RESUMEN

Objective:To explore the "Trinity" professional construction path of 1 + X elderly care and 1+ X dementia elderly care and nursing (elderly nursing direction), solve the problem that the three are difficult to be effectively compatible in the process of professional talent training, and cultivate more compound technical talents with "one specialty and multiple abilities" to meet the needs of aging society.Methods:In June 2021, 50 students from class 1 of 2020 nursing major (elderly nursing direction) of Pingxiang health vocational college were selected as the experimental group and 50 students from class 2 were selected as the control group. The nursing students in the experimental group were taught and trained by the newly revised talent training scheme, curriculum standards and a series of corresponding teaching reform measures. The students in the control group were trained by the conventional mode. After the course of the third semester, the passing rate of 1+X elderly care, the passing rate of 1+ X dementia elderly care and the score of geriatric nursing core competence evaluation scale were compared and analyzed between the two groups.Results:The total score of the test group was 156.60 ± 6.00 and that of the control group was 140.86 ± 9.20. The difference was statistically significant ( t=10.13, P<0.05). The passing rate of 1+ X elderly care in the experimental group was 96.43%(27/28), and that of 1 + X elderly care in the control group was 11/15. The difference was statistically significant ( χ2=5.07, P<0.05). The passing rate of care for the elderly with 1 + X dementia was 95.45%(21/22), and the passing rate of care for the elderly with 1 + X dementia was 8/13, the difference was statistically significant ( χ2=4.45, P<0.05). Conclusions:Based on the "Trinity" professional construction mode of nursing specialty (geriatric nursing direction) under the 1 + X certificate system, it can improve students′ core competence of geriatric nursing, improve the passing rate of vocational skill level certificate, and provide reference for the professional construction of nursing specialty (geriatric nursing direction) in medical and health higher vocational colleges.

Objective@#Through research on the geriatric nursing related literature in Chinese journals and medical care and elderly care combination related keywords from 1979 to 2017, this study provides the development trend of geriatric nursing research.@*Methods@#Searching articles in Wanfang, CNKI, VIP database, selecting the keywords and published time of the paper. Employing descriptive statistics to describe literatures information such as publication time, quantity of keywords, word frequency of keywords, heat degree of keywords, character of keywords and so on.@*Results@#Totally 134 791 articles were got and 58 031 articles were included in the analysis. The amount of literature growth was slow before 2005, The cumulative amount of literature in 1979-2005 was only 12.75% of the total amount of literature. After 2005, the amount of literature increased rapidly year by year. The number of literature published in 2017 has exceeded 12%. The amount of the geriatric nursing related literature was proportional to the number of people aged 65 and over in China. In the research of geriatric nursing, the content related to medical care and elderly care combination was quite dispersed, and it did not involve research at the level of Medical Association. The medical care and elderly care combination and Medical Association were new areas in the research of geriatric nursing.@*Conclusion@#The amount of the geriatric nursing related literature from 1979 to 2017 was proportional to the number of people aged 65 and over in China. There are few researches on how to carry out geriatric nursing under the policy of medical care and elderly care combination and Medical Association, which should be the focus of future research and exploration to actively respond to the challenge of aging population.

Objectives: To study family satisfaction of medical care to geriatric patients and to find out any difference in family satisfaction given by geriatric specialist and general medicine specialist. Indian data on family’s perception of geriatric care is lacking. Materials and methods: All geriatric patients who were admitted between March 2015 and September 2015 in medical wards and where hospital stay was more than 3 days were included. An internationally validated family satisfaction questionnaire was used and administered to family member of each patient admitted. Scores were graded on Likert scale under five important areas like staff interaction, support services, quality of medical care, medical communication, and facilities. Special comments were analyzed separately. Results: Patients’ family members were most satisfied with quality of medical care and facilities (95%), followed by staff interaction and support facilities. Satisfaction regarding medical communication and services was poor (60%). There was no significant difference for satisfaction/perception of quality of care given by geriatric specialist and general medicine specialist. Conclusion: Medical communication and support services of long-term care of geriatric patients are an important area of improvement. Family perception of geriatric care is vital to overall delivery of health care to this vulnerable group of patients. Support facilities specifically designed for geriatric patients need to be strengthened. There was no significant difference in perception of care offered by geriatric specialist and general medicine specialist.

Korea health care system and public welfare system have been changed slowly and developed step by step. It makes efforts to promote the welfare of the people, but does not provide good benefits for geriatric patients. In 2008, it was incompletely set up the long term care system in Korea. And the Law on Long-Term Care Insurance for Elderly was officially announced in Nov. 2011 and the law for dementia, Dementia Management Act, was drafted in Feb. 2012. Though there were many laws, the concept of long-term care hospital and geriatric care facility is still obscure. In long-term care hospital, there are big expansion of the number of beds and institutions in Korea. Over a half of the patients in long-term care hospitals are suffered from mixed neurological diseases. Especially, dementia is the comorbid state with other chronic geriatric diseases. In view of the aging population, the role of neurologist will continue to grow. So neurologists need more special training for the needed skills and competencies, and the specific care settings program in long-term care hospital. In this review, the history and current state of long-term care hospital, related laws, and the role of neurologist will be discussed.

In the future, geriatric care needs much more financial cost because korea is changing to an aging society rapidly and most of old people have health problems. It is important to have a comprehensive blueprint of the medical care of old patients, linking this with a welfare of old people. The government has to bring up and draw guidelines how to handle old patients and how much to treat them, before starting a national nursing insurance for old handicapped patients. Without a blueprint of old people care, we are in danger to encounter a huge financial leakage due to inefficient care system for old patients. Presently, old patient medical care are being randomly performed according to doctor's decision on a individual base. The multi-organ injury chronic diseased case tend to be rejected by doctors and will be referred to the higher level of medical center. Since old wealthy patient with a good economic and physical condition is a good target of making a money, this kind of old people will be welcomed and many lucrative cares will be applied under the name of anti-aging therapy. This consumptive environment must be changed to an efficient and economic system. The key content of a desirable system is a self-community of old-age people, which the lesser sick help the more sick and the lesser old patients help the more elderlies. The all around plan-building by the government is absolutely needed to provide a efficient medical treatment system for old patients with organizing the elderly as well as improving the regulations. Now, the government is ready to implement a national nursing insurance to solve the problems of the aging society. However, without a well prepared master plan for the future matters, we will meet other obstacles which could see in a tardy growing society.
