RESUMEN

Cemento-ossifying fi broma is a benign rare tumor of mesenchymal origin seen in the third or fourth decade in females. It arises from the mesenchymal blast cells of the periodontal ligament and can mature into cementum, bone, or fi brous tissue or varying proportions of each. Th e lesion is most commonly seen in the jaw. It appears as a well-defi ned circumscribed unilocular radiolucent lesion. Radio-opacity of the lesion depends on the concentration of cement or bone or in the concentration of both. We present the imaging features of cemento-ossifying fi broma in a 73-year-old female patient, involving the left maxilla.

RESUMEN

Florid cemento-osseous dysplasia is a condition of the bone which is rare and usually affects jaw bones. It is classified as sclerosing osteitis, multiple exostoses, diffuse chronic osteomyelitis, and gigantiform cementoma. The age group varies from 19 to 76 years and mostly presents in the fourth and fifth decade. Most commonly affects middle-aged women. It is identified on radiographic examination and manifested as diffuse irregularly shaped radiopacities in the alveolar processes. It usually presents as a bilaterally symmetrical lesion. This lesion is benign and requires treatment only if symptomatic or for cosmetic reasons. We report a case of an uncomplicated florid cemento-osseous dysplasia in a 35-year-old woman the diagnosis of our case was made radiographically.

RESUMEN

Cemento-ossifying fibroma is a slow growing benign tumor of mesenchymal origin. It is typically found in craniofacial bones, frequently involving the mandible and it tends to occur in third and fourth decade of life with predilection for women. Occurrence of this lesion in maxilla is a rare entity. We report one such case of cemento-ossifying fibroma of size 5x4 cm on upper left maxilla in 38 year old female patient which is an unusual presentation.

Cemento-ossifying fibromas are benign tumors, and, although cases of an aggressive type have been reported, no cases of cemento-ossifying fibroma transforming into osteosarcoma have been documented previously. Low-grade osteosarcoma is a rare type of primary bone tumor, representing 1%-2% of all osteosarcomas. A 45-year-old female patient was diagnosed with cemento-ossifying fibroma, treated with mass excision several times over a period of two years and eight months, and followed up. After biopsy gathered because of signs of recurrence, she was diagnosed with low-grade osteosarcoma. The patient underwent wide excision, segmental mandibulectomy, and reconstruction with fibula free flap. The aim of this report is to raise awareness of the possibility that cemento-ossifying fibroma can transform into osteosarcoma and of the consequent necessity for careful diagnosis and treatment planning.

RESUMEN

Cemento-ossifying fibromas (COFs) are benign lesions affecting the jaws and other craniofacial bones. They commonly affect adult females between the third and fourth decade of life, predominantly occurring in the premolar/molar region of the mandible. Most of the lesions typically show slow and often expansile growth, centrally within the jaw and characteristically behave in a benign form, but occasionally they may present as an aggressive gigantiform lesion. Radiographically, they appear as well-defined unilocular or multilocular intraosseous masses. The lesion is invariably encapsulated and of mixed radiolucent densities. The tumor may grow quite extensively; thus, the term "aggressive" is sometimes applied. Surgical resection is the treatment of choice. They are insensitive to radiotherapy and recurrences are uncommon. Clinical, radiographic and histopathologic features of COF and other fibro-osseous lesions are overlapping and may cause confusion in classification, diagnosis and treatment.

RESUMEN

Mandibular resection and reconstruction was done with rib graft on right side for a boy suffering from cemento-ossifying fibroma. The defect secondary to surgical ablation of mandible have far reaching consequences like speech, respiration, mastication, deglutition and cosmetics and growth disturbances. Restoring these functions is a challenging task. However, fairly regular crop of complications do occur during these surgical reconstructions. This article reports a rare case of an 11years old boy diagnosed as cement ossifying fibroma who underwent mandibular reconstruction with rib graft and the details of the follow up of the case for 6 months.

As lesões fibro-ósseas constituem um grupo de patologias caracterizadas pela substituição de tecido ósseo normal por tecido fibroso benigno contendo quantidades variadas de material mineralizado. A displasia fibrosa, o fibroma cemento-ossificante e a displasia cemento-óssea são as mais frequentes. Na literatura, a classificação e a terminologia das lesões fibro-ósseas apresentam-se ainda confusas e controversas, pois essas patologias exibem, frequentemente, características clínicas e morfológicas muito semelhantes. O fibroma cemento-ossificante é uma neoplasia benigna que faz parte do grupo das lesões fibro-ósseas. Caracteriza-se por crescimento bem delimitado e eventualmente encapsulado, apresentando quantidades variáveis de tecido mineralizado semelhante ao osso e ao cemento. São lesões incomuns, que tendem a ocorrer durante a terceira e a quarta décadas de vida, com uma predileção pelo gênero feminino, sendo a mandíbula o sítio de maior ocorrência. Este trabalho tem por objetivo descrever e discutir um caso clínico-cirúrgico sintomático de fibroma cemento-ossificante.


Fibro-osseous lesions are a group of diseases characterized by replacement of normal bone tissue for benign fibrous tissue containing different amounts of mineralized material. The most frequent lesions are fibrous dysplasia, cemento-ossifying fibroma and cemento-osseous dysplasia. In the literature, classification and terminology of these entities remain confusing and controversial, since these lesions show some clinical and morphologic similarities. Cemento-ossifying fibroma is a benign neoplasia that belongs to the group of fibro-osseous lesions. It is characterized by well delimited growth and occasionally is encapsulated, presenting changeable amounts of mineralized tissue similar to the bone and cementum. They are uncommon lesions that tend to occur during the 3rd and 4th decades of life, mainly in females and preferentially in the mandible. The aim of this work is to describe and discuss a clinical surgical case of symptomatic cemento-ossifying fibroma.
