas observadas após a criação da Equipe de Apoio à Gestão de Incidentes (setembro de 2022 a 2023) por meio de uma revisão e análise abrangentes de diversas fontes de dados e dados específicos de cada país fornecidos por meio do painel de vigilância da poliomielite. O objetivo foi compartilhar essas melhores práticas, destacando o apoio técnico e a implementação de medidas contra a poliomielite pelos Estados Membros. Apesar de vários desafios, a Região das Américas continua livre da poliomielite. Um levantamento de julho de 2023 demonstrou que o risco da poliomielite vem diminuindo, com menos países com risco médio, alto ou muito alto. Essa evolução é resultado de melhoras na cobertura vacinal, vigilância, contenção, preparação, determinantes de saúde e resposta a surtos. A Equipe de Apoio à Gestão de Incidentes de Poliomielite da OPAS foi fundamental para apoiar esses esforços.

Abstract In Brazil, current information about breastfeeding indicators among indigenous living in the urban areas is lacking. This article describes the duration of exclusive breastfeeding and its associations with mother and child characteristics in a cohort of Terena infants. The study enrolled infants born between June 2017 to July 2018 (n = 42) and living in villages of the urban area of Campo Grande, Mato Grosso do Sul State, Brazil. Information was collected in four time-points. Variables on maternal sociodemographics and on maternal and child health characteristics were collected, respectively, during the antenatal and the first-month interviews. Variables on breastfeeding practices and bottle use were collected during the first-, six- and 12-month interviews. Associations were examined using Wilcoxon, Kruskal-Wallis, Pearson's chi-square, and Fisher's exact tests. The prevalence of exclusive breastfeeding duration to the ages of three and six months were, respectively, 50% and 11.9%. Compared to infants never introduced to bottles during the first three months of life, those bottle-fed had lower median duration of exclusive breastfeeding (15 versus 150 days) and lower prevalence of exclusive breastfeeding duration to the age of three months (22.7% versus 80%). Most Terena infants fell short of meeting the international recommended duration of exclusive breastfeeding until six months of age and suggested the negative impact of bottle use in the duration of exclusive breastfeeding.


Resumo No Brasil, não há informações atuais sobre indicadores de aleitamento materno entre indígenas residentes em áreas urbanas. O objetivo deste estudo foi descrever a duração do aleitamento materno exclusivo e suas associações com características maternas e infantis em uma coorte de lactentes Terena. O estudo incluiu crianças nascidas de junho de 2017 a julho de 2018 (n = 42) e residentes em povoados da zona urbana de Campo Grande, Mato Grosso do Sul, Brasil. As informações foram coletadas em quatro momentos. Variáveis sociodemográficas maternas e características de saúde materno-infantil foram coletadas durante o pré-natal e o primeiro mês de entrevistas, respectivamente. As variáveis sobre práticas de amamentação e uso de mamadeira foram coletadas durante as entrevistas realizadas no primeiro mês, seis meses e 12 meses. As associações foram examinadas pelos testes de Wilcoxon, Kruskal-Wallis, qui-quadrado de Pearson e exato de Fisher. As prevalências de duração do aleitamento materno exclusivo até os três e seis meses de idade foram, respectivamente, de 50% e 11,9%. Em relação aos bebês que nunca foram introduzidos à mamadeira durante os três primeiros meses de vida, aqueles que usaram mamadeira tiveram menor duração média de amamentação exclusiva (15 versus 150 dias) e menor prevalência de duração de amamentação exclusiva até os três meses de idade (22,7% versus 80%). A maioria dos lactentes Terena não atingiu a duração recomendada internacionalmente para o aleitamento materno exclusivo até os seis meses, sugerindo um impacto negativo do uso da mamadeira na duração do aleitamento materno exclusivo.


Resumen En Brasil no existe información actual sobre los indicadores de lactancia materna entre los indígenas que viven en áreas urbanas. El objetivo de este estudio fue describir la duración de la lactancia materna exclusiva y sus asociaciones con las características maternas e infantiles en una cohorte de lactantes Terena. Este estudio incluyó a niños nacidos entre junio de 2017 y julio de 2018 (n = 42) y que vivían en aldeas del área urbana de Campo Grande, Mato Grosso do Sul, Brasil. La información se recopiló en cuatro momentos. Las variables sociodemográficas maternas y las características de salud materno-infantil durante la atención prenatal y el primer mes de entrevistas, respectivamente, se recogieron para este estudio. Las variables sobre prácticas de lactancia materna y alimentación con biberón fueron recolectadas de las entrevistas realizadas en el primer mes, seis meses y 12 meses. Las asociaciones pasaron por las pruebas de Wilcoxon, de Kruskal-Wallis, el chi-cuadrado de Pearson y la prueba exacta de Fisher. La prevalencia de duración de la lactancia materna exclusiva fue del 50% hasta los tres meses de edad y del 11,9% hasta los seis meses. En comparación con los bebés que no utilizaron biberón durante los primeros tres meses de vida, los que usaron biberón tuvieron una duración promedio más corta de lactancia materna exclusiva (15 versus 150 días) y una menor prevalencia de lactancia materna exclusiva hasta los tres meses de edad (22,7% versus 80%). La mayoría de los bebés Terena no alcanzaron la duración recomendada internacionalmente para la lactancia materna exclusiva hasta los seis meses, lo que sugiere un impacto negativo de la alimentación con biberón en la duración de la lactancia materna exclusiva.

Introduction. The legal declarations on functional foods of the four main economic integration organizations in Latin America and the Caribbean (LAC) ­ Pacific Alliance (PA), Caribbean Community (CARICOM), Southern Common Market (MERCOSUR) and Central American Integration System (SICA) - are based on the Codex Alimentarius system and do not regulate functional foods. The use of Codex in food marketing is an insufficient condition for its application in functional foods. Regulation based on scientific and technology results are required to be used in the economic integration organizations of LAC. Objective. The objective is to analyze the theoretical framework of the legal foundations that could govern the commercialization processes of functional foods, whose research advances have currently only been manifested in nutritional health. This article also seeks to address this gap through a systematic analysis of international regulations. Materials and methods. For this, a review of the literature emanating from two databases from 2018-2023 is carried by applying the legal-economic research method of documentary content analysis, applied to three general food marketing regulations: food safety declarations, regulations for inspections, food manufacturing and food labeling. Results. The results reveal the absence of specific legislation for functional foods in LAC economic integration organizations. Conclusions. The legal principle of marketing based on peremptory norm (also called jus cogens) can be facilitated through side letters, included in the contents of international contracts. along with the registration requirements of industrial property rights of the member countries associations(AU)


Introducción. Las declaraciones legales sobre alimentos funcionales de las cuatro principales organizaciones de integración económica de América Latina y el Caribe (ALC) ­ Alianza del Pacífico (AP), Comunidad del Caribe (CARICOM), Mercado Común del Sur (MERCOSUR) y Sistema de Integración Centroamericana (SICA) - se basan en el sistema del Codex Alimentarius y no regulan los alimentos funcionales. El uso del Codex en la comercialización de alimentos es condición insuficiente para su aplicación en alimentos funcionales. Se requiere que las regulaciones basadas en resultados científicos y tecnológicos sean utilizadas en los organismos de integración económica de ALC. Objetivo. Analizar el marco teórico de los fundamentos legales que podrían regir los procesos de comercialización de alimentos funcionales, cuyos avances en investigación actualmente solo se han manifestado en salud nutricional. Este artículo también busca abordar esta brecha a través de un análisis sistemático de las regulaciones internacionales. Materiales y métodos. Se realiza una revisión de la literatura emanada de dos bases de datos del período 2018-2023 aplicando el método de investigación jurídico- económica de análisis de contenido documental, aplicado a tres normas generales de comercialización de alimentos: declaraciones de seguridad alimentaria, normas para inspecciones, fabricación de alimentos y etiquetado de alimentos. Resultados. Los resultados revelan la ausencia de legislación específica para alimentos funcionales en las asociaciones comerciales de ALC. Conclusiones. El principio jurídico de comercialización basado en norma imperativa (también llamado jus cogens) puede facilitarse a través de cartas complementarias, incluidas en el contenido de los contratos internacionales. junto con los requisitos de registro de los derechos de propiedad industrial de las asociaciones de los países miembros(AU)

Purpose: To evaluate the accuracy of intraocular lens (IOL) power prediction of the formulas available on the American Society of Cataract and Refractive Surgery (ASCRS) post?refractive calculator in eyes with prior radial keratotomy (RK) for myopia. Methods: This retrospective study included 25 eyes of 18 patients whose status was post?RK for treatment of myopia, which had undergone cataract extraction with IOL implantation. Prediction error was calculated as the difference between implanted IOL power and predicted power by various formulae available on ASCRS post?refractive calculator. The formulas compared were Humphrey Atlas method, IOLMaster/Lenstar method, Barrett True?K no?history formula, ASCRS Average power, and ASCRS Maximum power on ASCRS post?refractive calculator. Results: Median absolute errors were the least for Barrett True?K and ASCRS Maximum power, that is, 0.56 (0.25, 1.04) and 0.56 (0.25, 1.06) D, respectively, and that of Atlas method was 1.60 (0.85, 2.28) D. Median arithmetic errors were positive for Atlas, Barrett True?K, ASCRS Average (0.86 [?0.17, 1.61], 0.14 [?0.22 to 0.54], and 0.23 [?0.054, 0.76] D, respectively) and negative for IOLMaster/Lenstar method and ASCRS Maximum power (?0.02 [?0.46 to 0.38] and ? 0.48 [?1.06 to ? 0.22] D, respectively). Multiple comparison analysis of Friedman抯 test revealed that Atlas formula was significantly different from IOLMaster/ Lenstar, Barrett True?K, and ASCRS Maximum power; ASCRS Maximum power was significantly different from all others (P < 0.00001). Conclusion: In post?RK eyes, Barrett True?K no?history formula and ASCRS Maximum power given by the ASCRS calculator were more accurate than other available formulas, with ASCRS Maximum leading to more myopic outcomes when compared to others

Aqui, fornecemos uma análise epidemiológica da leishmaniose em Pernambuco, Brasil, de 2015 a 2019. Nossos resultados revelaram 1.705 casos de leishmaniose tegumentar americana-LTA e 859 casos de leishmaniose visceral-LV. LV e LTA afetam crianças de 1 a 4 anos (23% e 3,2% dos casos, respectivamente) e adultos de 20 a 39 anos (26% e 28%, respectivamente), principalmente mulheres. O maior percentual (54,6%) de casos de LV foi encontrado na área urbana, enquanto LTA ocorreu principalmente na área rural (63,2%). Indivíduos com maior escolaridade foram menos acometidos pela leishmaniose e dados de prognóstico revelaram boas chances de cura clínica, mas óbitos ainda são registrados. Esses dados mostram que ações preventivas de saúde devem ser desenvolvidas para impedir a disseminação da leishmaniose, reduzir seu impacto na saúde pública e restaurar a qualidade de vida da população. Este estudo pode ser útil na elaboração de estratégias para minimizar os impactos da leishmaniose.


Here, we provide an epidemiological analysis of leishmaniasis in Pernambuco, Brazil, from 2015 to 2019. Our results revealed 1,705 of American cutaneous leishmaniasis-ACL and 859 cases of visceral leishmaniasis-VL. VL and ACL affect children aged 1­4 years (23% and 3.2% of all cases, respectively) and adults aged 20­39 years (26% and 28%, respectively), mainly women. The highest percentage (54.6%) of VL cases was found in urban areas, while ACL occurred mainly in rural areas (63.2%). Individuals with a higher level of education were less affected by leishmaniasis and data on prognosis revealed good chances of clinical cure, but deaths are still recorded. These data show that preventive health actions must be developed to hinder leishmaniasis spread, reduce its impact on public health, and restore the population's life quality. This study can be useful in further elaboration of strategies to minimize leishmaniasis impacts.

Purpose: Global ophthalmology educational activities often include the ability of ophthalmology residents to partake in low?resource clinical care at home and abroad. Low?resource surgical techniques have become a pillar of education during formalized global ophthalmology fellowships. A formal manual small?incision cataract surgery (MSICS) curriculum was started in the University of Colorado’s residency training program to meet the growing demand for this surgical skill and to allow for more sustainable outreach work from our graduates. The survey was conducted to collect evaluations on the value of formal MSICS training within a United States–based residency program. Methods: This was a survey study in a US ophthalmology residency program. A formal MSICS curriculum was created that included didactic lectures on epidemiology of global blindness, MSICS technique, and how MISCS compared to phacoemulsification in terms of cost and sustainability in low?resource settings, followed by a formal wet lab experience. Residents were then exposed to MSICS procedures in the operating room (OR) under supervision of an experienced MSICS surgeon. An anonymous online survey was conducted on three consecutive cohorts of recently graduated senior ophthalmology residents from 2019 to 2021 with the aim of eliciting opinions about and outcomes from the new curriculum. Results: Fifteen graduating senior residents comprised the three cohorts with a 100% survey response rate. All residents agreed or strongly agreed that “MSICS is a valuable skill to have”. Eighty percent of respondents agreed or strongly agreed that “exposure to MSICS has increased my likelihood of doing any type of outreach work in the future” and 86.67% agreed or strongly agreed that “exposure to MSICS increased my understanding about sustainable outreach work”. The average number of cases assisted or performed per resident was 8.2 (SD 2.7, range 4–12). Conclusion: A formal MSICS curriculum for US?based ophthalmology residents was well?received by the trainees. The majority felt it increased their likelihood of pursuing and improved their understanding of sustainable outreach work. The curriculum, which included lectures, wet lab training, and formal teaching in the OR, could add value to a residency program’s curriculum. Furthermore, a formal domestic program can avoid ethical pitfalls that can be seen with resident teaching during international mission work.

Este artículo tiene como objetivo examinar la valoración que se les da a las revistas científicas peruanas según los reglamentos actuales relacionados con ciencia y tecnología, lo que lleva a que existan clasificaciones de revistas y que las personas autoras se interesen en publicar en función del puntaje para el desarrollo de la carrera académica y de investigación. Metodológicamente se revisaron los reglamentos vigentes de dicha clasificación y calificación de investigadores, así como las características que debe tener una revista para incluirse en los mismos, para luego examinar el portal de ISSN y comparar la lista de revistas científicas registradas de manera que pueda observarse qué porcentaje de esas revistas se consideran para las clasificaciones vigentes. Se concluye que existe un alto porcentaje de revistas peruanas relegadas o fuera del sistema de clasificación, lo que hace que el proceso de crecimiento se vuelva complicado, haciendo el camino de la indexación más tormentoso para los equipos editoriales. Es necesario trabajar por el desarrollo de ciencia nueva y eso involucra dar la oportunidad a estas revistas para seguir avanzando en los procesos de gestión editorial.


This article aims to examine the valuation given to Peruvian scientific journals according to current regulations related to science and technology, which leads to the existence of journal classifications and that authors are interested in publishing based on the score for academic and research career development. Methodologically, the current regulations of said classification and qualification of researchers will be reviewed, as well as the characteristics that a journal must have to be included in them, to later examine the ISSN portal and compare the list of registered scientific journals so that it can be observed which percentage of those journals are considered for current rankings. It is concluded that there is a high percentage of peruvian journals relegated or outside the classification system, which makes the growth process complicated, making the path of indexing harder for editorial teams. It is necessary to work for the development of new science and that involves giving these journals the opportunity to continue advancing in editorial management processes.

This article is presented in the Keynote Lectures section because it is the text of a lecture given at the IV International Conference on Medical Education in the 21st Century during the IV International Health Convention, Havana, October 2022. It has been kept as an oral text. The topic on the Latin-American university and the social right to health is addressed, considering the relevance of health in the social and economic development of the population, as well as taking into account the importance of training human resources committed to health as a social right, a public good, and a responsibility of the State. This allows to analyze critically the public health space and the training of human resources in the twentieth century and the beginning of the twenty-first century, in order to contribute to the demand for a new strategic dimension that responds, in a committed manner, to the health needs and priorities of our populations(AU)

Background: Diabetes Mellitus refers to a group of common metabolic disorders that share the phenotype of Hyperglycemia. It is the leading cause of morbidity and mortality throughout the world with an estimated worldwide prevalence of 439 million by 2030 and 19% of world抯 DM patients are Indians. Magnesium is an important co-factor for various enzymes involved in Insulin secretion and is involved in sodium-potassium ATPase pump. 25%-38% of Type 2 DM patients had Hypomagnesemia, which has also contributed in developing microvascular complications such as Diabetic Retinopathy (DR) and Diabetic Nephropathy (DN). Various studies have suggested that Magnesium supplementation in Type 2 DM patients with Hypomagnesemia have shown beneficial effects on insulin sensitivity and glucose metabolism. Aim and objectives: To study the prevalence of Hypomagnesemia in Type 2 DM patients and to study the association of Hypomagnesemia with microvascular complications such as DR and DN. Materials and methods: It is a hospital based Observational study carried out in 2022 for a period of 1 year including 60 patients fulfilling the ADA criteria for diagnosing T2DM and patients with Diabetic Retinopathy and Diabetic Nephropathy, and excluding patients with Malabsorption, Chronic diarrhoea, Renal Failure on diuretic therapy, Sepsis, Pancreatitis. Serum Magnesium levels of 1.6 mg/dl � 2.6 mg/dl is considered as normal range. Serum Magnesium were measured using Xylidyl blue colorimetric method. Results: The Mean age of the patients in our study was 55.89 years. Among 60 patients diagnosed with Diabetes Mellitus, 42 patients had Hypomagnesemia, 18 patients had Normomagnesemia (p- value: <0.0001). Patients with an HbA1c levels > 7% had Hypomagnesemia when to compared to patients with HbA1c <7% with a significant p value of 0.009. Hypomagnesemia was also associated with Diabetic Retinopathy and Diabetic Nephropathy with a significant p-value of 0.013 and 0.009 respectively. Conclusion: In our study, it has shown that patients with uncontrolled T2DM had Hypomagnesemia, which is also associated with micro-vascular complications of T2DM such as DR and DN.

En Cuba, el primer departamento de rayos X se inauguró en el año 1907 por Francisco Domínguez Roldán. Poco se divulga un contacto previo durante el siglo XIX, en la guerra hispano-norteamericana. Este artículo describe el uso de los rayos X en la guerra hispano-cubano-norteamericana y su importancia en la historia de la Radiología. Para esta contienda, la parte norteamericana preparó sus barcos hospitales. El ejército habilitó el Relief y el Missouri, mientras que la marina lo hizo con Solace. También se convirtieron en buques hospitales el Olivette y el Bay State. Todos contaban con equipos de rayos X. El capitán Borden publicó un libro en 1900 "El uso de los rayos Roentgen por el departamento médico de la armada estadounidense en la guerra con España (1898)" que marcó un antes y un después en el uso de los rayos X. Este fue un hecho trascendental para la historia de la Radiología y favoreció un avance considerable en la sanidad militar y civil.


In Cuba, the first X-ray department was set up in 1907 by Francisco Domínguez Roldán. Little is reported about a previous contact during the 19th century in the Spanish-Cuban-American war. This article describes the use of X-rays in the Spanish-Cuban-American war and its importance in the history of Radiology. For this war the American side prepared its hospital ships. The Army fitted out the Relief and the Missouri, while the Navy did it with the Solace. The Olivette and the Bay State also became hospital ships. All were equipped with X-ray equipment. Captain Borden published a book in 1900 "The Use of Roentgen Rays by the Medical Department of the U.S. Navy in the War with Spain (1898)" which marked a before and after in the use of X-rays. This was a transcendental fact for the history of Radiology and favored a considerable advance in military and civilian health.

La inseguridad alimentaria (INSA) es la condición que resulta de un acceso incierto o nulo a alimentos nutricionalmente adecuados socialmente aceptables y/o insuficiente ingestión de alimentos. Las causas de la INSA son múltiples y difieren en cada país, entre ellas están inadecuado acceso, y disponibilidad a los alimentos nutricionalmente adecuados y a los capitales de desarrollo. Según los resultados de la Encuesta Nacional de Condiciones de Vida del 2014, la INSA afecta a más de la mitad de la población guatemalteca con repercusiones en su calidad de vida y desarrollo. El objetivo de la investigación fue determinar el nivel de INSA en hogares de los 29 municipios pertenecientes a 12 departamentos de cobertura del EPS de la Universidad de San Carlos de Guatemala. Se realizó un estudio de tipo cuantitativo, descriptivo, transversal, utilizando la metodología de recolección, validación y análisis de información establecida en el Manual de la Escala Latinoamericana y Caribeña de Seguridad Alimentaria (ELCSA). La muestra fue de 2,750 hogares, obtenida por medio de un muestreo probabilístico para poblaciones finitas. Los datos analizados fueron desagregados según sexo y edad de jefe de hogar, lugar de residencia, número de miembros en el hogar y presencia de menores de 18 años. La investigación mostró que, al momento de la entrevista, 2,035 (74%) [72.3, 75.7] hogares fueron clasificados en situación de INSA. Se presentó un mayor porcentaje de INSA en los hogares del área rural (85% [82.5, 87.4]) y en los hogares con niños menores de 18 años (80% [78.0, 81.5]). También se observó que, a mayor número de miembros del hogar, mayor porcentaje de INSA. Con los resultados obtenidos, se pretende motivar la implementación de estrategias que coadyuven a mejorar la situación de INSA en la población.


Food insecurity (FI) is the condition that results from uncertain or no access to socially acceptable, nutritionally adequate foods and/or insufficient food intake. The causes of FI are multiple and differ in each country, among them are inadequate access and availability to adequate nutritional foods and development capital. According to the results of the 2014 National Survey of Living Conditions, FI affects more than half of the Guatemalan population.The objective of the research was to determine the level of FI at household level of the 29 municipalities belonging to 12 departments covered by the Nutrition Supervised Professional Practice of the University of San Carlos of Guatemala, through a quantitative, descriptive, cross-sectional study, using the collection, validation and analysis of information established in the Manual of the Latin American and Caribbean Food Security Scale. The sample was 2,750 households, obtained by means of a probabilistic sampling for finite populations. The data analyzed were disaggregated according to the sex and age of the head of the household, place of residence, number of members in the household, and presence of children under 18 years of age. The investigation showed that, at the time of the interview, 2,035 (74%) [72.3, 75.7] households were classified as FI. There was a higher percentage of FI in households in rural areas (85% [82.5, 87.4]) and in households with children under 18 years of age (80% [78.0, 81.5]). It is intended that the results of this study contribute to motivate the implementation of strategies that help to improve the situation of FI in the evaluated population.

This study evaluated the dental prosthesis needs and associated factors in indigenous Brazilian adults from Northeastern Brazil. Dental prosthesis needs were assessed in a representative sample of 225 Indians. Edentulous area and current indication for tooth extraction according to periodontal status or non-restorable condition were registered in a clinical chart. Bivariate and logistic analyses were applied to assess associations of dental prosthesis needs and prosthesis type with age, sex, income, education, smoking, moderate or severe periodontitis, plaque index, dental caries experience, and previous dental visit. Prosthetic treatment was necessary in 83% of the participants. These individuals required 339 dental prostheses, including removable partial dentures (60.2%), fixed (33.6%), and complete dentures (6.2%). Regression analysis showed that age ≥35 years [odds ratio (OR) 5.95, 95% confidence interval (CI) 1.97­17.91, p=0.002] and caries experience ≥3 [OR 4.01, 95%CI 1.78­9.02, p=0.001] were significantly associated with prosthesis needs. The type of prosthesis required was associated with sex, age, educational level, periodontitis, caries experience, and plaque index (p≤0.03). In conclusion, the need for prosthetic rehabilitation was high in this population, and sociodemographic factors, dental plaque level, caries experience, and periodontitis were associated with the required type of prosthesis. These data can be used to plan the allocation of resources to promote preventive and curative strategies in this population.(AU)


Este estudo avaliou as necessidades de prótese dentária e fatores associados em adultos indígenas brasileiros do Nordeste do Brasil. As necessidades de prótese dentária foram avaliadas em uma amostra representativa de 225 índios. A área edêntula e a indicação para extração dental, de acordo com o estado periodontal ou condição não restaurável, foram registradas em um prontuário clínico. Análises bivariadas e logísticas foram aplicadas para avaliar as associações das necessidades de prótese dentária e tipo de prótese com idade, sexo, renda, escolaridade, tabagismo, periodontite moderada ou grave, índice de placa, experiência de cárie dentária e consulta odontológica anterior. O tratamento protético foi necessário em 83% dos participantes. Esses indivíduos necessitaram de 339 próteses dentárias, incluindo próteses parciais removíveis (60,2%), fixas (33,6%) e próteses totais (6,2%). A análise de regressão mostrou que idade ≥35 anos [odds ratio (OR) 5,95, intervalo de confiança de 95% (IC) 1,97­17,91, p=0,002] e experiência de cárie ≥3 [OR 4,01, IC 95% 1,78­9,02, p=0,001] foram significativamente associados às necessidades de prótese. O tipo de prótese necessária foi associado ao sexo, idade, escolaridade, periodontite, experiência de cárie e índice de placa (p≤0,02). Em conclusão, a necessidade de reabilitação protética foi alta nesta população e fatores sociodemográficos, nível de placa dentária, experiência de cárie e periodontite foram associados ao tipo de prótese necessária. Estes dados podem ser usados para planejar a alocação de recursos para promover estratégias preventivas e curativas nessa população.(AU)

ObjectiveTo explore the application of Brain and Spinal Injury Center (BASIC) score in evaluation of traumatic cervical spinal cord injury. MethodsFrom January, 2015 to December, 2021, 175 patients with traumatic cervical spinal cord injury in Beijing Bo'ai Hospital were analyzed. Gender, age, cause of injury, injury mechanism and American Spinal Injury Association Impairment Scale (AIS) grade were collected. The sagittal and axial T2 weighted imaging (T2WI) of the patients were evaluated with BASIC score, single/multi-segment injury, and with/without intramedullary hemorrhage. According to the injury mechanism, the patients were divided into two groups: with fracture/fracture dislocation (n = 92) and without fracture and dislocation (n = 83). The baseline demographic indicators and T2WI evaluation indicators were compared between the two groups, and the relationship between AIS grade and BASIC score, intramedullary hemorrhage, single/multi-segment injury were investigated. ResultsThere were significant differences in gender, age and AIS grade, BASIC score, and the rates of inntramedullary hemorrhage and single segment injury of T2WI between two groups (t = -10.276, χ2 > 8.703, P < 0.01); however, no difference was found in the cause of injury (P > 0.05). The AIS grade was significantly correlated with the BASIC score (r = 0.790, P < 0.001). There was significant difference in AIS grade between intramedullary hemorrhage or not, and single/multi-segment injury (χ2 > 5.516, P < 0.05). ConclusionThe BASIC score of T2WI is a predictor of the severity of spinal cord injury after traumatic cervical spinal cord injury, and is different with the injury mechanisms.
