RESUMEN

En este reporte presentamos tres pacientes en quienes ocurrieron condiciones inflamatorias perianales tardías, luego de la administración de sustancias modeladoras no identificadas en los glúteos. El diagnóstico inicial y supuesto no fue correcto, ya que no se investigó durante la evaluación preliminar el antecedente de la administración de elementos modeladores. Recomendamos que los pacientes con patologías inflamatorias del ano, sobre todo aquellos cuyo curso es extraño, se les pregunte acerca de la administración de agentes modeladores en los glúteos. Esta práctica puede contribuir a la eficacia del diagnóstico de manifestaciones perianales caracterizadas por flogosis, que se presentan de forma inusual. (AU)


In this report we present three patients in which late perianal inflammatory conditions occurred after administration of unidentified modeling agents to the buttocks. The initial diagnosis was not correct because of the administration of modeling agents was not investigated during the initial eval-uation. We recommend inquiring patients with inflammatory pathologies of the anus, especially those whose course is unusual, about the adminis-tration of modeling agents to the buttocks. This approach can contribute to the efficiency of the diagnosis of perianal complaints characterized by inflammation, but rare in its appearance. (AU)

Objective:To explore the clinical effect and nursing experience of papaverine intramuscular injection after finger replantation with self-designed hip intramuscular injection positioning device.Methods:Totally 216 cases of patients treated with palavering for anti-vasospasm after of severed fingers from January 2018 to December 2019 were randomly divided into control group (108 cases) and observation group (108 cases) by random number table method. The control group received intramuscular injection of buttock muscle according to the conventional method, and the observation group used the auxiliary intramuscular injection with self-designed glutei muscle positioning device. The incidence of postoperative hip indurations and ecchymosed, survival rate of and buttock were analyzed Pain scores were compared.Results:Totally 76 cases (70.37%) and 62 cases (48.15%) in the control group, 12 cases (11.11%) and 6 cases (5.56%) in the observation group. The difference between the two groups was statistically significant ( χ2 values were 78.545, 67.307, P<0.01). 78 cases (72.22%) survived in the control group and 96 cases (88.89%) survived in the observation group ( χ2 value was 11.416, P<0.01). After papaverine intramuscular injection, there was no significant difference in hip pain score between the two groups on the first and second day ( P>0.05), but there was significant difference between the two groups on the third to seventh day ( t values were 17.17 to 45.97, P<0.05). Conclusion:Palavering intramuscular injection can reduce the incidence of hip indurations and ecchymosed, relieve buttock pain and improve the survival rate of amputated finger, which is worthy of clinical application.

Resumen Objetivo: Describir las características clínicas, demográficas, resultados y complicaciones de una serie de pacientes operados de aumento de glúteo con implantes de silicona por medio de la técnica XYZ. Materiales y Método: Serie de casos retrospectiva de pacientes sometidos a gluteoplastía de aumento con implantes de silicona por medio de técnica intramuscular. Se describe la técnica quirúrgica, datos demográficos, antecedentes médico quirúrgicos y complicaciones posoperatorias. Resultados: Entre diciembre de 2014 y junio de 2017 se operaron 19 pacientes. La complicación más frecuente fue dehiscencia de herida operatoria, presentándose en 5 pacientes. Un paciente tuvo hematoma posoperatorio. Dos pacientes tuvieron seroma posoperatorio. Una paciente presentó rotación de un implante. Una paciente se perdió de seguimiento luego del control de retiro de puntos a la 6a semana. Una paciente necesitó reinternación para manejo del dolor. Ningún paciente necesitó reoperación. Ningún paciente presentó infección de herida operatoria. Ningún paciente presentó extrusión ni pérdida de implantes. Discusión: La técnica XYZ es un método efectivo para el tratamiento estético y reconstructivo de la región glútea especialmente en pacientes que no presentan zona dadora suficiente para injerto graso. Las complicaciones descritas son acordes a la literatura, por lo que se recomienda la técnica de gluteoplastía de aumento con prótesis de silicona intramuscular como una técnica reproducible y segura.


Objective: Describing the clinical features, demographics features, outcomes and complications from a serie of patients underwent buttock augmentation with silicone implants with XYZ technique. Material and Methods: Retrospective case series of patients underwent buttock augmentation with intramuscular technique. It Is described the surgical technique, demographics data, comorbidities and post operative complications. Results: Since December 2014 to June 2017 were operated 19 patients. The most frecuent complication was wound dehiscence, it was present in 5 patients. One patient had a postoperative hematoma. Two patients presented seroma. One patient presented implant rotation. One patient left controls after removal the suture in the sixth week. None of patients needed revisional procedures. None of patients presented wound infection. None of patients presented extrusión or implant removal. Discussion: The XYZ technique is a efective method to aesthetic and reconstructive treatment of buttock area, especially in patientes without a suficient donor area to fat graft. The complications described are accord with literatura, that is why buttock augmentation with intramuscular silicone implant is recomended as a reproducible and safe technique.

Objective: To evaluate the short-term effectiveness of percutaneous endoscopic lumbar discectomy (PELD) in treatment of buttock pain associated with lumbar disc herniation. Methods: Between June 2015 and May 2016, 36 patients with buttock pain associated with lumbar disc herniation were treated with PELD. Of 36 cases, 26 were male and 10 were female, aged from 18 to 76 years (mean, 35.6 years). The disease duration ranged from 3 months to 10 years (mean, 14 months). The location of the pain was buttock in 2 cases, buttock and thigh in 6 cases, buttock and the ipsilateral lower extremity in 28 cases. Thirty-four patients had single-level lumbar disc herniation, and the involved segments were L 4, 5 in 15 cases and L 5, S 1 in 19 cases; 2 cases had lumbar disc herniation at both L 4, 5 and L 5, S 1. The preoperative visual analogue scale (VAS) score of buttock pain was 6.1±1.3. VAS score was used to evaluate the degree of buttock pain at 1 month, 3 months, 6 months, and last follow-up postoperatively. The clinical outcome was assessed by the modified MacNab criteria at last follow-up. Results: All patients were successfully operated and the operation time was 27-91 minutes (mean, 51 minutes). There was no nerve root injury, dural tear, hematoma formation, or other serious complications. The hospitalization time was 3-8 days (mean, 5.3 days). All incisions healed well and no infection occurred. Patients were followed up 12-24 months (median, 16 months). MRI examination results showed that the dural sac and nerve root compression were sufficiently relieved at 3 months after operation. Patients obtained pain relief after operation. The postoperative VAS scores of buttock pain at 1 month, 3 months, 6 months, and last follow-up were 1.1±0.6, 0.9±0.3, 1.0±0.3, and 0.9±0.4 respectively, showing significant differences when compared with preoperative VAS scores ( P0.05). At last follow-up, according to the modifed MacNab criteria, the results were excellent in 27 cases, good in 9 cases, and fair in 2 cases, and the excellent and good rate was 94.4%. Conclusion: PELD can achieve satisfactory short-term results in the treatment of buttock pain associated with lumbar disc herniation and it is a safe and effective minimally invasive surgical technique.

We present two cases of percutaneous sacroplasty for sacral body fracture (Denis 3) and sacral alar fracture under multislice computed tomography (MSCT) guidance and discuss the clinical results and technical considerations. Sacroplasty is often recommended for the treatment of painful sacral insufficiency fractures, which destabilize the sacrum. However, sacroplasty for Denis zone 3 is rare because of the lack of validating controlled studies or unique technical considerations related to sacral anatomy. We performed sacroplasty for Denis zone 3 via the transiliac approach. Precise needle placement and polymethylmethacrylate cement injection were performed safely under the MSCT system. No complications occurred related to this procedure, such as iliac fracture, vascular leakage, or epidural leakage. MSCT-guided transiliac sacroplasty was a useful and effective solution in treating sacral body fracture.

PURPOSE: Driving is an important activity that is affected by various motor and cognitive deficits after stroke. On the other hand, there is no standard screening tool to evaluate the sitting asymmetry during driving, which is the stereotyped postural characteristic observed in patients with stroke. Therefore, this study compared the buttock pressure during simulated driving between healthy adults and patients with stroke. METHODS: Ten post-stroke patients and ten healthy subjects participated in the experiment. The participants experienced simulated driving of 6.1 km during approximately 5 minutes for adaptation. The driving scenario consisted of 3.5 km urban traffic conditions, 10 km straight highway, and 7 km curved or hilly rural roads. Force sensitive application (FSA) was used to analyze the distribution of the buttock pressure on the driver's seat. The symmetry index (SI) was determined using the average buttock pressure of each side of the buttock. The closer SI is to zero, the higher the symmetry of buttock pressure. RESULTS: These studies showed that the SI of healthy subjects was significantly closer to zero than that of the stroke patients. CONCLUSION: The buttock pressure of the stroke patients showed more asymmetry than that of the healthy subjects during simulated driving. Therefore, a therapeutic approach is needed for symmetrical sitting to improve the driving performance.

Introducción: El aumento de glúteo con implantes ha tomado fuerza en los últimos años con la aparición de la técnica intramuscular, que sugiere la creación de un bolsillo en el espesor del músculo glúteo mayor a 3 cm de profundidad. Por diferencias étnicas en el glúteo de pacientes chilenas, es necesario realizar mediciones para caracterizar el grosor del músculo glúteo mayor en nuestra población. Nuestro objetivo es conocer el espesor promedio del músculo glúteo mayor en mujeres chilenas. Material y método: Se diseñó un estudio descriptivo, retrospectivo, midiendo el espesor del músculo glúteo en todas las tomografías de abdomen y pelvis en prono, en mujeres entre 18 y 65 años, desde el 1 de enero al 31 de diciembre del año 2015 en el Hospital Clínico de la Universidad de Chile. Se determinó la asociación con la edad, el peso, la talla, el índice de masa corporal (IMC) y el espesor de la grasa abdominal y glútea de las pacientes. Resultados: El promedio del espesor del músculo glúteo derecho fue de 3,85 cm (IC 95%: 3,76-3,95) y el del glúteo izquierdo fue de 3,84 cm (IC 95%: 3,75-3,92). Existió asociación positiva entre el peso y el espesor del músculo, y entre el IMC y el espesor del músculo. Estas asociaciones fueron estadísticamente significativas. Discusión: Por las características de nuestra población, en la que el espesor del músculo glúteo mayor es menor que el descrito en la literatura, el bolsillo en el músculo debe ser realizado con precaución y a una profundidad menor a la descrita.


Introduction: Buttock augmentation with implants has become more frecuent in recent years with the emergence of the intramuscular technique, which suggests creating a pocket in the thickness of the gluteus maximus muscle, 3 cm deep. Ethnic differences in the gluteus of Chilean patients, makes measurements necessary to characterize the thickness of the gluteus maximus muscle in our population. Our objective is to know the average thickness of the gluteus maximus muscle in Chilean women. Material and method: A descriptive, retrospective study was designed by measuring the thickness of the gluteal muscle in all abdominal CT scans in prone position, in women aged 18 to 65, from January 1th, to December 31, 2015 at the Clinical Hospital the University of Chile. Association with age, weight, height, body mass index (BMI) and abdominal and gluteal fat thickness of patients was determined. Results: The average thickness of the right gluteal muscle was 3.85 cm (95% CI: 3.76-3.95) and the left gluteal muscle was 3.84 cm (95% CI: 3.75-3.92). There was a positive association between weight and thickness of the muscle, and between BMI and thickness of the muscle, these associations were statistically significant. Discussion: In our population, the thickness of the gluteus maximus is less than described in the literature, pocket in the muscle should be done with caution and shallowest than described.

Objective To compare the clinical effects of combined anesthesia of lumbar plexus-sciatic nerve and superior clunial nerve versus spinal-epidural analgesia in total hip replacement. Methods Fifty cases of total hip replacement were randomly divided into two groups. Patients in group A received combined anesthesia of lumbar plexus-sciatic nerve and superior clunial nerve,while patients in group B received spinal-epidural analgesia. Results There were no significant differences in preoperative HR, SBP,DBP and SpO2 between the groups(P > 0. 05). Compared with pre-anesthesia data,HR,SBP,DBP in group A were significantly lower during the anesthesia(P 0. 05). Patients′ heart rate in group A showed significant changes compared with that in group B. The differences in HR,SBP and DBP between group A and B at the same time points were significant(P < 0. 05). Superior rate of anesthesia in group B is higher than that in group A(P < 0. 05). Conclusion Combined anesthesia of lumbar plexus-sciatic nerve and superior clunial nerve has limited influence on the circulatory and respiratory systems,which can be used for total hip replacement.

<p><b>INTRODUCTION</b>Immediate postpartum haemorrhage (PPH) is the most common cause of maternal mortality worldwide. Most recommendations focus on its prevention and management. Visual estimation of blood loss is widely used for the early detection of PPH, but the most appropriate method remains unclear. This study aimed to compare the efficacy of visual estimation and objective measurement using a sterile under-buttock drape, to determine the volume of postpartum blood loss.</p><p><b>METHODS</b>This study evaluated patients aged ≥ 18 years with low-risk term pregnancies, who delivered vaginally. Immediately after delivery, a birth attendant inserted the drape under the patient's buttocks. Postpartum blood loss was measured by visual estimation and then compared with objective measurement using the drape. All participants received standard intra- and postpartum care.</p><p><b>RESULTS</b>In total, 286 patients with term pregnancies were enrolled. There was a significant difference in postpartum blood loss between visual estimation and objective measurement using the under-buttock drape (178.6 ± 133.1 mL vs. 259.0 ± 174.9 mL; p < 0.0001). Regarding accuracy at 100 mL discrete categories of postpartum blood loss, visual estimation was found to be inaccurate, resulting in underestimation, with low correspondence (27.6%) and poor agreement (Cohen's kappa coefficient 0.07; p < 0.05), compared with objective measurement using the drape. Two-thirds of cases of immediate PPH (65.4%) were misdiagnosed using visual estimation.</p><p><b>CONCLUSION</b>Visual estimation is not optimal for measurement of postpartum blood loss in PPH. This method should be withdrawn from standard obstetric practice and replaced with objective measurement using the sterile under-buttock drape.</p>

The word “hemangioma” comes from the Greek haema- (αιμα), “blood”; angeio (αγγειο), “vessel”; -oma (-ωμα), “tumor”. A Hemangioma is a benign and usually selfinvoluting tumor of the endothelial cells that lines blood vessels, and is characterized by increased number of normal or abnormal vessels filled with blood. It usually appears in the initial weeks of life and grows most rapidly over the first six months. Usually, growth is complete and involution has commenced by twelve months. Half of all infantile hemangiomas have completed involution by age five, 70% by age seven, and most of the remainder by age twelve years1. In more severe cases hemangiomas may leave residual tissue damage. In infancy, it is the most common tumor 2.

RESUMEN

Blue nevi can present clinically as blue, gray, brown, or black solitary nodules or plaques on the skin. Histologically, they represent collections of melanocytes and melanophages in the dermis. We presented here a case of a cellular blue nevus in a 20 years old man that presented as an enlarging blue-gray nodule on the right buttock. These cases can be challenging both clinically and histologically because malignant melanoma or malignant transformation of a blue nevus could be considered.

During routine dissection on 50 years old male cadaver, an accessory belly of piriformis was observed. This accessory belly was superior and parallel to the main piriformis muscle. This was associated with emergence of superior gluteal nerve and superior gluteal artery between the two bellies. Piriformis muscle and its relation to sciatic nerve has been suggested as a cause of piriformis syndrome. But interestingly in the present case, superior gluteal nerve was interposed between two bellies that may help the clinicians to establish a rare yet important cause of piriformis syndrome and a rare cause of undiagnosed chronic pain in gluteal region. As superior gluteal artery was also interposed, so this rare variation holds interest to surgeons especially in isolated buttock claudication despite otherwise normal vascular investigations.

[Objective]Summary of massage and composite therapy efficacy in the treatment of buttock epithelial neuritis, to explore the effective treatment of buttock epithelial neuritis.[Methods] To summarize the recent ten years related data on reports of buttock epithelial neuritis massage and combined thera-py.[Results] Single massage treatment can play a certain effect, but compared with the complex therapy, combined therapy effect is more remarkable. Com-bined therapy is the treatment of massage, acupuncture, acupotomy plus drug injection. Data showed that in addition to massage plus smal needle knife therapy and other complex therapy, the total effective rate was 100%.[Conclusion] Inflammation of buttock epithelial neuritis massage and composite thera-py is stable, and suitable for al ages, with fast recovery, less side effect, it can offer reasonable evidence-based foundation for clinical treatment.

RESUMEN

Steatocystoma multiplex is a clinical condition characterized by numerous dermal cysts that have their origin in the pilosebaceous duct, which appear and grow at puberty. The cystic nodules range from 3 mm to 3 cm in size, and they are soft and freely movable. Although there are numerous reports of different treatment methods, the long-term management of steatocystoma multiplex remains to be difficult. We, herein, report on a case of 23-year-old man with steatocystoma multiplex on the buttock. He was successfully treated with dot peeling of 100% trichloracetic acid after aspiration of the oily contents without any complications, such as atrophy, scarring and skin necrosis. We report the combination therapy of needle aspiration and trichloroacetic acid peeling as new therapeutic modality for multiple steatocystoma multiplex.

PURPOSE: Together with the breast, buttocks are an important element of attractive body contour. To make a beautiful buttocks, improvement of body contour around the buttock as well as buttock augmentation and lifting is also important. The authors investigated characteristic features of buttocks in Koreans and report about the results of liposculpture and autologous fat injection for improving buttock's contour. METHODS: We performed a retrospective study of 21 patients who would like to gluteal reshaping. We checked about buttock's ptosis, projection, depression, gluteal retraction and excessive fat accumulation around buttocks. Depending on it, we performed liposculpture and autologous fat injection. Under general anesthesia, we harvested fat from excessive fat accumulation areas around buttocks, and injected into buttocks medio-superiorly. Postoperatively, pillows were positioned on the bed not to press the buttocks which were injected with the fat. RESULTS: Based on the shape of buttocks, A-shape is seen in 4 cases(19%), V-shape 3 cases(14%), square-shape 9 cases(43%), round-shape 5 cases(24%). Based on the Gonzalez's ptosis grading method, 1 degree ptosis is 1 case(4%), 2 degree ptosis is 6 cases(29%), 3 degree ptosis is 8 cases(38%), 4 degree ptosis is 6 cases(29%). There were no complications such as infection, hematoma, pain, dysparethesia. The subjective assessment of surgical results by patients was excellent. CONCLUSION: To make a beautiful buttock, improvement of body contour around the buttock as well as buttock augmentation and lifting is also important. Liposculpture and autologous fat grafting are very safe, useful and easy methods for improving buttock's contour.

PURPOSE: The buttocks region has been associated with allure and sex appeal for centuries. Gluteal implants enable buttocks remodeling in a way that is not possible in other methods. One of the reasons that render gluteal implant surgeries unpopular is the fear of complications, the main problems being seroma, wound dehiscence, extrusion, and a visible or palpable implant. The authors present the XYZ technique, which provides anatomical reference points to guide the intramuscular dissection procedure in a feasible and safe way, resulting in a lower complication rate. METHODS: The XYZ procedure was done for buttock augmentation on 8 patients from December 2009 to June 2010. Patient's ages ranged from 27 to 44 with a mean age of 36. Seven patients were applied the 250cc sized Elastomer implants with one patient 290cc sized implant. Preoperative marking was done with the patients in the standing position. Bisection of the gluteus maximus muscle was done at the midpoint thickness to create a plane for implant insertion, which is called the sandwich plane. RESULTS: The 8 cases were performed safely without major complications. However one patient had minor wound dehiscence, and was managed with conservative treatment. Another patient had a discomfort on the buttocks for a long time. The patients were followed up for average 6 months. All patients were satisfied with the aesthetic results. CONCLUSION: In performing the XYZ technique for gluteal augmentation, the surgeon must split the muscle into two equal halves with anatomical reference points to guide the muscle detachment in a symmetrical way at an adequate depth. This method provides a guideline for the surgeon in determining the ideal plane during intramuscular dissection and gives predictable results with low complication rates. Intramuscular gluteoplasty with gluteal implants offers predictable, aesthetically pleasing results without contour irregularities and only a low incidence of major complications.

RESUMEN

Diffuse-type giant cell tumor of tendon sheath is an uncommon subtype of giant cell tumor of tendon sheath that shows destructive proliferation of synovial-like mononuclear cells, admixed with multinucleate giant cells, foam cells, siderophages and inflammatory cells. This tumor is commonly located in the periarticular soft tissues of knee, thigh and foot, but on rare occasions it can be purely subcutaneous or intramuscular. Diffuse-type giant cell tumor of tendon sheath affecting the buttock is extremely rare and only two cases, periarticular and intramuscular, have been described in the literature. We report the case of a 63-year-old male with diffuse-type giant cell tumor on the subcutaneous tissue of buttock.

RESUMEN

Sclerosing epithelioid fibrosarcoma (SEF) is a rare and poorly recognized variant of fibrosarcoma of deep soft tissue. We report the case of a 34-year-old woman who presented with a painful and palpable mass in the left buttock that was diagnosed as SEF.

RESUMEN

Kimura's disease is a rare inflammatory disorder of unknown etiology primarily seen in young Asian males. It is characterized by painless subcutaneous masses, blood and tissue eosinophilia, and markedly elevated serum immunoglobulin E (IgE) levels. Clinically, the subcutaneous nodules occur predominantly in the head and neck. However, we report the case of a five-year-old boy presenting with an asymptomatic solitary brown pigmented nodule on the left buttock diagnosed with Kimura's disease. Here we describe the diagnosis and treatment of this unusual case.

El polimetil-metacrilato es un polímero de alta resistencia al impacto, ampliamente utilizado en diferentes campos de la medicina, sin embargo, aún no está clara su indicación como relleno glúteo. A pesar de su aparente inocuidad, se han presentado complicaciones durante su uso como granulomas y nódulos palpables. Se han reportado casos de embolismo pulmonar por polimetil-metacrilato posterior a vertebroplastias. Sin embargo, no se encontró reporte bibliográfico de casos de embolismo pulmonar con su uso en procedimientos estéticos. Presentamos caso de paciente femenino de 31 años de edad, quien 4 horas luego de la inyección de 500 cm3 de polimetil-metacrilato en cada región glútea, presenta disnea en reposo de aparición súbita con tos seca y palpitaciones, disminución de agudeza visual y petequias generalizadas. Se diagnostica probable embolismo pulmonar por polimetil-metacrilato y retinopatía de Purtscher. Existe similitud clínica e imaginología del caso en estudio con embolismo por silicone, y las imágenes del fondo de ojo semejan la obstrucción de pequeñas arteriolas retinianas compatibles con este diagnóstico. Se sugieren nuevas investigaciones en el uso de polimetil-metacrilato como procedimiento estético en pro de la seguridad y en beneficio de los pacientes


Polymethyl-methacrylate is a high impact resistant polymer, widely used in different medicine fields, however its indication in buttock implants is still not clear. Even though its apparent innocuity, it has presented complications like granulomas and palpable nodules. There have been reported cases of pulmonary embolism caused by polimetil-metacrilato secondary to vertebroplasties. However, there have been no bibliographic cases of pulmonary embolism due to its use in aesthetic procedures. We present a case of a woman patient of 31 years old, who 4 hours after 500 cc injection of polymethyl- Methacrylate in each buttock, presents rest dyspnea with abrupt dry cough and palpitations, reduction in visual acuity and generalized petechiae. It is diagnosed probable pulmonary embolism by polimetil-metacrilato and Purtscher retinopathy. There is a clinical and imaginological similitude between this case and silicone embolism and the images of fondoscopy are similar to those of small retinian arteriole obstruction compatible with the diagnosis. Thereby, it is suggested new investigations in the use of polymethyl-methacrylate as esthetic procedure in favor of the patient’s security and benefit
