Objetivo: analisar o nível de estresse percebido e sofrimento psíquico em gestores de saúde na pandemia da Covid-19. Método: estudo descritivo, transversal com abordagem quantitativa. A coleta ocorreu de abril a setembro de 2021, com 40 gestores de serviço de saúde. Utilizou-se para a coleta de dados um instrumento para caracterização sociodemográfica e ocupacional e as escalas "Perceived Stress Scale-14" e "Self Reporting Questionnaire" para avaliação do estresse percebido e sofrimento psíquico. Os dados coletados foram analisados no Statistical Package for the Social Sciences versão 22.0. O presente estudo faz parte de um projeto intitulado "Trabalhadores dos Serviços de Saúde Frente à Pandemia de Covid-19", aprovado pelo Comitê de Ética em Pesquisa sob CAAE número 35260620.9.0000.5231. Resultados: a maioria dos profissionais eram do sexo feminino (90%, N=36), casados (70%, N=28), com filhos (80%, N=32), com média de idade de 45 anos e com pós--graduação (47,5%, N=19). A média dos escores relacionados ao estresse percebido foi 31,13 pontos (DP=3,77) sendo o mínimo 24 e máximo de 42 pontos. Com relação ao sofrimento psíquico, (40%, N=16) os gestores apresentaram prováveis casos de transtornos. A prática de atividades físicas e de lazer (p<0,05) tem papel importante na diminuição do estresse percebido e do sofrimento psíquico. Conclusão: os gestores em saúde apresentaram, durante a pandemia, estresse e sofrimento psíquico, resultados esses que devem ser considerados para promoção de autocuidado aos gestores de saúde, enfatizando a necessidade da realização de atividades físicas e de lazer.


Objective: to analyze the level of perceived stress and psychological suffering in health managers during the Covid-19 pandemic. Method: descriptive, cross-sectional study with a quantitative approach. The collection took place from April to September 2021, with 40 health service managers. An instrument for socio-demographic and occupational characterization and the "Perceived Stress Scale-14" and "Self Reporting Questionnaire" scales were used for the assessment of perceived stress and psychic suffering. The collected data were analyzed using the Statistical Package for the Social Sciences version 22.0. The present study is part of a project entitled "Health Service Workers in the Face of the Covid-19 Pandemic", approved by the Research Ethics Committee under CAAE number 35260620.9.0000.5231. Results: most professionals were female (90%, N=36), married (70%, N=28), with children (80%, N=32), with a mean age of 45 years and with a postgraduate degree. -graduation (47.5%, N=19). The average score related to perceived stress was 31.13 points (SD=3.77), with a minimum of 24 and a maximum of 42 points. With regard to psychic suffering, (40%, N=16) the managers presented probable cases of disorders. The practice of physical and leisure activities (p<0.05) plays an important role in reducing perceived stress and psychological distress. Conclusion: health managers presented, during the pandemic, stress and psychic suffering, results that should be considered for promoting self-care to health managers, emphasizing the need to carry out physical and leisure activities.Keywords: Health manager; Occupational stress; Covid-19; Coronavirus infections.

Abstract Background: With the identification of COVID-19 disease in China, a pandemic began that affected health-care systems. The Neonatal Intensive Care Unit (NICU) of the Hospital de Ginecobstetricia del Centro Médico Nacional de Occidente experienced an increase in patient flow as part of the COVID-19 strategy of the Instituto Mexicano del Seguro Social (IMSS). This study aimed to analyze the impact of the COVID-19 pandemic on neonatal care and mortality indicators in our unit. Methods: We conducted a retrospective study to compare the number of hospital births, pre-term newborns (PTNB), NICU admissions, and deaths. Changes in frequencies between 2019 and 2021 were analyzed using Poisson distribution. Changes in PTNB births, proportion of admissions, and deaths/NICU discharges were analyzed by z-test for two proportions. Results: Between 2019 and 2021, the number of births increased by more than 2-fold. NICU admissions increased from 770 in 2019 to 1045 in 2021 (p < 0.01). The ratio of deaths/discharge from the service was 16.9% in 2019 and 13.1% in 2021 (p = 0.02). Conclusions: Mortality indicators in the NICU decreased from 2019 to 2021, even with the increase in the number of patients admitted during the COVID-19 pandemic.


Resumen Introducción: Con la identificación de la enfermedad por COVID-19 en China, inició una pandemia que afectó a los sistemas de salud. La Unidad de Cuidados Intensivos Neonatales (UCIN) del Hospital de Ginecobstetricia del Centro Médico Nacional de Occidente del Instituto Mexicano del Seguro Social (IMSS) vio incrementado su flujo de pacientes como parte de la Estrategia COVID-19 del IMSS. El objetivo fue analizar el impacto de la pandemia COVID-19 en los indicadores de atención y mortalidad neonatal en nuestra unidad. Métodos: Se realizó un estudio retrospectivo para comparar el número de nacimientos en el hospital, nacimientos de recién nacidos prematuros (RNPT), ingresos a UCIN y defunciones. Se analizaron los cambios en frecuencias entre los años 2019 a 2021 mediante la distribución de Poisson. Los cambios en nacimientos de RNPT, proporción de ingresos y defunciones/egreso en UCIN se analizaron mediante prueba Z para dos proporciones. Resultados: Entre los años 2019 a 2021, el número de nacimientos incrementó más de 2 veces. Los ingresos a UCIN aumentaron de 770 en 2019, a 1045 en 2021 (p < 0.01). La proporción de defunciones/egreso del servicio fue de 16.9% en 2019, y 13.1% en 2021 (p = 0.02). Conclusiones: Los indicadores de mortalidad en la UCIN disminuyeron de 2019 a 2021, aun con el incremento en el número de pacientes atendidos durante la pandemia COVID-19.

Introducción: La infección por el virus SARS-CoV-2 se ha convertido en una pandemia que ha ocasionado gran número de complicaciones y suele ser fatal para embarazadas y puérperas. Objetivo: Describir las características clínicas y epidemiológicas de embarazadas y puérperas con covid-19 ingresadas en cuidados intensivos. Métodos: Se realizó un estudio observacional descriptivo, de corte transversal, de 56 embarazadas y puérperas con diagnóstico confirmado de covid-19, ingresadas en el Servicio de Cuidados Intensivos del Hospital Provincial Docente Dr. Joaquín Castillo Duany de Santiago de Cuba durante el 2021. Las variables analizadas fueron la edad, los antecedentes personales, la causa de ingreso, el estado al egreso y la principal causa de muerte. A tal efecto, se calcularon las frecuencias, los porcentajes y la tasa de letalidad. Resultados: Hubo predominio de las gestantes en todos los grupos de edades (58,9 %) en relación con las puérperas (41,1 %), principalmente en el grupo etario de 31-35 años (30,3 %). La hipertensión arterial resultó ser la comorbilidad más frecuente con 71,4 y 28,6 %, para embarazadas y puérperas, respectivamente; en tanto, 66,6 % de las primeras y 33,3 % de las segundas fueron ingresadas por neumonía. Todas las embarazadas egresaron vivas y 7 puérperas fallecieron. La tasa de letalidad fue de 12,5 y el síndrome de disfunción multiorgánica apareció en 57,2 % de las afectadas. Conclusiones: La hipertensión arterial fue la comorbilidad más frecuente, la neumonía viral, el principal diagnóstico al ingreso y el síndrome de disfunción multiorgánica, la primera causa directa de muerte.


Introduction: Infection due to SARS-CoV-2 virus has become a pandemic that has caused great number of complications and is usually fatal for pregnant and newly-delivered women. Objective: To describe the clinical and epidemiological characteristics of pregnant and newly-delivered women with Covid-19 admitted to intensive care units. Methods: An observational descriptive, cross-sectional study of 56 pregnant and newly-delivered women with confirmed diagnosis of Covid-19 was carried out; they were admitted to the Intensive Care Unit of Dr. Joaquín Castillo Duany Teaching Provincial Hospital in Santiago de Cuba during 2021. The analyzed variables were age, past health history, cause of admission, state when discharged from the institution and the main cause of death. For this purpose, the frequencies, percentages and case fatality rate were calculated. Results: There was a prevalence of pregnant women in all the age groups (58.9%) relative to the newly-delivered women (41.1%), mainly in the 31- 35 age group (30.3%). Hypertension was the most frequent comorbidity with 71.4 and 28.6%, for pregnant and newly-delivered women, respectively; as long as, 66.6% of the first ones and 33.3% of the second ones were admitted due to pneumonia. All the pregnant women were discharged alive and 7 newly-delivered women died. Case fatality rate was 12.5 and multiple organ dysfunction syndrome appeared in 57.2% of those affected. Conclusions: Hypertension was the most frequent comorbidity, viral pneumonia was the main admission diagnosis and multiple organ dysfunction syndrome, the first direct cause of death.

Introducción: Un elevado porcentaje de pacientes que han padecido la covid-19 refiere una serie de manifestaciones clínicas que persisten luego del cuadro original. Uno de los grupos poblacionales en el que esto ocurre con mayor frecuencia es el de adultos mayores, a los cuales se les debe prestar máxima atención. Objetivo: Evaluar la efectividad de la terapia multimodal en ancianos con manifestaciones psicosomáticas por síndrome poscovid-19. Métodos: Se realizó un estudio cuasiexperimental de intervención terapéutica, sin grupo de control, en el Hospital General Docente Dr. Juan Bruno Zayas Alfonso de Santiago de Cuba, durante el periodo de marzo a mayo de 2021. La población estudiada se conformó por 25 pacientes de 60 y más años de edad con síndrome poscovid-19, quienes presentaron manifestaciones psicosomáticas y recibieron tratamiento multimodal (psicoterapia grupal y terapia floral). Se analizaron las variables sexo, edad, manifestaciones psicosomáticas y respuesta al tratamiento, cuya efectividad se evaluó según la proporción de pacientes con respuesta satisfactoria, considerando un nivel de significación de 5 %. Resultados: Predominaron el sexo masculino y los grupos etarios de 65-69 y 70-74 años. Las manifestaciones más frecuentes fueron la disnea, la astenia y la ansiedad. Después de aplicar la terapia multimodal, se logró una proporción significativa de pacientes con condición favorable, pues disminuyó el número de manifestaciones psicosomáticas. Conclusiones: La terapia multimodal resultó ser efectiva para reducir el número de manifestaciones psicosomáticas en los adultos mayores con síndrome poscovid-19.


Introduction: A high percentage of patients that have suffered from covid-19 refer a series of clinical manifestations that persist after the original pattern. One of the population groups in which this happens most frequently is the elderly, to whom maximum care should be paid. Objective: To evaluate the effectiveness of the combined modality therapy in elderly with psychosomatic manifestations due to postcovid-19 syndrome. Methods: A quasi-experiment study without control group of therapeutic intervention was carried out at Dr. Juan Bruno Zayas Alfonso General Teaching Hospital in Santiago de Cuba, from March to May, 2021. The population studied was made up of 25 patients of 60 years and over with postcovid-19 syndrome, who presented psychosomatic manifestations and received combined modality treatment (group psychotherapy and floral therapy). Sex, age and psychosomatic manifestations were analyzed as variables, as well as the response to treatment, which effectiveness was evaluated according to the proportion of patients with a satisfactory response, taking into account a significance level of 5 %. Results: There was a prevalence of the male sex and the 65-69 and 70-74 age groups. The most frequent manifestations were dyspnea, asthenia and anxiety. After applying the combined modality therapy, a significant proportion of patients with a favorable condition was achieved, since the number of psychosomatic manifestations decreased. Conclusions: Combined modality therapy was effective to reduce the number of psychosomatic manifestations in elderly with postcovid-19 syndrome.

Fundamento la infección por SARS-CoV-2 es la enfermedad emergente más importante del presente siglo. En dicho contexto, el Grupo de las Industrias Biotecnológica y Farmacéutica Cubanas (BioCubaFarma) creó alternativas terapéuticas para combatir la COVID-19, entre ellas el uso de Jusvinza, la cual forma parte del protocolo utilizado en el país. Objetivo determinar la resolutividad terapéutica de Jusvinza en pacientes confirmados de COVID-19. Métodos se realizó un estudio descriptivo, durante el periodo de enero a septiembre de 2021, en el Hospital Militar Dr. Joaquín Castillo Duany, de Santiago de Cuba. El universo estuvo constituido por 166 confirmados de COVID-19, a quienes se administró Jusvinza. Se analizaron algunas variables clínicas (estado clínico, enfermedades no transmisibles asociadas, progresión clínica), epidemiológicas (edad, sexo) y farmacológicas (resolutividad, duración en días). Se utilizó una planilla de vaciamiento de datos, los cuales fueron tomados de las historias clínicas. Resultados la morbimortalidad por COVID-19 fue superior en el sexo masculino, asociado a comorbilidades y edad mayor de 60 años; el sexo femenino resultó el de mayor resolutividad al tratamiento con Jusvinza (71,7 %), la cual incrementó su porcentaje en ausencia de enfermedades crónicas no transmisibles asociadas (81,4 %). Los pacientes de alto riesgo fueron los de más baja mortalidad (15,8 %). Conclusiones la resolutividad en pacientes confirmados de COVID-19 tratados con Jusvinza fue más elevada en casos de alto riesgo que en graves y críticos.


Foundation SARS-CoV-2 infection is the most important emerging disease of this century. In this context, the Cuban Biotechnology and Pharmaceutical Industries Group (BioCubaFarma) created therapeutic alternatives to combat COVID-19, including the use of Jusvinza, which is part of the protocol used in the country. Objective to determine the Jusvinza therapeutic resolution in confirmed COVID-19 patients. Methods a descriptive study carried out from January to September 2021, at the Dr. Joaquín Castillo Duany Military Hospital, in Santiago de Cuba. The universe consisted of 166 confirmed COVID-19 cases, to whom Jusvinza was administered. Some clinical variables (clinical status, associated non-communicable diseases, clinical progression), epidemiological (age, sex) and pharmacological (resolving, duration in days) were analyzed. A data extraction form was used, which was taken from the medical records. Results morbidity and mortality from COVID-19 was higher in males, related to comorbidities and age over 60 years; The female sex was the one with the greatest response to treatment with Jusvinza (71.7%), which increased its percentage in the absence of associated chronic non-communicable diseases (81.4%). High-risk patients had the lowest mortality (15.8%). Conclusions resolution in confirmed COVID-19 patients treated with Jusvinza was higher in high-risk cases than in severe and critical cases.

OBJETIVO: Apontar os aspectos clínicos e epidemiológicos de crianças internadas por COVID-19 em um hospital público situado em um estado da Amazônia Brasileira. MÉTODO: Estudo observacional, descritivo, retrospectivo e documental com uma abordagem quantitativa dos casos de internação pediátrica por COVID-19. RESULTADOS: No Hospital da Criança e Adolescente, foram registrados um total de 5016 casos suspeitos de COVID-19 em crianças. Destes, 666 foram confirmados com a doença e resultaram em 140 internações. Analisamos 136 notificações de crianças internadas por COVID-19. A maioria dos pacientes era lactente (39%) e pré-escolar (36%), com prevalência do sexo masculino (67,6%) e raça/cor preta/parda (86%). Além disso, 83,1% delas residem em área urbana. Quanto ao desfecho, 96,67% evoluíram para a cura e 3,33% resultaram em óbito. CONCLUSÃO: No contexto amazônico, a análise das características clínicas e epidemiológicas deste grupo etário é essencial para orientar os cuidados clínicos, prever a gravidade da doença e determinar o prognóstico.


OBJECTIVE: To determine the clinical and epidemiologic aspects of children hospitalized for COVID-19 in a public hospital located in a state in the Brazilian Amazon. METHODS: Observational, descriptive, retrospective, and documentary study with a quantitative approach to pediatric hospitalization cases due to COVID-19. RESULTS: In the Hospital for Children and Adolescents, a total of 5016 suspected cases of COVID-19 in children were recorded. Of these, 666 were confirmed with the disease, resulting in 140 hospitalizations. We analyzed 136 reports of children hospitalized for COVID-19. Most patients were infants (39%) and preschool children (36%), with a prevalence of males (67.6%) and black/brown race/color (86%). In addition, 83.1% live in urban areas. Regarding the outcome, 96.67% were cured, and 3.33% resulted in death. CONCLUSION: In the Amazonian context, the analysis of this age group's clinical and epidemiologic characteristics is essential to guide clinical care, predict the severity of the disease, and determine the prognosis.

RESUMEN

Introducción: consecuencia de la pandemia a causa del coronavirus (SARS-CoV-2), desde el 2020 ha aumen-tado la generación de los residuos con riesgo biológico o infeccioso, usados en los protocolos de biosegu-ridad por parte de la ciudadanía en general y el talento humano en salud. Ello generó un alto riesgo para salud y un aumento en el índice de contaminación ambiental y degradación de los recursos naturales, que hasta el momento ha superado las expectativas para su mitigación. Desarrollo: para contrarrestar el impacto del aumento en el índice de contaminación ambiental, se reflexiona sobre la necesidad de buscar acciones medioambientales para mitigar el daño y ampliar el concepto sobre el medio ambiente y la importancia de la interacción y codependencia del ser humano con la naturaleza. Se plantea la idea de retomar elementos conceptuales del conocimiento de las culturas indígenas sobre la visión holística del ambiente, partiendo de una cosmovisión indígena del buen vivir, donde el ambiente y el ser humano se encuentran en constante equilibrio y armonía. Conclusión: se pretende encontrar, desde la cosmovisión indígena, posibles abordajes conceptuales que mitiguen el impacto de la contaminación ambiental a causa del coronavirus, así como la formulación de acciones estratégicas para la adecuada gestión de estos residuos


Introduction: Since 2020, due to the the Coronavirus (SARS-CoV-2) pandemic, there has been an increase in generation of waste with biological or infectious risk used in biosafety protocols by the community and human resources in health, which, in turn, generates a high rate of pollution and environmental degradation of natural resources that has so far exceeded expectations for mitigation.Development: To reduce the impact of this problematic situation, a reflection was made to strengthen the current public policies and implement environmental actions that seek to mitigate the damage, but, above all, to change the concept of the environment and increase awareness about the importance of interac-tion and codependency of human beings with nature. The idea of retaking conceptual elements of the knowledge of indigenous cultures regarding the holistic vision of the environment is proposed, starting from a Quechua worldview of good living, where the environment and the human being are in constant balance and harmony. Conclusions:This study is intended to determine, from the indigenous worldview, the possible conceptual approaches that mitigate the environmental impact, as well as to formulate strategic actions for the adequate management of the resultant waste.


Introdução: como consequência da pandemia causada pelo Coronavírus (SARS-CoV-2), desde 2020 houve um aumento na geração de resíduos com risco biológico ou infeccioso utilizados em protocolos de bios-segurança pelo público em geral e talentos humanos em saúde; o que gerou um alto risco à saúde e um aumento no índice de contaminação ambiental e degradação dos recursos naturais que até agora tem superado as expectativas para sua mitigação. Desenvolvimento: para neutralizar o impacto do aumento do índice de contaminação ambiental, é feita uma reflexão sobre a necessidade de buscar ações ambien-tais para mitigar os danos, e ampliar o conceito de meio ambiente e a importância da interação e code-pendência do ser humano com a natureza. Propõe-se a ideia de retomar elementos conceituais do conhe-cimento das culturas indígenas sobre a visão holística do meio ambiente, partindo de uma cosmovisão indígena do bem viver, onde o meio ambiente e o ser humano estão em constante equilíbrio e harmonia. Conclusão: pretende-se encontrar a partir da visão de mundo indígena, possíveis abordagens conceituais que mitiguem o impacto da contaminação ambiental devido ao coronavírus, bem como a formulação de ações estratégicas para o gerenciamento adequado desses resíduos

RESUMEN La pandemia de la Covid 19 forzó a las facultades de medicina a optar por una metodología virtual de enseñanza por la suspensión de las prácticas presenciales con pacientes reales en los establecimientos de salud, debido al confinamiento social y riesgo de contagio. Objetivo Determinar las ventajas y desventajas percibidas por los estudiantes de medicina en relación con la transición de las prácticas presenciales en hospitales a las sesiones virtuales de aprendizaje en un curso de semiología en una facultad de medicina de Lima, Perú. Material y métodos Estudio descriptivo de corte transversal realizado en estudiantes del 4to año de la carrera de Medicina. Los datos se recolectaron mediante una encuesta virtual no validada. Resultados 94 estudiantes respondieron la encuesta (tasa de respuesta: 51,1%). Las ventajas percibidas fueron: 57,4% tener mayor tiempo para estudio teórico y 38,3% en ahorro de tiempo en transporte y movilidad. Las desventajas fueron: 42,6% falta de contacto con pacientes reales y 39,4% no poder realizar una historia clínica adecuada. En cuanto al logro de objetivos de aprendizaje, el 72,3% consideró que logró identificar los problemas de salud del paciente, mientras que el 24,4% afirmó que logró realizar una adecuada historia clínica, y sólo el 9,6% que logró realizar un examen físico completo en pacientes. Conclusión La modalidad virtual de enseñanza permitió a los estudiantes tener más tiempo para revisar aspectos teóricos del curso, pero limitó la adquisición de habilidades prácticas, como realizar una anamnesis adecuada, presentar historias clínicas y examinar pacientes.


SUMMARY The COVID-19 pandemic forced the school of medicines to opt for a virtual teaching modality due to the suspension of face-to-face activities imposed by the lockdown. Objective To determine the advantages and disadvantages of the virtual teaching modality perceived by the students in an introduction to clinical medicine course of a school of medicine in Lima, Peru. Methods A virtual non-validated survey was circulated among fourth year medical students. Results 94 studentes answered the survey (51%). Perceived advantages were to have more time to study (57.4%) and saving time in transportation (39.4%). The disadvantages were lack of contact with real patients (42.6%) and not to be able to obtain a clinical history from patients (39.4%). The 72.3% of students were able to identify the medical problems of patients, but only 24.4% were able to obtain an adequate clinical history and just 9.6% performed an adequate physical examination. Conclusion The virtual teaching modality allowed the student to have more time for self-study but limited their abilities to obtain a clinical history and to perform a physical examination.

Resumo Objetivo Descrever a prevalência de lesão renal aguda em adultos jovens com diagnóstico da COVID-19 admitidos em unidade terapia intensiva. Métodos Estudo retrospectivo, quantitativo e analítico. A amostra foi de adultos jovens (20 a 40 anos) admitidos em unidades de terapia intensiva, com diagnóstico de infecção por SARS-CoV-2 entre março e dezembro de 2020. Os dados foram obtidos por meio do prontuário eletrônico, e a lesão renal aguda foi definida pelo valor da creatinina, segundo critérios das diretrizes da Kidney Disease Improving Global Outcomes. A significância estatística foi de p≤0,05. Resultados Foram internados 58 adultos jovens, sendo 63,8% do sexo masculino. A hipertensão arterial sistêmica esteve presente em 39,6%, a obesidade em 18,9% e o diabetes mellitus em 8,6%. A lesão renal aguda foi identificada em 55,1%, sendo o estágio 3 predominante em 43,1% deles. Nesses pacientes, o uso de ventilação mecânica e de drogas vasoativas foi significativo em 92%, assim como a disfunção orgânica respiratória (80%), seguida da renal (76%). Fatores de risco, como transplante renal ou doença renal crônica e obesidade, aumentaram em 12,3 e 9,0 vezes, respectivamente, a chance de desenvolver lesão renal aguda. Conclusão Este estudo demonstrou alta prevalência de lesão renal em adultos jovens e sua associação com comorbidades prévias. Obesidade, transplante renal e doença renal crônica elevaram a chance de o adulto jovem desenvolver lesão renal aguda, resultando em desfechos a favor da morbimortalidade.


Resumen Objetivo Describir la prevalencia de lesión renal aguda en adultos jóvenes con diagnóstico de COVID-19 admitidos en unidad de cuidados intensivos. Métodos Estudio retrospectivo, cuantitativo y analítico. La muestra fue de adultos jóvenes (20 a 40 años) admitidos en unidades de cuidados intensivos, con diagnóstico de infección por SARS-CoV-2 entre marzo y diciembre de 2020. Los datos se obtuvieron por medio de historias clínicas electrónicas, y la lesión renal aguda fue definida por el valor de la creatinina, de acuerdo con criterios de las directrices de la Kidney Disease Improving Global Outcomes. La significación estadística fue de p≤0,05. Resultados Hubo 58 adultos jóvenes internados, el 63,8 % de sexo masculino. La hipertensión arterial sistémica estuvo presente en el 39,6 %, la obesidad en el 18,9 % y la diabetes mellitus en el 8,6 %. Se identificó lesión renal aguda en el 55,1 %, de nivel 3 como predominante en el 43,1 % de los casos. En esos pacientes, el uso de ventilación mecánica y de drogas vasoactivas fue significativo en el 92 %, así como también la disfunción orgánica respiratoria (80 %), seguida de la renal (76 %). Los factores de riesgo, como trasplante renal o enfermedad renal crónica y obesidad, aumentaron 12,3 y 9,0 veces respectivamente la probabilidad de presentar lesión renal aguda. Conclusión Este estudio demostró alta prevalencia de lesión renal en adultos jóvenes y su asociación con comorbilidades previas. La obesidad, el trasplante renal y la enfermedad renal crónica aumentaron la probabilidad de que los adultos jóvenes presenten lesión renal aguda, lo que da como resultado desenlaces a favor de la morbimortalidad.


Abstract Objective To describe acute kidney injury prevalence in young adults diagnosed with COVID-19 admitted to the Intensive Care Unit. Methods This is a retrospective, quantitative and analytical study. The sample consisted of young adults (20 to 40 years old) admitted to Intensive Care Units, diagnosed with SARS-CoV-2 infection between March and December 2020. Data were obtained through electronic medical records, and kidney injury acute was defined by the creatinine value, according to the Kidney Disease Improving Global Outcomes guidelines criteria. Statistical significance was p≤0.05. Results A total of 58 young adults were hospitalized, 63.8% of whom were male. Hypertension was present in 39.6%, obesity in 18.9%, and diabetes mellitus in 8.6%. Acute kidney injury was identified in 55.1%, with stage 3 predominating in 43.1% of them. In these patients, the use of mechanical ventilation and vasoactive drugs was significant in 92% as well as respiratory organ dysfunction (80%), followed by renal organ dysfunction (76%). Risk factors such as kidney transplantation or chronic kidney disease and obesity increased by 12.3 and 9.0 times, respectively, the chances of developing acute kidney injury. Conclusion This study demonstrated a high kidney injury prevalence in young adults and its association with previous comorbidities. Obesity, kidney transplantation and chronic kidney disease increased the chance of young adults to develop acute kidney injury, resulting in outcomes in favor of morbidity and mortality.

ABSTRACT Objective: To identify the scientific evidence on the impacts caused by the use of screens during the COVID-19 pandemic in children and adolescents, raising reflections for future interventions with this public. Data source: This is an integrative literature review, conducted in the databases Medical Literature Analysis and Retrieval System Online (MEDLINE), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Cumulative Index to Nursing and Allied Health Literature (CINAHL), United States National Library of Medicine (PubMed), Scopus, Web of Science, and Embase, published from March 2020 to January 2022, in Portuguese, English and Spanish. Data synthesis: The search strategies allowed retrieving 418 articles, of which 218 were duplicates. The analysis of titles and abstracts resulted in the maintenance of 62 studies. Of these, 31 were excluded from the reading of the full text, since they did not clearly present the phenomenon investigated. Thirty-one were eligible, resulting in five categories: eye consequences; increased sedentary behavior and weight; change in eating habits; implications for sleep quality and impacts on mental health. Conclusions: The excessive use of screens during the pandemic led to numerous consequences for children and adolescents, with a higher incidence of visual damage, sedentary lifestyle, inadequate eating habit and increased weight gain, in addition to impaired sleep quality and mental health. This study provides subsidy for health professionals to carry out continuing education focused on this theme, and elaborate effective interventions for this public in this transition to the post-pandemic period.


RESUMO Objetivo: Identificar as evidências científicas sobre os impactos causados pelo uso de telas durante a pandemia da COVID-19 em crianças e adolescentes, almejando reflexões para futuras intervenções com esse público. Fontes de dados: Trata-se de uma revisão integrativa da literatura, realizada nas bases de dados Medical Literature Analysis and Retrieval System Online (MEDLINE), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Cumulative Index to Nursing and Allied Health Literature (CINAHL), United States National Library of Medicine (PubMed), Scopus, Web of Science e Embase, publicados de março de 2020 a janeiro de 2022, em português, inglês e espanhol. Síntese dos dados: As estratégias de busca possibilitaram recuperar 418 artigos, sendo 218 duplicados. A análise de títulos e resumos resultou na manutenção de 62 estudos. Destes, após a leitura do texto integral foram excluídos 31, uma vez que não apresentavam com clareza o fenômeno investigado. Foram elegíveis 31 estudos, emergindo cinco categorias: consequências oculares; aumento do comportamento sedentário e do peso; alteração dos hábitos alimentares; implicações na qualidade do sono e impactos na saúde mental. Conclusões: Percebe-se que o uso telas em excesso durante a pandemia trouxe inúmeras consequências para o público infantojuvenil, com maior incidência de acometimentos visuais, sedentarismo, alimentação inadequada e, por consequência, maior ganho de peso, além de prejuízos à qualidade do sono e à saúde mental. Este estudo fornece subsídios para que os profissionais da saúde realizem educação continuada voltada para essa temática e elaborem intervenções efetivas para esse público nesta transição para o período pós-pandêmico.

Resumo Objetivo Analisar a prevalência e os fatores associados à hospitalização de idosos com COVID-19 no estado do Paraná, PR, Brasil. Métodos Estudo transversal vinculado à coorte "Acompanhamento Longitudinal de adultos e idosos que receberam alta da internação hospitalar por COVID-19", realizado por meio de informações contidas nas fichas de notificação compulsória do Sistema de Informação de Agravos de Notificação. As análises foram realizadas através de frequências relativas e absolutas, com aplicação do teste de qui-quadrado adotado no modelo de regressão logística. A população do estudo englobou pessoas residentes no Estado do Paraná com idade de 60 anos ou mais, hospitalizadas por COVID-19 no período de março de 2020 a setembro de 2021. Resultados Foi identificada maior prevalência de hospitalização entre idosos com escolaridade igual ou maior a oito anos. Indivíduos não vacinados contra COVID-19 apresentaram maior chance de internação. O sexo masculino apresentou mais chance de admissão em Unidade de Terapia Intensiva em comparação com o sexo feminino. Doenças cardiovasculares, pneumopatia e obesidade aumentaram a prevalência da forma grave da doença. Conclusão Fatores tais como escolaridade e não adesão à vacinação contra COVID-19 podem aumentar o risco de hospitalização pela doença. Pessoas idosas do sexo masculino apresentam maior chance de hospitalização na UTI se comparadas às do sexo feminino; além disso, a não utilização de antivirais pode contribuir para o agravamento do estado de saúde.

Métodos Estudio transversal vinculado a la cohorte "Seguimiento longitudinal de adultos y personas mayores que recibieron alta de internación hospitalaria por COVID-19", realizado mediante información contenida en las fichas de notificación obligatoria del Sistema de Información de Agravios de Notificación. Los análisis fueron realizados a través de frecuencias relativas y absolutas, con aplicación de la prueba ji cuadrado adoptada en el modelo de regresión logística. La población del estudio incluyó personas residentes del estado de Paraná, de 60 años o más, hospitalizadas por COVID-19 en el período de marzo de 2020 a septiembre de 2021. Resultados Se identificó mayor prevalencia de hospitalización en personas mayores con escolaridad igual o mayor a ocho años. Individuos no vacunados contra COVID-19 presentaron mayor probabilidad de internación. El sexo masculino presentó más probabilidad de admisión en Unidad de Cuidados Intensivos en comparación con el sexo femenino. Enfermedades cardiovasculares, neumopatía y obesidad aumentaron la prevalencia de la forma grave de la enfermedad. Conclusión Factores tales como escolaridad y no adhesión a la vacunación contra COVID-19 pueden aumentar el riesgo de hospitalización por la enfermedad. Personas mayores de sexo masculino presentaron mayor probabilidad de hospitalización en la UCI al compararlas con las de sexo femenino. Además, la no utilización de antivirales puede contribuir al agravamiento del estado de salud.


Abstract Objective To analyze the prevalence and factors associated with hospitalization of elderly people with COVID-19 in the State of Paraná, PR, Brazil. Methods A cross-sectional study linked to the cohort "Longitudinal Monitoring of adults and elderly people who were discharged from hospital admission due to COVID-19", was carried out using information contained in the compulsory notification forms of the Notifiable Diseases Information System. Analyzes were carried out using relative and absolute frequencies, applying the chi-square test adopted in the logistic regression model. The study population included people aged 60 years or over and residing in the State of Paraná, who were hospitalized for COVID-19 from March 2020 to September 2021. Results A higher hospitalization prevalence was identified among elderly people with eight years of education or more. Individuals not vaccinated against COVID-19 had a greater chance of hospitalization. Males had a greater chance of admission to the Intensive Care Unit compared to females. Cardiovascular diseases, lung disease, and obesity have increased the prevalence of the severe form of the disease. Conclusion Factors such as education and non-adherence to vaccination against COVID-19 can increase the risk of hospitalization due to the disease. Elderly people of the male sex have a greater chance of hospitalization in the ICU compared to the female sex. Furthermore, not using antivirals can contribute to worsening health status.

ABSTRACT Purpose: To evaluate macular chorioretinal flow changes on optical coherence tomography angiography, in participants who received inactivated and messenger RNA (mRNA) vaccines to prevent coronavirus disease 2019 (COVID-19). Methods: In this prospective cohort study, healthy participants who received two doses of an inactivated COVID-19 vaccine (CoronaVac) and then one dose of an mRNA vaccine (BNT162b2) were examined before and after each vaccination. Ophthalmologic examination and imaging with optical coherence tomography angiography were performed during each visit. We evaluated vascular densities in the superficial and deep capillary plexuses in foveal, parafoveal, and perifoveal areas; the foveal avascular zone; and choriocapillaris flows (in 1- and 6-mm-diameter areas). Results: One eye in each of the 24 participants was assessed. Superficial capillary plexus vascular densities in the parafoveal area were significantly lower after the second dose of the CoronaVac vaccine than after the first dose. In the deep capillary plexus, vascular attenuation was observed only in the parafoveal region after the first CoronaVac dose. However, in all regions, the deep capillary plexus vascular densities and subfoveal choriocapillaris flow were significantly decreased after the second CoronaVac dose. After the BNT162b2 dose, the superficial capillary plexus vascular densities, the deep capillary plexus vascular densities, and subfoveal choriocapillaris flow of most regions were significantly lower than those before vaccinations. Conclusion: Vascular attenuation, observed particularly after the second dose of the CoronaVac vaccine, may explain the pathogenesis of postvaccine ocular ischemic disorders reported in the literature. However, these disorders are extremely rare, and the incidence of thrombotic events caused by COVID-19 itself is higher.

ABSTRACT Objective: To develop and validate predictive models to estimate the number of COVID-19 patients hospitalized in the intensive care units and general wards of a private not-for-profit hospital in São Paulo, Brazil. Methods: Two main models were developed. The first model calculated hospital occupation as the difference between predicted COVID-19 patient admissions, transfers between departments, and discharges, estimating admissions based on their weekly moving averages, segmented by general wards and intensive care units. Patient discharge predictions were based on a length of stay predictive model, assessing the clinical characteristics of patients hospitalized with COVID-19, including age group and usage of mechanical ventilation devices. The second model estimated hospital occupation based on the correlation with the number of telemedicine visits by patients diagnosed with COVID-19, utilizing correlational analysis to define the lag that maximized the correlation between the studied series. Both models were monitored for 365 days, from May 20th, 2021, to May 20th, 2022. Results: The first model predicted the number of hospitalized patients by department within an interval of up to 14 days. The second model estimated the total number of hospitalized patients for the following 8 days, considering calls attended by Hospital Israelita Albert Einstein's telemedicine department. Considering the average daily predicted values for the intensive care unit and general ward across a forecast horizon of 8 days, as limited by the second model, the first and second models obtained R² values of 0.900 and 0.996, respectively and mean absolute errors of 8.885 and 2.524 beds, respectively. The performances of both models were monitored using the mean error, mean absolute error, and root mean squared error as a function of the forecast horizon in days. Conclusion: The model based on telemedicine use was the most accurate in the current analysis and was used to estimate COVID-19 hospital occupancy 8 days in advance, validating predictions of this nature in similar clinical contexts. The results encourage the expansion of this method to other pathologies, aiming to guarantee the standards of hospital care and conscious consumption of resources.

ABSTRACT Objective: This study analyzed the impact of sex on self-reported health and lifestyle parameters in peripheral artery disease patients at two periods of the COVID-19 pandemic. Methods: In this longitudinal study, 99 patients with peripheral artery disease (53 men and 46 women) were evaluated during two periods of the COVID-19 pandemic ( i.e ., at onset: May to August 2020, and on follow-up: May to August 2021). Patients were interviewed via telephone, and information regarding lifestyle and health parameters was obtained. Results: At the onset of the COVID-19 pandemic, health and habit parameters were similar between women and men, with 63.0% and 45.3% indicating frequent fatigue, 73.9% and 84.9% reporting increased sitting time, and 23.9% and 39.6% practicing physical activity, respectively. At follow-up, difficulties in physical mobility (women: from 26.1% to 73.9%, p<0.001; men: from 39.6% to 71.7%, p=0.001) and the frequency of hospitalization for reasons other than COVID-19 increased similarly in women and men (women: from 4.3% to 21.7%, p=0.013; men: from 9.4% to 24.5%, p=0.038). The other parameters were similar between the periods. Conclusion: Self-reported physical mobility difficulties and hospitalization frequency increased in women and men with peripheral artery disease.

ABSTRACT Objective To review the long-term outcomes (functional status and psychological sequelae) of survivors of critical illnesses due to epidemic viral pneumonia before the COVID-19 pandemic and to establish a benchmark for comparison of the COVID-19 long-term outcomes. Methods This systematic review of clinical studies reported the long-term outcomes in adults admitted to intensive care units who were diagnosed with viral epidemic pneumonia. An electronic search was performed using databases: MEDLINE®, Web of Science™, LILACS/IBECS, and EMBASE. Additionally, complementary searches were conducted on the reference lists of eligible studies. The quality of the studies was assessed using the Newcastle-Ottawa Scale. The results were grouped into tables and textual descriptions. Results The final analysis included 15 studies from a total of 243 studies. This review included 771 patients with Influenza A, Middle East Respiratory Syndrome, and Severe Acute Respiratory Syndrome. It analyzed the quality of life, functionality, lung function, mortality, rate of return to work, rehospitalization, and psychiatric symptoms. The follow-up periods ranged from 1 to 144 months. We found that the quality of life, functional capacity, and pulmonary function were below expected standards. Conclusion This review revealed great heterogeneity between studies attributed to different scales, follow-up time points, and methodologies. However, this systematic review identified negative long-term effects on patient outcomes. Given the possibility of future pandemics, it is essential to identify the long-term effects of viral pneumonia outbreaks. This review was not funded. Objective Considering the psychosocial repercussions of the COVID-19 pandemic on children, this case study aims to describe the work process involved in the construction and execution of the Children in the COVID-19 Pandemic module, which comprised the National Course on Mental Health and Psychosocial Care in COVID-19, hosted by the Fundação Oswaldo Cruz. Method Data from the materials that made up the module (booklet, video lesson, forum, and live broadcast), the profile of those enrolled, as well as emerging themes from participants' questions and comments (forum and live broadcast) were analyzed. Results Thirty-one percent of the course participants, who were predominantly female psychologists, completed the module. The booklet, developed with an accessible language, met the general guidelines for the development of manuals aimed at providing guidance on health care, seeking to address emerging demands in order to qualify the care for children in the context of COVID-19. Conclusion There was a great diversity of themes addressed in the material resulting from the forum and the live broadcast, which were discussed in order to contribute to the planning of psychosocial care strategies aimed at mitigating the negative repercussions of the pandemic on child development, as well as to offer a space for sharing knowledge and experiences on professional practice during this major public health emergency.


Objetivo Considerando as repercussões psicossociais da pandemia de COVID-19 às crianças, este estudo de caso tem o objetivo de descrever o processo de trabalho envolvido na construção e execução do módulo Crianças na Pandemia COVID-19, que compôs o Curso Nacional de Saúde Mental e Atenção Psicossocial na COVID-19, promovido pela Fundação Oswaldo Cruz. Método Analisaram-se dados provenientes dos materiais que compuseram o módulo (cartilha, videoaula, fórum e live), o perfil dos inscritos, bem como as temáticas emergentes em questionamentos e comentários dos participantes (fórum e live). Resultados Concluíram o módulo 31% dos participantes do Curso, os quais eram, predominantemente, mulheres psicólogas. A cartilha, desenvolvida em linguagem acessível, atendeu às diretrizes gerais para elaboração de manuais voltados à orientação sobre cuidados em saúde, buscando abordar demandas emergentes, no sentido de qualificar a atenção às crianças no contexto da COVID-19. Conclusão Houve grande diversidade de temáticas abordadas no material decorrente do fórum e da live, as quais foram discutidas com vista a contribuir para o planejamento de estratégias de atenção psicossocial voltadas a mitigar repercussões negativas da pandemia para o desenvolvimento infantil, bem como oferecer um espaço para compartilhamento de conhecimentos e experiências de atuação profissional frente a essa grave emergência de saúde pública.

Resumo Objetivo Identificar a frequência de lesão renal aguda (LRA) em pacientes hospitalizados com COVID-19, as características associadas, a mortalidade e a letalidade. Métodos Revisão realizada nas bases de dados CINAHL, Embase, LILACS, Livivo, PubMed, SCOPUS, Web of Science e, na literatura cinzenta (Google Acadêmico) em 12 de janeiro de 2022. Foram incluídos artigos em inglês, espanhol e português, publicados a partir de novembro 2019 até janeiro de 2022, em pacientes maiores de 18 anos com COVID-19 hospitalizados e LRA conforme critério Kidney Disease Improving Global Outcomes (KDIGO). Os estudos selecionados foram lidos na íntegra para extração, interpretação, síntese e categorização conforme nível de evidência. Resultados 699 artigos encontrados e 45 incluídos. A idade avançada, sexo masculino, hipertensão, doença renal crônica, ventilação mecânica, aumento da proteína C reativa, uso de drogas vasoativas e de determinadas classes de anti-hipertensivos foram associados a LRA. A LRA está relacionada à maior frequência de mortalidade. Em 30% dos pacientes hospitalizados com COVID-19 houve LRA. A taxa de mortalidade por LRA foi de 5% e a letalidade de 18%. Conclusão Estes resultados ressaltam a relevância da LRA como uma complicação significativa da COVID-19 e sugerem que um controle mais cuidadoso e precoce dos fatores associados poderia potencialmente reduzir a mortalidade e a letalidade. É crucial intensificar a pesquisa nesse campo para esclarecer melhor os mecanismos envolvidos na lesão renal em pacientes com COVID-19, bem como identificar estratégias terapêuticas mais efetivas para sua prevenção e tratamento nesse contexto.

Los estudios seleccionados fueron leídos en su totalidad para extracción, interpretación, síntesis y categorización según el nivel de evidencia. Resultados Se encontraron 699 artículos y se incluyeron 45. Los factores relacionados con la LRA fueron: edad avanzada, sexo masculino, hipertensión, enfermedad renal crónica, ventilación mecánica, aumento de la proteína C reactiva, uso de drogas vasoactivas y de determinadas clases de antihipertensivos. La LRA está relacionada con mayor frecuencia de mortalidad. En el 30 % de los pacientes hospitalizados con COVID-19 hubo LRA. La tasa de mortalidad por LRA fue de 5 % y la letalidad de 18 %. Conclusión Estos resultados resaltan la relevancia de la LRA como una complicación significativa de COVID-19 y sugieren que un control más cuidadoso y temprano de los factores asociados podría reducir potencialmente la mortalidad y la letalidad. Es crucial intensificar la investigación en este campo para explicar mejor los mecanismos relacionados con la lesión renal en pacientes con COVID-19, así como identificar estrategias terapéuticas más efectivas para su prevención y tratamiento en este contexto.


Abstract Objective To identify the frequency of acute kidney injury (AKI) in patients hospitalized with COVID-19, associated characteristics, mortality and lethality. Methods Integrative review carried out in the databases CINAHL, Embase, LILACS, Livivo, PubMed, SCOPUS, Web of Science and in the grey literature (Google Scholar) on January 12, 2022. Articles were included in English, Spanish and Portuguese, published from November 2019 to January 2022, in hospitalized patients over 18 years old with COVID-19 and AKI according to the Kidney Disease Improving Global Outcomes (KDIGO) criteria. The selected studies were read in full for extraction, interpretation, synthesis and categorization according to the level of evidence. Results A total of 699 articles were found and 45 included. Older age, male gender, hypertension, chronic kidney disease, mechanical ventilation, increased C-reactive protein, use of vasoactive drugs and certain classes of antihypertensives were associated with AKI. AKI is related to a higher frequency of mortality. AKI occurred in 30% of patients hospitalized with COVID-19. The mortality rate from AKI was 5% and the case fatality rate was 18%. Conclusion These results highlight the relevance of AKI as a significant complication of COVID-19 and suggest that more careful and early control of associated factors could potentially reduce mortality and lethality. It is crucial to intensify research in this field to better clarify the mechanisms involved in kidney injury in COVID-19 patients, as well as to identify more effective therapeutic strategies for its prevention and treatment in this context.
