RESUMEN

A proptose do globo ocular é uma das consequências comuns do trauma e a enucleação é um procedimento de escolha em caso de impossibilidade de reversão do quadro. Nota-se a infrequência de relatos de enucleação do globo ocular resultante de proptose traumática, o que torna importante a descrição deste caso, a qual objetiva fornecer relevantes informações e contribuições para o desenvolvimento da oftalmologia e clínica cirúrgica veterinária. Uma cadela filhote foi atendida no HVU - UFPI/CPCE, apresentando o globo ocular direito prolapsado. A enucleação foi selecionada em decorrência do intervalo prolongado entre a detecção da lesão e a busca por assistência médica, da presença de uma alta carga de corpos estranhos observados e da ausência de reflexos pupilares. A cirurgia iniciou-se com a cantotomia seguida da dissecação da musculatura do globo ocular. Foi realizado o pinçamento dos vasos sanguíneos e do nervo óptico, e fez-se a ressecção do globo ocular. Depois de uma ligadura invaginante e redução do espaço morto, removeu-se as bordas palpebrais e realizou-se a blefarorrafia. Cerca de 40 dias após a enucleação, a cadela apresentou-se estável e com uma evolução cicatricial satisfatória do ferimento cirúrgico. Esse procedimento, foi realizado de forma semelhante ao que é visto na literatura, embora, majoritariamente, seja recomendada a enucleação em decorrência de afecções diferentes da proptose traumática.


Proptosis of the eyeball is one of the common consequences of trauma and enucleation is the procedure of choice if it is impossible to reverse the condition. There are few reports of enucleation of the eyeball resulting from traumatic proptosis, which makes it important to describe this case, which aims to provide relevant information and contributions to the development of ophthalmology and veterinary surgical practice. A female puppy was seen at the HVU - UFPI/CPCE, presenting with a prolapsed right eyeball. Enucleation was selected due to the prolonged interval between detecting the lesion and seeking medical assistance, the presence of a high foreign body burden and the absence of pupillary reflexes. Surgery began with canthotomy followed by dissection of the eyeball muscles. The blood vessels and optic nerve were clamped and the eyeball was resected. After an invaginating ligature and reduction of the dead space, the eyelid edges were removed and blepharorrhaphy was performed. Around 40 days after enucleation, the dog was stable and had satisfactory healing of the surgical wound. This procedure was carried out in a similar way to that seen in the literature, although enucleation is mostly recommended for conditions other than traumatic proptosis.


La proptosis del globo ocular es una de las consecuencias comunes de los traumatismos y la enucleación es el procedimiento de elección si es imposible revertir la condición. Existen pocos relatos de enucleación del globo ocular resultante de proptosis traumática, lo que torna importante la descripción de este caso, que pretende proporcionar informaciones relevantes y contribuciones para el desarrollo de la oftalmología y de la práctica quirúrgica veterinaria. Una cachorra fue atendida en el HVU - UFPI/CPCE con prolapso del globo ocular derecho. Se optó por la enucleación debido al prolongado intervalo entre la detección de la lesión y la búsqueda de asistencia médica, la presencia de una elevada carga de cuerpo extraño y la ausencia de reflejos pupilares. La cirugía comenzó con una cantotomía seguida de la disección de los músculos del globo ocular. Se pinzaron los vasos sanguíneos y el nervio óptico y se resecó el globo ocular. Tras una ligadura invaginante y la reducción del espacio muerto, se retiraron los bordes de los párpados y se realizó una blefarorrafia. Unos 40 días después de la enucleación, el perro estaba estable y la herida quirúrgica había cicatrizado satisfactoriamente. Este procedimiento se llevó a cabo de forma similar a lo visto en la bibliografía, aunque la enucleación se recomienda sobre todo para afecciones distintas de la proptosis traumática.

Purpose: To discuss the novel swept?source anterior segment optical coherence tomography (SS?ASOCT)?guided surgical approach in slipped medial rectus muscles. Methods: Prospectively (between February 2020 and July 2022), six patients with a clinical suspicion of slipped medial rectus muscle were recruited. After complete ophthalmic and orthoptic evaluation, the missing medial rectus muscle is screened using Anterior Segment Optical Coherence Tomography (ASOCT). In presence of a traceable muscle, its morphology, depth, and distance from a fixed anatomical landmarks were noted; in its absence, the status of other recti was noted. Intraoperatively, the features were confirmed and the intended intervention was performed. Results: The mean age of six patients was 25.66 ± 9.72 years, two with surgical trauma and four with penetrating trauma (66.66%). In five patients, the ASOCT traced the slipped medial rectus muscle successfully (83.33%); intraoperatively, the same was confirmed (within 1–2 millimeters) with favorable outcomes. ASOCT made a significant contribution in all subjects by reducing the number of interventions and muscle surgeries. Conclusions: In eyes with slipped medial rectus muscle, especially those which are within a finite distance from the angle can be traced using ASOCT. This approach impacts the outcomes in many ways

Purpose: To evaluate safety profile and surgical outcomes of loop myopexy with concurrent intra? ocular lens implantation in cases of myopic strabismus fixus (MSF). Methods: A retrospective chart review of patients who underwent loop myopexy with concurrent small incision cataract surgery with intra?ocular lens implantation between January 2017 and July 2021 for MSF at a tertiary eye care centre was undertaken. A minimum of 6 months of follow?up after surgery was required for inclusion. The main outcome measures were improvement in alignment postoperatively, improvement in extra?ocular motility postoperatively, intraoperative and postoperative complications and post?operative visual acuity. Results: 12 eyes of 7 patients (male (6): female (1)) underwent modified loop myopexy at a mean age of 46.86 years (range 32?65 years). 5 patients underwent bilateral loop myopexy with intra?ocular lens implantation whereas 2 patients underwent unilateral loop with intra?ocular lens implantation. All eyes underwent additional medial rectus (MR) recession with lateral rectus (LR) plication. At the last follow?up, mean esotropia improved to 16 prism dioptres (PD) (Range: 10?20 PD) from 80 PD (Range:60?90PD), P = 0.016; and success (deviation ?20PD) was achieved in 73% (95% CI 48 to 89%). Mean hypotropia at presentation was 10 PD (range 6?14 PD), which improved to 0 PD (range 0?9 PD), P = 0.063. Mean BCVA improved from 1.08 LogMar to 0.3 LogMar units. Conclusion: Loop myopexy combined with intra?ocular lens implantation is a safe and effective procedure in the management of patients who have Myopic Strabismus Fixus with visually significant cataract and improves both visual acuity and ocular alignment significantly.

Learning about human eye movements broadens our comprehension of the visuomotor system and aids in the effective management of strabismus. One’s clinical practice is improved by a dynamic simulation of human eye movements using physical models of the extraocular muscles (EOMs). We use our eyeball model to teach the basics of strabismus to undergraduate students and ophthalmology residents. In Listing’s plane, extraocular movements of each muscle and the angle demonstration are being used to familiarize students with their knowledge. The degree of the residents’ understanding of strabismus is significantly influenced by the eyeball strabismus simulator. This model is an inexpensive, Do It Yourself (DIY) model that is simple to build.

Thyroid-associated ophthalmopathy(TAO)is an autoimmune inflammatory disease involving multiple orbital tissues with a variety of clinical manifestations, which has serious effects on the life quality of patients.Interventions of TAO mainly include medical treatment to stabilize thyroid function, reduce inflammation and regulate immune function, as well as surgical treatment to relieve ocular symptoms. Botulinum toxin type A can paralyze muscles by blocking nerve impulse conduction at the neuromuscular junction, which is of certain therapeutic value for restrictive strabismus due to extraocular muscle involvement and upper eyelid retraction due to involvements of levator palpebrae superioris and Müller's muscle in TAO patients, especially when they have surgical contraindications, lack surgical opportunity, or refuse surgery. This paper reviews the application of botulinum toxin type A in the treatment of TAO, focusing on its pharmacological mechanism, dosage, effectiveness, and possible complications when treating restrictive strabismus and upper eyelid retraction, and discussing potential therapeutic values of botulinum toxin type A for intraocular pressure elevation, glabellar frown lines and dry eye caused by extraocular muscle compression in TAO patients, in order to provide a reference for clinical intervention.

As a primary treatment for strabismus, extraocular muscle surgery can achieve the purpose of correcting the eye position, improving the appearance and reconstructing the third-level visual function. Previous studies have found that the vascular density(VD)and thickness of retina increased in the early stage after extraocular muscle surgery, where multiple mechanisms involved. In recent years, with the appearance of detection means such as optical coherence tomography angiography(OCTA), our quantitative understanding of retinal microscopic changes and their mechanisms brought about by traditional extraocular muscle surgery has become more and more profound. The increase of retinal VD in the early postoperative period may be closely related to the recovery of postoperative visual function. However, the related studies are few, and the association between microscopic changes and visual function after extraocular muscle surgery and its mechanism need to be further clarified. This article will review the microscopic changes of retina and their mechanisms after extraocular muscle surgery from multiple perspectives to improve our understanding of the relationship between the mechanism of its influence and visual function, with a view to provide references for the choice of extraocular muscle surgery scheme and related clinical research.

Purpose: To assess the diagnostic accuracy of forced eyelid closure test (FECT), ice pack test (IPT), repetitive nerve stimulation test (RNS), and acetylcholine receptor (AchR) antibody test in patients with suspected ocular myasthenia. To assess the clinical utility of AchR antibody test in predicting disease progression. Methods: Retrospective cohort study of patients diagnosed with ocular myasthenia at a South?Indian neuro?ophthalmology tertiary eye clinic. Baseline characteristics; ocular myasthenia symptoms; results of FECT, IPT, RNS, and AchR antibody test; and progression time to generalized myasthenia (GM) over 36 months from the time of diagnosis were recorded and analyzed using receiver operator curve analysis, multiple logistic regression, and Kaplan朚eier survival analysis. Results: FECT had a sensitivity of 96.7% (95% CI: 88.5�.6) and a specificity of 75% (95% CI: 34.9�.8). Combination of FECT and IPT, using the positivity of at least one test, increased the sensitivity to 98.3% (95% CI: 91�0), reducing the specificity to 50% (95% CI: 15.7�.3), whereas using the positivity of both tests, we obtained a sensitivity of 71.7% (95% CI: 58.6�.5) and a specificity of 100% (95% CI: 63.1�0). In the subset of patients with double negative RNS and AchR antibodies, the positive predictive value of combined FECT and IPT (double positive) was 100%. Patients who developed GM were more likely to have a positive AchR antibody test result (P = 0.001). Conclusion: Combined FECT and IPT (double positive) has high diagnostic accuracy even among patients with normal RNS and negative AchR antibodies. Despite low sensitivity, AchR?antibody test has a significant predictive value in disease progression.

La fibrosis congénita de los músculos extraoculares se caracteriza por una oftalmoplejía externa congénita, no progresiva, generalmente bilateral, con ptosis y alteración de los movimientos oculares. Existen varios tipos en dependencia del gen afectado y puede estar acompañada de hallazgos adicionales tanto oculares como generales. Para el manejo de los casos se realiza cirugía de la ptosis, alineación ocular y corrección del torticolis, además de su corrección óptica y rehabilitación para maximizar el resultado visual. Con el objetivo de estar al día sobre esta enfermedad se realiza una revisión de las publicaciones de los últimos cinco años en este tema. Actualmente no hay tratamientos que puedan restaurar la funcionalidad completa y el rango de movimiento de los músculos extraoculares, pero mejorar su funcionalidad visual es primordial(AU)


Congenital fibrosis of the extraocular muscles is characterized by congenital external ophthalmoplegia, nonprogressive, usually bilateral, with ptosis and altered eye movements. There are several types depending on the affected gene and it may be accompanied by additional ocular or general findings. For the management of these cases, ptosis surgery, ocular alignment and torticollis correction are performed, as well as optical correction and rehabilitation to maximize visual outcome. In order to be updated about this disease, a review of the publications on this subject within the last five years is carried out. Currently, there are no treatments that can restore full functionality and range of motion of the extraocular muscles, but improving their visual functionality is paramount(AU)

SUMMARY: The purpose of this study was to reveal the overall distribution pattern of the intramuscular nerves of each extraocular muscle and provide morphological guidance for the selection of the neuromuscular compartment during extraocular muscle transplantation and target localization of the botulinum toxin A injection to correct strabismus. We studied 12 Chinese head specimens that were fixed with formalin. The extraocular muscles from both sides of each head were removed, and a modified Sihler's staining technique was used to reveal the overall distribution pattern of the intramuscular nerves. We observed an intramuscular nerve-dense region formed by the intramuscular arborized branches in the semitransparent superior rectus, inferior rectus, medial rectus, lateral rectus, superior oblique, inferior oblique, and levator palpebrae superioris muscles with Sihler's staining technique. The seven extraocular muscles can each be divided into two neuromuscular compartments. The intramuscular nerve-dense regions of the superior, inferior, medial, and lateral rectus and the superior oblique, inferior oblique, and levator palpebrae superioris muscles were positioned at 33.50 % -72.72 %, 40.21 % - 66.79%, 37.92 % - 64.51 %, 31.69 % - 56.01 %, 26.35 % - 64.98 %, 40.46 % - 73.20 %, and 27.72 % - 66.07 % of the lengths of the muscle bellies, respectively, and the centers of intramuscular nerve dense regions were located at 59.50 %, 54.18 %, 51.68 %, 50.08 %, 48.38 %, 56.49 %, and 50.77 % of the length of each muscle belly, respectively. The aforementioned values are the means of the actual values. These results suggest that when the strabismus is corrected with muscle transplantation, the extraocular muscle should be transplanted based on the neuromuscular compartment, which would benefit the function of both donor and recipient muscles. The localization of these nerve dense regions is recommended as an optimal target for the injection of botulinum toxin A to treat strabismus.


RESUMEN: El objetivo de este estudio fue revelar el patrón de distribución de los nervios intramusculares de cada músculo extraocular y, proporcionar una guía morfológica para la selección del compartimento neuromuscular durante el trasplante de músculo extraocular, y la localización de la inyección de toxina botulínica A para corregir el estrabismo. Estudiamos 12 muestras de cabezas de individuos chinos fijadas en formalina. Se extrajeron los músculos extraoculares de ambos lados de cada cabeza y, se utilizó una técnica de tinción de Sihler modificada para revelar el patrón de distribución general de los nervios intramusculares. Observamos una región densa en nervios intramusculares formada por los ramos intramusculares en los músculos recto superior semitransparente, recto inferior, recto medial, recto lateral, oblicuo superior, oblicuo inferior y elevador del párpado superior con técnica de tinción de Sihler. Los siete músculos extraoculares se pueden dividir cada uno en dos compartimentos neuromusculares. @#AIM: To investigate the related-factors analysis of extra-ocular muscle paralysis in pediatric ocular myasthenia gravis(OMG). <p>METHODS: A retrospective, observational cohort study was performed of 203 pediatric patients diagnosed with OMG between Nov.2011 and May 2020 at Wuhan Children's Hospital. Data on clinical features and laboratory indicators of children with OMG were statistically analyzed between extra-ocular muscle paralysis group and non-extra-ocular muscle paralysis group, which were classified by pediatric ophthalmologists based on ocular manifestations. The Logistic regression analysis was performed immediately after that to determine the independent factors. <p>RESULTS: Totally 203 pediatric OMG, 97(47.8%)had extra-ocular muscle paralysis with various clinical features including strabismus(<i>n</i>=69, 71.1%), torticollis(<i>n</i>=18, 18.6%). Among the 97 children, 79(81.4%)patients were presented with monocular involvement, of which 53(54.6%)had single extra-ocular muscle paralysis. The most common was medial rectus muscle(<i>n</i>=19, 35.8%). Between extra-ocular muscle paralysis group and non-extra-ocular muscle paralysis group, there were statistically significant difference(<i>P</i><0.05)in age, serum immunoglobulin M(IgM), serum free triiodothyronine(FT3), serum thyroglobulin, and glucocorticoid combination pyridostigmine bromide therapy(72.2% <i>vs</i> 38.7%). Furthermore, the level of FT3(<i>OR</i>=2.006, 95%<i>CI</i>:<i> </i>1.233-3.263)and glucocorticoid combined treatment(<i>OR</i>=4.328, 95%<i>CI</i>: 1.936-9.677)were the related-factors affecting the extra-ocular muscle paralysis of children with ocular myasthenia gravis. <p>CONCLUSION: Extra-ocular muscle paralysis was the common ocular manifestation in pediatric OMG. Monocular involvement was more common than binocular involvement especially the medial rectus muscle, while diplopia was rare. The level of FT3 can be considered as an important immune indicator to evaluate extra-ocular muscle paralysis in children with ocular myasthenia gravis.

OBJECTIVE: We aimed to investigate the ability of readout-segmented echo-planar imaging (rs-EPI)-based diffusion tensor imaging (DTI) in assessing the microstructural change of extraocular muscles (EOMs) and optic nerves in patients with thyroid-associated orbitopathy (TAO) as well as in evaluating disease activity.MATERIALS AND METHODS: We enrolled 35 TAO patients and 22 healthy controls (HCs) who underwent pre-treatment rs-EPI-based DTI. Mean, axial, and radial diffusivity (MD, AD, and RD) and fractional anisotropy (FA) of the medial and lateral EOMs and optic nerve for each orbit were calculated and compared between TAO and HC groups and between active and inactive TAO groups. Factors such as age, sex, disease duration, mediation, and smoking history between groups were also compared. Logistic regression analysis was used to evaluate the predictive value of significant variables for disease activity.RESULTS: Disease duration was significantly shorter in active TAOs than in inactive ones (p < 0.001). TAO patients showed significantly lower FA and higher MD, AD, and RD than HCs for both medial and lateral EOMs (p < 0.001), but not the AD value of lateral EOMs (p = 0.619). Active patients had significantly higher FA, MD, and AD than inactive patients for medial EOMs (p < 0.005), whereas only FA differed significantly in the lateral EOMs (p = 0.018). The MD, AD, and RD of optic nerves were significantly lower in TAO patients than HCs (p < 0.05), except for FA (p = 0.129). Multivariate analysis showed that the MD of medial EOMs and disease duration were significant predictors for disease activity. The combination of these two parameters showed optimal diagnostic efficiency for disease activity (area under the curve, 0.855; sensitivity, 68.4%; specificity, 96.9%).CONCLUSION: rs-EPI-based DTI is promising in assessing microstructural changes of EOMs and optic nerves and can help to indicate the disease activity of TAO, especially through the MD of medial EOMs.

@#AIM: To investigate the corneal curvature and refractive changes after microscopically extraocular muscle surgery in adults.<p>METHODS: To observe 62 cases of 85 eyes underwent microscopic extraocular muscle surgery in the Second People's Hospital of Yunnan Province from December 2016 to May 2017, age 18 to 43, according to the surgical method, patients were divided into horizontal rectus surgery: Ⅰ group(simple migration group)32 eyes, Ⅱ group(rectus monocular migration combined with its antagonist muscle excision group)34 eyes, the oblique muscle group: Ⅲ group(single oblique muscle tendon broken group)10 eyes, Ⅳ group(oblique muscle transposition group)9 eyes; Subjective optometry was performed preoperatively and postoperatively at 1d, 1wk, 1mo, 3mo and 6mo, respectively, to obtain the refractive state changes and the curvature value of the anterior surface of the cornea(0-3mm, 5mm, 7mm)collected by corneal topography. The dynamic changes of the refractive state and corneal morphology of the patients in each group at different time points were compared. Anova and LSD-<i>t</i> test were used for comparison among time points and within each group.<p>RESULTS: Changes of corneal curvature in horizontal rectus muscle can be induced in a short period after the operation, which is limited to the central 5mm area of the cornea, and basically restored to the preoperative level 1mo after the operation. There was no significant effect on corneal curvature after inferior oblique operation. Astigmatism value increased in the early postoperative period of horizontal rectus muscle; Myopic drift occurred and recovered to preoperative level 1mo after surgery. After the operation of the inferior oblique muscle, there was no significant change in the spheroscopy and astigmatism.<p>CONCLUSION:Corneal curvature and refractive changes may occur after extraocular muscle in adults.

@#Anterior segment ischemia(ASI)is a rare but serious complication after extraocular muscle surgery. Due to its complex clinical manifestations, it is easy to be misdiagnosed. If not treated in time, it may lead to blindness such as vision loss. This paper mainly reviews the concept, clinical manifestations, pathogenesis, risk factors, diagnosis and treatments of ASI after extraocular muscle surgery, so as to improve people's understanding of ASI and minimize or avoid the occurrence of ASI in clinical work.

A 23 year female presented with bilateral recurrent swelling of eyelids along with ptosis and proptosis for last 3 years. She also had swellings over the right cheek, parotid gland, and retro auricular area along with regional lymphadenopathy. Systemic laboratory workup revealed raised serum IgE and a high peripheral eosinophil count. Computed tomography and magnetic resonance imaging showed bilateral enlargement of extraocular muscles, lacrimal glands, and ipsilateral parotid gland. Excision biopsy of the retro-auricular lymph node was suggestive of Kimura's disease (KD). The patient responded well to systemic corticosteroid. KD rarely affects orbit, but it should be included in the differential diagnosis of inflammatory diseases of the orbit. To our knowledge, this is the first reported case of KD from India involving the orbit, lacrimal gland, extraocular muscles, parotid gland and buccal area.

ABSTRACT Granulomatosis with polyangiitis (GPA) is a systemic vasculitis with multiorgan involvement. Although ocular involvement has frequently been described, there are few reported cases of extraocular myositis in this disease. The case is presented of a Mexican woman with GPA who debuted with extraocular myositis.

El compromiso ocular ha sido descrito frecuentemente, sin embargo, existen pocos casos reportados de miositis extraocular en esta enfermedad. RESUMEN

Hands-on resident surgical training for various ocular procedures is essential to impart good surgical skills to the budding ophthalmologists. Here in this report, we demonstrate a simple and inexpensive technique of performing extraocular muscle surgery on goats' eye. These animal eyes possess soft tissue resemblance to that of human eyes to a greater extent in terms of scleral rigidity, muscle elasticity, its width, thickness, and its insertion onto the sclera. Therefore, rectus muscle recession, resection, and plication surgeries can be performed repeatedly to improve an individual's orientation and practical experience before performing the procedure on human eyes.

RESUMEN

Orbital pseudotumor is a non-granulomatous inflammation of orbital soft tissue for unknown etiology. In the first case, a 35-year-old female presented with bilateral painless upper eyelid swelling since 3years.No proptosis, no extra ocular movements restriction of both eyes was noted. Best corrected visual acuity both eye: 6/6, N6. USG B-Scan and CECT revealed bilateral lacrimal gland enlargement with bulky muscles and tendons. USG guided FNAC confirmed pseudotumor which showed prompt response to steroids but recurred after 1month of stopping of steroid therapy. In the second Case, a 15year old female presented with unilateral proptosis left eye for 3 months with painful extraocular movements and restricted in all gazes. Best corrected visual acuity in L/E - 6/12, N6, R/E-6/9, N6. USG B-scan and CECT revealed soft tissue lesion encasing Optic nerve. CEMR revealed bulky lacrimal gland, muscles and lesion extending up to orbital apex. Prompt response to steroid with no recurrence till date.

RESUMEN La caracterización de las poleas de los músculos extraoculares ha sido de gran trascendencia en la fisiopatología del equilibrio óculo-motor. El efecto mecánico de la polea funcional es cambiar el eje de acción del músculo de acuerdo con la orientación que el globo ocular presente. Los trastornos en la localización o estabilidad de las poleas pueden crear patrones de estrabismo incomitante. Es importante el conocimiento de estas anomalías porque los procedimientos quirúrgicos estándar no corrigen los desplazamientos de las poleas. Se realizó una búsqueda del tema de los últimos 10 años utilizando la plataforma de infomed(AU)


ABSTRACT Characterization of extraocular muscle pulleys has had great relevance in the physiopathology of ocular motor balance. The mechanical effect of the functional pulley is to change the axis of action of the muscle in keeping with the orientation of the eyeball. Pulley location or stability disorders may create incomitant strabismus patterns. It is important to be aware of these anomalies because standard surgical procedures do not correct pulley displacement. A search was conducted on the Infomed platform of papers published about the topic in the past ten years(AU)

RESUMEN

A 60-year-old man presented with sudden-onset proptosis of the left eye and intermittent diplopia of 2 months duration. Ophthalmic examination revealed bilateral eyelid retraction, left eye proptosis and a firm, non-tender mass (2 cm × 1.5 cm) in left supero-medial orbit with restricted extraocular movements. Contrast-enhanced computed tomography showed a well-defined, enhancing antero-medial orbital mass which was removed via anterior orbitotomy approach. The histopathology/immunohistochemistry showed adenocarcinoma; metastasis of urothelial origin. The oncology consultation and metastatic workup revealed a urinary bladder carcinoma with distant metastasis. Our patient expired within 6 months of diagnosis. The ophthalmic symptoms due to orbital metastasis may be the first presentation of some cancer patients.
