Objective@#To analyze and evaluate the clinical application effect of bioceramic material mineral trioxide aggregate and iRoot BP plus on the formation of apical barrier in adult teeth with incomplete apical foramen.@*Methods@#A total of 200 permanent teeth with apical periodontitis whose apical foramen were not closed were randomly divided into two groups, and the apical barrier was prepared with MTA and iRoot BP plus, respectively. The operation time of MTA and iRoot BP plus and effects of different types of apical foramen destruction, lesion range of apical area, the filling of apical area and patients' age on the treatment success rate was analyzed and compared. @*Results@#The operation time of preparing apical barrier in iRoot BP plus group was shorter than that in MTA group, and the difference was statistically significant (P<0.05). However, different types of apical foramen destruction, lesion scope of apical area, filling of apical area and patients' age had no effect on the treatment success rate of the two groups (P>0.05). @*Conclusion@#The clinical effects of MTA and iRoot BP plus in the treatment of apical periodontitis of permanent teeth with unclosed apical foramen are similar, but the operation performance of iRoot BP plus is better.

Foraminal enlargement has been recommended to optimize the disinfection of infected root canals, although some authors still claim that the foramen should be kept in its original shape and position. This study aimed to evaluate morphological alterations of apical foramen after foraminal enlargement through a systematic review. An electronic search was conducted until April 2022. Ex vivo studies evaluating influence of foraminal enlargement in the morphologic changes of apical foramen were included. Studies without a control group or available full text were excluded. Foraminal deformation and area increase were considered as primary outcomes. Risk-of-bias assessment was performed according to a modified Joanna Briggs Institute's Checklist. From 702 studies retrieved, five were eligible. Most studies used single-rooted teeth, and rotary systems for instrumentation ranging from ­ 2 mm to + 1 mm to the apex. All studies found increased major foramen deformation after foraminal enlargement. Among four studies that evaluated foraminal area, all found increased area after foraminal enlargement. Insufficient data for touched/untouched walls by instruments and dentinal microcrack formation was observed. A low risk of bias was found. Foraminal enlargement during root canal preparation seems to increase deformation and major apical foramen area. Future investigations with standardized methodologies are encouraged (AU)


A ampliação foraminal tem sido recomendada para otimizar a desinfecção de canais radiculares infectados, embora alguns autores ainda afirmem que o forame deve ser mantido em sua forma e posição originais. Este estudo teve como objetivo avaliar alterações morfológicas do forame apical após ampliação foraminal por meio de uma revisão sistemática. Uma busca eletrônica foi realizada até abril de 2022. Foram incluídos estudos ex vivo que avaliaram a influência da ampliação foraminal nas alterações morfológicas do forame apical. Foram excluídos estudos sem grupo controle ou com texto completo indisponível. A deformação foraminal e o aumento da área foram considerados desfechos primários. A avaliação do risco de viés foi realizada de acordo com uma lista de verificação modificada do Instituto Joanna Briggs. Dos 702 registros recuperados, cinco foram elegíveis. A maioria dos estudos utilizou dentes unirradiculares e sistemas rotatórios para instrumentação, com comprimento de trabalho variando de ­ 2 mm a + 1 mm até o ápice. Todos os estudos encontraram aumento da deformação do forame maior após ampliação foraminal. Dos quatro estudos que avaliaram a área foraminal, todos encontraram aumento de área após alargamento foraminal. Foram observados dados insuficientes para paredes tocadas/intocadas pelos instrumentos e formação de microfissuras dentinárias. Um baixo risco de viés foi encontrado. A ampliação foraminal durante o preparo do canal radicular parece aumentar a deformação e a área do forame apical. Futuras investigações com metodologias padronizadas são incentivadas (AU)

Introdução: A infecção endodôntica pode alcançar a saída foraminal e, inclusive, ir além dela. Logo, a determinação da constrição apical como o limite ideal para instrumentação e obturação tem sido questionada. A instrumentação foraminal intencional é realizada com o intuito de diminuir o contingente microbiano a níveis mais favoráveis ao reparo. Entretanto, repercussões locais e sistêmicas estão associadas à sua execução. Objetivo: Realizar uma revisão crítica da literatura sobre repercussões locais e sistêmicas relativas à instrumentação foraminal intencional. Métodos: Em março de 2018, uma busca eletrônica realizada na base de dados PUBMED utilizando os termos "foraminal enlargement" OR "foraminal widening" OR "apical limit" AND "endodontics" identificou 74 artigos científicos. Esses artigos, a análise de suas referências bibliográficas e a utilização de mais 5 artigos base resultaram nos 111 estudos consultados para a realização dessa pesquisa. Resultados: A ampliação foraminal intencional nem sempre pode ser praticada em virtude de razões anatômicas e morfológicas. Quanto maior a ampliação do forame apical, maior a possibilidade de extravasamento de substâncias e/ou materiais utilizados para a realização do tratamento endodôntico. A instrumentação foraminal intencional parece ser contraindicada em pacientes que fazem ou fizeram uso de bisfosfonatos recentemente, com distúrbios de coagulação e/ou sob uso crônico de anticoagulantes e com alto risco de bacteremia. Conclusões: Os impactos da instrumentação foraminal intencional sobre o sucesso do tratamento endodôntico devem ser investigados. Contudo, os delineamentos metodológicos dos estudos clínicos devem ser cuidadosos, principalmente no tocante às condições sistêmicas dos pacientes que farão parte do universo amostral (AU).


Introduction: endodontic infection can reach and even go beyond the apical foramen. Therefore, determining apical constriction as the ideal limit for instrumentation and obturation has been questioned. Intentional foraminal enlargement is performed for the purpose of reducing the microbial contingent to levels more favorable to repair. However, local and systemic repercussions have been associated with this approach. Objectives: to critically review the literature on local and systemic repercussions relative to intentional foraminal enlargement. Methods: in March 2018, an electronic search performed on the PUBMED database using the terms "foraminal enlargement" OR "foraminal widening" OR "apical limit" AND "endodontics" identified 74 scientific articles. These articles, analysis of their references and use of another 5 base articles resulted in the 115 studies used for performing this research. Results: intentional foraminal enlargement cannot always be performed due to the anatomical and morphological conditions. The greater the apical foramen enlargement, the greater the possibility of extrusion of substances and/or materials used to perform endodontic treatment. Intentional foraminal enlargement seems to be contraindicated in patients who are taking or have recently used bisphosphonates, those with coagulation disorders and/ or under chronic use of anticoagulants and at high risk for bacteremia. Conclusions: the impacts of intentional foraminal enlargement on the success of endodontic treatment should be investigated. However, the methodological procedures of clinical studies should be carefully designed, especially taking into consideration the systemic conditions of patients who will be part of the sample (AU).

The aim of this study was to evaluate apical transportation and apical root canal sealing after root canal filling in human teeth prepared with MTwo® Rotary System with and without apical foramen enlargement. Twenty mandibular premolars were divided into two groups (n=10). Group 1 had root canals prepared 1mm beyond the apical foramen. Group 2 had root canals prepared 1mm below the root canal length. After chemo-mechanical preparation, samples were submitted to scanning electronic microscopy. Apical foramen images had 75x magnification at standardized positions, allowing measurements from the apical foramen area before and after root canal preparation, and after root canal filling. Apical foramen shape and apical transportation, as well as its level of circumferential filling after root canal preparation were accessed using the Image Subtraction System. Scanning electronic microscopy analysis demonstrated that samples of Group 1 showed larger foraminal diameter than samples of Group 2 (p<0.05). Apical foramen transportation was statistically different between Groups 1 and 2 (p=0.0108). Furthermore, the apical foramen sealing also differed statistically between groups 1 and 2 (p=0.0007) and 100% of samples of Group 1 showed apical root canal sealing. Apical root canal sealing was more effective when the root canal was prepared with apical foramen enlargement, even when the apical transportation was detected.

Revitalization procedures have been extensively studied during the last decade and offers several advantages over root canal treatment, such as the recovery of the natural immune system. Mature teeth have a small apical foramen diameter (AFD), which could impair the ingrowth of tissue into the root canal. We analysed three methods for apical foramen enlargement by instrumentation in in situ human teeth and evaluated the damage over hard tissues produced by the techniques. Tooth length (TL), defined as the length from the most coronal part of the crown to the point at which the file abandons the root canal, was calculated. Forty-four in situ teeth were randomized: Group I: instrumentation 0.5 mm coronal to TL; Group II: at TL level; Group III: 0.5 mm beyond TL. Teeth were instrumented up to K-file #80. The mandibles were scanned in a micro-CT device before and after treatment. Group I: Only 20 % of teeth presented an enlarged AFD, with augmentation of 0.09 mm. No damage to hard tissues was observed. Group II: 71.4 % of the teeth presented an enlarged AFD with augmentation of 0.42 mm. 35.7 % presented damage to periapical tissues. Group III: 86.7 % presented an enlarged AFD with augmentation of 0.43 mm. 46.7 % presented damage to periapical tissues. All groups presented similar prevalence of teeth with dentine thickness less than 1mm. All mandibular incisors presented areas of thickness less than 1mm. Instrumentation 0.5 mm beyond TL is the most effective technique.


La revitalización de dientes necróticos ha sido ampliamente estudiada durante la última década y ofrece varias ventajas sobre la endodoncia convencional, tal como la recuperación del sistema inmunitario natural del diente. Los dientes maduros tienen un diámetro de foramen apical (FA) pequeño, lo que podría afectar el crecimiento de tejido en el conducto. Se evaluaron tres métodos para la ampliación del foramen apical en dientes humanos in situ y se evaluó el daño sobre los tejidos duros producidos por las técnicas. Mediante radiografía periapical se calculó la longitud del diente (TL), definida como la longitud desde la parte más coronal de la corona hasta el punto en que la lima abandonó el conducto radicular. En el estudio fueron aleatorizados 44 dientes in situ: Grupo I: instrumentación 0,5 mm coronal a TL; Grupo II: a nivel TL; Grupo III: 0,5 mm más allá de TL. Los dientes fueron instrumentados hasta la lima K #80. Las mandíbulas se escanearon en un dispositivo de microCT antes y después del tratamiento. Grupo I: solo el 20 % de los dientes presentaron un diámetro de FA ensanchado, con un aumento de 0,09 mm. No se observó daño a los tejidos duros. Grupo II: el 71,4 % de los dientes presentaban un FA ensanchado con un aumento de 0,42 mm. El 35,7 % presentó daño a los tejidos periapicales. Grupo III: el 86,7 % presentó un FA ensanchado con un aumento de 0,43 mm. El 46,7 % presentó daño a los tejidos periapicales. Todos los grupos presentaron una prevalencia similar de dientes con un espesor de dentina inferior a 1mm. Todos los incisivos mandibulares presentaban áreas de grosor inferior a 1mm. La instrumentación 0,5 mm más allá de TL es la técnica más efectiva, aunque se debe tener especial consideración en aspectos como el debilitamiento de la estructura dentaria.

The determination of working length and its maintenance during cleaning and shaping procedures is a key factor for successful endodontic treatment. The aim of this in- vitro study was to evaluate the utility and accuracy of Raypex5 electronic apex locator in determining the working length in comparison to conventional radiography and radiovisiography in posterior teeth. Methods: In this study sixty extracted mandibular first and second molars were used. Diagnostic images were obtained both by conventional radiography and radiovisiography (RVG) and tentative working length was obtained and recorded. After access opening actual root canal length was determined by both conventional radiography and RVG and recorded. Four blinded observers determined the working length in all samples with Raypex5 electronic apex locator. Then at that length file position was checked using RVG. Results: All the data collected was statistically analyzed using one way analysis of variance (ANOVA), Cronbach’s alpha reability analysis and student paired t-test. Conclusion: In this study it was found that Electronic apexlocator Raypex 92 %, RVG 74% and conventional radiograph was 70% accurate in measuring root canal length in posterior teeth.

Objetivo: Evaluar radiográfica y macroscópicamente la desembocadura del foramen apical en caninos superiores. Materiales y métodos: Se seleccionaron 300 caninos superiores recientemente extraídos, siguiendo las normas del Comité de Ética de la Universidad Maimónides. Los dientes fueron conservados en una solución fisiológica y glicerina (ana/ana) hasta el momento de su utilización. Se tomaron radiografías preoperatorias en sentido ortorradial y próximo-proximal. Las aperturas fueron realizadas con piedra redonda de diamante #3, y la preparación de los tercios coronario y medio, con fresas de Gates Glidden #1, 2 y 3. Luego se introdujo una lima K #15 hasta que se constató visualmente su salida por el foramen apical mediante una lupa X6 (Ballon). Después se procedió a la toma radiográfica en sentido vestíbulo-palatino y próximo-proximal. Los criterios de valoración radiográfica fueron: incidencia radiográfica ortorradial: mesial (M), distal (D) y coincidencia (C); incidencia radiográfica próximo-proximal: vestibular (V), palatino (P) y coincidencia (C). Los criterios de valoración macroscópica fueron: vestibulodistal (DV), distopalatino (DP), mesiovestibular (MV), mesiopalatino (MP), mesial (M), palatino (P), vestibular (V), distal (D) y coincidencia (C). La desembocadura de los forámenes fue evaluada macroscópicamente con una lupa estereoscópica (ZTX-F2, Lancet) a 30X. Los datos de las evaluaciones fueron registrados en una planilla de Excel (Microsoft). Para efectuar el análisis estadístico, se empleó la prueba de frecuencia relativa en porcentaje, con intervalo de confianza de 95%. Resultados: Se encontraron diferencias estadísticas (P<0,0001) entre la incidencia radiográfica bucolingual y la vista macroscópica. No se encontraron diferencias estadísticas (P>0,5) entre la incidencia radiográfica mesiodistal y la vista macroscópica. Se encontraron diferencias estadísticas al comparar todas las vistas macroscópicas con todas las coincidencias radiográficas (P<0,0001). Conclusiones: En las condiciones en que se realizó este estudio, la terminación más frecuente del foramen apical en una incidencia radiográfica ortorradial fue hacia distal (43%), y en una vista próximo-proximal, hacia vestibular (47%). Macroscópicamente, la desembocadura más frecuente del foramen apical fue hacia distovestivular (27,7%), seguida de una coincidencia con el ápice anatómico (25,3%) (AU)


Aim: To evaluate the localization of the opening of the apical foramen of maxillary canines. Materials and methods: 300 recently extracted maxillary canines were selected and stored in a/a saline solution/glicerine. The teeth were radiographed in two directions: mesiodistal and buccolingual directions. The access cavity was prepared with a diamond round bur 3, and Gates Glidden burs 1, 2 and 3 were used to shape the coronal and middle third of the root canal. A K-file 15 was introduced in the root canal until its tip was seen at the opening of the apical foramen. Then, buccolingual and mesiodistal radiographs were taken. The evaluation followed these radiographic criteria: Buccolingual direction: mesial (M), distal (D), and coincidence (C). Mesiodistal direction: buccal (V), lingual (P), and coincidence (C). The macroscopic criteria: distobuccal (DV), distolingual (DP), mesiobuccal (MV), mesiolingual (MP), mesial (M), lingual (P), buccal (V), distal (D) and coincidence (C). The openings of the apical foramen were macroscopically evaluated using magnification (30X). Data were statistically analyzed using the relative frequency tests and 95% confidence intervals. Results: Statistical differences were found (P<0.0001) ortobetween buccolingual incidence and macroscopic view. No statistical differences were found (P>0.05) between mesiodistal and macroscopic view. Statistical differences were found comparing all macroscopic views with all radiographic coincidences (P<0.0001). Conclusions: The most frequent localization of the opening of the apical foramen was distal in a buccolingual radiographic direction (43%), and buccal in a mesiodistal view (47%). Macroscopically, the most frequent localization of the opening of the apical foramen was distobuccal (27.7%), followed by coincidence with the anatomic apex (25.3%) (AU)

The purpose of this study was to analyze the ratio and rate of apical closure when inducing root growth of necrotic immature permanent teeth using alternative triple antibiotics. 24 permanent teeth in the treatment group and 27 premolars in the control group were retrospectively studied using periapical radiographs for more than 300 days after the first visit. The difference in the growth rate between the two groups was statistically compared using the Mann-Whitney test at a significance level of 0.05. There were no statistically significant differences between the two groups in the first month and during months 1 – 3, 3 – 6, and 6 – 12. After 12 months, the cumulative rate of decrease in the apical foramen width in the treatment group was 50.59% and that in the control group was 71.82%, which revealed a significant difference between the two groups. There were significant differences in the rates of decrease in the apical foramen width after 3, 6 months, and later period in the treatment group, respectively. The cumulative rate of increase in the root dentin area presented no statistically significant differences between the treatment group and control group during the entire period of examination.

Objetivo: Instrumentos endodônticos de movimentos reciprocante estão entre as últimas inovações para o preparo do sistema de canais radiculares. O objetivo desse estudo piloto foi comparar o desvio apical promovido por sistemas reciprocantes e rotatório em canais simulados. Material e método: Quarenta e cinco canais simulados foram distribuídos aleatoriamente em três grupos experimentais (n=15), de acordo com o sistema utilizado para realização do preparo: Grupo PT - sistema ProTaper; Grupo RP - sistema Reciproc e Grupo WO - sistema WaveOne. O desvio apical foi avaliado por meio da análise de imagens obtidas pré e pós-instrumentação de cada canal simulado, com auxílio do programa ImageJ. Resultados: Todos os grupos apresentaram algum nível de desvio apical. O grupo PT apresentou maior desvio aos 2 mm, com diferença estatisticamente significante em relação aos demais grupos (p<0,05). Quando comparada a média dos 3 mm apicais, somente houve diferença significativa entre os grupos PT e RP (p<0,05). Conclusão: Nenhum dos sistemas testados, reciprocante ou rotatório, foi capaz de manter a posição original do forame apical após o preparo do canal simulado.


Objective: Endodontic instruments of reciprocating motion are among the latest innovations for root canal system preparation. The aim of this pilot study was to compare the apical deviation promoted by reciprocating and rotary systems in simulated canals. Methods: Forty-five simulated canals were randomly distributed into three experimental groups (n=15), according to the system used for preparation: PT group - ProTaper system; RP group - Reciproc and WO group - WaveOne system. The apical deviation was assessed by analysis of pre- and post-instrumentation images of each simulated canal, with the aid of the ImageJ software. Results: All groups presented some level of apical deviation. The PT group presented greater deviation at 2 mm, with statistically significant difference in comparison with the other groups (p<0.05). When compared the average of the final apical 3 mm, there was significant difference only between PT and RP groups (p<0.05). Conclusion: None of the tested systems, reciprocating or rotary, were able to maintain the original position of the apical foramen after preparation of the simulated canal.

El primer premolar maxilar es considerado un diente con alta variación en su anatomía radicular y morfología de canales radiculares (Chaparro et al., 1999). Por esto, toma gran relevancia el conocimiento de su anatomía al realizar tratamientos endodónticos. El objetivo de éste estudio fue describir frecuencia de número y disposición de forámenes apicales en la región apical radicular en primeros premolares maxilares. Se utilizó microscopia óptica para estudiar en detalle la anatomía radicular apical de 141 raíces de primeros premolares maxilares unirradiculares (n=51) y birradiculares (n=49) de nacionalidad chilena; en donde se observó número de forámenes apicales mayores y accesorios (foraminas), ubicación del foramen mayor, y distancia de éste al ápice anatómico. En dientes unirradiculares se encontró una mayor frecuencia de uno o dos forámenes mayores, y un promedio de 2,5 foraminas accesorias por diente analizado. En dientes birradiculares se encontró una mayor frecuencia de un foramen mayor por raíz, y un promedio de 1,3 foraminas por raíz. Además, se encontró que la ubicación del foramen mayor fue más frecuente en las superficies mesial-distal-palatino en dientes unirradiculares, no encontrando diferencias significativas entre éstas superficies. En cambio en dientes birradiculares se observó que el foramen apical mayor en todas las superficies radiculares sin haber diferencias significativas entre ellas. La distancia promedio entre el foramen mayor y el ápice anatómico fue de 0,819 mm en uni y birradiculares. En conclusión los primeros premolares maxilares representativos de población chilena, presentan una anatomía apical variable y compleja, en cuanto al número de forámenes mayores, número de foraminas, y su ubicación.


The first maxillary premolar tooth has a high variation in its anatomy and morphology. Therefore, knowledge of its anatomy is important at the time of endodontic treatment. The purpose of this study was to describe frequency, number and arrangement of apical foramen in external root apical region in maxillary first premolar. We studied under microscopy (4x) of 141 root of first maxillary premolar single-rooted teeth (51) and double-rooted teeth (49) in Chilean population; where the number of major apical foramen and accessories (foraminas), major foramen location and distance from it to the anatomical apex was observed. In single-rooted teeth the emergence of one or two major foramen was found, averaging 2.5 foramina accessory. In double-rooted teeth the emergence of one foramen was observed with an average of 1.3 foraminas by root. It was established that there were no significant differences in the location of the major foramen between the mesial-distal-palatal surfaces in single-rooted teeth. In double-rooted teeth the major apical foramen was observed in all root surfaces without significant differences between them. The average distance between the major foramen and the anatomic apex foramen was 0.819 mm. In conclusion, the first premolars of Chilean population, have a changeable and complex apical anatomy, in relation to the number of foramina, the number of foramen and its location.

Aim: The present study evaluated the influence of the working length and foraminal enlargement on the sealing ability and anatomy of the apical region of the root. Materials and Methods: Fifty‑five roots were divided into three groups: G1, instrumentation 1 mm short of the major foramen; G2, instrumentation in the limit of the apical foramen; and G3, instrumentation 1 mm beyond the foramen. All groups were prepared using nickel‑titanium rotary files and obturated with AH Plus and Gutta‑percha. Photomicrographs were taken using a scanning electron microscopy (SEM) before instrumentation, after instrumentation with each file, and after root canal filling. Moreover, bacterial microleakage with Enterococcus faecalis was performed. Results: The results were analyzed using Mann–Whitney, Friedman, Kruskal–Wallis and Kaplan– Meier tests at a significance level of 5%. The cemental canal was uninstrumented in G1. No statistical differences regarding foramen deviation was observed when compared G2 and G3 (P > 0.05). SEM analysis showed that G2 and G3 resulted in good apical foramen obturation. Microleakage showed no statistically significant differences between all of the groups tested. Conclusions: Under the conditions of this study, it can be concluded that foramen enlargement resulted in more apical deviation; however, no differences in bacterial microleakage was observed among the experimental groups.

Resumen El conocimiento de ia anatomía radicular de cada uno de los grupos de dientes, es un aspecto importante que contribuye juntos con otros aspectos clínicos, a un mejor resultado de los procedimientos de micro-cirugía endodóntica (MCE). Variaciones anatómicas entre individuos de una misma población, suscitan para el endodoncista, una evaluación diagnóstica detallada de cada caso en particular para facilitar el abordaje quirúrgico. El objetivo de esta revisión es realizar una descripción de la anatomía radicular y del sistema de conductos radiculares de los diferentes grupos de dientes y sus variaciones anatómicas según la literatura científica disponible a la fecha, que permitirá al clínico tener una visión más amplia y detallada, que podrá aplicar para la planeación y ejecución de los procedimientos de MCE.


Knowing the root anatomy of each groups of teeth, it is an important aspect that contributes with other clinical aspects, to a better outcome of the procedures in endodontic micro-surgery(MCE). Anatomical variations between individuals of the same population, arise in the endodontist, a detailed diagnostic evaluation of each particular case, which will facilitate the surgical approach. The objective of this review is to describe the anatomy and root canal system of different groups of teeth and anatomical changes according to available scientific literature to date, allowing the clinician to have a more comprehensive and detailed vision, you can apply for the planning and execution of procedures for MCE.

The aim of this study was to compare in vivo the accuracy of two electronic foramen locators (EFLs) based on different operation systems - Root ZX and Propex II. Ten healthy adult patients needing premolar extractions due to orthodontic reasons participated in the study, providing a sample of 17 noncarious, non-restored, vital teeth (n= 24 canals). After coronal access preparation and cervical preflaring and prior to tooth extraction, the root canal length was measured alternating the two EFLs. All measurements were performed with K-files well fitted to the canal diameter at the level that each EFL indicated the apical foramen in their display (APEX or 0.0). The last K-file were fixed in place with cyanoacrylate, the tooth was extracted, and the apical 4 mm of each root were resected to measure the distance between the file tip and the apical foramen. The mean errors based on the absolute values of discrepancies were 0.30 ± 0.29 mm (Root ZX) and 0.32 ± 0.27 mm (Propex II). Analysis by the Wilcoxon test for paired samples showed no statistically significant differences between the electronic canal measurements performed with the EFLs (p=0.587). The apical foramen was accurately located in 75% (Root ZX) and 66.7% (Propex II) of the cases, considering a ±0.5 mm error margin, with no statistically significant difference by the chi-square test. Despite having different measurement mechanisms, both EFLs were capable of locating the apical foramen with high accuracy in vivo. Under the tested clinical conditions, Root ZX and Propex II displayed similar results.


O presente estudo teve como objetivo avaliar, in vivo, a precisão de dois localizadores eletrônicos foraminais (LEFs) baseados em diferentes mecanismos de funcionamento, Root ZX e Propex II, na determinação do forame apical, sendo utilizados nos mesmos dentes. Após o acesso coronário e o pré-alargamento, e anteriormente à exodontia, os comprimentos de 24 canais radiculares foram determinados eletronicamente alternando-se os dois LEFs. As odontometrias foram realizadas até que os dispositivos apontassem o FA (APEX), utilizando-se limas tipo-K ajustadas. O último instrumento utilizado foi fixado em posição, o dente extraído e os 4,0 mm apicais de cada canal desgastados de forma a possibilitar a determinação da distância entre a ponta dos instrumentos e o forame apical. Os erros médios em função dos valores absolutos das discrepâncias foram, respectivamente, 0,30 ± 0,29 mm (Root ZX) e 0,32 ± 0,27 mm (Propex II). A análise estatística realizada por meio do teste de Wilcoxon para amostras pareadas demonstrou a semelhança entre as determinações do forame apical realizadas pelos dois LEFs (p=0,587). O comprimento radicular até o forame apical foi corretamente determinado em 75% (Root ZX) e 66,7% (Propex II) dos casos, considerando margem de ± 0,5 mm, sem diferenças estatísticas quando analisados pelo teste qui-quadrado. Os LEFs avaliados e consequentemente seus mecanismos de funcionamento, foram capazes de determinar o comprimento dos canais radiculares com precisão em condições in vivo. Nas condições do presente estudo, Root ZX e Propex II apresentaram resultados semelhantes.

OBJECTIVES: The aim of this study was to investigate the relationship between the apical foramen morphology and the length of merged canal at the apex in type II root canal system. MATERIALS AND METHODS: This study included intact extracted maxillary and mandibular human premolars (n = 20) with fully formed roots without any visible signs of external resorption. The root segments were obtained by removing the crown 1 mm beneath the cementum-enamel junction (CEJ) using a rotary diamond disk. The distance between the file tip and merged point of joining two canals was defined as Lj. The roots were carefully sectioned at 1 mm from the apex by a slow-speed water-cooled diamond saw. All cross sections were examined under the microscope at x50 magnification and photographed to estimate the shape of the apical foramen. The longest and the shortest diameter of apical foramen was measured using ImageJ program (1.44p, National Institutes of Health). Correlation coefficient was calculated to identify the link between Lj and the apical foramen shape by Pearson's correlation. RESULTS: The average value of Lj was 3.74 mm. The average of proportion (P), estimated by dividing the longest diameter into the shortest diameter of the apical foramen, was 3.64. This study showed a significant negative correlation between P and Lj (p < 0.05). CONCLUSIONS: As Lj gets longer, the apical foramen becomes more ovally shaped. Likewise, as it gets shorter, the apical foramen becomes more flat shaped.

Since instrumentation of the apical foramen has been suggested for cleaning and disinfection of the cemental canal, selection of the file size and position of the apical foramen have challenging steps. This study analyzed the influence of apical foramen lateral opening and file size can exert on cemental canal instrumentation. Thirty-four human maxillary central incisors were divided in two groups: Group 1 (n=17), without flaring, and Group 2 (n=17), with flaring with LA Axxess burs. K-files of increasing diameters were progressively inserted into the canal until binding at the apical foramen was achieved and tips were visible and bonded with ethyl cyanoacrylate adhesive. Roots/files set were cross-sectioned 5 mm from the apex. Apices were examined by scanning electron microscopy at ×140 and digital images were captured. Data were analyzed statistically by Student’s t test and Fisher’s exact test at 5% significance level. SEM micrographs showed that 19 (56%) apical foramina emerged laterally to the root apex, whereas 15 (44%) coincided with it. Significantly more difficulty to reach the apical foramen was noted in Group 2. Results suggest that the larger the foraminal file size, the more difficult the apical foramen instrumentation may be in laterally emerged cemental canals.


Tendo em vista que a instrumentação do forame apical tem sido sugerida para a limpeza e desinfecção do canal cementário, a seleção do calibre do instrumento e a posição do forame apical representam passos desafiantes. Este estudo analisou a influência que a saída lateral do forame apical e o calibre do instrumento podem exercer na instrumentação do canal cementário. Trinta e quatro incisivos centrais superiores foram divididos em dois grupos: Grupo 1 (n=17), sem preparo cervical, e Grupo 2 (n=17), com preparo cervical com brocas LA Axxess. Limas K com aumentos de diâmetro foram progressivamente inseridas no canal até ajustar no forame apical e as pontas ficarem visíveis e foram fixadas com adesivo de etil cianoacrilato. Os conjuntos raízes/limas foram seccionados transversalmente a 5 mm aquém do ápice. Os ápices foram examinados por microscopia eletrônica de varredura com aumento de 140x e imagens digitais foram capturadas. Os dados foram examinados estatisticamente pelo teste t de Student e teste exato de Fischer com nível de significância de 5%. A microscopia eletrônica de varredura mostrou que 19 (56%) dos forames apicais saíram lateralmente em relação ao ápice radicular, enquanto que 15 (44%) coincidiram com ele. Dificuldade significantemente maior para chegar ao forame apical foi observada no Grupo 2. Os resultados sugerem que quanto mais calibroso for o instrumento foraminal, mais difícil poderá ser a instrumentação do forame apical em canais cementários que apresentam saída lateral.

Introduction: There are few researches in literature that mention the use of the apex locator in deciduous teeth and working length is obtained through radiographies. Objective: The purpose of this research was to compare the radiographic and the electronic method to obtain the working length in deciduous molars. Material and methods: Twelve molar teeth were used. The specimens in the visual method had their root length measured through the passive insertion of a 10 K-file with a silicone stop within root canal until its tip was seen at the apical foramen. The working length was measured through radiographs or using the apex locator Root ZX II. The mean between the examiners was submitted to the variance analysis (ANOVA). Results: Statistically significant differences were found between the visual method and the radiographic method (p < 0.001). There was no significant difference between the working length measurements in visual method and those obtained with the apex locator (p = 0.1319). Conclusion: The apex locator is indicated as a clinical implementation for endodontic treatment in primary teeth.

Considering that instrumentation of the apical foramen has been suggested for root canal infection control, this study analyzed the relationship between the files that bind at the apical foramen and the foraminal openings in 50 maxillary central incisors. After preparation of the pulp chamber, access to the canal was obtained with #1 and 2 LA Axxess and K-files with tip cut were inserted up to the apical foramen until binding was felt. The files were fixed with methyl cyanoacrylate and the tooth-file sets were cross-sectioned 10 mm short of the apex. Scanning electron microscopic analysis was carried out and files and foraminal areas were measured using Image Tool software. Statistically significant difference (p<0.0001) was found between files and the apical foraminal areas. The mean foraminal area was 3.8 times larger than the mean file area. The results of this study suggest that it would require 4 files of greater size beyond the one that bound to the foramen in order to allow a better relationship between files and apical openings of maxillary central incisors.


Uma vez que a instrumentação do forame apical tem sido sugerida para o controle de infecção do canal radicular, este estudo analisou a relação entre as limas que se ajustam no forame apical e a abertura foraminal em cinquenta incisivos centrais superiores. Após o preparo da câmara pulpar, foi feito o acesso radicular com as brocas LA Axxess #1 e 2 e limas K com a ponta cortada foram inseridas até que oferecessem a sensação tátil de ajuste no forame apical. Foram fixadas com cianoacrilato de metila e o conjunto dente-lima foi seccionado a 10 mm aquém do ápice. Foi feita a microscopia eletrônica de varredura e as áreas das limas e dos forames foram medidas por meio do Image Tool software. A análise estatística demonstrou diferença significante entre as áreas das limas e dos forames (p<0,0001). A média da área dos forames apicais foi 3,8 vezes maior que a das limas. Os resultados deste estudo sugerem que seriam necessários 4 instrumentos de calibre maior além do que se ajustou para que haja melhor relação entre as limas e as aberturas foraminais nos incisivos centrais superiores.

Foram utilizados 30 incisivos inferiores permanentes unirradiculados de humanos, extraídos, com raízes íntegras e ápices completamente formados e portadores de um único canal. Por meio de um paquímetro, as espessuras radiculares dos dentes foram medidas, no sentido mésio-distal a 1,0 e a 4,0mm aquém do forame apical. Após a abertura coronária, uma lima tipo K no 10, munida de limitador de penetração, foi introduzida no canal radicular até que sua extremidade pudesse ser visualizada na altura do forame, com o auxílio de um microscópio óptico com aumento de 7,8X. Dessa medida, subtraiu-se 1,0mm, estabelecendo-se o comprimento de trabalho. A dilatação do canal radicular foi feita, inicialmente, com brocas de Gates Glidden, em ordem numérica decrescente, da número 5 até a número1, até 4,0mm aquém do forame apical. Os dentes foram fixados em um modelo experimental especialmente desenvolvido para permitir a medição com os localizadores foraminais eletrônicos. Tal modelo era constituído por dois segmentos de PVC: um de menor calibre, com diâmetro correspondente a meia polegada por 2,0cm de comprimento, com as duas extremidades abertas e outro, de maior calibre, com uma das extremidades fechada e com diâmetro interno equivalente ao diâmetro externo do primeiro segmento (3/4 de polegada). No segmento de maior diâmetro, foi feito um orifício lateral que permitiu o posicionamento do eletrodo labial do localizador foraminal eletrônico e, para a medição, no seu interior, foi colocado alginato e, então, encaixado o componente de menor diâmetro, fazendo com que o ápice radicular ficasse imerso no alginato. This study evaluated the influence of the apical root dentin thickness, apical constriction and diameter of the apical foramen in the accuracy of readings obtained using the electronic apex locators Mini Apex Locator and Root ZX II ®. The study was conducted on 30 extracted human single-rooted permanent mandibular incisors, with intact and completely formed roots and presenting a single canal. The root thickness of the teeth was measured with a pachymeter in mesiodistal direction, at 1.0 and 4.0mm from the apical foramen. After coronal opening, a 10 K file with a stop was introduced in the root canal until its end could be observed at the level of the apical foramen, with aid of a light microscope with 7.8X magnification. One millimeter was subtracted from this measurement for establishment of the working length. Enlargement of the root canal was initially performed using Gates Glidden burs, in decreasing order, from number 5 to number 1, up to 4.0mm beyond the apical foramen. The teeth were fixated in an experimental model especially designed to allow the measurement with the electronic apex locators. This model was composed of two PVC segments: one smaller, with diameter corresponding to half inch with 2.0cm length, with both ends open; and the other, with larger diameter, with one end closed and internal diameter similar to the external diameter of the first segment (3/4 inch). In the segment with greater diameter, a lateral orifice was made to allow positioning of the lip electrode of the electronic apex locator. For the measurement, alginate was poured and the component with smaller diameter was fitted, so as the root apex was immersed in alginate. RESUMEN

The final preparation (MAF) size in infected root canals is still controversial. Nonetheless, recent studies demonstrated that larger apical preparation sizes produces a greater reduction in remaining bacteria and dentinal debris as compared to smaller apical preparation sizes. Therefore, clinicians should be practiced with treatment strategies guided by evidence-based information, especially in infected/failed root canals.
