Introducción: El virus chikungunya (CHIKV), es un alfavirus con genoma de ARN monocatenario, perteneciente a la familia Togaviridae, transmitido a través de la picadura de mosquitos del género Aedes sp., caracterizada por fiebre y dolores articulares incapacitantes. Objetivo: Determinar las características clínicas y epidemiológicas del virus chikungunya en pacientes pediátricos que acudieron a un Hospital Regional de Paraguay entre enero y junio del 2023. Material y Métodos: Estudio cuantitativo, observacional, descriptivo de corte transversal. Se incluyeron en el estudio todos los pacientes que presenten variables de interés; sexo, edad, procedencia, hallazgos clínicos y síntomas atípicos. Resultados: Se ha realizado la revisión de 465 fichas clínicas de pacientes con cuadro confirmado de chikungunya. La edad mínima de los pacientes fue de 4 días y la máxima de 17 años, la mitad de los pacientes estuvieron ubicados entre 1 y 10 años de edad. El 62,6% tuvo una presentación típica del cuadro de infección por chikungunya, el 37,4% tuvo una forma de presentación atípica. El 4,3% presentó complicaciones entre las que se citan encefalitis-meningoencefalitis y miocarditis. El 10,8% tuvo desenlace fatal, obitó. Conclusión: En cuanto a los datos demográficos, hubo predominio del sexo masculino y procedencia urbana. En cuanto a la presentación clínica, se reportaron formas típicas con fiebre, polialtralgias y mialgias. Las formas atípicas se presentaron con alteración del sistema cardiovascular, discrasia sanguínea y neumonías asociadas. Los casos que se consideraron complicados representan el 4,3% de la muestra y fue del 10,8% la mortalidad.
Introduction: Chikungunya virus (CHIKV) is an alphavirus with a single-stranded RNA genome belonging to the Togaviridae family. It is transmitted through the bite of mosquitoes of the Aedes sp. genus and is characterized by fever and disabling joint pain. Objetive: Determine the clinical and epidemiological characteristics of the chikungunya virus in pediatric patients who attended a Regional Hospital in Paraguay between January and June 2023. Materials and method: Quantitative, observational, descriptive cross-sectional study. All patients with variables of interest were included in the study; sex, age, origin, clinical findings and atypical symptoms. Results: A total of 465 clinical records of patients with confirmed chikungunya were reviewed. The minimum age of the patients was four days, and the maximum was 17 years; half of the patients were between 1 and 10 years of age. Male predominance in 52.7%, patients from urban areas in 72.5%. 62.6% had a typical presentation of chikungunya infection, and 37.4% had an atypical presentation. Fever, polyarthralgia, myalgia, headache, vomiting, and nausea predominated in the usual manner. In the atypical form, alterations of the cardiovascular system, bleeding dyscrasias, and pneumonia, among others, predominate. 4.3% presented complications, including encephalitis-meningoencephalitis and myocarditis. There was a death rate of 10.8%. Conclusion: Male sex predominated, coming from urban areas with typical presentation of chikungunya, with predominant symptoms of fever, polyarthralgia, and myalgia. Atypical form alteration of the cardiovascular system, bleeding dyscrasias, pneumonia. 4.3% presented complications, 10.8% died.

O uso indiscriminado de psicotrópicos tem importância econômica, social e requer atenção dos profissionais de saúde. Nessa perspectiva, a pesquisa tem como objetivo avaliar a prescrição e o perfil de utilização de psicotrópicos na Farmácia Básica do município de Catolé do Rocha/PB. O presente estudo é transversal, quantitativo do tipo descritivo. A coleta de dados foi realizada por meio de questionários aplicados no período de abril a maio de 2021. Foram entrevistados 200 usuários, selecionados com base em critérios de inclusão e exclusão. Os resultados apontaram a prevalência do sexo feminino (63,0%), casados (53,5%), residentes da zona urbana (70,5%), baixa escolaridade (56,5%) e aposentados (44,5%). Em suma, as prescrições eram obtidas em UBS (81,0%) por queixas de ansiedade (24,0%) e insônia (23,5%). Observou-se maior frequência de prescrição para clonazepam (18,6%) e alprazolam (15,9%), ambos considerados MPI. Estes foram prescritos principalmente por clínicos gerais (71,8%). Assim, evidencia-se a importância do farmacêutico na APS, tanto na sensibilização dos prescritores com relação à escolha e prescrição adequada destes medicamentos quanto na orientação dos pacientes com vistas à promoção do uso racional de psicotrópicos.
The use of psychotropic drugs has economic and social importance and requires attention from health professionals. he objective of this research was to evaluate the prescription and the profile of use of psychotropic drugs in the Basic Pharmacy in the municipality of Catolé do Rocha/PB. This is a cross-sectional, quantitative, descriptive study. Data collection was carried out through questionnaires applied from April to May 2021. Two hundred users were interviewed, selected based on inclusion and exclusion criteria. The results showed a prevalence of females (63.0%), married (53.5%), residing in the urban area (70.5%), with low education (56.5%), and retired (44.5%). The prescriptions were obtained at the Basic Health Unit (81.0%) for complaints of anxiety (24.0%) and insomnia (23.5%). We observed a higher frequency of prescriptions for clonazepam (18.6%) and alprazolam (15.9%), both considered potential inappropriate drugs. These were mainly prescribed by general practitioners (71.8%). Thus, the importance of pharmacists in Primary Health Care is evident both to increase the awareness among prescribers regarding the choice and proper prescription of medications and to guide patients with a view to promoting the rational use of psychotropic drugs.

Background: The study delves into the pressing issue of Road Traffic Accidents, a significant public health concern worldwide, particularly in low- and middle-income nations. With the backdrop of the 2030 Agenda for Sustainable Development, road safety has gained paramount importance. This research aims to comprehensively examine the multitude of factors influencing RTAs by analyzing data sourced from individual interviews, case summaries, and medico-legal records from a tertiary care hospital's Emergency Medicine Department. Methods: A hospital-based cross-sectional study conducted at JSS hospital, Mysuru. Data from 1525 retrospective cases spanning (2019-2021) and 114 prospective cases from December 2022 to January 2023 were analyzed. A pre-tested, semi-structured proforma gathered socio-demographic variables, etc. Analysis revealed a surge in accidents in 2021 compared to previous years, with a majority occurring between 8 pm to 8 am. Primarily in the 21-30 years age group. Over the three-year period, 72 fatalities were recorded, predominantly among males. Results: The study identified significant associations between RTAs and factors such as gender, vehicle type, mode of transportation, and referring hospital. Lower education levels correlated with increased risk, highlighting the importance of road safety education. Additionally, a striking urban-rural disparity was noted, with majority of RTAs occurring in rural areas. Conclusions: Overall, this study underscores the complex nature of RTAs and highlights the importance of addressing multiple risk factors to effectively mitigate road traffic injuries. By understanding these factors, policymakers and healthcare professionals can develop targeted interventions aimed at reducing the burden of RTAs and promoting safer road practices.

Background and objectives: During the SARS-CoV-2 pandemic, reduction in detection of other Respiratory Viruses (RV) was observed. Epidemiological studies are needed to understand the impact of the pandemic on the circulation of RV. The aim of this study is to analyze the epidemiological profile of cases of severe acute respiratory infection (SARI) associated with the main RV in hospitalized patients from Rio Grande do Sul, Brazil (RS), between 2010 and 2019 (period A) and between 2020 and 2021 (period B). Methods: Data related to SARI cases in RS were retrieved from SIVEP-Gripe. Results: In period A there were more infections with Influenza, Parainfluenza, Adenovirus and Respiratory Syncytial Virus, while in period B most cases were of SARS-CoV-2 infection. The most affected age groups were individuals <5 years old (67.1%) in period A, and >60 years old (50%) in period B. The main symptoms were fever and cough in period A, and dyspnea and O2 saturation <95% in period B. The most reported comorbidities were lung diseases and chronic cardiovascular diseases in period A, and chronic cardiovascular diseases and diabetes mellitus in period B. Importantly, a higher fatality rate was observed in period B. Most cases occurred between May and July in period A, and in November and December 2021 in period B. Conclusion: This study reveals that the COVID-19 pandemic changed the epidemiological profile of SARI in RS, and most cases were in the elderly with chronic cardiovascular disease and diabetes mellitus.(AU)
Justificativa e Objetivos: Durante a pandemia de SARS-CoV-2 foi observada redução na detecção de outros vírus respiratórios (VR). Estudos epidemiológicos são importantes para uma melhor compreensão dos impactos da pandemia sobre a circulação de VR. O objetivo deste estudo foi analisar o perfil epidemiológico dos casos de síndrome respiratória aguda grave (SRAG) associados aos principais VR em pacientes internados no Rio Grande do Sul, Brasil (RS), entre 2010 e 2019 (período A) e entre 2020 e 2021 (período B). Métodos: Dados relacionados a casos de SRAG no RS foram obtidos do SIVEP-Gripe. Resultados: No período A houve mais infecções por Influenza, Parainfluenza, Adenovírus e Vírus Sincicial Respiratório, enquanto no período B a maioria foi por SARS-CoV-2. Os grupos etários mais afetados foram de indivíduos <5 anos de idade (67,1%) no período A, e >60 anos (50%) no período B. Os principais sintomas foram febre e tosse no período A, e dispneia e saturação de O2 <95% no período B. As principais comorbidades no período A foram pneumopatias e cardiopatias, enquanto no período B foram cardiopatias e diabetes mellitus. A mortalidade foi maior no período B. A maioria dos casos no período A foram entre maio e julho, e no período B entre novembro e dezembro de 2021. Conclusão: Este estudo revela que a pandemia de COVID-19 alterou o perfil epidemiológico de SRAG no RS, sendo a maioria dos casos em indivíduos idosos com doença cardiovascular e diabetes. (AU)

Este estudio analizó variables epidemiológicas asociadas al Síndrome Respiratorio Agudo Grave (SRAG) en Rio Grande do Sul (RS), Brasil, antes de la aparición del SARS-CoV-2 (período A, 2010-2019) y durante la pandemia (período B, 2020-2021). Métodos: Los datos relacionados con los casos de SRAG en RS se obtuvieron de SIVEP-Gripe. Resultados: En el período A hubo más infecciones por Influenza, Parainfluenza, Adenovirus y Virus Respiratorio Sincitial, mientras que en el período B la mayoría de los casos fueron causados por SARS-CoV-2. Los grupos de edad más afectados fueron <5 años (67,1%) en el período A, y >60 años (50%) en el período B. Los principales síntomas fueron fiebre y tos en el período A, y disnea y saturación de O2 <95% en el período B. Las principales comorbilidades en el período A fueron enfermedades pulmonares y cardíacas, y en el período B fueron enfermedades cardíacas y diabetes mellitus. While the Internet has brought convenience and speed to human life, it has also led to frequent privacy violations. In the context of epidemiological investigations and information disclosure regarding confirmed Covid-19 patients, many individuals have utilized the Internet as a means to disseminate information and engage in cyber manhunts, resulting in breaches of privacy for those involved. This phenomenon is particularly prevalent within the realm of the Internet, where the boundaries of privacy invasion become blurred. Various types of privacy infringements, both active and passive negligence, are evident on social networking platforms. The juxtaposition of the virtual world of the Internet with real-life scenarios presents novel challenges in the realm of privacy violations. The Internet era, coupled with the widespread use and integration of big data, has diminished the absolute right to privacy on the Internet. This paper examines the challenge of safeguarding the identity information of infectious patients through the lens of two theoretical frameworks -Kantianism and Utilitarianism- in an effort to address this ethical dilemma.
Aunque Internet ha aportado comodidad y rapidez a la vida humana, también ha dado lugar a frecuentes violaciones de la intimidad. En el contexto de las investigaciones epidemiológicas y la divulgación de información relativa a pacientes confirmados de covid-19, muchas personas han utilizado Internet como medio para difundir información y participar en cibercacerías, lo que ha dado lugar a violaciones en la intimidad de los implicados. Este fenómeno prevalece en el ámbito de Internet, donde los límites de la invasión de la intimidad se vuelven vagos. En las redes sociales, se manifiestan diversos tipos de violaciones de la intimidad, tanto por negligencia activa como pasiva. La yuxtaposición entre el mundo virtual de Internet con escenarios de la vida real plantea nuevos retos en el ámbito de las violaciones de la intimidad. La era de Internet, junto con el uso generalizado y la integración del bigdata, han mermado el derecho absoluto a la privacidad. Este artículo examina el reto de salvaguardar la información sobre la identidad de los pacientes infecciosos a través de la lente de dos marcos teóricos -el kantianismo y el utilitarismo- en un esfuerzo por abordar este dilema ético.

No contexto de investigações epidemiológicas e divulgação de informações em relação a pacientes confirmados de Covid-19, muitos indivíduos utilizaram a Internet como um meio para disseminar informação e participar de uma caçada cibernética, resultando em violações da privacidade para aqueles envolvidos. Esse fenômeno é particularmente prevalente no âmbito da Internet, onde os limites de invasão da privacidade se tornaram borrados. Vários tipos de infrações da privacidade, tanto negligências ativa como passiva, são evidentes em plataformas de redes sociais. A justaposição do mundo virtual da Internet com cenários da vida real apresenta novos desafios no âmbito das violações da privacidade. A era da Internet, juntamente com o amplo uso e integração de megadados (big data), diminuiu o direito absoluto à privacidade na Internet. Esse artigo examina o desafio de proteger a informação de identidade de pacientes infectantes através das lentes de dois enquadres teóricos -Kantianismo e Utilitarismo- em um esforço para abordar esse dilema ético.

Abstract Objectives: Fungal infections (FI) pose a public health concern and significantly increase mortality rates, especially within Neonatal Intensive Care Units (NICU). Thus, this study aimed to investigate epidemiological indicators, risk factors, and lethality predictors associated with FI in a NICU. Methods: This study included 1,510 neonates admitted to the NICU of a reference hospital in Brazil between 2015 and 2022. Demographic data, such as sex, birth weight, gestational age, and use of invasive devices were analyzed. Results: Thirty neonates developed invasive FI, totaling 33 episodes and an incidence of 1.2 per 1,000 patient days. Candida albicans was the most frequent species (52.9 %), the bloodstream was the most affected site (78.9 %), and 72.7 % of infections occurred between 2015 and 2018. The lethality rate associated with FI was 33.3 %, and 90 % of deaths occurred within 30 days of diagnosis of infection. Weight < 750 g, prolonged hospital stay, use of parenteral nutrition, and broad-spectrum antimicrobials were independent risk factors for infection occurrence, especially glycopeptides and 4th generation cephalosporins, having a considerable role in the increase in fungal infections. Weight < 750 g was considered a significant predictor of lethality, and C. albicans had the highest lethality rate (40 %). Conclusion: These findings highlight the elevated lethality rate associated with these infections, reinforcing the importance of developing strategies to control FI within NICU.

Background: Childhood diarrhoea mainly caused by Group A Rotavirus, is a major global health issue, especially for children under five. In India, RVA-induced diarrhoea causes numerous deaths, hospitalizations, and outpatient visits annually. Vaccination is crucial in preventing RVA, with WHO-approved oral vaccines significantly reducing global mortality and morbidity. However, challenges persist in implementing vaccines in regions like sub-Saharan Africa due to factors like malnutrition and unsanitary conditions. Despite this, since 2009, low-income countries have seen a decline in RVA-related illness. Methods: Over 18 months, from January 2021 to June 2022, a study at the Post Grad Dept. of Microbiology, Index Medical College, Hospital and Research Centre in Indore, MP, involved 250 children under five with acute gastroenteritis. Ethical clearance and parental consent were obtained. Data included demographic, antenatal, diarrhoea l symptoms, feeding, hygiene, physical exams, and stool analysis. Results: 250 children under five were screened for Rotavirus. 60 tested positive, mostly in 6–12-month-olds during cooler months in urban areas. 80% were from low socioeconomic backgrounds. Exclusive breastfeeding linked to lower incidence. Vomiting and severe dehydration more frequent in positive cases. ELISA and ICG methods equally effective. Conclusions: Childhood diarrhoea, primarily caused by Rotavirus, remains a leading cause of under-five deaths, totalling 600,000 annually. Among 250 children studied, 60 tested positive for Rotavirus, especially among males aged 7-12 months, with infections peaking in cooler months. Both ELISA and ICG were equally effective in detection. Treatment primarily involves oral rehydration with low osmolarity ORS. Predominant strains were G1 P (8) and G2 P (4). Global endorsement of rotavirus vaccines like Rotarix and Rotateq, with Rotavac showing promise in India, signals progress in fighting rotavirus, potentially improving public health via inclusion in state immunization programs.

Objetivo: Analisar a incidência de sífilis congênita e descrever o perfil epidemiológico das mães e recém-nascidos diagnosticados no município de Sorocaba, SP. Métodos: Estudo descritivo e inferencial, utilizando dados provenientes da notificação compulsória de casos confirmados de sífilis congênita. A coleta dessas informações ocorreu no período de 2018 a 2022, a partir do banco de dados do Centro Municipal de Atenção Especializada. Resultados: Foram notificados 164 casos e a incidência de sífilis congênita foi de 3,66 novos casos. O perfil sociodemográfico das mães, predominantemente jovens, de etnia branca e com escolaridade desconhecida, reflete desafios na identificação e tratamento. A associação significativa entre o esquema de tratamento materno e a escolaridade, especialmente no ensino superior, destaca a importância do acesso à informação na prevenção da transmissão vertical. Apesar da cobertura universal do pré-natal, a presença majoritária de tratamentos inadequados ou não realizados sugere barreiras práticas e socioeconômicas. A significativa proporção de parceiros não tratados ou desconhecidos destaca a necessidade de abordagens abrangentes. Conclusão: Os resultados destacam a importância de estratégias integradas que ultrapassem o âmbito clínico. A implementação de medidas preventivas, educacionais e de apoio socioeconômico é imperativa para avançar em direção à redução efetiva da sífilis congênita em Sorocaba. (AU)
Objective: To analyze the incidence of congenital syphilis and to describe the epidemiological profile of mothers and newborns diagnosed in the city of Sorocaba, Southeastern Brazil. Methods: This was a descriptive and inferential study using data from the compulsory notification of confirmed cases of congenital syphilis. This information was collected from 2018 to 2022, using the database of the Municipal Center for Specialized Care. Results: 164 cases were reported and the incidence of congenital syphilis was 3.66 new cases. The sociodemographic profile of the mothers, who were predominantly young, of white ethnicity and with unknown schooling, reflects challenges in identification and treatment. The significant association between maternal treatment regimen and schooling, especially in higher education, highlights the importance of access to information in the prevention of mother-to-child transmission. Despite universal prenatal coverage, the majority of inadequate or non-performing treatments suggests practical and socioeconomic barriers. The significant proportion of untreated or unknown partners highlights the need for comprehensive approaches. Conclusion: The results highlight the importance of integrated strategies that go beyond the clinical scope. The implementation of preventive, educational and socioeconomic support measures is imperative to move towards the effective reduction of congenital syphilis in Sorocaba. (AU)

Métodos: Estudio descriptivo e inferencial con datos de la notificación obligatoria de casos confirmados de sífilis congénita. Esta información se recolectó de 2018 a 2022, utilizando la base de datos del Centro Municipal de Atención Especializada. Resultados: Se notificaron 164 casos y la incidencia de sífilis congénita fue de 3,66 casos nuevos. El perfil sociodemográfico de las madres, predominantemente jóvenes, de etnia blanca y con escolaridad desconocida, refleja dificultades en la identificación y el tratamiento. La asociación significativa entre el régimen de tratamiento materno y la escolaridad, especialmente en la educación superior, pone de relieve la importancia del acceso a la información en la prevención de la transmisión maternoinfantil. A pesar de la cobertura prenatal universal, la mayoría de los tratamientos inadecuados o ineficaces plantean barreras prácticas y socioeconómicas. La importante proporción de parejas no tratadas o desconocidas pone de relieve la necesidad de adoptar enfoques integrales. Conclusión: Los resultados ponen de manifiesto la importancia de las estrategias integradas que van más allá del ámbito clínico. La implementación de medidas de apoyo preventivo, educativo y socioeconómico es imprescindible para avanzar hacia la reducción efectiva de la sífilis congénita en Sorocaba. (AU)

Background: According to World Health Organization (WHO), rabies is a vaccine-preventable, zoonotic, viral disease caused by Lyssavirus type 1. It causes 20,000 deaths annually in India. MBBS interns represent the physicians who will graduate and practice medicine from next year. Hence, keeping this in mind, the present study was carried out to assess the knowledge regarding epidemiological determinants, prevention and management of rabies among MBBS interns. Methods: A cross-sectional study was conducted between April 2023 to June 2023 among 200 MBBS interns of Indira Gandhi Government Medical College, Nagpur, Maharashtra. Data was collected by the self-administered, pre-designed, pre-structured questionnaire and was analysed using statistical package for the social sciences (SPSS) software version 25. Results: It was seen that the mean age of participants was 23.34 (±0.859) years. The majority of participants were males (51.5%). The majority (80%) of interns knew the correct causative agent of rabies, while only 40% knew the correct incubation period of rabies. The majority, that is 63.5%, 85.5% and 75% of interns, knew the correct route, site and schedule of post exposure prophylaxis (PEP) respectively. Only 26.5% interns knew the correct management of category I animal bites, while 74.5% and 89% knew the correct management of category II and category III animal bites, respectively. The mean total knowledge score among interns was 13.17±2.63. Conclusions: The study found that interns with experience in dealing with animal bite cases had significantly higher total knowledge scores. To improve knowledge, interactive animal bite case-dealing sessions and regular CMEs should be conducted.

INTRODUCCIÓN: La comprensión del comportamiento de la respuesta humoral en COVID-19 continúa siendo un desafío para la producción de vacunas que proporcionen inmunidad más duradera. OBJETIVO: Describir la respuesta humoral natural inducida por SARS- CoV-2 en personal de salud con base en el perfil epidemiológico y clínico. MATERIALES Y MÉTODOS: Estudio transversal en personal de salud de hospitales públicos de referencia del Departamento de Alto Paraná, Paraguay. Se incluyeron 962 participantes, mediante muestreo no probabilístico de tipo consecutivo, aplicación de cuestionario y toma de muestras sanguíneas. Se buscaron anticuerpos por ensayo inmunocromatográfico para detección de IgM e IgG contra SARS- CoV-2 y por el método ELISA de captura de IgG específicos contra la proteína spike (SARS-CoV-2) y se evaluaron factores asociados a la seropositividad. RESULTADOS: La seroprevalencia global fue 36,5% (IC 95%: 33,4 - 39,5); 59,3% (n: 571) de los encuestados refirió haber tenido síntomas compatibles al COVID-19 entre el inicio de la pandemia y la fecha de toma de muestra, de estos 44% (n: 251) resultó seropositivo; 10,4% (n: 100) manifestó no haber tenido síntomas en el periodo estudiado, pero tuvo un resultado positivo. Los factores asociados a la seropositividad fueron: presencia de síntomas (p 90 días). CONCLUSIONES: Las características clínicas fueron mayormente asociadas con la seropositividad y la seropreva- lencia en los sintomáticos varió de acuerdo con el tiempo transcurrido desde el inicio de los síntomas y la serología.
BACKGROUND: Understanding the behavior of humoral response in COVID-19 continues to be a challenge to produce vaccines that provide long-lasting immunity. AIM: To describe the natural humoral response induced by SARS-CoV-2 among healthcare workers based on epidemiological and clinical profiles. METHODS: Cross-sectional study in healthcare workers from public hospitals in the Department of Alto Paraná, Paraguay, 962 participants were recruited through consecutive sampling, using a questionnaire and blood sampling. Antibodies were determined by immunochromatography assay for detection of IgM and IgG and by SARS-CoV-2 IgG anti-spike capture ELISA method and factors associated with seropositivity were evaluated. RESULTS: The overall seropositivity was 36.5% (95% CI: 33.4 - 39.5); 59.3% (n: 571) of respondents reported symptoms compatible with COVID-19 since the start of the pandemic and the date of blood draw, 44% (n: 251) of them tested positive; 10.4% (n: 100) who reported no history of symptoms tested positive. The factors associated with seropositivity were the presence of symptoms (p 90 days). CONCLUSIONS: Clinical characteristics were mostly associated with seropositivity and sero prevalence in symptomatic participants varied according to the time elapsed from the onset of symptoms to serology.

Objetivou-se analisar o perfil epidemiológico das notificações de acidentes de trabalho com exposição a materiais biológicos entre profissionais de enfermagem no Paraná, por meio da série histórica determinada pelos anos de 2013 a 2022. Estudo epidemiológico observacional descritivo do tipo levantamento, fundamentado na epidemiologia descritiva, a partir de dados do Sistema de Informações de Agravos de Notificação e do Cadastro Nacional de Estabelecimentos de Saúde. Foram Identificados 48.405 casos, com prevalência de 98,54/1.000 profissionais. Destaca-se maior ocorrência no ano de 2019, na Regional de Saúde Metropolitana, entre técnicos de enfermagem, do sexo feminino, com faixa etária entre 31 e 40 anos. Entre enfermeiros, a faixa etária prevaleceu entre 18 e 30 anos, com evolução de alta sem conversão sorológica e fontes negativas para doenças. Destaca-se o alto número de tratamentos abandonados e subnotificações. Investir na formação profissional, na educação permanente, em equipamentos de proteção de qualidade e na efetivação dos protocolos para tratamento profilático é fundamental para prevenir o agravo.
The objective was to analyze the epidemiological profile of notifications of work accidents with exposure to biological material among nursing professionals in Parana, through the historical series determined for the years 2013 to 2022. Descriptive observational epidemiological study of the survey type, based on descriptive epidemiology, based on data from the System Information on Notifiable Diseases and the National Registry of Health Establishments. 48,405 cases were identified, with a prevalence of 98.54/1,000. More occurrences among female nursing technicians aged between 31 and 40 years old, from the Metropolitan Health Regional Facilities, in year 2019, stand out. Among nurses, the age range was between 18 and 30 years old, with discharge progressing without serological conversion and negative sources of disease. The high number of abandoned treatments and underreporting stands out. Investing in professional training, continuing education, quality protective equipment and implementing protocols for prophylactic treatment are essential to prevent the disease.
El objetivo fue analizar el perfil epidemiológico de las notificaciones entre profesionales de enfermería de Paraná, a través de la serie histórica determinada para los años 2013 a 2022. Estudio epidemiológico observacional descriptivo, tipo encuesta, con base en epidemiología descriptiva, a partir de datos del Sistema de Información de Enfermedades de Notificación Obligatoria. Se identificaron 48.405 casos, con una prevalencia de 98,54/1.000. Sobresalen más ocurrencias en 2019 entre técnicos de enfermería, del sexo femenino, edades entre 31 y 40 años, en la Regional de Salud Metropolitana. Entre los enfermeros, el rango de edad estuvo comprendido entre 18 y 30 años, progresando el alta sin conversión serológica y focos negativos de enfermedad. Destaca elevado número de tratamientos abandonados y subregistro. Invertir en formación profesional, educación continua, equipos de protección de calidad e implementar protocolos de tratamiento profiláctico son fundamentales para prevenir la enfermedad.

Introdução: As doenças do sistema respiratório se mostram como uma das causas mais preocupantes de internações hospitalares no país. Nessa perspectiva, o perfil das internações por doenças respiratórias em crianças permite observar os parâmetros desta problemática, fornecendo um conhecimento amplo acerca do processo saúde e doença nessa população. Objetivo: Analisar o perfil epidemiológico das internações hospitalares por causas do aparelho respiratório em crianças de 0 a 9 anos no Brasil e regiões, entre os anos de 2013 e 2022. Metodologia: Trata-se de um estudo ecológico, realizado no Brasil, a partir de dados secundários do Sistema de Informações Hospitalares. A população escolhida para este estudo foram crianças de 0 a 9 anos deidade. Os dados foram coletados em 26 de novembro de 2022. As variáveis dependentes do estudo são as internações de crianças de 0 a 9 anos por doenças do aparelho respiratório no Brasil e suas regiões. Já, como variáveis independentes, tem-se o tempo, do período de janeiro de 2013 a setembro de 2022, regiões e faixa etária. Resultados: Há uma tendência de queda das internações por causa respiratória até o ano de 2016, seguido de um crescimento gradativo até 2019. Entretanto, em 2020, a taxa de hospitalização reduziu drasticamente em todas as localidades. As regiões Sul, Norte e Centro-Oeste permaneceram com taxas maiores que o Brasil em todo o período estudado. A internação em menores de 1 ano representa o maior quantitativo de internações sendo a pneumonia a causa mais prevalente. Conclusões: As taxas de internação infantil por doenças respiratórias representam importante preocupação para saúde pública. Assim, destaca-se a relevância da efetividade da Atenção Primária à Saúde, que possui grande impacto no desfecho dos adoecimentos em crianças, especialmente das doenças respiratórias (AU).
Introduction: Respiratory system diseases are one of the most worrying causes of hospital admissions in the country. From this perspective, the profile of hospitalizations for respiratory diseases in children allows us to notice the parameters of this problem, providing a broad understanding of the health and disease process in this population.Objective: To analyze the epidemiological profile of hospital admissions due to respiratory causes in children aged 0 to 9 in Brazil and its regions between 2013 and 2022. Methodology: This is an ecological study held in Brazil using secondary data from the Hospital Information System. The population chosen for this study consisted of children aged 0 to 9. Data were collected on November 26, 2022. The dependent variables of this study are hospitalizations of children aged 0 to 9 due to respiratory diseases in Brazil and its regions. The independent variables are time, from January 2013 to September 2022, regions, and age group.Results: There was a downward trend in hospitalizations due to respiratory causes until 2016, followed by a gradual increase until 2019. Nonetheless, in 2020, the hospitalization rate fell dramatically in all locations. The South, North and Mid-West regions remained with higher rates than Brazil throughout the studied period. Hospitalization of children under 1 year old represents the largest number of admissions, with pneumonia being Revista Ciência Plural. 2024; 10(1): e31414 3the most prevalent cause.Conclusions: Hospitalization rates during childhood due to respiratory diseases represent a major public health concern. Thus, one can highlight the importance of the effectiveness of Primary Health Care, which has a major impact on the outcome of illnesses in children, especially respiratory diseases (AU).

Las regiones Sur, Norte y Medio Oeste se mantuvieron con tasas másaltas que Brasil durante todo el período estudiado. Las hospitalizaciones en niños menores de 1 año representan el mayor número de internaciones, siendo la neumonía la causa más prevalente.Conclusiones: Las tasas de hospitalización infantil por enfermedades respiratorias representan un importante problema de salud pública. Así, se subraya la importancia de la eficacia de la Atención Primaria de Salud, que tiene un gran impacto en el resultado de las enfermedades en los niños, especialmente las respiratorias (AU).

Objective: To characterize cases of Surgical Site Infections (SSI) in patients undergoing hip and knee arthroplasties. Method: A cross-sectional, retrospective, and quantitative study conducted in a public, teaching, and high-complexity hospital in the southern region of Brazil. Data collection took place between 2020 and 2022 from records contained in 91 medical records and post-discharge forms within 90 days after surgery for prosthesis implan-tation. Data were analyzed by descriptive statistics and SSI incidence rate. Results: 49 (53.8%) knee arthroplasty records and 42 (46.2%) hip arthroplasty records were analyzed. Five cases developed infection, all detected at the post-arthroplasty knee outpatient visit, resulting in an SSI incidence rate in arthroplasties of 5.5% (n=5). Infections were characterized as deep incisional (40%; n=2), organ or cavity (40%; n=2), and superficial (20%; n=1), resul-ting in readmission in 80% of cases and a corresponding average hospitalization time of 11 days (SD=4.2). Conclusion: The significant rate of SSI in clean surgeries points to the need to intensify good surgical practices. Outpatient surveillance is emphasized as a strategy for building realistic indicators and providing support for prevention efforts. (AU)
Objetivo: Caracterizar los casos de Infecciones del Sitio Quirúrgico (ISQ) en pacientes sometidos a artroplastia de cadera y rodilla. Método: Investigación transversal, retrospectiva y cuantitativa realizada en un hospital público, docente y de alta complejidad en la región sur de Brasil. La reco-lección de datos se realizó entre 2020 y 2022 en registros contenidos en 91 historias clínicas y formularios posteriores al alta hospitalaria en un período de hasta 90 días después de la cirugía destinada al implante de la prótesis. Los datos fueron analizaron mediante estadística descriptiva y tasa de incidencia de ISQ. Resultados: Se analizaron 49 (53,8%) registros de artroplastia de rodilla y 42 (46,2%) de cadera. Cinco casos evolucionaron con infección, todos detectados en el seguimiento ambulatorio después de la artroplastia de rodilla, lo que resultó en una tasa de incidencia de ISQ en artroplastias del 5,5% (n=5). Las infecciones se caracterizaron como incisionales profundas (40%; n=2), de órgano o cavidad (40%; n=2) y superficiales (20%; n=1); resultando en reingreso en el 80% de los casos y el correspondiente tiempo promedio de hospitalización de 11 días (SD=4,2). Conclusión: La tasa significativa de ISQ en cirugías limpias apunta a la necesidad de intensificar las buenas prácticas quirúrgicas. Se reitera la vigilancia ambulatoria como estrategia para la construcción realista de indicadores y apoyo a la prevención. (AU)

Os dados foram analisados por estatística descritiva e taxa de incidência de ISC. Resultados: Foram analisados 49 (53,8%) regis-tros de artroplastia de joelho e 42 (46,2%) de quadril. Cinco casos evoluíram com infecção, todos detectados no retorno ambulatorial pós artroplastia de joelho, resultando em taxa de incidência de ISC em artroplastias de 5,5% (n=5). As infecções foram caracterizadas como incisional profunda (40%; n=2), de órgão ou cavidade (40%; n=2) e superficial (20%; n=1); decorrendo em reinternação em 80% dos casos e correspondente tempo médio de hospitaliza-ção de 11 dias (DP=4,2). Conclusão: O expressivo índice de ISC em cirurgias limpas direciona para a necessidade de intensificar boas práticas cirúrgicas. Reitera-se a vigilância ambulatorial como estratégia para a construção realística de indicadores e subsídio para a prevenção. (AU)

Objetivo: descrever e analisar os fatores de risco associados aos óbitos por COVID-19 no município de Barreiras-BA. Método: estudo de coorte em que foram analisadas as notificações de casos positivos da doença no período de março de 2020 a dezembro de 2022. As variáveis incluídas foram: sexo, idade, raça/cor, sintomas apresentados e condições de saúde. Para verificar a associação entre as variáveis, foi utilizada Regressão de Poisson, com estimativa do risco relativo. Resultados: Ocorreram 348 óbitos no período analisado, sendo estes mais frequentes em homens (60,6%), idosos (58,9%), pretos/pardos (88,1%). Os principais sintomas apresentados pelos indivíduos que vieram a óbito foram a tosse (67,8%), a dispneia (62,9%) e a febre (53,4%), e as principais comorbidades as doenças cardíacas (39,1%) e o diabetes (21,8%). Foram encontrados como fatores de risco associados à ocorrência do óbito ser do sexo masculino e idoso e ter apresentado sintomas como dispneia e febre, além de apresentar comorbidades como doenças respiratórias, cardíacas, diabetes, renais, imunossupressão, doenças cromossômicas e obesidade (p < 0,05). Ter tido sintomas como dor de garganta, dor de cabeça, coriza e distúrbios olfativos demonstraram menor risco de morte (p < 0,05). Conclusão: O número de mortes ocorridas por COVID-19 foi considerado baixo, com risco maior para homens e idosos e indivíduos com outras doenças prévias. Apresentação de sintomas mais graves, como febre e dispneia, também aumentam o risco de morte.
Objective: the objective of this study was to describe and analyze the risk factors associated with deaths from COVID-19 in the city of Barreiras-BA. Methods: a cohort study in which notifications of positive cases of the disease were analyzed from March 2020 to December 2022. The variables included were sex, age, race/color, presented symptoms, and health conditions. Poisson Regression was used to verify the association between variables with relative risk estimation. Results: There were 348 deaths in the period analyzed, these being more frequent in men (60.6%), elderly people (58.9%), black/brown people (88.1%). The main symptoms presented by the individuals who died were cough (67.8%), dyspnea (62.9%) and fever (53.4%), and the main comorbidities were heart disease (39.1%) and diabetes (21.8%). Risk factors associated with the occurrence of death were found to be male and elderly and having symptoms such as dyspnea and fever, in addition to presenting comorbidities such as respiratory, cardiac, diabetes, kidney diseases, immunosuppression, chromosomal diseases and obesity (p < 0 .05). Having had symptoms such as sore throat, headache, runny nose and olfactory disorders demonstrated a lower risk of death (p < 0.05). Conclusion: The number of deaths caused by COVID-19 was considered low, with a higher risk for men and the elderly and individuals with other previous illnesses. Presentation of more serious symptoms, such as fever and dyspnea, also increases the risk of death.

Objective To analyze the seasonal and epidemiological characteristics of community-acquired pneumonia (CAP) in Chuzhou from 2010 to 2022. Methods The epidemiological data of CAP in Chuzhou from 2010 to 2022 were obtained from the center for diseases control and prevention of Chuzhou City, Anhui Province. After inclusion and exclusion, a total of 1 053 cases were enrolled. General data were collected. Then the characteristics of CAP patients in terms of gender, age, regional and seasonal distribution and pathogenic bacteria distribution were analyzed. Results A total of 316 patients with CAP, with a prevalence rate of 30.01%, including 152 males (48.10%) and 164 females (51.90%). Regarding age, 86 cases (27.22%) at 19-40 years, 106 cases (33.54%) at 41-60 years, and 124 cases (39.24%) at >60 years, suggesting a statistical difference in the CAP detection rate among different genders and ages (P<0.05). Seasonally, 118 cases (37.34%) were detected in spring, 13 cases (4.11%) in summer, 49 cases (15.51%) in autumn, and 136 cases (43.04%) in winter, with the highest CAP detection rate in winter, followed by spring. Pathogenic bacteria were positive in 198 of 316 patients, with a detection rate of 62.66%. A total of 125 strains of pathogenic bacteria were detected in sputum culture, of which 138 cases were Gram-negative, mainly Escherichia coli (24.24%, 48/198), and 60 cases were Gram-positive, mainly Streptococcus pneumoniae (14.14%, 28/198). Among 198 patients positive for pathogen detection, 41 cases were detected in spring, 37 cases in summer, 56 cases in autumn and 64 cases in winter. The drug sensitivity results showed that Escherichia coli had the highest resistance rate to ampicillin and cefazolin, and was sensitive to imipenem and other antibiotics; Streptococcus pneumoniae has the highest resistance rate to penicillin and erythromycin, and is sensitive to vancomycin. Conclusion CAP is quite common in elderly population in Chuzhou from 2010 to 2022, with a high prevalence rate in spring and winter, and the prevention work of high-risk groups should be strengthened.

Objective To understand the epidemiological characteristics of new occupational pneumoconiosis in Zigong City from 2018 to 2022, and to provide the basis for further prevention and treatment of local pneumoconiosis. Methods The information of newly diagnosed and reported cases of pneumoconiosis in Zigong City from 2018 to 2022 was collected through the occupational disease and occupational health information monitoring system, and the characteristics of the distribution of pneumoconiosis in three regions, the composition of diseases and the length of service of exposure to dust were analyzed. Results From 2018 to 2022, the top 3 newly diagnosed pneumoconiosis diseases in Zigong City were silicosis, coal workers' pneumoconiosis and asbestosis. Silicosis cases were mainly distributed in small and medium-sized employers, accounting for 81.41%. Coal workers' pneumoconiosis was mainly distributed in large and medium-sized employers, accounting for 97.24%. Asbestosis mainly distributed in large scale employers, accounting for 96.36%. There was significant difference in dust handling age of different scale employers (H=11.453, P<0.05). The median ages of silicosis, coal workers' pneumoconiosis and other pneumoconiosis were 47.0 years, 52.0 years and 48.2 years, respectively. The median age of dust handling was 3.3 years, 22.0 years and 23.2 years, respectively. The age of onset of coal workers' pneumoconiosis was higher than that of silicosis and other pneumoconiosis (H=72.547, P<0.05), and the age of dust exposure of silicosis was shorter than that of coal workers' pneumoconiosis and other pneumoconiosis (H=10.453, P<0.05). Conclusion The current situation of pneumoconiosis in Zigong City is still severe, with obvious clustering in disease types and industries. Prevention and treatment of pneumoconiosis in key industries should be further strengthened to protect the health rights and interests of workers.

@#Objective To analyze the epidemiological and etiological characteristics of hand,foot and mouth disease(HFMD)in Honghe Prefecture,Yunnan Province from 2013 to 2021,and provide basis for the development of prevention and control strategies for HFMD. Methods The case data of HFMD in Honghe Prefecture,Yunnan Province from 2013 to2021 were extracted from the“China Disease Prevention and Control Information System”. Descriptive epidemiological analysis method was used to analyze the incidence rate,severe rate,mortality rate. The nucleic acids of enterovirus-A71(EV-A71)and Coxsackievirus-A16(CV-A16)were detected by Real-time PCR in throat swabs,anal swabs or stool samples from 16 586 cases of HFMD,and the etiological characteristics were analyzed. Results A total of 78 356 cases of HFMD were reported from 2013 to 2021 in Honghe Prefecture,Yunnan Province,with 354 severe cases and 11 deaths. The average annual incidence rate of HFMD was 187. 47/100 000,with an upward trend from 2013 to 2015,and reached the first peak in 2015,which showed a downward trend from 2016 to 2017,while increased,reached the second peak,and then gradually decreased and tended to be flat in 2018. The severe and death rates have been declining since 2015. The age group of 0~7 years old showed high incidence,which accounted for 97. 85%(76 670/78 356)of the total incidence,and the average annual incidence of 1~2 years old group was the highest(4 397. 98/100 000). Generally,the time distribution showed a bimodal distribution,which were from April to July(summer peak)and from October to December(autumn peak),and the peak in summer was higher than that in autumn. The number of cases and severe cases in males were significantly higher than those in females(χ~2= 1 154. 436 and 5. 183,respectively,each P < 0. 05). The average annual incidence and severe rate in the northern part of Honghe Prefecture were significantly higher than those in the southern part(χ~2= 18 573. 742 and 11. 036,respectively,each P < 0. 05). Among 16 586 HFMD cases,EV-A71,CV-A16 and other enteroviruses accounted for 18. 71%,20. 38% and 60. 91% of cases,respectively,with significant difference(χ~2= 5 426. 965,P < 0. 05). The dominant pathogen was EV-A71 in 2013 and 2015,while CV-A16 in 2014,and other enteroviruses became the dominant pathogens in 2016 — 2021. Conclusion The epidemic trend of HFMD in Honghe Prefecture,Yunnan Province from 2013 to 2021 was seasonal and regional,with children under the age of 7 being the key population,and other enteroviruses have become the dominant pathogens after 2016. HFMD prevention and control knowledge publicity should be strengthened,good hygiene habits should be advocated,and parents' awareness of disease prevention should be raised. EV-A71 vaccination should be strengthened to reduce EV-A71 infection and effectively prevent severe cases and deaths.

Background In recent years, regional high-temperature weather in summer occurs frequently in China. Heat stroke is the most representative meteorological disease caused by high temperature. In order to improve monitoring, early warning, prevention, and control of heat stroke, it is of great significance to understand the epidemiological characteristics of heat stroke and the associated impact of heatwave. Objective To understand the epidemiological characteristics of heat stroke cases in Jinan City, and to explore the effects of heatwave exposure on heat stroke. Methods Case reports of heat stroke and daily data of meteorological factors in Jinan City from 2017 to 2022 were collected. We described the temporal, population, and regional distribution characteristics of heat stroke cases in Jinan City, and used a time-stratified case-crossover design combined with conditional logistic regression model to explore the effects of heatwave exposure on heat stroke under 12 heatwave definitions (different combinations of intensity and duration). The cut-off percentiles used for heatwave definitions were the 90th (P90), 95th (P95), 97.5th (P97.5), and 99th (P99) percentiles of daily mean temperature; the durations were ≥ 2 d, ≥ 3 d, and ≥ 4 d, respectively. Pi(k), where i is temperature threshold, and k is duration. For example, the definition of a heatwave was notated as P90(2), indicating that the daily mean temperature is ≥ P90 and lasts for ≥ 2 d. Alternatively, lag01 denotes the cumulative lag effect with a 1 d lag, and so on. Results A total of 1394 cases of heat stroke were reported in Jinan City from 2017 to 2022, including 581 mild cases and 813 severe cases, and 85 deaths were reported, with a cumulative fatality rate of 6.10%. The cases of heat stroke reported each year during the study period were concentrated from June to August and peaked in July (665 cases, 47.70%). The sex ratio of males to females in heat stroke cases was 2.02:1. A high incidence of heat stroke was in 50-89 years, with a smaller peak occurring in the age group of 50-59 years and a larger peak in the age group of 70-79 years, respectively. The high-incidence areas of heat stroke were distributed in the western part of Jinan City where city centers situated (Tianqiao District, 274 cases, 19.66%; Huaiyin District, 223 cases, 16.00%) and in the surrounding rural areas (Pingyin County, 254 cases, 18.22%). The effect of heatwave exposure on heat stroke was statistically significant during the study period. The largest effect estimates for the effect on heat stroke occurred under the heatwave definitions of P99(2), P97.5(3), and P97.5(4) at lag04, lag03, and lag04, where corresponding OR (95%CI) values were 9.27 (4.71, 14.24), 8.95 (6.17, 12.98), and 8.22 (4.91, 13.78), respectively. The exposure-response curve showed that the risk of heat stroke tended to increase with the increase of average daily temperature. Conclusion July is the key period for the occurrence of heat stroke among Jinan City residents, while male cases are predominant, more serious cases, age concentration in the 50-89 years. The occurrence of heatwave can further increase the risk of heat stroke with a significant lag effect.

Hepatitis E virus infection is a common cause of acute viral hepatitis. In recent years, the incidence of hepatitis E has shown an increasing trend, which has gradually become an important cause of acute viral hepatitis worldwide. Age, sex, intensity of immunosuppression and socio-economic factors are all risk factors for hepatitis E virus infection. Liver transplant recipients require long-term use of immunosuppressive drugs for anti rejection treatment, prone to hepatitis E virus infection and at the risk of liver fibrosis and cirrhosis due to immunosuppression status. Therefore, special attention should be paid to liver transplant recipients in clinical practice. Meantime, related risk factors should be identified to assist diagnosis and take stricter preventive measures. According to literature review, the etiological characteristics of hepatitis E virus and the epidemiological characteristics, clinical manifestations, diagnosis and treatment of hepatitis E virus infection in liver transplant recipients were reviewed, aiming to properly monitor, treat and prevent hepatitis E virus infection in liver transplant recipients in clinical practice, improving the prognosis of liver transplant recipients.

To clarify the causal factors of a case with severe Chlamydia psittaci pneumonia in Jiujiang District, Jiangxi Province and provide a scientific basis for effective prevention and control. Basic information and epidemiological data of the case were collected by epidemiological survey, and the samples from the close contacts and environment were collected for pathogen detection. A 37-year-old female patient was admitted to the hospital due to "fever with cough for five days and worsening dyspnea". The results of targeted next-generation sequencing (tNGS) of pathogenic microorganisms in the bronchoalveolar lavage fluid of the case indicated that the patient was infected with Chlamydia psittaci. Our epidemiological investigation revealed a clear history of chicken and duck exposure. Four throat swab samples of close contacts and one spleen, liver and lung sample of sick chicken and duck were collected for testing. One of the duck tissue samples tested positive, while the other samples were negative. The severe pneumonia was likely caused by exposure to the external environment contaminated by Chlamydia psittaci. Poultry farming should be regulated to prevent the transmission infection from poultry to humans.Additionally, awareness among medical staff should be increased to prevent severe outcomes.
