RESUMEN

PURPOSE: The purpose of this study was to explore and analyze the latest trends in domestic research related to integrated nursing care service and to present a practical direction for future research.METHODS: This study was focused on domestic nursing research related to integrated nursing care service as published in journals until March 20, 2019. After reviewing the quality of the papers, 34 papers were finally selected for analysis. In order to analyze the overall trends, the analysis was reviewed by year, source, research design, and subjects, and content analysis was used to identify trends by topic.RESULTS: From the content analysis, the top categories were divided into nurses' view and patients' view, with both nurses' view and patients' view for the subcategories of ‘input’, ‘process’ and ‘output’ respectively. From both the nurse and patient view, the sub-category ‘input’ included ‘service environment’, ‘process’ and ‘service throughput’ and for the ‘output’ subcategory, three subjects, ‘performance’, ‘service-related perception change’ and ‘social psychological change’ were derived from the nurses' view, and two subjects ‘performance’ and ‘service-related perception change’ were derived from the patients' view.CONCLUSION: In order for future studies to provide practical basic data for stable settlement and improvement of integrated nursing care service, it is necessary to develop and study various aspects without bias, and in a balanced and integrated manner.

Objective@#To explore the influence of integrated nursing care mode on the incidence of deep vein thrombosis in lower extremities in bedridden patients.@*Methods@#82 cases of chronic bed rest patients in our hospital were selected as clinical subjects. The time of admission was from December 2016 to December 2017. The patients were divided into control group (routine nursing, 41 cases) and observation group (medical nursing integrated nursing, 41 cases). To compare the influence of nursing intervention on the incidence of deep venous thrombosis of the lower extremities.@*Results@#After treatment, the incidence of deep vein thrombosis in the control group was 24.39% (10/41), and that in the observation group was 4.88% (2/41). The difference was statistically significant (χ2=6.248, P<0.05). The prevention disease score and nursing satisfaction score of the observation group were (35.01±4.21) points and (87.02±5.22) points, and the control group were (28.65±5.87) points and (68.75±4.64) points. The difference was statistically significant (t=5.638, 16.750, P<0.01). The DD dimer, aPTT and PT of the observation group were (448.82±56.27) ug/L, (29.75±2.01) s, (12.88±0.65) s, and the control group were (610.32±72.88) ug/L, (26.81±1.98) s, (10.54±0.59) s. The differences were statistically significant (t=11.231, 6.672, 17.068, P<0.05).@*Conclusion@#For the patients in bed, the effect of nursing intervention is obvious, which can effectively reduce the incidence of deep venous thrombosis and improve the blood coagulation function of the patients.

Objective To discuss the effect of self-efficacy on patients with coronary heart disease cured in general practice department based on Hospital-Community-Patient Integrated Nursing Mode. Methods From January to April in 2018, 106 patients (51 males and 55 females) with coronary heart disease hospitalized in general practice of hospital were selected as subjects of study. Random number table method was used to divide the patients into control group and intervention group, 53 cases in each group. The intervention group adopted the hospital-community-patient integrated nursing model, while the control group adopted the traditional health education mode after discharge. Self-efficacy evaluation was conducted before intervention, 3 months after intervention and 6 months after intervention. Results The total score of self-efficacy in the two groups was higher than that before intervention, but the increase in the intervention group was significantly better than that in the control group, the difference was statistically significant (F=34.681, P < 0.01). The total scores of self-efficacy in intervention group were (30.35 ± 2.58), (33.59 ± 2.68) points respectively 3 months and 6 months after intervention, which were higher than (28.95 ± 2.42), (29.10 ± 2.12) points in control group. The difference was significant (t =3.702, 13.494, P<0.01). Conclusion Hospital-community-patient integrated nursing model is superior to traditional health education model after discharge, which can significantly improve the self-efficacy of patients with coronary heart disease discharged from general practice department.

Objective@#To explore the application experience of integrated nursing mode in the treatment of extremely severe burn patients in August 2nd Kunshan factory aluminum dust explosion accident.@*Methods@#On August 2nd, 2014, 35 extremely severe burn patients involved in the August 2nd Kunshan factory aluminum dust explosion accident were admitted to Wuxi Third People′s Hospital, including 18 males and 17 females, aged 21-50 years. According to the characteristics of the wounded, the situation of the nursing staff, and the characteristics of the nursing work, the integrated nursing mode was constructed and implemented to improve overall nursing quality. The standardized management measures such as cluster management of facilities and equipments in wards, improving and unifying nursing system, standardized training, drawing up " Nurses Compulsory Reading" , optimizing nursing shift handover and so on were taken. Professional quality control groups such as continuous renal replacement therapy (CRRT) group, static therapy group, airway group, and burn group were established, and standardized writing nursing group, wound nursing group, psychological nursing group, and enteral nutrition nursing group were set up under burn group. The treatment outcomes of patients and effects of nursing management, nursing methods, and specialty nursing were recorded.@*Results@#Twenty-seven patients survived the shock period, infection period, and recovery period smoothly. The success rate of rescue was 77.14%. During the treatment, the ward was in good order. The implementation rate of disinfection and isolation system, the completion rate of shift handover, the standard rate of intravenous therapy, the implementation rate of bed head elevation, the correct rate of posture placement, and the success rate of CRRT were all 100%. Successful turn over of rotating bed without interruption of CRRT for 24 hours was implemented in two patients. In many cases, the single filter for hemodialysis continuously run for more than 72 hours. The airway mucosa of patients healed around 20 days after injury. No adverse nursing events such as tracheal cannula detachment/blockage, respiratory distress, atelectasis, lung consolidation, aspiration by mistake, rotating bed rollover, ear chondritis, nasal septal pressure ulcer, vacuum sealing drainage (VSD) catheter blockage, VSD dressing leakage, severe abdominal distension/diarrhea, non-planned extubation/blockage of various intravenous treatment catheters implanted into deep veins and arteries were observed.@*Conclusions@#The integrated nursing mode significantly optimizes the nursing work process in the treatment of extremely severe mass burns, clarifies the duties of nursing staff, and improves the quality of nursing. This mode is worthy of taking reference by other burn treatment units.

RESUMEN

Objective To investigate the influence of integrated nursing intervention on cognitive function in patients with chronic schizophrenia. Methods 64 patients with chronic schizophrenia in our hospital from December, 2016 to December, 2017 were selected. All the patients met the diagnostic criteria of CCMD-3 chronic schizophrenia. Patients in two groups were treated with routine psychotropic medication. Patients in control group received general nursing care and routine nursing care including giving prescribed medicine on time. Patients in intervention group were treated with integrated nursing intervention in addition. The total score of WMS-RC and score of every term in SSPI before and after intervention in two groups were compared. Results The total score of WMS-RC and score of every term in SSPI after intervention in two groups increased compared with those before intervention, and the total score of WMS-RC and score of every term in SSPI in intervention group were significantly higher than those in control group(P<0. 05). Conclusion Integrated nursing intervention along with active application of medication in patients with chronic schizophrenia could improve the cognitive function of patients so as to promote the recovery and the early return to society.

RESUMEN

PURPOSE: This study was done to evaluate the adequacy of nurse staffing in integrated nursing care. METHODS: Statistical data on integrated nursing care from the National Health Insurance Corporation was used in this study. We extracted hospital data and patient data related to patient needs for nursing care. We analyzed the differences in patient needs by staffing level of each type of medical institution. RESULTS: Approximately 70% of medical institutions provided nursing care by the mid level of nurse staffing, which was 1:6, 1:10 and 1:12 in the tertiary hospital, general hospital and semi-hospital, respectively. The patients' characteristics were significantly different by hospital type. Especially, the distribution of the main diagnosis was completely different between the tertiary hospital and semi-hospital. In the tertiary hospital, the patient needs measured by severity and activities of daily living dependency were higher at higher staffing level than at lower staffing level. However, the nurse staffing was less relevant to the patient needs in the general hospital and semi-hospital. CONCLUSION: To provide high-quality nursing care, accurate workload forecasting is required at the start, and then the standard nurse staffing level can established based on workload forecasting.

Objective To explore the role conversion, emotion regulation and maternal lactation effect of obstetrics in-tegrated nursing model on maternal. Methods A total of 160 cases maternal delivery in our hospital from May 2011 to May 2013 were randamly divided into observation group 80 cases and control group of 80 cases, the control group was treated with routine nursing, the observation group based on the control group underwent integrated nursing model. The maternal role conversion and emotion regulation effect was compared between two groups, and lactation maternal initial time, lactation and serum PRL levels were observed. Results The role conversion and neonatal care ability score in ob-servation group were significantly higher than the control group, and the difference between two groups with statistically significant(P<0.05). Two groups of maternal SAS, SDS score was significantly lower, and the observation group SDS, SAS score was significantly lower than the control group(P<0.05). The observation group in the initial time of lactation signifi-cantly earlier than the control group, postpartum 24 h, 48 h, 72 h lactation quantity and the level of serum PRL was significantly higher than that of the control group, and each index of two group with statistical significance(P<0.05). Conclusion Integration nursing mode can be accelerate maternal role conversion, improve maternal negative emotion, promote milk secretion,the method is easy to learn, and worthy of clinical application.

Objective To explore the influence of comprehensive nursing intervention on anxiety and adverse reaction in patients undergoing painless gastroscopy. Methods In our hospital from January 2011 to January 2014 for painless gastroscopy patients 120 cases as the observation object, the two groups underwent painless gastroscopy, while the control group was given routine care, the intervention group on the basis of the control group implementation of comprehensive nursing intervention, adverse reactions during examination, SAS score were compared between the two groups. Results After the intervention, two groups of patients with SAS scores were(36.34±7.32),(41.78±8.26), the difference was sta-tistically significant between two groups (P<0.05). And after checking, SAS score of the patients in the intervention group was significantly lower than that of the control group. None of the patients developed delirium in intervention group, control group of 2 patients with delirium. Agitation, decreased heart rate, respiratory depression, the incidence of nausea and vomiting in the intervention group patients was significantly lower than the control group, the difference was statistically significant (P<0.05). Conclusion For the first time in painless gastroscopy patients implement the compre-hensive nursing intervention during perioperative period can alleviate the patients psychological anxiety, reduce the in-cidence of adverse reaction during examination, so as to ensure the safe,effective check completed successfully.

Objective Discuss recovering guidance of effect of coronary heart disease and angina Patients. Methods 120 patients with coronary heart disease and angina were randomly selected for recover self health at home recovering guidance. The process includes data collection, evaluation, action and analyzing the feedback of the home practice in the recovering guidance application within two years when the patient is out of the ward. Results Recover self health at home Recovering Guidance for coronary heart disease and angina patients is effective on preventing and reducing the disease relapse, the neopathy of the brain, kidney,liver and mortality.Conclusions Recover self health at home Recevering Guidance for coronary heart disease and angina patients could help to short patient’s in-ward period, speed in and out ward circulation, relieve his distress and treatment cost of the patients, strengthen the communication between the patient and nurses. It is also a way to improve the comprehensive nursing ability of the nurses.

Em 1996, um grupo de docentes das áreas de Imunologia, Histologia, Nutrição, Patologia e Fundamentos de Enfermagem, iniciou o Projeto de Ensino junto ao Programa de Apoio a Projetos Integrados de Ensino e Serviços de Saúde (PAPIENS), no Curso de Enfermagem da Universidade Estadual de Londrina. Este Programa está inserido no Projeto UNI subsidiado pela Fundação Kelloggs, que tem possibilitado inovações no processo ensino-aprendizagem e mudanças curriculares nos cursos da área de Saúde desta Universidade. Este Projeto tem como conteúdo integrador o tema Feridas e como objetivo principal propiciar ao aluno a construção deste conhecimento através da articulação e integração das diferentes áreas envolvidas, partindo da observação da prática dos pacientes portadores de feridas, para a busca do conhecimento. Concluise que o Projeto tem facilitado a aprendizagem dos alunos, além de preparar os professores para o desafio do Currículo Integrado do Curso de Enfermagem a ser implantado no ano 2,000.


In 1996, a group of professors of the areas of Immunology, Histology, Nutrition, Pathology and Foundations of Nursing, began the Educational Project with the Support Program to Integrated Projects of Education and Health Services (PAPIENS), in the Nursing graduation course of the State University of Londrina. This Program is inserted in the project UNI subsidized by the Kellogg Foundation, which has propitiated innovations in the teaching-learning process, as well as changes in the curriculum of courses in the health area of the university mentioned before. This project has as its linking content the subject wounds. The main objective is to propitiate to students the construction of knowledge, regarding this theme, through the articulation and integration of the different areas involved, as well as through the observation of patients who carry wounds. It was observed that the project has facilitated student's learning and also prepared teachers for the challenge of the Nursing Integrated Curriculum to be implemented in the year 2000.


En 1996 un grupo de docentes en Imunologia, Histología, Nutrición, Patología y Fundamentos de Enfermería empezó el Proyecto de Enseñanza junto al Programa de Apoyo a Proyectos Integrados de Enseñanza y Servicios de Salud (PAPIENS), en el Curso de Enfermería de la Universidad Estadual de Londrina. El Programa se insiere en el Proyecto UNI, subvencionado por la Fundación Kelloggs, y ha posibilitado innovaciones y cambios curriculares en cursos de la Salud de esta Universidad. El Proyecto tiene como contenido integrador el tema Heridas y el objetivo principal es el de proporcionarle al alumno la construcción de ese saber, mediante la articulación e integración de las distintas áreas que lo integran, partiendo de la observación y práctica con los pacientes portadores de heridas. Se concluye que dicho Proyecto facilita el aprendizaje de los alumnos, además de preparar a los profesores para el desafío del Currículo Integrado del Curso de Enfermería, que debe implantarse este mismo año.
