RESUMEN

Objective:To survey the training needs for chronic diseases among family health care workers in Beijing.Methods:A questionnaire survey on training needs for chronic disease was conducted from September to November 2018 among 820 family health care workers from 15 community health service centers in Beijing selected by stratified random sampling. The questionnaire consisted of demographic information and training needs related to chronic diseases. The training needs included: the knowledge and skills related to chronic diseases: the types of chronic diseases expected to learn, the basic knowledge of chronic diseases (7 items), preventive health care knowledge (7 items), nursing and rehabilitation skills (9 items); the duration, frequency, method, time and location of training arrangement.Results:The survey showed that chronic disease that family health care workers most wanted to learn was hypertension (90.3%, 717/794); among the basic knowledge of chronic diseases, the highest need was disease risk factor (65.5%, 520/794); among the knowledge of preventive health care, the highest need was home safety protection (87.3%, 693/794); among nursing and rehabilitation skills, the highest need was blood glucose measurement (83.1%, 660/794). The female respondents had higher training need for basic knowledge of chronic diseases than males ( Z=2.51, P=0.012). There were significant differences in the needs for preventive health care knowledge among respondents of different gender ( Z=2.72, P=0.007), occupation ( H=15.02, P=0.001) and educational level ( H=12.01, P=0.002). Respondents with different age ( H=6.49, P=0.039), occupation ( H=52.93, P<0.001) and educational level ( H=9.56, P=0.008)) had different needs for nursing and rehabilitation skills. Among the respondents, 58.2% (462/794) indicated that duration of each course should be 30-59 min, 34.0% (270/794) wanted to participate in the training once a month, 53.1% (422/794) wanted to participate in the training in the form of large class, 39.2% (311/794) were willing to participate in the training on working days and 48.7% (387/794) wanted to be trained in community health service institutions. Conclusions:There are diverse training needs of family health care workers. Attention should be paid to the implementation of need-oriented training. It is crucial to promote the motivation of family health care workers to participate in the training, which can improve the training effect and facilitate their role in family health management.

RESUMEN

O presente artigo trata da prática educativa gerencial do enfermeiro no Programa Saúde da Família (PSF). Resgata a realidade deste Programa no cenário institucional de Natal/RN, suscita alguns questionamentos e faz uma análise da sua ação educativa no espaço cotidiano das Unidades de Saúde da Família (USF), à luz da literatura. Objetiva contribuir com a reflexão acerca do fazer gerencial do enfermeiro nesse cenário, articulando-o à ação pedagógica/educacional. Como resultados, identifica que o caráter inovador da Estratégia Saúde da Família (ESF) evidencia desafios que se relacionam à necessidade de se definir as competências necessárias aos profissionais, aos seus processos de formação, educação continuada e permanente e aos novos modelos gerenciais para a enfermagem, que atendam, especificamente, as demandas desse cotidiano.


This article deals with the educational and managerial actions of nurses in the Family Health Program (FHP). It traces the reality of the FHP within the institutional setting of the city of Natal/RN, raises some questions and analyzes the educational action of the program in the daily routine of the Health Units in the light of the literature. Its objective is to contribute to the reflection about the managerial action of the nurse in that setting, in connection with pedagogical and educational action. The study demonstrates that, as a result of its innovative nature, the Family Health Strategy is facing challenges such as the need to define the profile of competencies for these professionals, their qualification processes, continued and permanent education, and new managerial models for nursing that should specifically meet the daily demands.

RESUMEN

A necessidade de um processo de especialização em larga escala em Medicina de Família e Comunidade (MFC) para os médicos da Estratégia Saúde da Família no Brasil (ESF) é identificada nesse documento. Os programas de residência médica (PRMs) seguem como padrão-ouro de especialização, mas não são suficientes para atingir o objetivo acima. Estimula-se que o processo seja acreditado pela Sociedade Brasileira de Medicina de Família e Comunidade (SBMFC) e que seus concluintes sejam certificados por meio do concurso de titulação TEMFC. Modalidades à distância com momentos presenciais são as mais adequadas, observando-se propiciar acesso a profissionais já inseridos em serviço, valorizar sua continuidade nos mesmos, orientar o aprendizado para uma integralidade inteligente e coordenar diferentes recursos para viabilizar o ensino-aprendizagem. As competências buscadas são as definidas pela WONCA, adaptadas à realidade nacional. Aprender a aprender como um MFC é o principal objetivo visado. Casos complexos,construídos de acordo com uma matriz de agravos e problemas diversos, atendidas às realidades regionais, são a principal ferramenta do processo. A SBMFC propõe-se a impulsionar tal estratégia


This document identifies the need for large-scale specialization in Family and Community Medicine (FCM) for the physicians of the Brazilian Family Health Program (FHP). The medical residency programs (MRP) continue the golden-standard for graduate education, but they are not enough to fill that gap. It is suggested that the Brazilian Society of Family and Community Medicine (SBMFC) shall be in charge of certifying the process. E-learning with some presential moments is considered the most suitable modality for propitiating access to health professionals already in service; valuing their continuity in the services; directing the teaching-learning process towards comprehensive intelligence and for coordinating different learning resources. The competences are those defined by the WONCA, adapted to the Brazilian reality. .Learn to learn. as a Family Doctor is the main goal. Complex cases constructed according to a framework of different clinical problems adapted to regional realities are the main tool for this process and the SBMFC offers itself to encourage such strategy.
