RESUMEN

Objective To evaluate the treatment effect of bladder instillation chemotherapy on glandular cystitis. Methods The clinical data of glandular cystitis of 54 cases were retrospectively analyzed, all cases were treated individually and the applications of chemotherapeutic drugs bladder instillation were not adopted. Results Follow-up period ranged from 2.5 to 9.0 years, and the average was 4.6 years. Lower urinary tract symptoms score before treatment, 3 month , 6 month, 2 years , 4 years after treatment was (8.5 ± 3.7), (5.7 ± 2.3), (3.9 ± 1.3), (4.0 ± 1.9), (4.2 ± 1.9) scores, and the scores after treatment were improved significantly compared with that before treatment (P < 0.05). Recurrence rate 3 months , 6 months, 2 years and 4 years after treatment was 3.7%(2/54), 13.0%(7/54), 5.6%(3/54) and 1.9%(1/54). Conclusions Eliminating the inducements and improvement of symptoms provides a significantly curative effect in glandular cystitis. Postoperative bladder instillation chemotherapy is not recommended.

RESUMEN

Objective To investigate the manifestations and features of CT for glandular cystitis as well as cystic cystitis.Methods Clinical manifestations and CT imaging features of 39 cases with biopsy-proved glandular cystitis or cystic cystitis were analyzed retrospectively.Results Among 39 cases,33 were glandular cystitis and 6 were cystic cystitis.Two out of the 33 cases with glandular cystitis had a negative CT scan,and 31 had a positive CT scan among which 4 cases with extensive lesions showed diffuse thickening of the bladder wall,and 27 were with localized lesions.Furthermore,19 out of the 27 cases showed localized thickening of the bladder wall with smooth edge,which was a continuation of surrounding normal bladder wall;8 showed nodular lesions.17 of the 33 cases with glandular cystitis underwent contrast-enhanced CT scan which showed mildly enhancement consistent with or slightly stronger than the normal bladder wall in 15 cases and moderate uneven enhancement in 2 cases.The 6 cases with cystic cystitis showed diffuse thickened rough bladder wall.There were cystic shadows of various sizes in the inner wall of the bladder partially protruding into the bladder,which presented as a"beaded shape"manifestation.Conclusion The characteristic manifestations of glandular cystitis as well as cystic cystitis on CT scan are of great significance in diagnosing both of the diseases.

RESUMEN

Objective To investigate the clinical effects of transurethral resection of cystitis after gynecological cystitis after transurethral resection of bladder,and to observe the effect on patients' anxiety(SAS)and quality of life index(QOL).Methods The clinical data of 62 patients with cystitis glandularis admitted to hospital from January 2011 to December 2016 were retrospectively analyzed.The control group was treated with plasma ablation alone,with plasmakinetic resection of Kangfuxin liquid combined with gemcitabine intravesical therapy as the treatment group,31 cases in each group.Statistics of two groups of patients with clinical efficacy,followed up for 12 months,and the anxiety self-rating scale(SAS)was used to evaluate the anxiety of the two groups before and after treatment,QOL was used to evaluate the quality of life before and after treatment in two groups of patients.The recurrence rate of two groups were recorded.Results After treatment,the effective rate of the treatment group was 80.65%,slightly higher than the control group 77.42%,the difference was statistically significant(P<0.05),the SAS score in the treatment group was slightly higher than before treatment,but the difference was not statistically significant,the SAS score in the control group was significantly lower than before treatment,after treatment between the groups,the difference was statistically significant(P<0.05).The QOL index of the treatment group decreased slightly after treatment,but the difference was not statistically significant.The QOL index of the treatment group decreased significantly after treatment,and compared between the two groups after treatment,the difference was statistically significant(P<0.05).Follow-up of 12 months,the relapse rate in the treatment group was 11.11%,slightly lower than the control group 18.75%,the difference was statistically significant(P<0.05).Conclusion Compared to pure plasmapheresis,the use of postoperative intravesical instillation of bladder irrigation with the same effect,but the latter may increase the degree of anxiety in patients,thus affecting the quality of life of patients,so for the non-mandatory use of drugs treatment,surgery may be given priority to surgery alone.

RESUMEN

Se presenta un caso de cistitis glandular, enfermedad rara, generalmente asintomática, que consiste en un desorden proliferativo de las glándulas que producen moco dentro de la mucosa y submucosa del epitelio urinario de la vejiga. La irritación crónica constituye el factor de riesgo de mayor importancia en su aparición. La evolución hacia la malignidad (adenocarcinoma) es excepcional y ocurre en caso de la persistencia del factor de predisposición. Para llegar a establecer el diagnóstico es necesario el examen clínico, radiológico e histológico. A este caso se le realizó ecografía vesical y se encontró tumoración pequeña de 3 cm en fondo vesical, que primeramente se le efectuó biopsia por cistoscopia y después se resecó totalmente. Confirmado el diagnóstico, el tratamiento fue esencialmente quirúrgico. La vigilancia anual por cistoscopia con biopsias de vejiga resulta necesaria en estos casos. La forma florida es mucho más rara e incapacitante pues precisa resección amplia de las lesiones


This is the case of glandular cystitis a uncommon disease generally asymptomatic consisted of a proliferative disorder of glands producing mucus within mucosa and submucosa of vesical urinary epithelium. Chronic irritation is the risk factor more significant in its appearance. Course to malignancy (adenocarcinoma) is exceptional and occur in case of persistence of the predisposition factor. To made diagnosis it is necessary the clinical, radiological and histological examination. This case undergoes a vesical echography noting a 3 cm small tumor in vesical fundus and a biopsy by cystoscopy after a total resection. Diagnosis was confirmed and treatment was essentially surgical. Annual surveillance by cystoscopy with vesical biopsies is necessary in these cases. The progressive way is rarer and incapacitant since a wide resection of lesions is essential

RESUMEN

Objective To investigate the use of cystoscopy in the diagnosis and treatment of glandular cystitis. Methods A total of 38 cases of glandular cystitis was pathologically diagnosed under cystoscopy.The patients were treated by transurethral electric resection.After surgery a chemotherapy with mitomycin-c bladder irrigation was given once weekly for 8 times and then once monthly for 10 times.During the follow-up a cystoscopy was periodically performed and biopsy samples were pathologically examined with all the cases.Results The follow-up checkups were made for 6~12 months in 38 cases.Of the 38 cases,29 cases were cured(with transitional cell surface covering the bladder musoca gradually),5 were symptomatically relieved,and 4 were found relapsed. Conclusions Cystoscopy can offer valuable help for the diagnosis of glandular cystitis.Treatments with transurethral electric resection and mitomycin-c bladder irrigation are effective for glandular cystitis.
