RESUMEN

Objective:To investigate the long-term outcomes of laparoscopic ventral rectopexy (LVR) for obstructive defecation with overt pelvic structural abnormalities. Methods:The retrospective cohort study was conducted. The clinical data of 31 obstructive defecation patients with overt pelvic structural abnormalities who were admitted to the Renji Hospital of Shanghai Jiaotong University School of Medicine from June 2014 to August 2020 were collected. There were 6 males and 25 females, aged 59(range, 32?81)years. All 31 patients underwent LVR through transabdominal approach. Observation indicators: (1) the Cleveland clinic constipation score (CCCS); (2) severity of obstructive defecation; (3) patients assessment of constipation quality of life (PAC-QoL). Follow-up was conducted using telephone interview and outpatient examination up to October 2021. One professional researcher assessed the constipation symptoms and quality of life of patients through outpatient interview or mobile software platform of Questionnaire Star. Measurement data with skewed distribution were represented as M(range), and comparison before and after operation was conducted using the Wilcoxon sign rank test. Results:(1) The CCCS. All 31 patients underwent LVR for the first time and were followed up for 61.8(range, 11.0?87.0)months. The constipation symptoms of the 22 patients were improved. The CCCS of the 31 patients before surgery and at the last follow-up time were 15.8(range, 8.0?26.0) and 10.7(range, 2.0?20.0), respectively, showing a significant difference ( Z=?3.98, P<0.05). (2) Severity of obstructive defecation. The severity scores of frequency of bowel movements, difficult of bowel movements, sensation of incomplete defecation, abdominal distension or pain, time of each bowel movements, daily unsuccessful times of defecation, artificial assisted defecation for the 31 patients were 2.9(range, 1.0?4.0), 3.0(range, 1.0?4.0), 1.9(range, 0?3.0), 0.5(range, 0?3.0), 2.6(range, 2.0?4.0), 2.0(range, 0?4.0), 0.9 (range, 0?2.0) before surgery, versus 1.7(range, 0?4.0), 1.6(range, 0?4.0), 1.2(range, 0?4.0), 0.3(range, 0?3.0), 1.7(range, 0?3.0), 1.4(range, 0?3.0), 0.7(range, 0?2.0) after surgery, respectively. There were significant differences in the frequency of bowel movements, difficult of bowel movements, sensation of in-complete defecation, abdominal distension or pain, time of each bowel movements, daily unsuccessful times of defecation for the 31 patients before and after surgery ( Z=?3.38, ?3.80, ?2.54, ?2.31, ?3.64, ?2.75, P<0.05) and there was no significant difference in the artificial assisted defecation for the 31 patients before and after surgery ( Z=?1.31, P>0.05). (3) PAC-QoL. The score of physical discomfort, satisfaction, worries and concerns, psychological discomfort for the 31 patients were 2.3(range, 1.0?4.0), 3.2(range, 1.0?4.8), 2.2(range, 0.6?4.0), 1.8(range, 0.4?3.9) before surgery, versus 1.6(range, 0?4.0), 2.3(range, 0?4.0), 1.7(range, 0?4.0), 1.3(range, 0?4.0)after surgery, respectively, showing significant differences before and after surgery ( Z=?3.49, ?2.17, ?2.50, ?3.05, P<0.05). RESUMEN

Aim To observe the influences of dexmen-detomidine on the spontaneous contraction of duodenal smooth muscle of rabbits in vitro and explore the mech-anisms.Methods The rabbits ( male or female ) were stunned and the duodenums were isolated .The sam-ples of duodenal segments were connected with tension transducer , which were then put into oxygen saturation Krebs-Henseleit ( K-H) solution .The influences of dex-mendetomidine on amplitude ( AM ) and frequency ( FR ) of duodenal smooth muscle were recorded by BL-420 F biological signal processing system .The cu-mulative dosing method was used to observe the differ-ent concentrations of dexmedetomidine on duodenal smooth muscle spontaneous contraction .Glibenclamide ( Gli) was added to K-H solution before dexmendeto-midine.In the calcium-free K-H solution, calcium chloride and rynodine were added before dexmendeto-midine.The mechanisms of dexmendetomidine were studied .Results ① Dexmendetomidine reduced the amplitude of spontaneous contraction of duodenal smooth muscle in rabbits in a dose-dependent manner ( P0.05 ) .② Gli ( P <0.05 ) partly abolished the inhibitory effects of dexmendetomi-dine on duodenal smooth muscle .③ Dexmendetomi-dine inhibited the contraction of duodenum smooth muscle induced by calcium chloride ( P <0.05 ) and rynodine ( P<0.05 ) application into calcium-free K-H solution. Conclusion Dexmendetomidine inhibits the spontaneous contraction of duodenal smooth muscle of rabbits in vitro. The mechanisms may be related to ac-tivating ATP sensitive potassium channels , inhibition of the extracellular calcium influx via cell membrane and intracellular calcium release via sarcoplasmic reticulum in duodenal smooth muscle .

RESUMEN

PURPOSE: To assess clinical outcome of clear lens extraction in high myopia after epikeratophakic (EKP) lenticule removal and to evaluate the change of cornea in EKP. METHODS: The retrospective study reports 5 eyes of 3 patients who underwent clear lens extraction (CLE) after EKP lenticule removal to treat myopia. The changes in best corrected and uncorrected visual acuity, spherical equivalent and corneal refractivity were evaluated before and after EKP, EKP lenticule removal and CLE. RESULTS: The cornea was clear, but the refractive power and astigmatism of cornea increased after EKP lenticule removal. The degree of change of the cornea seemed to be associated with the period between the EKP lenticule removal and the CLE. The BCVA in 3 of 5 eyes returned to the pre-EKP level after CLE. The eye that underwent CLE without EKP lenticule removal revealed severe overcorrected hyperopia and was corrected after CLE. The eye that had CLE without the EKP lenticule removal showed the overcorrection postoperatively but it was corrected after the EKP lenticule removal. CONCLUSIONS: The CLE following EKP lenticule removal can be an effective refractive surgery in complicated EKP patients. To obtain the accurate refractive estimate postoperatively, it is better to perform EKP lenticule removal preceding CLE.

RESUMEN

AIMTo investigate whether DADS induce MGC803 cell apop tosis and cell cycle arrest. METHODSMGC803 cell growth inhibitio n was measured by MTT assay. Flow cytometry and acridine orange fluorescent stai ning method were used to determine the induction of apoptosis and the change of cell cycle. RESULTSMTT assay showed that adding 20,30,40 mg?L -1 DADS for 72 h suppressed MGC803 growth by 25 7%,58 6%,69 0% respective ly. Partial cells presented the characteristic morphological changes of apoptosi s under the fluorescent microscope. The apoptosis rate ncreased in time-depende nt manner. Flow cytometry analysis revealed that treating MGC803 cell with DADS significantly increased in the percentage of cells in the G 2/M phase. The proportion of cells in the G 2/M phase after treatment with 30 mg?L -1 DADS for 24 hours was comparable (46 0%), and more than four times that occu rring in untreated cells (9 9%). Furthermore, flow cytometry analysis also demonstrated that DADS induced apoptosis of MGC803 cell in time-dependent manner. T he pencentage of apoptotic cell was 3 53% after 0 h of 30 mg?L -1 DADS tr eatment. This pencentage of apoptotic cell rose steadily over time reaching 9 8 % after 24 h and 39 5% after 48 h. CONCLUSIONDADS could induce apoptosis of MGC803 cells and block the cell cycle at G 2/M phase.

RESUMEN

Chilblain lupus erythematosus(CLE) is a chronic unremitting from of LE seen predomin antly in women, The chilblain lesion occurs commonly on the digits, calves and heels. Chronic facial discoid LE usually appears before the chilblain form. A 23-year-old female had relatively well defined, erythematous discoid plaques on her both cheeks and scattered erythema multiforme-like ring lesions on her right hand and right wrist. There were also chilblain lesions showing multiple, purple colored macules on her knees, lower legs and periungual areas of fingers and toes. The skin lesions developed at November, 1982 and then the skin lesions remitted during the next summer. The skin lesions recurred at December, 1983.
