RESUMEN

Objective To explore the diagnostic value of artificial intelligence (AI) cytology combined with DNA-image cytometry (DNA-ICM) auxiliary diagnostic system for the identification of benign and malignant pleural effusion and ascites. Methods Liquid-based cytology technology (LCT), DNA-ICM, AI, and AI combined with DNA-ICM were used to identify benign and malignant pleural effusion and ascites specimens in 360 cases, and their sensitivity, specificity, accuracy, Kappa value, Youden index and AUC were statistically analyzed. Results The sensitivity, specificity, and accuracy of AI combined with DNA-ICM in detecting benign and malignant pleural effusion and ascites were 95.23%, 94.12%, and 94.44%, respectively, which were higher than those of the three other separate detection methods (all P < 0.05). The kappa values of LCT, DNA-ICM, and AI were 0.646, 0.642, and 0.586; their Youden index values were 0.693, 0.687, and 0.676, and their AUC values were 0.846, 0.843, and 0.838, respectively. The Kappa value of AI combined with DNA-ICM was 0.869, the Youden index was 0.893, and AUC was 0.947, which were all higher than those of the three detection methods alone. Conclusion Among the three separate detection methods, LCT has the highest reliability, authenticity, and diagnostic value, and it can be used as a common method for the clinical identification of benign and malignant pleural effusion and ascites. The diagnostic performance of AI combined with DNA-ICM auxiliary diagnosis system in identifying benign and malignant pleural effusion and ascites is better than those of the three separate detection methods and can be used as a reliable method for the clinical identification of benign and malignant pleural effusion and ascites.

RESUMEN

Objective To determine the application value of quantitative detection of DNA in cervical lesion screening. Methods Retrospective analysis of 82 518 cases of cervical lesions was carried out.The sensitivity, specificity, positive predictive value (PPV),and negative predictive value (NPV) of DNA quantification and ThinPrep cytology test (TCT) for the diagnosis of cervical lesions were compared with the results of biopsy. The accuracy of the two methods was evaluated by applying the receiver operating characteristic curve. Results The sensitivity of DNA quantification,TCT,and combined diagnosis was 77.37%,70.97%,and 90.24%,respectively,and the specificity was 65.59%,70.42%,and 45.06%,respectively.The sensitivity of cell DNA quantitative analysis was higher but the specificity was lower than that of TCT,while the sensitivity of combined diagnosis was the highest. In DNA quantitative analysis of patients with and without atypical squamous cells of undetermined significance (ASCUS),the positive rates of pathology were 42.71% and 12.5%,respectively.The misdiagnosis rate of cell DNA quantitative analysis for screening cervical adenocarcinoma (39.13%) was higher than that for screening cervical squamous cell carcinoma (15.11%).In a cytologic smear, the misdiagnosis rate of cells < 5 000 (57.83%) was much higher than that of cells > 5 000 (22.63%).However,a combined diagnosis with TCT could reduce the misdiagnosis rate from 20.19% to 13.38%.Conclusion Cell DNA quantification is more sensitive than TCT,but the specificity of cell DNA analysis is lower than that of TCT,and the screening rate for cervical adenocarcinoma is high.Quantitative DNA analysis combined with TCT can significantly improve the screening sensitivity.

RESUMEN

Objective To investigate the application value of the liquid‐based cytology test (TCT) and the DNA quantitative analysis in cervical lesions screening .Methods 2 883 cases of TCT ,1 742 cases of DNA quantitative analysis and 333 cases of TCT combined with the DNA quantitative analysis were performed the retrospective analysis for investigating their clinical significance in diagnosing the cervical lesions .Results The positive coincidence rates of TCT ,DNA quantitative analysis and their combined detec‐tion were 43 .86% ,68 .04% and 81 .16% respectively .There was statistically significant difference in the positive coincidence rates for diagnosing CIN Ⅰand above between TCT and the DNA quantitative analysis (P<0 .01);the positive coincidence rates of the combined detection had statistical difference compared with TCT and the DNA quantitative analysis (P<0 .01) .The sensitivity and the specificity of TCT for discovering the cervical lesions were 69 .44% and 92 .42% respectively ;which of the DNA quantitative a‐nalysis were 85 .71% and 87 .89% respectively ;while which of combined detection were 96 .55% and 95 .89% respectively .Conclu‐sion The DNA quantitative analysis and TCT have the higher clinical diagnostic value in the cervical lesion screening .Their com‐bined detection can more effectively increase the detection rate of cervical lesions .

RESUMEN

Objective To compare the application and clinical significance of the liquid based cytology examination and the DNA quantitative analysis in female cervical lesions. Methods The cervical cell samples were collected from 879 women participating in the comparison by the cervical brush and performed the the liquid-based thin layer section preparation for conducting Papanicolaou staining and DNA staining respectively. The liquid based cytology examination was performed on the Papanicolaou staining section and the fully automatic scanning diagnosis was performed on the DNA staining section. Results The cases of above atypical squa-mous cells of undetermined significance(ASCUS)detected by the liquid based cytology examination and the partial cases of hetero-ploid cell detected by the fully automated DNA ploidy analysis system were recommended to further perform colposcopy and cervi-cal biopsy.28 women were performed the pathological biopsy. With the cytological examination result as the standard, the detection rate of above ASCUS cervical lesions detected by the cellular DNA quantitative analysis was calculated. Conclusion The combined application of the cellular DNA quantitative analysis method and the liquid based cytology examination can obviously increase the positive detection rate of cervical cancer and precancerous lesion, which has important significance for the prevention and treatment of female cervical cancer in our country.
