RESUMEN

El artículo tiene como objetivo analizar los indicadores asociados al bienestar materno-infantil en los protocolos cubanos de actuación para la Covid-19. Con este fin, se realizó un análisis cualitativo de contenido que combinó una lógica deductiva derivada de la revisión bibliográfica de la literatura sobre bienestar materno-infantil y experiencia positiva del parto, y otra inductiva, con base en el procesamiento de la información empírica de los protocolos cubanos, según las recomendaciones de la OMS y la OPS. El análisis reveló que las guías enfatizan en la vida de la madre y el niño y priorizan la atención de las embarazadas como parte del acceso universal y gratuito a la salud; pero descuidan aspectos cualitativos del bienestar y la calidad del servicio, y con ello, los derechos de las gestantes y madres, y sus necesidades físicas, emocionales, psicológicas, sociales y espirituales. O artigo tem como objetivo analisar os indicadores associados ao bem-estar materno-infantil nos protocolos de ação cubanos para o Covid-19. Por tanto, foi realizada uma análise de conteúdo qualitativa que combinou uma lógica dedutiva derivada da revisão bibliográfica da literatura sobre bem-estar materno-infantil e uma experiência positiva de parto, e uma indutiva, baseada no processamento de informações empíricas de protocolos cubanos, a luz das recomendações da OMS e da OPAS. A análise revelou que as orientações enfatizam a vida da mãe e do filho e priorizam o cuidado à gestante como parte do acesso universal e gratuito à saúde. Porém, negligenciam os aspectos qualitativos do bem-estar e da qualidade do serviço e, com isso, os direitos da gestante e da mãe, suas necessidades físicas, emocionais, psicológicas, sociais e espirituais. The article aims to analyze indicators associated with maternal and child wellbeing in the Cuban Covid-19 action protocols. We carried out a qualitative content analysis that combined a deductive logic derived from a bibliographic review of the literature on maternal and child wellbeing and positive childbirth experiences, and an inductive logic based on the processing of empirical information from Cuban protocols, in light of the recommendations issued by WHO and PAHO. The analysis revealed that the instructions emphasize the mother's and child's life and prioritize care provided for pregnant women as part of free and universal access to health. However, they neglect the qualitative aspects of wellbeing and service quality and, with this, they also neglect the pregnant woman's and mother's rights, as well as their physical, emotional, psychological, social and spiritual needs. Adolescence is a period of rapid development where there is an acquisition of new capacities. It is a phase that presents not only opportunities for progress but also risk to health and well-being. During this stage, there is a tendency to try on various roles as adolescents struggle with who they want to be and how they want to live. This is a phenomenological study of seven adolescent mothers aimed at understanding their lived experience with regard to their pregnancy and childbirth. Themes that emerged from the data generated in the course of the various conversations with the mothers were (a) "finding encouragement" , (b) "the downside of my childbirth experience", (c) "point of no return", (d) "hope for the time to come". The participants found encouragement from their child, from their immediate family, from the father of the baby; for the married and those who are in committed relationship, from life's changes, and from themselves. Some of the negative aspects of their childbirth experience include "having to step their studies", and 'strained relationships with the people who matter". Difficulties arise from "burden of child care", specially during illness, breast feeding and financial dependency. "Loss of my youth", where the participants were forced into maturity as their perform adults roles and "feeling sorry" were the other categories under the theme downside experience. A recollection of the passed is the described in the theme, "point no return". Family problems, bad influence, bad influence and wrong choices happen to be the precursors to unintended pregnancy. Fears of the consequences of pregnancy and thoughts of abortion demonstrated that the pregnancies were unintended. Emotions felt during labor and delivery were pain, fear, loss of patience and relief. The health workers comment, instructions and actions made an impact on the labor and delivery experiences of the adolescents mothers. The theme "hope the time that is to come" describes the goals and aspirations and coping and family support needed to achieve all of they hope for their child and their own selves. A working framework was also developed towards understanding such a phenomenon,m serving as a springboard for future studies towards a grounded theory of understanding experiences of adolescent mothers. Interventions to prevent adolescents pregnancy should be utmost importance. However, when pregnancy occurs, the adolescents should be helped from pregnancy onwards. Need of a support person, efforts to lessen the pain and attempts to address the fears of the adolescent during labor and delivery can greatly improve her childbirth experience. Adolescents mother can benefit from the services rendered by health workers when they display a more caring attitude that includes providing nonjudgmental care despite the adolescent's lifestyle choices, being available and maintaining a presence for constant support and allowing the adolescent to be part of the decision-making process when possible.

PURPOSE: This study aimed to provide fundamental information about childbirth and antenatal care for pregnancy women and to find differences in mother's antenatal care and delivery service satisfaction between vaginal delivery and cesarean section. METHODS: This study was conducted in 4 residential areas and a study sample of 184 postpartum mothers who agreed to collect data. Data was collected from September 1 to October 20 2007 and a structured questionnaire were recruited by the survey. The data was analyzed by t-test and chi-squire test using SPSS/WIN 12.0. RESULTS: There was a significant difference in delivery place between vaginal delivery and cesarean delivery. Only 10.7% of vaginal delivery group delivered in general hospitals, however 24.5% of the cesarean section group delivered in general hospitals. Early antenatal care also showed statistical difference in mode of delivery. 43.5% of vaginal delivery mothers visited hospitals for the detection of pregnancy but 28.3% of cesarean section mothers did that. Vaginal delivery mothers more satisfied with her own delivery method and suggested a vaginal delivery to others. CONCLUSION: These results suggest that cesarean section mother's sensitivity of early antenatal care was less than vaginal delivery mother. Satisfaction related to delivery care services were higher in the vaginal delivery group.

RESUMEN

PURPOSE: This study is designed to test the effects of a full body massage on Apgar score, postpartum mother's childbirth experience, and mother-infant attachment for primipara during labor. METHOD: Data were collected using a quasi-experiment method (nonequivalent control group, pre-post test design) from November 1, 2001 to July 31, 2002. The subjects for this experiment consist of 28 women in the experimental group and 29 in the control group, out of 57 primipara hospitalized at the U OB & GYN hospital in Inchon. A 20 minute full body massage was given to the experimental group on each of three delivery phases (latent, active, and transition), and a conventional delivery care to the control group. The experimental tools for the study are Apgar score scale developed by Apgar(1958), postpartum mother's childbirth experience scale developed by Marut and Mercer(1979), and mother-infant attachment scale developed by Cropley et al(1981). The data collected were analyzed using t-test, chi2 test, and ANCOVA on the SPSS program. RESULT: The results of the experiment are as follows: One minute(t=6.756, p=.000; F=9.181, p=.004), and five minute(t=6.129, p=.000; F=7.918, p=.007) Apgar score are increased significantly. Postpartum mother's childbirth experience is significantly increased(t=11.691, p=.000; F=11.741, p=.001). Verbal touch score(t=5.256, p=.000; F=7.169, p=.010), skin touch score(t=11.332, p=.000; F=27.073, p=.000), and total score(t=12.105, p=.000; F=30.104, p=.000) of postpartum mother's childbirth experience are significantly increased, but not for eye touch score(t=3.236, p=.002; F=.800, p=.375). CONCLUSION: This study shows that a full body massage has a significant positive effect on Apgar score, postpartum mother's childbirth experience. and mother-infant attachment. Therefore, it is suggested that the full body massage might be used clinically to help primipara during labor.
