RESUMEN

Abstract INTRODUCTION: Loxoscelism is a clinical condition involving spiders of the genus Loxosceles. One of the most severe complications is acute kidney injury (AKI). This study aimed to investigate AKI and other complications associated with loxoscelism. METHODS: We analyzed cases diagnosed with loxoscelism in an area where most accidents were caused by Loxosceles amazonica from January 2010 to December 2015. AKI was defined according to the KDIGO criteria. RESULTS: Forty-five patients were recorded: 95.6% presented characteristic necrotic skin lesions and 13.3% AKI. CONCLUSIONS: Loxoscelism could cause kidney involvement which is uncommon and could lead to the death of these patients.

RESUMEN

Venous air embolism (VAE) is a life-threatening complication of some surgical procedures. Though occurrence of VAE is frequent during neurosurgical procedures, coagulopathy following VAE has not previously been reported. Coagulation abnormalities are more commonly reported associated with fat or amniotic fluid embolism, but rarely with VAE. We present a case of massive VAE in sitting position leading to fatal coagulopathy even after successful resuscitation following the event. Coagulation abnormalities and bleeding can produce catastrophic consequences in neurosurgical patients. This report emphasizes the possibility of this potentially fatal complication in patients who have sustained a massive VAE.

RESUMEN

A follow-up study of the dynamic changes with multiple parameters of blood coagulation and plasma limulus assay was carried out in 20 patients with decompen-sated hepatic cirrhosis. It was found that the levels of endotoxin in peripheral blood and factor VIII-related antigen increased significantly, prothrombin time and kaolin partial thromboplastin time prolonged, whereas fibrinogen, factor II, hepaplastin test, antithrombin-III and fibronectin decreased. Our data suggested that( 1 )both the coagulation abnormalities and endotoxemia were coexisted and correlated progressively with the disease process. ( 2 )the incidence and severity of endotoxemia were closely correlated with the degree and severity of hepatic decompensation; ( 3 )dynamic charges of endotoxemia and coagulation abnormalities were of some prognostic significance in decompensated hepatic cirrhosis
