RESUMEN

RESUMEN: Proponer un plan de tratamiento con técnicas quirúrgicas y protocolos protésicos predecibles representa un gran desafío profesional, especialmente en pacientes con un maxilar atrófico. Entre las alternativas terapéuticas, la prótesis híbrida sobre cuatro implantes dentales correctamente distribuidos permite formar un área de distribución de carga, la cual guarda relación con las necesidades biomecánicas del elemento protésico.


ABSTRACT: Proposing a treatment plan with surgical techniques and predictable prosthetic protocols means a great professional challenge, especially in patients with an atrophic maxilla. Among the therapeutic alternatives, the hybrid prosthesis on four correctly distributed dental implants, allows to form a load distribution area, which is related to the biomechanical needs of the prosthetic element.

RESUMEN

In edentulous patients, the advantages of prosthodontic therapy using oral implants include both stability and comfort. Advantages suggested for this approach include the prevention of mandibular bone resorption and increased masticatory function. RESUMEN

Down syndrome patients generally have poor oral hygiene due to lack of awareness and ability to perform oral care, and there is a high risk of tooth loss. Also, they are characterized by small and irregular teeth and oligodontia, which makes it difficult to do prosthetic treatment. This article reports a case of implant hybrid prosthesis treatment of maxillary edentulous area in Down syndrome patient. The external shape of the final prosthesis was determined by using the mandibular artificial teeth to reflect the oligodontia of the mandible and the pontic areas were adjusted to facilitate oral hygiene care. As a result, aesthetically and functionally satisfactory results were obtained.

RESUMEN

Prótesis híbridas metal-acrílico implanto-retenidas han brindado solución funcional y estética para rehabilitar arcos completos severamente atróficos, constituyendo una alternativa a cirugías reconstructivas más complejas. La subestructura protésica debe ajustar pasivamente sobre los implantes. Con la tecnología CAD-CAM el diseño manual de las subestructuras, mediante cera perdida, se ha reemplazado por diseño digital mediado por software. La tecnología de sinterizado láser en la confección de subestructuras metálicas tiene como ventajas la disminución de acumulación de errores, rapidez y diseño de geometrías complejas según las necesidades del caso. Caso Paciente desdentada total. Se planifica prótesis híbrida maxilar (8 implantes) y mandibular (Revitalize®) con subestructuras en aleación Cr-Co mediante sinterizado láser, que presenta resultados a largo plazo similares a estructuras nobles, con menor costo y alta predictibilidad. Conclusiones Prótesis híbridas con subestructura Cr-Co mediante sinterizado láser podrían mejorar el pronóstico al incorporar CAD-CAM en su confección al obtener ajustes de precisión.


Abstract Implant-retained metal-acrylic hybrid prostheses provide a functional and aesthetic solution to rehabilitate severely atrophic complete arches, and are an alternative to more complex reconstructive surgeries. the prosthetic substructure must adjust passively on the implants. With the CAD-CAM technology, the manual design of the substructures, by lost wax, has been replaced by digital design. Laser sintering technology in the production of metal substructures has advantages, such as, decreased accumulation of errors, fabrication speed and design of complex geometry, according to the needs of the case. Case report Hybrid prostheses in the maxilla (8 implants) and jaw (Revitalize®) were planned in a patient with no teeth, using Co-Cr alloy substructures by laser sintering, which have similar long-term results as obtained with noble structures, but with lower cost and high predictability. Conclusions Hybrid prostheses with Co-Cr substructures using laser sintering could improve prognosis by incorporating CAD-CAM in their preparation to obtain fine adjustments.

RESUMEN

Loss of teeth may not only imply impaired oral function and loss of alveolar bone but is also often accompanied by reduced self-confidence. This results in a larger problem with the fully edentulous patient. The patient introduced in this study showed multiple missing teeth and mobility of remaining teeth and wanted to have fixed dental prosthesis using implants. Remaining teeth were extracted because of periodontally bad prognosis. This article reports a satisfactory clinical and esthetic outcome of full mouth rehabilitation using implant hybrid prosthesis in fully edentulous patient.

PURPOSE: The aim of this study was to evaluate clinical outcomes of implant supported fixed-hybrid prostheses (FHP) in the fully edentulous arches. MATERIALS AND METHODS: Patients in this retrospective study were restored with fixed-hybrid prostheses supported by 4 to 6 implants and functioned more than 1 year of loading. Outcome measures were marginal bone change of implant related with sex, anatomical location (maxilla vs. mandible), opposing teeth, loading time of patients, tilting of posterior implant by Mann-Whitney U test and cantilever length of superstructure by regression analysis, and complication rates. Significance level was set P.05), and cantilever length was not significantly related with a marginal bone loss of implant next to cantilever (P>.05). Complication was shown in 11 patients and veneer fracture and dislodging of artificial teeth were most prevalent. CONCLUSION: Within the limitations of this study, although marginal bone loss of FHP was very little, complication rates were high. Irrespective of tilting of most posterior implants, marginal bone loss of most posterior implants next to cantilever was less than those of the other implants positioned anteriorly. Cantilever length (<17 mm) did not affect a marginal bone loss of most posterior implants.

Las prótesis removibles de extremidad libre son las que más sujetas a movimientos están, por las diferentes fuerzas que actúan sobre ellas, en las excursiones funcionales que realiza el paciente en su actividad diaria al hablar, masticar, deglutir, etc. La unión de estas prótesis con los dientes pilares remanentes que le sirven de anclaje ha sido y es preocupación de los protesistas por la tracción ejercida sobre ellos. Este trabajo presentó un caso del uso de medios de conexión “rompe fuerzas” muy estudiado, pero poco visto tanto por especialistas, como residentes que se forman en este campo. Se presentó la rehabilitación de un paciente con prótesis híbrida.


The free ending removable prosthesis are those more subject to movements, because of the different forces acting on them in the functional excursions the patient makes in its daily activities when speaking, masticating, swallowing, etc. The splitting of these prostheses with the remnant abutment teeth serving as anchorage has been and is preoccupation of the prosthetics because of the traction exerted on them. In this work we present a case of the usage of “breaking forces” splitting means that have been studied very much, but almost never seen by the specialists, like residents forming in our field. We present the rehabilitation of a patient with hybrid prosthesis.
