RESUMEN

Recurrent erythema nodosum leprosum (ENL) is a type 2 leprosy reaction, which often poses a challenge for the treating physician. Patient may be non-ambulatory due to systemic symptoms like fever, body aches, joint pains, and painful skin lesions. Moreover, drugs like corticosteroids and thalidomide in refractory cases pose an increased risk of deep vein thrombosis (DVT). We have described a case who developed DVT in due course of the treatment.

@#Objective To study the influence of bleeding after using low molecular weight heparin (LMWH) to prevent deep vein thrombosis (DVT) in patients with thoracolumbar spinal bursting fracture and spinal cord injury operated by rear internal fixation. Methods 120 cases were divided into the observation group (64 cases) and control group (56 cases). The observation group accepted hypodermic injection of LMWH to prevent DVT, while the control group did not. Results The volume of bleeding and blood transfusion, the change of red blood cell count (RBC) and hemoglobin were compared between two groups before and after operation. There is no statistical significance. Conclusion During the period of the operation of rear internal fixation, low molecular weight heparin cannot increase the bleeding and is safe to prevent deep vein thrombosis.

RESUMEN

PURPOSE: To evaluate the accuracy and availability of Multidetector-Row Computed Tomography (MDCT) and Color Doppler Sonography as a method for making an early diagnosis of DVT after joint arthroplasty. MATERIALS AND METHODS: A total of 71 cases (63 patients) were selected. 32 cases (32 patients) underwent a THRA and 39 cases (31 patients) underwent a TKRA between April 2004 to August 2004. All cases underwent both Color Doppler Sonography and MDCT, before and after surgery. No prophylactic medications for DVT were given. Patients who previously had DVT or pulmonary embolism or a medication history of anticoagulation therapy were excluded. RESULTS: DVT was found by MDCT in 33 cases (46.5%) and by Color Doppler Sonography in 15 cases (21.1%). In the 33 cases of DVT diagnosed by MDCT, 24 cases developed in the calf vein, among them, only 4 cases were positive and remaining 20 were negative in Color Doppler Sonography. In the 15 cases of DVT diagnosed by Color Doppler Sonography, 13 cases were positive and only 2 cases were negative in MDCT. CONCLUSION: MDCT is effective in diagnosing DVT after joint arthroplasty in terms of the objectivity, efficacy and accuracy.

RESUMEN

Objective To investigate the clinical effects of surgical treatment of acute deep vein thrombosis (DVT) in the lower limbs. Methods Twenty four cases with DVT were treated with a 2~3cm incision at the cross of superficial and deep femoral veins to take the thrombasis out. Moulding thrombasis were pulled up with ova circular tongs, and fragmented thrombi were spilled out of the incision with the prelum arteriol winding from foot to thigh. The artery was rinsed with heparinized saline for three minuets till the femoral vein was completely unobstructed. The vein was sutured, and the wound was drained under negative pressure. Results The near future effects: Limb swelling and pain disappeared in seventeen cases, conditions turned for the better in six cases, and there was no improvement in one case. The long term effects: Twenty one were followed up for an average of one year and three months. All patients had excellent walking and standing except two cases who suffered lower limb swelling and pain occasionally. Conclusions The operation of vein thrombectomy is a simple, easy and excellent approach to treat DVT, especially within six days after the the condition.
