RESUMEN

By summarising Professor WANG Xianbo's clinical experience in treating hepatitis B virus associated acute-on-chronic liver failure (HBV-ACLF), it is believed that HBV-ACLF is a syndrome of root-cause deficiency and manifestation excess, with spleen deficiency as the root cause and dampness-heat-toxicity-blood stasis as the manifestation, and the therapeutic methods proposed as “detoxification and cooling of the blood to promote circulation of the internal organs, and strengthening spleen and resolving dampness to take care of the middle energizer”. In the treatment of HBV-ACLF, for syndrome of stasis-heat-toxicity mass, it was common to use the Jiedu Liangxue Formula (解毒凉血方) by detoxifying and cooling blood; For syndrome of dampness-heat-toxicity mass, it was common to use in the Jiedu Liangxue Lishi Formula (解毒凉血利湿方) by detoxifying and cooling the blood, strengthening the spleen and resolving dampness; For syndrome of spleen-deficiency and dampness-heat, it was common to use in the Jiedu Liangxue Jianpi Formula (解毒凉血健脾方, also known as Zhonggan No.2 Formula) by strengthening the spleen and reple-nishing qi, clearing heat and resolving dampness.

RESUMEN

Objective:To investigate the clinical effect of Tiaomai mixture combined with metoprolol tartrate on premature ventricular contraction in coronary heart disease (CHD) due to Qi-Yin deficiency and stagnated heat in blood vessel. Method:A total of 95 patients with CHD complicated with premature ventricular contraction were randomized into a treatment group and a control group. Four cases dropped out, leaving 91 cases (45 in the treatment group and 46 in the control group) included in the follow-up. On the basis of routine treatments for CHD, patients in the control group were further treated with metoprolol tartrate, while those in the treatment group received metoprolol tartrate plus Tiaomai mixture. Such curative effect and safety indexes as traditional Chinese medicine (TCM) syndrome score, electrocardiogram (ECG), and 24 h dynamic ECG were observed before and after four-week treatment. Result:After treatment, the therapeutic effect on arrhythmia in the treatment group was better than that in the control group(<italic>P</italic><0.05). The treatment group was superior to the control group in reducing the frequency of premature ventricular contraction (<italic>P</italic><0.05), improving the Lown grade (<italic>P</italic><0.01), increasing the heart rate variability index (<italic>P</italic><0.05), and ameliorating the QT dispersion in ECG (<italic>P</italic><0.05), hypersensitive C-reactive protein, and homocysteine(<italic>P</italic><0.05). As revealed by comparison with those before treatment, both interventions improved TCM syndrome, with better outcomes observed in the treatment group (<italic>P</italic><0.01), manifested as the alleviation of shortness of breath, fatigue, dry mouth with desire to drink, and tongue and pulse manifestations (<italic>P</italic><0.01). Conclusion:Tiaomai mixture improves the clinical efficacy against arrhythmia in CHD patients by regulating the heart rate variability index, inhibiting inflammatory cytokines, lowering homocysteine, and relieving clinical symptoms, which is worthy of clinical promotion and application.

Objective: To explore the relationship between clearing heat and cooling blood of shikonin (SK) and regulation of blood redox homeostasis in blood-heat syndrome mice model induced by active yeast. Methods: Mice were randomly divided into control group, model group, SK (10 mg/kg) group, aspirin positive (200 mg/kg) group, L-buthionine sulfoximine (BSO, 600 mg/kg) group, N-acetyl-L-cysteine (NAC, 400 mg/kg) group, BSO combined with SK group, NAC combined with SK group. Mice were sc with 0.2 g/mL active yeast (10 mL/kg) on the neck, mice were ip SK at 0.5 h, ig Asp at 8 h, ip BSO and NAC before the beginning of the experiment (twice a day for one week). The rectal temperature (tR), general activity, metabolism, coagulation, and blood cell counting were determined. The levels of GSH and GSSG were determined by OPA assay. Results: Compared with the control group, tR of mice was significantly increased at 4 h (P < 0.05) and reached (38.07 ± 0.11)℃ at 10 h in model group; Irritability, redder lips, inclined to drinking, reduced carbon powder propelling rate, decreased pellet moisture capacity and prolonged coagulation time were observed (P < 0.05); RBC and inflammatory cells infiltrated were observed in lung tissue sections; The levels of LPO and MDA in serum were significantly increased, the activity of SOD was decreased, GSH/GSSG was decreased (P < 0.05). Compared with model group, tR of mice was significantly reduced and reached (37.51 ± 0.12)℃ at 10 h in SK group; The symptoms were significantly attenuated; The level of LPO and MDA in serum were decreased; The activity of SOD and GSH/GSSG ratio were increased (P < 0.05). BSO significantly reduced the GSH/GSSG ratio (P < 0.05), reduced the effect of SK on blood-heat syndrome mice model. NAC increased the level of GSH in serum and GSH/GSSG ratio (P < 0.05), enhanced the SK treatment effect. Conclusion: SK can attenuate hyperpyrexia, hemorrhage and congestion, oxidative damage in blood-heat syndrome mice model by regulating the blood redox homeostasis, and thereby the effect of clearing heat and cooling blood work.

Objective To investigate the protective effect of Shenxi Dan,a recipe for cooling blood,activating blood and inducing resuscitation,on multiple organ damage of rats with endotoxic shock.Methods The SD rats were randomly divided into normal group,model group and Shenxi Dan group.Except for the normal group,the rats in the other groups were given intraperitoneal injection of D-galactosamine (GalN) and intravenous injection of lipopolysaccharide (LPS) to induce endotoxic shock.The changes in mean arterial pressure (MAP) during the entire process were monitored.Serum samples and liver and lung tissues were collected after infusion for 0 h,2 h,6 h,respectively,and then the amount of whole blood cells,serum contents of total bilirubin (TB),lactate dehydrogenase (LDH),aspartate aminotransferase (AST),and alanine aminotransferase (ALT) were detected.Meanwhile,the pathological changes in lung and liver tissues after hematoxylin-eosin (HE) staining were observed under light microscope.Results Compared with the model group,the decrease degree of the blood pressure was mild,platelet count(plt) was not decreased obviously,while the increase of serum TB,LDH,AST,ALT contents was significantly inferior,and inflammatory pathological changes of lung and liver tissues significantly were much relieved in Shenxi Dan group.Conclusion Shenxi Dan can improve lung and liver function of endotoxic shock rats,reduce the pathological damage,and improve microcirculation,which is helpful to clinical treatment of endotoxic shock.

Objective Few reports are seen about the effects of cooling blood and removing stasis on nephrin and podocin . This study was to evaluate the therapy of cooling blood and removing stasis for Henoch -Sch?nlein purpura nephritis ( HSPN) and its ac-tion mechanisms by observing the effects of Danshao Granular Ⅲ on hematuresis , proteinuria and the expressions of nephridial nephrin and podocin in HSPN rats . Methods Twenty-one SD male rats were e-qually randomized to a blank control , an HSPN model , and a Dan-shao group.At 13 weeks after modeling , the animals in the model group were treated intragastrically with distilled water , while those in the Danshao group with Danshao Granular Ⅲtwice daily for 4 weeks. Then, the urinary red blood cell ( RBC) count was examined , the 24 h urinary protein quantity determined , the glomerular mesangial changes observed under the light microscope , the protein expres-sions and distributions of nephrin and podocin detected by indirect immunofluorescence , and their mRNA expressions determined by re-al-time PCR. Results The urinary RBC count and 24 h urine protein quantity were significantly higher in the HSPN model than in the blank control group (26.5/HP vs 0.3/HP and [2.214 ±1.090]g/24 h vs [0.624 ±0.354]g/24 h, both P0.05).The distributions of nephrin and podocin were improved after Danshao treatment .The mRNA expressions of nephrin and podocin were markedly higher in the Danshao group than in the HSPN models (0.530 ±0.089 vs 0.117 ±0.021 and 0.490 ±0.160 vs 0.033 ±0.025, P<0.05). Conclusion Danshao Granular Ⅲ, with its main action mechanisms of cooling blood and removing stasis , can effectively reduce urinary RBC count and urinary protein quantity and improve the symptoms of HSPN in rats .

Objective To observe the effect of expelling wind, cooling blood and invigorating kidney prescription on the marrow megakaryocyte differentiation maturity and TGF-β1 in patients with chronic idiopathic thrombocytopenic purpura (CITP). Methods Fifty CITP patients were randomly divided to three groups, including 20 patients administrated with the expelling wind, cooling blood and invigorating kidney prescription as TCM group, 15 patients with the same decoction and prednisone as combine group, and 15 patients with prednisone as western medicine group, 4 weeks as one treatment course. After 3 courses, the change of platelet counts, marrow megakaryocyte amount, platelet-produced megakaryocyte proportion and TGF-β1 level in the marrow were observed. Results The fine efficiency of TCM group (55.0%, 11/20) excelled western medicine group (20.0%, 3/15) significantly and close to that of combine group (60.0%, 9/15). Platelet counts of TCM group was raised obviously after treatment (P<0.01), better than that of western medicine group (P<0.05), and was similar to that of combine group. The marrow megakaryocyte amount of TCM group was effectively decreased and the platelet-produced megakaryocyte proportion was increased greatly (P<0.01), similar to that of combine group, which of western medicine group had no significant difference between pre-therapy and post-therapy. TGF-β1 of TCM group and combine group degraded obviously (P<0.05) compared with that before treatment, and was significant deviation compared with that of western medicine group (P<0.05). Conclusion The expelling wind, cooling blood and invigorating kidney prescription can effectively degrade the value of TGF-β1 of CITP patients, facilitate the maturity of the marrow megakaryocyte amount and raise the platelet counts.

Objective To study the clinical effect of nourishing kidney, securing essence and cooling blood on incipient diabetic nephropathy. Methods A total of 110 patients with incipient diabetic nephropathy were randomly recruited into a treatment group and a control group. The treatment group was treated with the method of nourishing kidney, securing essence and cooling blood and benazepril hydrochloride tablets. The control group was treated with bcnazepril hydrochloride tablets Exclusively. Urina microalbumin was detected in all patients 24 h before and after treatment. Results The effective rate was 94.7% and 86.8% in the treatment group and the control group respectively, showing significant difference (t= 9.31, P<0.01) . Conclusion The method of nourishing kidney, securing essence and cooling blood is effective to decrease urina microalbumin in incipient diabetic nephropathy.

Objectives To study the effect of cooling blood and invigorating circulation method on prevention and treatment of pulmonary fibrosis,and explore its mechanism.Methods Male ICR mice were randomly divided into 4 groups:negative control group,pulmonary fibrosis model group,cooling blood and invigorating circulation group and prednisone positive control group.The pulmonary fibrosis model of mice were established by BLM nasal instillation.After 8 hours,the mice were given prescription of cooling blood and invigorating circulation(Xijiao Dihuang decoction) by gastric perfusion.At the 28th day after treatment,the indexes were detected.The TGF-?1 content of bronchoalveolar lavage fluid(BALF) was tested by enzyme linked immunosorbent assay,and hydroxyproline(HYP) was examined by alkaline hydrolysis.Results The degree of airsacculitis and fibrosis in model group are obviously higher than the negative control group by pathological observation.Compared with model group,HYP content was decreased obviously in herb group(P

【Objective】To observe the jaundice-relieving effect of therapy of clearing heat and draining dampness combined with cooling blood and normalizing gallbladder function for severe icterohepatitis with damp-heat in liver and gallbladder.【Methods】Seventy-one severe icterohepatitis patients with damp-heat in liver and gallbladder were randomized into groups A(n=38) and B(n=33).The two groups received routine basic treatment and symptomatic treatment,group A was treated with heat-clearing,dampness-draining,blood-cooling and bile-draining herbs,and group B with intravenous drip of small-dose 654-2 additionally.Serum levels of total bilirubin(TBil),direct bilirubin(DBil),glutamic pyruvic transaminase(ALT),prothrombin activation(PTA) and jaundice duration were observed for the evaluation of jaundice-relieving effect.Meanwhile,the changes of gallbladder ultrasonograms were observed.【Results】After treatment,serum levels of TBil and ALT were decreased in both groups(P0.05)but the markedly effective rate in group A was higher than that in group B(P

[Objective] To explore the effect of wind-expelling and blood-cooling therapy in treating chronic idiopathic thrombocytopenia purpura (ITP) . [Methods] Thirteen cases of chronic ITP differentiated in TCM as heat-induced blood extravasation (group A) were treated with wind-expelling and blood-cooling therapy. Objective To observe the influence of wind-dispelling therapy on therapeutic effect of blood-cooling and kidney-tonifying herbs for chronic idiopathic thrombocytopenic purpura(CITP),and to investigate its immunoregulatory effect.Methods Forty-two CITP patients were randomized into 2 groups:the treatment group(N=22) received therapy of dispelling wind,cooling blood and tonifying kidney,and the control group(N=20) received therapy of cooling blood and tonifying kidney.Six weeks constituted one treatment course,and the treatment lasted 3 courses.Serum interleukin 2(IL-2),soluble IL-2 receptor(sIL-2R),Fas protein,Fas-L protein,and platelet-associated immunoglobulin G(PAIgG) levels and lymphocyte subtypes were compared before and after treatment.Results In the treatment group,14 patients were clinically cured,5 markedly effective,2 effective,1 ineffective and the total effective rate was 95.5%;in the control group,8 patients were clinically cured,4 markedly effective,3 effective,5 ineffective and the total effective rate was 75.0%.The therapeutic effect in the treatment group was superior to that in the control group(P

RESUMEN

[Objective] To observe the effect of Zicao Tianhuafen Decoction (ZTD), mainly composed of Radix Arnebiae seu Lithospermi, Radix Trichosanthis, Rhizoma Bletillae, Radix Notoginseng and Radix Codonopsis Pilosulae, on abortion induced by mifepristone. [Methods] One hundred and sixty cases were randomly allocated to two groups. After administration of mifepristone in the two groups, Group A was given ZTD while Group B was not. Complete abortion rate, incomplete abortion rate, ineffective rate, vaginal bleeding volume and bleeding time were observed after treatment. [Results] The complete abortion rate was 96.25% in Group A and 86.25% in Group B (P = 0.058). Vaginal bleeding volume was less and bleeding time shorter in Group A than those in Group B(P = 0.001). [Conclusion] ZTD can decrease the vaginal bleeding volume and shorten the bleeding time and tends to increase the complete abortion rate.

Conclusion The herbal decoction L iangxue Huayu Formula is e ffective in the 6-month observation period for the wet AMD cases through sta bilizing the conditions of retinal hemorrhage, exudation, and accumulated damage with low incidence of untoward reactions. It's a safe method for treatment.
