RESUMEN

ObjectiveTo explore the clinical experience and characteristics of treating phlegm syndrome with Erchen Decoction (二陈汤) by ZHU Zhenheng, a famous doctor in Jin-Yuan Dynasty, and provide ideas for treating various diseases from phlegm in clinical practice. MethodsThis study comprehensively collected the medical cases and discussions of Erchen Decoction in treating phlegm syndrome in ZHU Zhenheng's six types of medical works. With the help of qualitative grounded theory method, pathogenesis and treatment were coded by Nvivo 11, and finally the pathogenesis and treatment system of ZHU Zhenheng using Erchen Decoction in the treatment of phlegm syndrome was constructed. ResultsA total of 48 medical case reports of ZHU Zhenheng using Erchen Decoction to treat phlegm were collected, having 52 discussions about Erchen Decoction, and involving 38 miscellaneous internal diseases such as vomiting, fullness, and stroke, six gynecological diseases, four pediatric diseases and four surgical diseases. According to the pathogenesis, ZHU Zhenheng mainly used Erchen Decoction in modifications to treat the tangible phlegm such as damp phlegm (28), cold phlegm (17), hot phlegm (17), wind phlegm (15), food phlegm (13), phlegm stasis (7), and phlegm-rheum (3), as well as invisible phlegm such as latent phlegm mainly composed of qi phlegm (27), and deficiency phlegm (15). Seven methods are suggested to treat corresponding syndromes based on the different pathogenesis, which are fortifying the spleen and reinforcing healthy qi (28), warming the center and dissipating cold (15), nourishing blood and supplementing yin (10), rectifying qi and relieving constraint (34), raising the clear and directing the turbid downward (11), dispelling wind and eliminating dampness (39), and discharging heat and moving stagnation (17). ConclusionZHU Zhenheng widely uses Erchen Decoction dissolving phlegm, draining phlegm and dispelling phlegm methods to treat internal, surgical, gynecological and pediatric diseases caused by tangible phlegm such as damp phlegm, qi phlegm, cold phlegm, hot phlegm, wind phlegm, deficiency phlegm and invisible phlegm.

RESUMEN

Objective To explore the targets, metabolites, and signal pathways of Erchen Decoction based on network pharmacology and to reveal the molecular mechanism of Erchen Decoction in the treatment of coronary heart disease (CHD) by “phlegm and blood stasis”. Methods TCMSP, TCM-PTD, and TCMID were used to find the main ingredients of Erchen Decoction. Targets of Erchen Decoction were predicted by using ChemMapper database. The targets related with coronary heart disease were obtained by searching through TTD, Drugbank, and DisGeNET databases. The common targets of disease-drug component were selected by intersecting disease targets with drug targets. A protein-protein interaction (PPI) network model was constructed using the STRING platform. The metabolites of above-mentioned and selected components were analyzed using the MetaboAnalyst database to screen out the main metabolites. MetScape and ClueGO APP in Cytoscape were used to analyze the gene function and metabolic pathway of Erchen Decoction in the treatment of CHD, and to construct the main drug component-metabolite-target-signal pathway topology network of Erchen Decoction. Results Eight main drug ingredients of Erchen Decoction, 56 predicted targets related with CHD, and 13 metabolites and 13 signal pathways related to the main components of Erchen Decoction were screened out by network analysis at the same time. The molecular network of compounds of Erchen Decoction-metabolites-CHD targets-signal pathways were constructed based on all the targets. The results showed that Erchen Decoction may play a therapeutic role in CHD by protecting vascular endothelium, anti-oxidative stress, anti-inflammatory and injury repair by multi-targets and multi-pathways. Conclusion This research explores the possible molecular mechanism of Erchen Decoction in the treatment of CHD by “phlegm and blood stasis” and reveals the pharmacological effects of Erchen Decoction to provide direction for carrying out the follow-up evaluation of curative effect of Erchen Decoction in clinic.

RESUMEN

Erchen Decoction is widely used to dampness-phlegm syndrome and it can be flexibly used in clinical application by my supervisor Professor OU Zheng-wu, especially in cough, pneumonia, asthma and influenza, which has obvious efficacy in treatment. This article reviewed five medical cases during the follow-up process with Professor OU in application of Erchen Decoction, with a purpose to share experience of famous doctors and improve TCM diagnosis and treatment level together.

RESUMEN

Objective To explore the effects of the Chinese medicine, modified Erchen decoction, on the serum lipid spectrum of ApoE-/-mice, and to explore its possible anti-atherosclerotic mechanism.Methods Forty-four male 7-8-week old ApoE-/-mice were used in this experiment.ApoE-/-mouse models of atherosclerosis were generated by high-cholesterol diet for 4 weeks.And then, they were given simvastatin or modified Erchen decoction by gavage.The body weight of mice was recorded every week, The mice were sacrificed after treated with the drugs for 8 weeks continuously, and the plasma lipid was determined by enzymatic method.The aortic valves and arches were stained with oil red O to depict atherosclerotic plaques and liver structural changes of the mice were examined by pathology.Results Modified Erchen decoction lowered plasma lipid ( including TCHOL and LDL-C ) significantly ( P<0.01 ) .The body weight was increased in the mice of all groups, but it was more pronounced in the mice of model group than in the blank and modified Erchen decoction groups.The serum CHOL and LDL-C levels were significantly lowered in the modified Erchen decoction group (P<0.01).The area of atherosclerotic plaques in the aortic wall was significantly reduced in the mice of modified Erchen decoction group as shown by oil red O staining.The pathological changes of hepatocytes were less severe and the structure of hepatic lobules was better preserved in the mice of modified Erchen decoction group.Conclusions The Chinese medicin modified Erchen decoction can effectively reduce serum lipids, regulate lipid metabolism, and ameliorate the process of atherosclerosis in ApoE-/-mice.

To explore indications regularity of analogous decoctions of Erchen Decoction.323 analogous decoctions of Erchen Decoction were found in Concise Thesaurus of TCM Formula.Diseases and symptoms of TCM were analyzed.22 diseases and 118 symptoms of TCM were found.The main indication of analogous decoctions of Erchen Decoction included spleen-stomach and lung system,other symptoms were broadly involved.Erchen Decoction was the most appropriate for phlegm.
