ABSTRACT Hyperprolactinemia is a frequent cause of menstrual irregularity, galactorrhea, hypogonadism, and infertility. The most common etiologies of hyperprolactinemia can be classified as physiological, pharmacological, and pathological. Among pathological conditions, it is essential to distinguish prolactinomas from other tumors and pituitary lesions presenting with hyperprolactinemia due to pituitary stalk disconnection. Proper investigation considering clinical data, laboratory tests, and, if necessary, imaging evaluation, is important to identify the correct cause of hyperprolactinemia and manage the patient properly. This position statement by the Brazilian Federation of Gynecology and Obstetrics Associations (Febrasgo) and Brazilian Society of Endocrinology and Metabolism (SBEM) addresses the recommendations for measurement of serum prolactin levels and the investigations of symptomatic and asymptomatic hyperprolactinemia and medication-induced hyperprolactinemia in women.

@#ObjectiveTo prepare colloidal gold immunochromatographic test paper for rapid detection of Legionella pneumophila(LP)and test its performance to ensure that it meets the national clinical diagnostic standards.MethodsLP colloidal gold immunochromatographic test paper was prepared based on double antibody sandwich ELISA,and tested for the cross reactivity,anti-interference,sensitivity,hook effect,stability and other aspects.ResultsLP colloidal gold immunochromatography test paper showed no cross reaction with 22 common pathogens in respiratory tract such as Moraxella catarrhalis,and was not affected by internal and external interferences in respiratory tract;The minimum detection limit for LP was 2.00 × 105cfu/mL,with good sensitivity and no hook effect;Under the conditions of accelerated aging at 45 ℃,simulated high temperature transportation and frozen transportation,the repeatability and stability of test paper were not affected,and the stability was good in the same batch and between different batches.ConclusionThe prepared LP colloidal gold immunochromatographic test paper realized rapid detection of LP,which was simple to operate and had good application prospect and popularization value.

OBJECTIVE@#To study the chemical constituents of the roots of Angelica dahurica, a well-known Chinese herbal medicine named Baizhi in Chinese.@*METHODS@#Compounds were separated by various chromatographies, and the structures of new compounds were elucidated based on the analysis of their spectroscopic and spectrometric data (1D, 2D NMR, HRESI MS, IR, and UV). The absolute configurations of new compounds were determined by the calculated electronic circular dichroism and chemical derivatization. The inhibitory activities of all isolates against nitric oxide (NO) production were evaluated using lipopolysaccharide-activated RAW 264.7 macrophage cells.@*RESULTS@#Seven new 3,4-dihydro-furanocoumarin derivatives ( 1a/ 1b, 2a/ 2b, 3a/ 3b, 4) together with a known furanocoumarin ( 5) were isolated from the roots of A. dahurica. The new compounds included three pairs of enantiomers, (4S, 2''R)-angelicadin A ( 1a)/(4R, 2''S)-angelicadin A ( 1b), (4S, 2''S)-angelicadin A ( 2a)/(4R, 2''R)-angelicadin A ( 2b), and (4S, 2''S)-secoangelicadin A ( 3a)/(4R, 2''R)-secoangelicadin A ( 3b), together with (4R, 2''R)-secoangelicadin A methyl ester ( 4). The known xanthotoxol ( 5) inhibited the NO production with the half-maximal inhibitory concentration (IC50) value of (32.8 ± 0.8) µmol/L, but all the new compounds showed no inhibitory activities at the concentration of 100 µmol/L.@*CONCLUSION@#This is the first report of the discovery of 3,4-dihydro-furanocoumarins from A. dahurica. The results are not only meaningful for the understanding of the chemical constituents of A. dahurica, but also enrich the reservoir of natural products.

BackgroundIn order to implement the spirit of the 20th National Congress of the Communist Party of China, and the Opinions on Promoting the Inheritance, Innovation and Development of Traditional Chinese Medicine(TCM), to regularly summarize the research results of TCM, to present the academic progress of TCM dynamically, and to give full play to the academic leadership of academic groups, the China Association of Chinese Medicine had organized the selection of the top 10 academic progress of TCM in 2022. The selection work adhered to the four orientations, eliminated the four only, highlighted the solution of clinical problems, answered scientific questions, led the development of the industry, reflected the exploratory and forward-looking, innovative and breakthrough, focused on new laws, new discoveries, new methods, new products, new theories in the field of basic research and applied basic research in TCM. After dynamic collection, preliminary examination, review and final judgment, the top 10 academic progress of TCM in 2022 were determined.

Polysaccharide of Balanophora involucrata Hook. f. (BPS), the major component of Balanophora involucrata Hook. f., was confirmed the protective effect on liver injury in our previous study. This research aimed to investigate the protective mechanism of BPS on experimental liver injury by attenuating cell ferroptosis through modulating solute carrier family 7 member 11/glutathione peroxidase 4 (SLC7A11/GPX4) pathway. The animal experiment was approved by the Experimental Animal Ethical Committee of Hubei Minzu University and all rats had received human care in compliance with the institutional animal care guidelines. Rats were given intraperitoneal injection of (D-galactosamine, D-GalN) solution (800 mg·kg-1) one time to establish the acute liver injury model. The results showed aspartate amino transferase (AST), alanine aminotransferase (ALT) and 4-hydroxynonenal (4-HNE) levels in serum were decreased, and the contents of reactive oxygen species (ROS), Fe2+, malondialdehyde (MDA) and lipid peroxide (LPO) in liver tissues also decreased and glutathione (GSH) level increased after BPS administration with 200 mg·kg-1. Besides, BPS reduced iron deposition and increased the expression of SLC7A11 and GPX4 proteins in liver tissue. In conclusion, BPS ameliorated experimental liver injury by alleviating cell ferroptosis through SLC7A11/GPX4 pathway. The present study pointed to the possibility of utilizing BPS for protection against liver injury in clinic.

@# Objective    To systematically evaluate the application effect of CT-guided Hook-wire localization and CT-guided microcoil localization in pulmonary nodules surgery. Methods    The literatures on the comparison between CT-guided Hook-wire localization and CT-guided microcoil localization for pulmonary nodules were searched in PubMed, EMbase, The Cochrane Library, Web of Science, Wanfang, VIP and CNKI databases from the inception to October 2021. Review Manager (version 5.4) software was used for meta-analysis. The Newcastle-Ottawa Scale (NOS) was used to evaluate the quality of studies. Results    A total of 10 retrospective cohort studies were included, with 1 117 patients including 473 patients in the CT-guided Hook-wire localization group and 644 patients in the CT-guided microcoil localization group. The quality of the studies was high with NOS scores>6 points. The result of meta-analysis showed that the difference in the localization operation time (MD=0.14, 95%CI −3.43 to 3.71, P=0.940) between the two groups was not statistically significant. However, the localization success rate of the Hook-wire group was superior to the  microcoil group (OR=0.35, 95%CI 0.17 to 0.72, P=0.005). In addition, in comparison with Hook-wire localization, the microcoil localization could reduce the dislocation rate (OR=4.33, 95%CI 2.07 to 9.08, P<0.001), the incidence of pneumothorax (OR=1.62, 95%CI 1.12 to 2.33, P=0.010) and pulmonary hemorrhage (OR=1.64, 95%CI 1.07 to 2.51, P=0.020). Conclusion    Although Hook-wire localization is slightly better than microcoil localization in the aspect of the success rate of pulmonary nodule localization, microcoil localization has an obvious advantage compared with Hook-wire localization in terms of controlling the incidence of dislocation, pneumothorax and pulmonary hemorrhage. Therefore, from a comprehensive perspective, this study believes that CT-guided microcoil localization is a preoperative localization method worthy of further promotion.

A four-month-old prepubertal female goat was presented to RVP IVRI Bareilly, with the history of an abnormal growth in the vulva. On clinical examination the growth was found as prominent clitoris, and two peanut sized soft structures were palpable at the subcutaneous in inguinal region. Transabdominal B mode real time ultrasound of inguinal region revealed testis like morphology and presence of uterus and ovaries structures subcutaneously. The case was subsequently diagnosed as hermaphrodism.

Manual small-incision cataract surgery (MSICS) preserves its utility as a cost-effective and efficient strategy for mitigating cataract-associated blindness. Numerous techniques of nucleus delivery in MSICS have been described in the literature. The fish hook technique of nucleus extraction was advocated in 1997 and has been popular for high-volume surgery in limited pockets of the world. This article describes in detail the steps involved in the construction of a fish hook, tips of nucleus extraction with the help of a fish hook, and the pros and cons of fish hook-based MSICS through text, diagram, and video supplement.

Aim To explore the efficacy-toxicity of Tripterygium wilfordii Hook.f.in intervention of lupus nephritis by the method of network pharmacology.Methods Firstly, the active components were searched and the action targets of Tripterygium wilfordii Hook.f.were predicted through the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP).Secondly, the target genes of LN were collected through CoolGeN, OMIM and Gene Cards databases.And then, it was mapped to Tripterygium wilfordii Hook.f.targets and the drug component-disease target interaction network was constructed with Cytoscape software, and STRING database was used to analyze the protein interaction network.Finally, the common targets were analyzed by GO and KEGG pathway enrichment analysis using DAVID database to explore the potential mechanism of Tripterygium wilfordii Hook.f.in the treatment of LN.Results A total of 52 active components of Tripterygium wilfordii Hook.f.and 38 targets for the treatment of LN were screened.Most of the components had potential therapeutic effects on LN, but the effects of triptolide, tripterine, kaempferol and β-sitosterol may not be conducive to the improvement of LN.The results of KEGG analysis showed that efficacy-toxicity mainly involved NOD-like receptor signaling pathway, p53 signaling pathway, Toll-like receptor signal pathway and so on.Conclusions Tripterygium wilfordii Hook.f.plays the efficacy-toxicity effect on LN by regulating immune inflammation, cell proliferation and apoptosis, and its overall intervention effect needs further experimental study.

To explore the mechanism of ovarian toxicity of Hook. F. (TwHF) by network pharmacology and molecular docking. The candidate toxic compounds and targets of TwHF were collected by the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP) and the Comparative Toxicogenomics Database (CTD). Then, the potential ovarian toxic targets were obtained from CTD, and the target genes of ovarian toxicity of TwHF were analyzed using the STRING database. The protein-protein interaction (PPI) network was established by Cytoscape and analyzed by the cytoHubba plug-in to identify hub genes. Additionally, the target genes of ovarian toxicity of TwHF were subjected to Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses by using the R software. Finally, Discovery Studio software was used for molecular docking verification of the core toxic compounds and the hub genes. Nine candidate toxic compounds of TwHF and 56 potential ovarian toxic targets were identified in this study. Further network analysis showed that the core ovarian toxic compounds of TwHF were triptolide, kaempferol and tripterine, and the hub ovarian toxic genes included , , , , , , , , and . Besides, the GO and KEGG analysis indicated that TwHF caused ovarian toxicity through oxidative stress, reproductive system development and function, regulation of cell cycle, response to endogenous hormones and exogenous stimuli, apoptosis regulation and aging. The docking studies suggested that 3 core ovarian toxic compounds of TwHF were able to fit in the binding pocket of the 10 hub genes. TwHF may cause ovarian toxicity by acting on 10 hub genes and 140 signaling pathways.

OBJECTIVE@#To evaluate the efficacy and prognosis of double button plate combined with strengthened anchor technique and clavicular hook plate internal fixation for complete acromioclavicular joint dislocation.@*METHODS@#From July 2017 to September 2020, a total of 42 patients with acromioclavicular joint dislocation treated by surgery were choosen as study objects and divided into observation group (21 cases) and control group (21 cases). In the observation group, there were 14 males and 7 females, aged 21 to 63 years old, with an average of (45.05±8.70) years old. In the control group, there were 16 males and 5 females, aged 25 to 68 years old, with an average of(45.00±9.44) years old. The patients in the observation group were treated with double button plate combined with strengthened anchor technique, whereas those in the control group received clavicular hook plate internal fixation. The two groups were compared in terms of operating time, intraoperative blood loss, postoperative hospital stay, shoulder pain(visual analogue scale, VAS) score, shoulder function Constant-Murley score and postoperative complications.@*RESULTS@#There was no significant difference between the two groups in intraoperative blood loss or postoperative hospital stay(P>0.05). The operating time of double button plate combined with strengthened anchor technique group (65.24±5.36) min was significantly longer than that of the clavicular hook plate group (48.81±4.72) min, and the difference was statistically significant (P<0.05). There was no significant difference in shoulder function or pain degree between the two groups before operation (P>0.05). After 1 month, 3 months and 6 months, the Constant-Murley score of the observation group was 73.29±2.15, 85.43±1.47, 93.86±1.24 separately, which were significantly higher than those of the control group;and the VAS score was 2.76±0.62, 1.71±0.64, 0.57±0.51 separately, which were significantly lower than those of the control group (P<0.05). One instance of shoulder discomfort was found in the observation group, while 5 cases of shoulder pain, 2 cases of restricted shoulder mobility, and 1 case of subacromial bone absorption were found in the control group. In both group, there was no loss of reduction.@*CONCLUSION@#In the treatment of complete acromioclavicular joint dislocation double button plate combined with strengthened anchor technique achieves favorable clinical result. Internal fixation does not need to be removed. The recovery of shoulder joint function and pain relief are superior than clavicular hook plate internal fixation, which is more worthy of clinical promotion.

AIM:To evaluate the application effect of iris hooks combined with suture fixation of capsular tension ring(CTR)in the treatment of phacoemulsification with secondary glaucoma associated with lens subluxation.METHODS: Retrospective case series. A total of 18 eyes of patients with serious secondary glaucoma associated with lens subluxation were enrolled in Aier Eye Hospital(Changchun)from October 2017 to May 2020 as the observation group. All the patients had iris hooks inserted to support the capsule during the phacoemulsification combined with intraocular lens(IOL)implantation. Iris hooks were inserted through the incisions and placed in the capsulorhexis to support the capsule, then CTR was implanted and sutured onto the sclera of the dislocation side. A total of 11 eyes of patients had intracapsular cataract extraction combined with IOL suspension were enrolled in Aier Eye Hospital(Changchun)from October 2015 to October 2017 as the control group. The postoperative complications were recorded. The postoperative examinations included visual acuity, intraocular pressure(IOP)and positions of the capsule and IOL.RESULTS: The postoperative follow-up at least 12(12-40)mo, both the IOP of observation group and control group returned to normal, with statistically significant difference compared with pre-operation(t=9.994, 8.790, all P&#x003C;0.001). At 12mo after operation, the best corrected visual acuity of the two groups were significantly improved compared with surgery before(Z=-3.900, P&#x003C;0.001; Z=-3.002, P=0.003). The IOL in the observation group of the 18 eyes were in the expected position without complications such as an obvious tilt or displacement. In the control group with 11 eyes, the position of IOL in 8 eyes were centered, 3 eyes were slightly biased, and 2 eyes with cystoids macular edema.CONCLUSION: The application of suture fixation capsular tension ring and iris hooks for secondary glaucoma associated with lens subluxation in patients can be used as an effective auxiliary method, it could effectively control the IOP and maintain long-term stability of the capsule.

Objective To explore the influence of hook deflection angle and inclination angle on mechanical properties of the soft tissue suture passer hook. Methods Taking the end face far away from the tip of the needle (end face 1) as the study object, a mathematical model was established with the moment as dependent variable and the hook deflection angle and inclination angle as the independent variable. The moment was solved by the mathematical model with the deflection angle and inclination angle of 0°, 10°, 20° and 30°. Based on the finite element analysis method, 16 three-dimensional geometric hook models with deflection angle and inclination angle of 0°, 10°, 20° and 30° were established by SolidWorks. The stress analysis was carried out by ANSYS Workbench. Under the same puncture force, the maximum von Mises stress of each hook and the reaction moment of end face far away from the tip of the needle were calculated. Results The results from theoretical analysis and numerical simulation showed that the reaction moment of end face 1 increased with the increase of deflection angle, and increased with the decrease of inclination angle. The hook with deflection angle of 0° and inclination angle of 30° had the minimum reaction moment. The finite element analysis results showed that with the deflection angle of 0°, the maximum von Mises stress of the hook was the smallest and did not change with the inclination angle of the hook changing. Conclusions The established mathematical model can accurately explain the relationship between the moment at the end face of the hook and the deflection angle and inclination angle of the hook. This study provides the theoretical basis for designing hook geometry of the soft tissue suture passer, and improves the safety of the soft tissue suture passer in operation process.

Many variations of ovariohysterectomy techniques have been described, including the traditional one and minimally invasive procedures. Non-laparoscopic Snook hook technique is an alternative for performing minimally invasive ovariohysterectomy. Few studies have been carried out in order to assess pain in animals submitted to minimally invasive surgeries, especially involving one of the most performed surgical procedures in veterinary practice. The aim of this study was to evaluate surgical duration, intraoperative nociception and acute postoperative pain after traditional ovariohysterectomy or minimally invasive non-laparoscopic technique in dogs using Snook hook. The hypothesis is that non-laparoscopic minimally invasive ovariohysterectomy would be faster and less painful than the conventional technique. Thirty dogs were divided into Traditional Group (TG = 15) and Minimally Invasive Group (MIG = 15). Heart rate, respiratory rate, systolic blood pressure, body temperature, oxyhemoglobin saturation, end-tidal carbon dioxide concentration (ETCO2) and end-tidal isoflurane concentration were evaluated before the surgery begins (M0), during incision (M1), clamping of the first ovarian pedicle (M2), second ovarian pedicle (M3), uterine cervix (M4), abdominal suture (M5) and at the end of surgery (M6). The modified Glasgow Pain Scale was used for acute postoperative pain assessment and Visual Analogue Scale (VAS) was used to assess the sensitivity of surgical wound. The level of significance established for all statistical analysis was 5%. Statistical differences were not observed between groups considering total surgical time and postoperative acute pain intensity (P > 0.05), in spite of MIG having shorter duration of surgery. There was no statistical difference between groups considering all intraoperative parameters except respiratory rate (TG < MIG; P < 0.05) and ETCO2 (MIG < TG; P < 0.05) at the moment of traction of the first ovarian pedicle (M2). Pain assessment by VAS showed statistical difference 24h after the end of surgery (TG < MIG) (P < 0.05). Both procedures were similar regarding intraoperative nociception and acute postoperative pain. It is possible that the sensation of pain in both procedures was blocked by the effectiveness of analgesics, once they might cause an inhibition of painful behaviors limiting a possible difference in pain identification. Higher respiratory stress observed in MIG at M2 and higher pain score by VAS noted in MIG 24 h after the end of surgery can be justified by greater traction of ovarian pedicle, due to limited surgical access of minimally invasive technique. Minimally invasive ovariohysterectomy non-laparoscopic seems to be potentially faster, probably due to the smaller size of the abdominal incision, which takes less time to be closed. In the present study, both techniques were performed by an experienced surgeon, providing safe procedures, nevertheless it is important to emphasize that iatrogenic injury can be caused by surgeons non-proficient in the Snook hook technique, considering the limited visualization of anatomical abdominal structures. Data obtained indicate that traditional ovariohysterectomy and non-laparoscopic Snook hook technique promote similar intraoperative nociception and acute postoperative pain, however minimally invasive procedure is potentially faster with less surgical trauma.(AU)

Resumen En el boxeo se requieren acciones precisas en un período muy corto de tiempo porque, en la medida que sea más rápido el accionar técnico en los ataques, imposibilita al contrario defenderse y contraatacar de manera eficiente durante el combate. Mejorar el proceso de preparación técnica en el boxeo desde el entrenamiento deportivo, con énfasis en la ejecución de los golpes en gancho en los boxeadores de la categoría pioneril que entrenan en el Gimnasio de boxeo "Giraldo Córdova Cardín" de Pinar del Río, es el objetivo de este trabajo. Se partió del diagnóstico del estado en que se encuentra este elemento técnico, en la muestra de diez atletas, en el que se aplicó la observación a 24 sesiones de entrenamiento, así como una entrevista a los cuatro entrenadores de boxeo que trabajan con esta categoría en dicho gimnasio. Se constataron una serie de debilidades al realizar el trabajo con la citada técnica en la unidad de entrenamiento. Como resultado final, a fin de posibilitar la corrección de las deficiencias existentes, se implementa una metodología contentiva de un aparato cognitivo y uno instrumental, metodológico desde los ejercicios que se les presentan y demuestran a los entrenadores y los resultados en las competencias del ciclo 2019-2020 que muestran la cuota de ganancia para los entrenadores y atletas.


Resumo No boxe, são necessárias acções precisas num período de tempo muito curto porque, na medida em que a ação técnica nos ataques é mais rápida, torna impossível defender e contra-atacar eficazmente durante a luta. O objectivo deste trabalho é melhorar o processo de preparação técnica no boxe a partir do treino desportivo, com destaque para a execução dos golpes de gancho nos boxeadores da categoria pioneira que treinam no ginásio de boxe "Giraldo Córdova Cardín" em Pinar del Río. Baseou-se no diagnóstico do estado deste elemento técnico, na amostra de dez atletas, na qual a observação foi aplicada a 24 sessões de treino, bem como numa entrevista aos quatro treinadores de boxe que trabalham com esta categoria naquele ginásio. Foi encontrada uma série de pontos fracos quando se trabalhou com esta técnica na unidade de formação. Como resultado final, a fim de tornar possível a correção das deficiências existentes, é implementada uma metodologia contendo um aparelho cognitivo e instrumental, metodológico a partir dos exercícios que são apresentados e demonstrados aos treinadores e os resultados nas competições do ciclo 2019-2020 que mostram a quota de ganho para os treinadores e atletas.


Abstract In boxing, precise actions are required in a very short period of time because, to the extent that the technical action in the attacks is faster, it makes it impossible to defend and counterattack efficiently during the fight. The objective of this work is to improve the technical preparation process in boxing from sports training, with emphasis on the execution of hook blows in the pioneer category boxers who train in the "Giraldo Córdova Cardín" boxing gym in Pinar del Río. It was based on the diagnosis of the state of this technical element, in the sample of ten athletes, in which the observation was applied to 24 training sessions, as well as an interview with the four boxing trainers who work with this category in that gym. A series of weaknesses were found when working with this technique in the training unit. As a final result, in order to make possible the correction of the existing deficiencies, a methodology containing a cognitive and an instrumental apparatus is implemented, methodological from the exercises that are presented and demonstrated to the coaches and the results in the competitions of the cycle 2019-2020 that show the quota of gain for the coaches and athletes.

Lateral end clavicle fractures are of three types (Neer type- I, II, & III). In Neer Type-II, the fractures are very unstable due to damage to the coracoclavicular (CC) ligament. So, it is always treated surgically. This comparative cross-sectional prospective study compares the outcome after internal fixation of these fractures with clavicular hook plate and precontoured anatomical locking plate.METHODSThirty-two (32) patients attending the OPD and ER of R.G. Kar Medical College, Kolkata, having Neer Type-II fractures, were treated surgically within three weeks of injury using either clavicular hook plate (17 patients) or precontoured anatomical locking plate (15 patients) and then systematically reviewed. The relevant literature of the two fixation methods were studied to note union, infection, hardware prominence, implant failure, metaphyseal fracture and other complications. Constant-Murley score was also used for evaluation.RESULTSIn our study, with both the fixation modalities, union was achieved in all cases by three months. No impingement was noted. Patients having fixation with precontoured anatomical locking plate had better scores and returned earlier to better function than the other group (p value <0.0001).CONCLUSIONSPrecontoured anatomical locking plate might be a better option which provides better functional outcome.

RESUMEN

Tripterygium wilfordii Hook F has significant anti-inflammatory and immunosuppressive properties and is widely used for treating autoimmune and inflammatory diseases such as rheumatoid arthritis, systemic lupus erythematosus, and kidney disease, especially in traditional Chinese medicine. The mechanisms underlying its effects may be diverse but they remain unclear, and its toxicity and side effects limit its wider clinical application. This review summarizes the clinical application of Tripterygium wilfordii Hook F in recent years, as well as the results of studies into its mechanisms and toxicity, to provide a reference for its future clinical application.

RESUMEN

OBJECTIVE: Because the double Endobutton plate has more advantages than the clavicle hook plate in the treatment of acromioclavicular joint dislocation, and the trauma is small; it does not need to be removed through secondary surgery. Thus, it has been widely used in clinical practice in recent years. Meta-analysis was used to investigate whether there were differences in the treatment of acromioclavicular dislocation between double Endobutton plate and clavicle hook plate so as to provide effective guidance for clinical treatment. METHODS: The database Cochrane library, PubMed, EBSCO, CNKI, VIP and Wanfang were searched from the database inception to January 2020. All the literatures related to the treatment of acromioclavicular dislocation with double Endobutton plate and clavicle hook plate were collected. The literatures were selected according to the inclusion and exclusion criteria to extract the data and to carry out the relevant analysis. The literatures that meet the requirements were screened out. The quality of the selected relevant literatures was assessed. All outcome indicators were analyzed by RevMan 5.3 software. RESULTS: (1) A total of 38 articles were included, and the total number of cases was 2 199, of which 1 035 cases were in the double Endobutton plate group and 1 164 cases were in the clavicle hook plate group. (2) The results showed that the double Endobutton plate group was superior to the clavicle hook plate group in the excellent and good rate [RR=1.19, 95%CI(1.13, 1.26), P < 0.000 01], operation time [MD=10.54, 95% CI(6.09, 15.00), P < 0.000 01], intraoperative blood loss [MD=-14.83, 95%CI(-20.54,-9.13), P < 0.000 01], hospitalization time [MD=-1.44, 95%CI(-2.65,-0.23), P=0.02], incision length [MD=-2.87, 95%CI (-3.60, -2.14), P < 0.000 01], hospitalization cost [MD=-2 442.10, 95% CI(-4 466.34, -417.86), P=0.02], shoulder pain after operation [RR=0.34, 95%CI(0.22, 0.53), P < 0.000 01], visual analogue scale score [MD=-0.85, 95% CI(-1.28, -0.41), P=0.000 2], final Constant-Murley score at 6 months postoperatively [MD=8.82, 95%CI(1.91, 15.72), P=0.01; MD=6.66, 95%CI(4.46, 8.86), P < 0.000 01]CONCLUSION: The treatment of Rockwood III type and above acromioclavicular joint dislocation with double Endobutton plate is superior to that clavicular hook plate. It has the advantages of short operation time, less intraoperative blood loss, less trauma, short hospitalization time and less cost, and good functional recovery of shoulder joint after operation.

@#AIM:To analyze the effectiveness and safety of different surgical strategies for cataract combined with subluxated lens. <p>METHODS: This is a retrospective study. Data are acquired from patients who diagnosed with cataract combined with subluxated lens between November 2018 to May 2019 in the ophthalmological center of the First Affiliated Hospital of Zhejiang University of Traditional Chinese Medicine. There are 11 eyes in 11 patients(9 males and 2 females), 10 eyes were caused by trauma and 1 eye was unknown. Cataract combined with subluxated lens were treated with different surgery procedures according to the degree of dislocation. Four eyes with lens dislocation larger than 270° underwent intraocular lens suspension; Four eyes with lens dislocation about 180° underwent IOL and tension ring implant in capsular, at the same time, the iris hook was hooked to the lens capsule bag and fixed to the sclera. Three eyes with lens dislocation less than 180° underwent IOL implantation combined with 5-0 polypropylene suture, iris hook was also hooked to the lens capsule bag and fixed to the sclera. If the anterior chamber has vitreous prolapse, anterior vitrectomy was performed. Best corrected visual acuity(BCVA)and intraocular pressure(IOP)were observed before and after surgery. <p>RESULTS: All eyes were successfully implanted with intraocular lens(IOL). Best corrected visual acuity(LogMAR)increased from 0.77±0.26 to 0.35±0.28. Mean IOP decreased from 24.33±13.55 to 13.85±3.80mmHg. No intraoperative complications occurred in all cases.<p>CONCLUSION: In the treatment of lens dislocation with cataract, individualized surgical plan and flexible treatment measures can make cataract surgery safe and effective.

BACKGROUND: At present, most of the treatment of unstable dislocation of sternoclavicular joint is to insert the hook plate of acromioclavicular joint into the cavity of sternal bone marrow or put it behind the sternum. The difficulty of operation is increased because of the insufficient application of steel plate, which reduces the effect of joint reduction. OBJECTIVE: The research team has developed a new type of sternoclavicular hook plate (Patent No. ZL201520515237.3), and compared the biomechanical properties of acromioclavicular hook plate and new sternoclavicular hook plate to fix sternoclavicular joint from the aspect of biomechanics. METHODS: The sternum and clavicle at both ends of the sternoclavicular joint were replaced with a rectangular artificial bone. Eighteen pairs of artificial sternoclavicular joints were randomly divided into two groups. In control group (n=6), the plate screw hole section of the shortest segment 3-hole common acromioclavicular hook plate was fixed to the artificial clavicle end; and the hook end was placed on the back side of the artificial sternum. Load and displacement changes in artificial sternal fractures were measured with MTS biomaterial test system. In the experimental group (n=12), the plate screw hole section of the shortest rod section 2 hole new sternoclavicular hook plate was fixed to the artificial clavicle end; and the hook end was inserted into the artificial sternum. The MTS biomaterial test system was used to detect changes in load and displacement of the anterior cortical bone (n=6) and posterior cortical bone (n-6). RESULTS AND CONCLUSION: In the control group, the mean of maximum destructive force was (390±51) N. The mean of maximum destructive force of anterior cortical bone was (421±55) N. In the experimental group, the mean of maximum destructive force of posterior cortical bone was (437±56) N. There were no significant differences between the three groups (P > 0.05). Results indicated that the new sternoclavicular hook plate did not increase the risk of sternum fracture, and it was a safe and effective internal fixation material.
