Introducción: La cirugía actual de la catarata no comprende solo restablecer la visión en su mayor cantidad posible, sino que incluye además la psicofísica de la visión. Objetivo: Evaluar los resultados de la cantidad, calidad visual y de vida en la cirugía refractiva bilateral simultánea del cristalino en pacientes hipermétropes. Métodos: Se realizó un estudio descriptivo longitudinal prospectivo de serie de casos en 108 ojos de 54 pacientes con catarata bilateral e hipermetropía operados por cirugía bilateral simultánea en el Instituto Cubano de Oftalmología "Ramón Pando Ferrer" desde noviembre 2018 hasta abril 2022. Se emplearon los porcentajes y los números absolutos para resumir las variables cualitativas. En el caso de las cuantitativas se usó la media con su desviación estándar (DE) y el intervalo de confianza del 95 por ciento. Resultados: Fue más frecuente el sexo femenino con una edad media de 69,1 ± 8,1 años. El 55,11 por ciento de los pacientes tuvo dureza NO3. La agudeza visual monocular mejoró en el 97,2 por ciento y el 100 por ciento la mejoró de forma bilateral. Hubo una pérdida celular endotelial de 10,19 por ciento. La visión de colores y la sensibilidad al contraste mostraron de igual manera mejoría significativa. Conclusiones: La cirugía bilateral simultánea del cristalino en pacientes hipermétropes contribuye de manera significativa a la mejoría de la visión en cantidad y calidad, por lo que permite recuperar la calidad de vida(AU)

Monocular visual acuity improved in 97.2 percent and 100 percent improved bilaterally. There was an endothelial cell loss of 10.19 percent. Color vision and contrast sensitivity likewise showed significant improvement. Conclusions: Simultaneous bilateral lens surgery in hyperopic patients, contributes significantly to the improvement of vision in quantity and quality, thus allowing recovery of quality of life(AU)

@#Objective     To assess the safety and clinical outcomes of segmentectomy in one- or two-staged video-assisted thoracoscopic surgery (VATS) for bilateral lung cancer. Methods    We retrospectively enrolled 100 patients who underwent VATS segmentectomy for bilateral lung cancer at the Department of Thoracic Surgery of Peking Union Medical College Hospital from December 2013 to May 2022. We divided the patients into two groups: a one-stage group (52 patients), including 17 males and 35 females with a mean age of 55.17±11.09 years, and a two-stage group (48 patients), including 16 males and 32 females with a mean age of 59.88±11.48 years. We analyzed multiple intraoperative variables and postoperative outcomes. Results     All 100 patients successfully completed bilateral VATS, and at least unilateral lung received anatomical segmentectomy. Patients in the one-stage group were younger (P=0.040), had lower rate of comorbidities (P=0.030), were less likely to have a family history of lung cancer (P=0.018), and had a shorter interval between diagnosis and surgery (P=0.000) compared with patients in the two-stage group. Wedge resection on the opposite side was more common in the one-stage group (P=0.000), while lobectomy was more common in the two-stage   group. The time to emerge from anesthesia in the one-stage group was longer than that in the first and second operations of the two-stage group (P=0.000, P=0.002). Duration of surgery and anesthesia were similar between two groups (P>0.05). Total number of lymph node stations for sampling and dissection (P=0.041) and lymph nodes involved (P=0.026) were less in the one-stage group. Intraoperative airway management was similar between two groups (P>0.05). The one-stage group was associated with lower activities of daily living (ADL) scores. Conclusion     Segmentectomy is safe in one- or two-staged VATS for bilateral lung cancer, including contralateral sublobectomy and lobectomy. Duration of surgery and perioperative complications are similar between two groups, but the one-stage group is associated with lower ADL scores. On the basis of comprehensive consideration in psychological factors, physical conditions and personal wishes of patients, one-staged sequential bilateral VATS can be the first choice.

Objetivo: Analizar la presión intraocular posterior a la cirugía bilateral simultánea del cristalino en pacientes hipermétropes. Métodos: Se realizó un estudio prospectivo y analítico de serie de casos, con 108 ojos de 54 pacientes con catarata bilateral e hipermetropía, operados por cirugía bilateral simultánea, en el Instituto Cubano de Oftalmología "Ramón Pando Ferrer", desde noviembre de 2018 hasta abril de 2022. Se analizaron las variables edad, sexo, dureza nuclear, mejor agudeza visual con y sin corrección, monocular y binocular, profundidad de cámara anterior y longitud axil, todo esto relacionado a la presión intraocular. Resultados: Se emplearon los porcentajes y los números absolutos para resumir las variables cualitativas; en el caso de las cuantitativas se usó, la media con su desviación estándar y el intervalo de confianza del 95 por ciento. El sexo femenino fue el más frecuente (57,4 por ciento), la edad media fue de 69,1 ± 8,1 años. El 55,11 por ciento de los pacientes tuvo dureza NO3. La mejor agudeza visual corregida monocular y bilateral mejoraron de manera significativa, hubo una pérdida celular endotelial del 10,19 por ciento. No se observó asociación significativa de la presión intraocular con la longitud axil, la profundidad de la cámara anterior, esfera refractiva, ni la dureza del cristalino, la cual disminuyó después de la cirugía. Conclusipor cientoones: La cirugía bilateral simultánea del cristalino en pacientes hipermétropes, contribuye de manera significativa a la mejoría de la visión y a la disminución de la presión intraocular posoperatoria(AU)

There was no significant association of intraocular pressure with axillary length, anterior chamber depth, refractive sphere, nor lens hardness, which decreased after surgery. Conclusions: Simultaneous bilateral lens surgery in hyperopic patients significantly contributes to improved vision and decreased postoperative intraocular pressure(AU)

Objetivo: Analizar la recuperación y la calidad visual posoperatoria en la cirugía bilateral del cristalino por facoemulsificación. Métodos: Se realizó un estudio prospectivo y analítico de serie de casos en 40 pacientes con el diagnóstico de catarata bilateral, operados por cirugía bilateral simultánea del cristalino, atendidos en el Centro de Microcirugía Ocular del Instituto Cubano de Oftalmología "Ramón Pando Ferrer", durante el periodo comprendido desde noviembre del año 2018 hasta marzo de 2020. Se analizaron las variables edad, sexo, dureza nuclear según LOCS III, mejor agudeza visual con y sin corrección, astigmatismo medio inducido, visión de colores, sensibilidad al contraste, velocidad de lectura y estudio de la calidad de vida por la encuesta FV-14. Se emplearon los porcentajes y los números absolutos para resumir las variables cualitativas. En el caso de las cuantitativas se usó la media con su desviación estándar y el intervalo de confianza del 95 por ciento. Resultados: Fue más frecuente el sexo femenino en el 65 por ciento; la edad media fue de 69,1 ± 8,1 años. El 55,11 por ciento de los pacientes tuvo dureza NO3. La media de la mejor agudeza visual sin corrección mejoró a 0,89 ± 0,25; la mejor agudeza visual con corrección mejoró a 0,96 ± 0,03; el resto de las variables estudiadas mostró mejoría significativa. Conclusiones: La cirugía bilateral simultánea del cristalino personalizada contribuye de manera significativa a la mejoría de la visión en cantidad y calidad, lo que permite recuperar la calidad de vida de los pacientes(AU)


Objective: Analyze postoperative recovery and visual quality in bilateral crystalline lens surgery by phacoemulsification. Methods: An analytical prospective study was conducted of a case series of 40 bilateral cataract patients undergoing simultaneous bilateral crystalline lens surgery at the Center for Ocular Microsurgery of Ramón Pando Ferrer Cuban Institute of Ophthalmology from November 2018 to March 2020. The variables analyzed were age, sex, nuclear hardness by LOCS III, best visual acuity with and without correction, induced medial astigmatism, color vision, contrast sensitivity, reading speed and quality of life by the VF-14 survey. Percentages and absolute numbers were used to summarize qualitative variables, whereas mean and standard deviation were used for quantitative variables, with a 95 percent confidence interval. Results: Female sex prevailed with 65 percent; mean age was 69.1 ± 8.1 years. Hardness was NO3 in 55.11 percent of the patients examined. Mean best visual acuity without correction improved to 0.89 ± 0.25; best corrected visual acuity improved to 0.96 ± 0.03; the remaining variables studied showed significant improvement. Conclusion: Simultaneous personalized bilateral crystalline lens surgery significantly contributes to visual improvement both quantitatively and qualitatively, making it possible for patients to recover their quality of life(AU)

Objetivo: describir los resultados de la cirugía bilateral secuencial inmediata del cristalino por facoemulsificación y su beneficio económico. Métodos: se realizó un estudio observacional, descriptivo y longitudinal, en una muestra de 20 pacientes con catarata bilateral en el Servicio de Microcirugía Ocular del Instituto Cubano de Oftalmología Ramón Pando Ferrer, en el período de septiembre del año 2013 a septiembre de 2015. Se evaluaron la edad, el sexo, la dureza del cristalino según LOCS III y los resultados refractivos pre y posoperatorios. Resultados: se evidenció el predominio del sexo masculino con 11 pacientes para un 55,0 por ciento y una media de edad de 67,7 años en un rango de 28 a 87 años. El estudio aportó resultados significativos en la estadificación de LOCS III en el grupo de opalescencia nuclear y color nuclear 2 y 3 con un valor de 30,0 y 22,5 pr ciento, respectivamente. Los resultados refractivos mostraron mejoría significativa y la técnica produjo una reducción del costo. Conclusiones: la cirugía bilateral secuencial inmediata del cristalino por facoemulsificación es una técnica válida que permite obtener buenos resultados refractivos de forma casi inmediata, reduce el costo, aumenta el grado de satisfacción del paciente y le permite incorporarse a su vida habitual en un período de tiempo más corto(AU)


Objective: to describe the results of the immediate sequential bilateral surgery of the crystalline lens by using phacoemulsification technique and its economic advantages. Methods: Longitudinal, descriptive and observational study of 20 patients with bilateral cataract conducted at the Eye Microsurgery Service of Ramon Pando Ferrer Cuban Institute of Ophthalmology from September 2013 to September 2015. The evaluated variables were age, sex, crystalline lens hardness according to LOCS III and the pre and postoperative results. Results: Males predominated in the sample with 11 patients (55 percent) and the average age was 67.7 years for a range of 28-87 years. The study yielded significant results in the staging of LOCS III in terms of nuclear opalescence and nuclear color 2 and 3 with 30.0 and 22.5 percent, respectively. The refractive results showed significant improvement and the use of this technique brought cost savings. Conclusions: Immediate sequential bilateral surgery of the crystalline lens by using phacoemulsification technique represents a valuable method that allows achieving good refractive results almost immediately after surgery and the patient to return to his/her normal life in a shorter term(AU)

RESUMEN

Objective To evaluate the impact of different surgical approach on postoperative renal function in patients with synchronous sporadic bilateral renal cell carcinoma ( SSBRCC) .Methods The medical records of 28 patients with SSBRCC were reviewed.Of all these cases, 21 patients were male and 7 were female, and the mean age was 51 ( 25-63 ) years.None of these cases had family history of renal carcinoma.The treatment and other clinical data were analyzed retrospectively.Results All of the 28 patients underwent bilateral surgeries.Single procedures were performed on 16 patients, of which, one underwent bilateral radical nephrectomy ( RN) , 8 bilateral nephron-sparing surgery ( NSS) and 7 unilateral NSS combined with contralateral RN.Staged procedures were performed on 12 patients, of which bilateral NSS was on 5 patients (NSS-NSS group), NSS followed by RN (NSS-RN group) on 3 patients and RN followed by NSS ( RN-NSS group) on 4 patients.In term of postoperative renal function preservation for SSBRCC patients, the standard NSS was better than RN.Between different staged procedures, NSS-NSS was the preferred choice, and the NSS-RN was superior to the RN-NSS.Conclusions In term of preservation of postoperative renal function, the staged procedures were recommended, and NSS was preferred in the first stage.In deciding the second stage surgical approaches, the surgeons should take the renal function preservation and the tumor size into consideration.

Dissociated vertical deviation(DVD)is a kind of vertical strabismus of an upward drift of a nonfixating eye, and requires operation when it is cosmetically unacceptable. The authors compared the results of unilateral superior rectus recession with those of bilateral recession in the treatment of DVD, and analyzed the effects and complications of the superior rectus recession. The charts of 58 patients(94 eyes)who underwent superior rectus recession for DVD were retrospectively reviewed, and minimum follow-up was 6 months after operation. In 22 patients, the recession was performed unilaterally, and in 36 patients bilaterally. Satisfactory alignment under 9 PD was obtained in 77.3%of the unilateral cases and 77.8%of the bilateral cases. DVDs developed in unoperated eye in 9 unilateral cases. Graduated superior rectus recession was a quite predictable and effective procedure in the treatment of DVD. In unilateral surgery, it is important to keep in mind that the unmasking of DVD in the fellow eye can occur, but unilateral surgery may be indicated if only when the obvious deviation is diagnosed preoperatively in one eye.

RESUMEN

PURPOSE: This study compares the outcomes of unilateral cataract surgery to those of bilateral cataract surgery in patients with cataract of both eyes. METHODS: Among a total of 153 patients, 61 underwent unilateral surgery and 92 bilateral surgery. RESULTS: The unilateral and bilateral surgeries were similar in demographic and ocular characteristics including visual acuity and visual function-14, except patient-reported factor, such as trouble with vision at baseline. The patient-reported factor was more serious in bilateral surgery than in unilateral surgery. The changes of three outcomes were associated not with unilateral eye or bilateral eye surgery, but with each baseline condition. CONCLUSION: The patient-reported factors such as symptom score, trouble with vision were the significant outcomes to decide unilateral or bilateral surgery in patients with cataract of both eyes. The result of unilateral or bilateral surgery did not affect the outcomes of cataract surgery after 12 months.

RESUMEN

This survey of 374 patients, of which 47 underwent bilateral cataract surgery in one sitting and 42 in different sittings reveal that there is no differences in the incidence of complications following cataract extraction. The apparent slight discrepancies are more due to the inequality of patient reaction and variability of situations before surgery rather than from the unilaterality and bilaterality of surgery itselfThe advantages of simultaneous surgery to name a few, are (1) less mental trauma, (2) shorter hospitalization period, (3) less hospital and surgery expenses, (4) much wider field of vision and (5) earlier return to workThis study definitely disproves the contention that there are more complications when 2 eyes are done simultaneously than when the surgery is done with some time intervalIt may therefore, be safely concluded that bilateral cataract surgery may be done safely in one sitting if the proper presurgical and postoperative care are followed faithfully. (Summary and conclusion)
