Resumo Contexto Pacientes com isquemia crítica (IC) dos membros inferiores (MMII) precisam de arteriografia para o planejamento da cirurgia de revascularização. A ultrassonografia Doppler (UD) não é invasiva e, através da aferição do índice de resistência (IR), pode fornecer informações sobre as artérias distais. Objetivos Correlacionar a Classificação Angiográfica de Rutherford com o IR na avaliação do leito arterial distal dos MMII. Métodos Estudo transversal, realizado em hospital público terciário, com 120 pacientes portadores de IC dos MMII, entre setembro de 2019 a abril de 2022. Foi comparado o IR das artérias da perna passíveis de serem receptoras de revascularização com a imagem obtida através da arteriografia dessas artérias em acordo com a Classificação Angiográfica de leito distal de Rutherford. Resultados Foram avaliados 120 MMII em 120 pacientes com idade média de 68,6 anos. A amostra foi composta de 50,0% de pacientes do sexo masculino. Na amostra, 90,0% pacientes encontravam-se na classe cinco de Rutherford. Os valores do IR encontrados para as artérias de perna apresentaram uma correlação positiva, estatisticamente significativa, quando comparados com a Classificação de Rutherford (tibial anterior, p< 0,01; tibial posterior, p = 0,012 e fibular, p = 0,034 e artéria dorsal do pé, p < 0,001). Conclusões Neste estudo, os IRs das artérias da perna obtidos através da ultrassonografia Doppler apresentaram uma correlação positiva quando comparados à classificação de Rutherford. Em pacientes com isquemia crítica, esse índice pode ser útil na avaliação do leito arterial distal dos membros inferiores.


Abstract Background Patients with chronic limb threatening ischemia (CLTI) of the lower limbs (LL) undergo arteriography for revascularization surgery planning. Doppler ultrasound (DU) is non-invasive and can provide information about the distal arteries through measurement of the resistance index (RI). Objectives To correlate the Rutherford Angiographic Classification with the RI for assessment of the distal arterial bed of the LL. Methods A cross-sectional study, conducted at a public tertiary hospital with 120 patients with LL CLTI, from September 2019 to April 2022. The RI of arteries that were candidates for revascularization was compared with the images of the same arteries obtained using arteriography, using the Rutherford Angiographic Classification of the distal bed. Results A total of 120 LL were assessed in 120 patients with a mean age of 68.6 years. The sample was 50.0% male and 90.0% of the patients in the sample were classified as Rutherford category five. The RI values found for the arteries of the leg exhibited a statistically significant positive correlation with the Rutherford Classification (anterior tibial, p< 0.01; posterior tibial, p = 0.012 fibular, p = 0.034; and dorsalis pedis, p < 0.001). Conclusions In this study, RIs for the arteries of the leg measured using Doppler ultrasound exhibited a positive correlation with the Rutherford Classification. This index could be useful for assessment of the distal arterial bed of the lower limbs of patients with chronic limb threatening ischemia.

SUMMARY: The aim of this study was to determine influence of upper limbs on the ball throwing velocity. A total of 10 professional handball players (25.74±4.84 years) participated in this study. All of them were playing in the top Montenegrin professional handball league. The results obtained in this study shows that upper limbs have high influence on ball throwing velocity. This study provides normative data and performance standards for professional handball. Coaches can use this information to determine the type of anthropometric characteristics that are needed for handball. Anthropometric parameters such as arm length, wrist diameter, hand length and arm span are the most relevant aspects related to ball throwing speed, given that these parameters cannot be changed through training, they should be taken into account when discovering talents.


El objetivo de este estudio fue determinar la influencia de los miembros superiores sobre la velocidad de lanzamiento de la pelota. En el estudio participaron un total de 10 jugadores profesionales de balonmano (25,74±4,84 años). Todos ellos jugaban en la principal liga profesional de balonmano de Montenegro. Los resultados obtenidos mostraron que los miembros superiores tienen una alta influencia en la velocidad de lanzamiento de la pelota. Este estudio proporciona datos normativos y estándares de rendimiento para el balonmano profesional. Los entrenadores pueden utilizar esta información para determinar el tipo de características antropométricas necesarias para el balonmano. Los parámetros antropométricos como la longitud del brazo, el diámetro de la muñeca, la longitud de la mano y la envergadura del brazo son los aspectos más relevantes relacionados con la velocidad de lanzamiento de la pelota, dado que estos parámetros no se pueden cambiar mediante el entrenamiento, deben tenerse en consideración a la hora de descubrir talentos.

Abstract Introduction: Complex regional pain syndrome (CRPS), also known as Sudeck syndrome, is a chronic painful condition usually affecting the limbs after trauma or surgery. Its presentation is heterogeneous and its physio pathology, diagnosis and treatment remain controversial. The objective of this study was to analyze a group of patients with this rare syndrome, describing in detail the results of the dual energy X-ray absorptiometry (DXA) and the response to bisphosphonate treatment. Method: We retrospectively analyzed 54 patients with CRPS, taking into account their demographic features, inciting events and diagnostic tests. As regards treat ment, we analyzed the results and adverse events of the use of bisphosphonates Results: We found a female predominance (74%), with 55 ± 13 years. The most common inciting event was trauma (59%), followed by surgery. The difference in bone mineral density between the affected limb and the healthy one was 12 to 15%. Forty-four patients were treated with bisphosphonates (pamidronate, iban dronate, zoledronic acid) and showed a clinical im provement, mainly in terms of pain intensity. Only six patients presented with adverse events, like pseudoflu syndrome and acute phase symptoms. Conclusion: In our cohort, lower limbs CRPS pre dominantly affects middle aged women. DXA scans are tests used to quantify bone loss and the response to treatment. The use of bisphosphonates is an interest ing therapeutic option to improve clinical symptoms in most patients. Future prospective randomized studies will be needed to confirm our results.


Resumen Introducción: El síndrome doloroso regional complejo (SDRC), también conocido como síndrome de Sudeck, es una enfermedad dolorosa crónica que generalmente afecta a las extremidades luego de un trauma o cirugía. Su presentación es heterogénea y existen controversias sobre su fisiopatología, adecuado diagnóstico y trata miento. El objetivo de este trabajo es describir un grupo de pacientes con SDRC en miembros inferiores, describi endo los resultados de la densitometría mineral ósea (DMO) y la respuesta al tratamiento con bifosfonatos. Método: Analizamos retrospectivamente 54 pacientes con SDRC, teniendo en cuenta características demográ ficas, factores desencadenantes y estudios diagnósticos. En relación al tratamiento, analizamos los resultados y efectos adversos del uso de bifosfonatos. Resultados: Encontramos un predominio femenino (74%), con una edad de 55 ± 13 años. Los factores des encadenantes más comunes fueron los traumatismos (59%) y la cirugía. La diferencia de densidad mineral ósea entre el miembro comprometido y el sano fue 12 a 15%. En los 44 pacientes fueron tratados con bifosfona tos (pamidronato, ibandronato y ácido zoledrónico), su uso se asoció a mejoría clínica, especialmente del dolor. Seis pacientes tuvieron efectos adversos como sindrome pseudogripal y síntomas de fase aguda. Conclusión: En nuestra población, el SDRC de miem bros inferiores predomina en mujeres de edad media. La DMO es un método que permite cuantificar la pérdida ósea y la respuesta al tratamiento. Los bifosfonatos son una buena opción terapéutica para el control de síntomas. Son necesarios futuros estudios de naturaleza prospectiva y aleatorizada para confirmar nuestros resultados.

RESUMEN

Sirenomelia also called mermaid syndrome is a congenital, rare, lethal, multisystemic human malformation of unknown etiology. The characteristic feature of sirenomelia is the fusion of the lower limbs, resulting in the appearance of a mermaid’s tail, and thus the name “mermaid syndrome.” This condition is also characterized by various severe urogenital abnormalities and the presence of a singular umbilical cord blood vessel, and it is more common in infants of diabetic mothers and in monozygotic twins. The incidence is around 1 in 60,000–70,000 pregnancies. The majority of affected fetuses are stillborn, whereas the rest of the live-born die in the early neonatal period due to complications of the gastrointestinal and urogenital systems. We are reporting a case of sirenomelia in a neonate born to an unregistered multipara mother. The baby had perinatal asphyxia, sirenomelia, dextrocardia, low set ears, lymphatic malformation, bilateral renal agenesis, absent external genitalia and anus, single umbilical artery, and congenital corneal clouding. There was no antenatal ultrasonography done and the baby died at 6 h of life.

Introducción: La enfermedad arterial periférica es una de las afecciones más prevalentes. Resulta habitual su coexistencia con la enfermedad vascular en otras localizaciones. El diagnóstico precoz tiene importancia para mejorar la calidad de vida del paciente y reducir el riesgo de eventos secundarios mayores, como el infarto agudo de miocardio o el ictus. Objetivo: Caracterizar el comportamiento de la isquemia arterial aguda trombótica en miembros inferiores en pacientes que ingresaron en el Instituto Nacional de Angiología y Cirugía Vascular. Métodos: Se realizó un estudio observacional, descriptivo y retrospectivo, con el fin de determinar factores pronósticos en la evolución final del tratamiento quirúrgico de la isquemia arterial aguda trombótica en miembros inferiores, en pacientes que ingresaron en los servicios de Arteriología y Angiopatía Diabética del Instituto Nacional de Angiología y Cirugía Vascular durante un período de cuatro años. Resultados: El grupo de edades más afectado estuvo entre 40 y 59 años, con un predomino del sexo masculino. El hábito de fumar fue el factor de riesgo vascular más frecuente; y la amputación mayor, el procedimiento quirúrgico más empleado, por lo que el patrón oclusivo femoropoplíteo resultó el más prevalente. Conclusiones: Predominaron el sexo masculino, el hábito de fumar, la amputación mayor y el patrón oclusivo femoropoplíteo(AU)


Introduction: Peripheral arterial disease is one of the most prevalent conditions. Its coexistence with vascular disease in other locations is common. Early diagnosis is important to improve the patient's quality of life and reduce the risk of major secondary events, such as acute myocardial infarction or stroke. Objective: To characterize the behavior of acute thrombotic arterial ischemia in lower limbs in patients admitted to the National Institute of Angiology and Vascular Surgery. Methods: An observational, descriptive and retrospective study was conducted in order to determine prognostic factors in the final evolution of surgical treatment of acute thrombotic arterial ischemia in the lower limbs in patients admitted to the Arteriology and Diabetic Angiopathy services of the National Institute of Angiology and Vascular Surgery for a period of four years. Results: The most affected age group was the one of 40 to 59 years, with a predominance of males. Smoking was the most frequent vascular risk factor; and major amputation, the most used surgical procedure, so the femoropopliteal occlusive pattern was the most prevalent. Conclusions: Male sex, smoking habit, major amputation and femoropopliteal occlusive pattern predominated(AU)

ABSTRACT Older adults should perform physical activity to improve cardiorespiratory functions. Aerobic exercises improve cardiovascular function, decrease basal heart rate and blood pressure, and improve mood and energy. To facilitate the execution of aerobic physical activity in older adults, the objective of this research was to design an exercise device for older adults. Starting from design specifications such as comfort, size, weight, resistance, and cost, we developed the detailed design of a horizontal bicycle. The designed device, thanks to its horizontally disposed structure, its adequate resistance to pedaling, its dimensioning based on anthropometric measurements of the Chilean population, and the disposition of sensors for heart rate and oxygen saturation, is suitable for older adults. Its weight and size also make it convenient for use inside the home, and the use of materials, components, and manufacturing processes available nationally and within the cheapest options, increases the possibility of access by older adults.


RESUMEN Es fundamental que los adultos mayores realicen actividad física para mejorar las funciones cardiorrespiratorias, particularmente los ejercicios aeróbicos mejoran la función cardiovascular, disminuyen la frecuencia cardíaca basal y la tensión arterial, además de mejorar su estado de ánimo y energía. Con la finalidad de facilitar la ejecución de actividad física del tipo aeróbica en adultos mayores, se planteó el objetivo de este trabajo, centrado en diseñar un dispositivo para su ejercitación. Partiendo de especificaciones de diseño como comodidad, tamaño, peso, resistencia y costo, se desarrolló el diseño de detalle de una bicicleta horizontal. El dispositivo diseñado, gracias a la disposición horizontal de la estructura, la resistencia adecuada al pedaleo, su dimensionamiento con base a medidas antropométricas de población chilena, y la disposición de sensores para heart rate and oxygen saturation, es adecuado para adultos mayores. Además, su peso y tamaño lo hacen conveniente para el uso dentro de las viviendas, del mismo modo, el uso de los materiales, componentes y proceso de manufactura disponibles nacionalmente y dentro de las opciones más económicas, aumentan la posibilidad de acceso por parte de los adultos mayores.

O ajuste à amputação envolve tanto questões físicas quanto psicossociais e a satisfação com o membro artificial. Objetivo: Revisar sistematicamente a literatura acerca dos instrumentos que avaliam o ajuste psicossocial à amputação e uso da prótese e a satisfação com a prótese em pessoas com amputação de membro inferior. Métodos: Fonte de dados: Medline via Pubmed, Web of Science e Scopus. Critérios de elegibilidade: estudos originais que utilizaram questionários para avaliar o ajustamento psicossocial a amputação e uso da prótese e a satisfação com a prótese. Participantes: pessoas com amputação de membro inferior. Métodos de avaliação e síntese dos estudos: todas as análises foram realizadas por três avaliadores, de forma independente, sendo os resultados apresentados de forma descritiva, em tabelas. Resultados: Foram encontrados 239 artigos na busca inicial, sendo incluídos 12 artigos ao final da revisão. Nestes, foram identificados 14 questionários que avaliam o ajuste psicossocial e a satisfação com a prótese, porém, somente 5 são validados especificamente para a população amputada. Conclusão: A Trinity Amputation and Prosthesis Experience Scale (TAPES) e o Prosthesis Evaluation Questionnaire (PEQ) são os instrumentos mais utilizados, sendo sempre importante uma seleção criteriosa dos instrumentos a serem utilizados nas pesquisas e intervenções a fim de se obter dados válidos, confiáveis e comparáveis. The adjustment to amputation involves physical and psychosocial issues and the satisfaction with the artificial limb. Objective: To systematically review the literature about the instruments that assess psychosocial adjustment to amputation and use of the prosthesis and satisfaction with the prosthesis in people with lower limb amputation. Methods: Data sources: Medline via Pubmed, Web of Science and Scopus. Study eligibility criteria: Original studies that using questionnaires to evaluated the psychosocial adjustment to amputation and prosthesis use and about prosthesis satisfaction. Participants: people with lower limb amputation. Study appraisal and synthesis methods: all analyzes were performed independently by three evaluators, and the results were presented descriptively, in tables. Results: Were found 239 articles in the initial search, with 12 articles included at the end of the review. In these, 16 questionnaires were identified that assess the psychosocial adjustment and satisfaction with the prosthesis, however, only five are validated specifically for the amputated population. Conclusion: The Trinity Amputation and Prosthesis Experience Scale (TAPES) and the Prosthesis Evaluation Questionnaire (PEQ) are the most used instruments, being always important to carefully select the instruments to be used in research and interventions in order to obtain valid, reliable and comparable data. OBJECTIVE@#To observe the intervention effect of damage control orthopaedic(DCO) strategy on fat embolism syndrome(FES) associated with long shaft fracture of lower limbs.@*METHODS@#Retrospective analysis was made on the clinical data of 163 patients with FES associated with lower limb long shaft fractures admitted from January 2015 to May 2021. They were divided into two groups based on the time point of implementing DCO strategy in January 2018. Total of 92 patients were admitted from January 2015 to December 2017 as the control group, and other 71 patients were admitted from January 2018 to May 2021 as the intervention group. The hospital mortality, arterial oxygen saturation (SaO2), arterial partial pressure of oxygen (PaO2) and oxygenation index (OI), hemoglobin (Hb), platelet count(PLT), Harris score of hip joint, HSS score of knee joint, AOFAS score of ankle joint, clinical efficacy and complications were observed and compared between two groups.@*RESULTS@#Total of 163 patients were followed up for 12 to 18 months with an average of (16.91±1.22) months. The in-hospital mortality rate in the intervention group was 2.82% (2/71), and that in the control group was 16.30% (15/92), the difference between two groups was statistically significant(χ2=6.455, P<0.05). After the intervention, SaO2, PaO2 and OI in two groups were higher than those before the intervention(P<0.05), and after the intervention, SaO2, PaO2 and OI in two groups were statistically significant(P<0.05). Hb and PLT in two groups after intervention were higher than those before intervention (P<0.001), and there was statistically significant difference in Hb and PLT between two groups after intervention (P<0.05). The Harris score of hip joint, HSS score of knee joint and AOFAS score of ankle joint in both groups after 3 months of treatment were better than those before treatment (P<0.05). The total clinical effective rate of the intervention group was higher than that of the control group(χ2=4.194, P<0.05). The total incidence of complications in the intervention group was lower than that in the control group(χ2=4.747, P<0.05).@*CONCLUSION@#DCO strategy is helpful to reduce the in-hospital mortality of patients with FES associated with long shaft fracture of lower extremities, eliminate FES symptoms and stabilize vital signs, gain time advantage for phase Ⅱ definitive surgery, and has significant clinical intervention effect, which is worth popularizing.

ObjectiveTo observe the effect of bilateral sequential repetitive transcranial magnetic stimulation on the motor function of upper limbs in stroke patients. MethodsFrom December, 2020 to December, 2022, 62 stroke inpatients in the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine were included. They were randomly divided into control group (n = 31) and observation group (n = 31). Both groups accepted conventional medicine and rehabilitation, as well as electroacupuncture antagonistic muscle therapy. Before electroacupuncture, the observation group acceped low-frequency repetitive transcranial magnetic stimulation at primary motor cortex (M1) on the healthy side, followed by intermittent theta burst stimulation at M1 on the affected side, for four weeks. They were assessed with Fugl-Meyer Assessment-Upper Extremities (FMA-UE), modified Barthel Index (MBI) and modified Ashworth Scale (MAS), and motor evoked potential (MEP) latency was compared before and after treatment. ResultsOne patient dropped down in the observation group, and no adverse event happened. After treatment, the scores of FMA-UE and MBI significantly increased (|t| > 9.953, P < 0.001), and the score of MAS and the latency of MEP significantly decreased (|t| > 5.043, P < 0.001) in both groups; while all of them were better in the observation group than in the control group (|t| > 2.237, P < 0.05). ConclusionBilateral sequential repetitive transcranial magnetic stimulation can effectively promote the recovery of upper limb motor function in stroke patients.

Objective:To investigate the clinical effect of a 3D reconstruction assisted preoperative optimal design of anterolateral abdomen cross-region free perforator flaps for repair of soft tissue defects of limbs.Methods:Twenty patients who were treated for soft tissue defects of hand, forearm, foot and ankle in the Department of Hand Surgery, the Sixth Hospital of Ningbo from October 2017 to January 2020 were included in this study. Among the patients,17 had soft tissue defects in hand and forearm and 3 with composite tissue defects including ankle and soft tissue defect of foot. The sizes of soft tissue defects in limbs ranged from 6 cm × 8 cm - 36 cm × 18 cm. Twenty free cross-area perforator flaps were optimal designed with CTA and 3D assisted reconstruction before surgery. Following combinations of flaps were designed: (1) Free perforator flap with inferior abdominal artery and superior abdominal artery; (2) Free perforator flap with superficial iliac circumflex artery and deep iliac circumflex artery; (3) Free perforator flap with superficial iliac circumflex artery and inferior abdominal wall perforator artery; (4) Free perforator flap with perforators of superficial iliac circumflex artery plus superficial abdominal artery; (5) Free perforator flap with perforating artery of lower abdominal wall and superficial artery of abdominal wall. The overall nutritional area of a combined flap were 272.3 cm 2± 12.5 cm 2, 107.4 cm 2± 9.3 cm 2, 193.6 cm 2± 24.2 cm 2, 155.2 cm 2± 20.1 cm 2 and 203.7 cm 2± 16.3 cm 2, respectively. All the donor sites were sutured directly in one stage. The appearance, texture, blood supply, colour, joint movement of affected limbs, recovery and function of donor sites were observed through postoperative follow-up visits at the outpatient clinic. Results:Among the 20 anterolateral transventral perforator flaps, 18 flaps survived successfully; One had partial necrosis after surgery, and healed after dressing change. Subcutaneous haematoma occurred in 1 flap, and survived after drainage. In this study, there was no postoperative infection of flap. A total of 19 flaps healed in one stage, except 1 that had a delayed healing and the flap wound was closed after dressing change for 1 week. According to Disability of Arm, Shouder and Hand (DASH) questionnaire evaluation, which is widely used in the world to evaluate the therapeutic effect after limb injury, combined with the 6-12 months of follow-up, the functional recovery of 17 patients with upper limbs iniury was 7 in excellent, 9 in good and 1 in poor. The overall excellent and good rate achieved 94.1%. All the 3 patients with foot injury recovered well, and the walking and jumping were not significantly affected. The results were all excellent according to the Maryland Foot Function Scoring. Sensation of flaps was evaluated according to the British Sensory Function Evaluation, it showed: 3 in S 2, 15 in S 3 and 2 in S 3+. All 20 flaps had good blood supply, in soft texture, good colour, feeling, thickness and movement. The donor sites all healed well. Conclusion:Combined with an optimal preoperative design, the perforator flap of anterior lateral wall cross-region can obtain a satisfactory clinical efficacy in repair of large area soft tissue defects. It is a feasible treatment method.

Objective:To investigate the results of coryoint flap harvested from lower abdominal wall for covering extremely soft tissue circular defects on limbs.Methods:From March 2018 to June 2020, 15 patients who suffered from severe degloved injury were admitted into the Department of Hand Surgery, The Affiliated Traditional Chinese Medicine Hospital of Southwest Medical University. The injuries were characterised as extreme circular defects on limbs. The dimension of defects ranged from 25.0 cm×9.0 cm to 30.0 cm×18.0 cm. All wounds were taken through emergency debridement and managed by VSD. Using lower abdomen as a donor site, a conjoined flap was dissected when the wound surface became granulating. The perforator vessels of the flaps included vessels of deep inferior epigastric artery(DIEA), superficial inferior epigastric artery(SIEA) and superficial circumflex iliac artery(SCIA). The donor sites were primary closed. Postoperative follow-ups were conducted by the surgeons in the same surgical team at outpatient clinic.Results:Fourteen flaps survived completely without significant complications. Distal necrosis occurred in 1 flap, which healed with a skin graft in the second stage surgery. All flaps were reviewed during the postoperative follow-up that lasted for 18-24(mean 20) months. The aesthetic outcomes were achieved on the recipient site without hairy nor hyper-pigmentation. A concealed linea scare left at the donor sites without hernia and limited function. At the last follow-up, 5 patients were in excellent and 2 in good evaluated by following the Disabilities of the arm, shoulder and hand(DASH). With the Lower extremity functional scale(LEFS), 5 patients were in excellent and 3 in good.Conclusion:The simultaneous reconstruction of extremely large soft tissue circular defects on limbs with best possible salvage surgery can be achieved by a conjoined flap. A conjoined flap offers a concealed donor site, easy to design, flexible perforators design and larger size of soft tissue.

RESUMEN

Objective:To evaluate the therapeutic effect of excimer laser ablation (ELA) in the treatment of lower extremity arterial ischemic diseases.Methods:The clinical data of 44 patients with lower extremity ischemic diseases treated with ELA in the People′s Hospital of Xinjiang Uygur Autonomous Region from December 2020 to April 2021 were analyzed retrospectively. Among the 44 patients, there were 29 patients in lower extremity arteriosclerosis obliterans (ASO), including 3 patients with femoral artery stent occlusion. 8 patients of diabetes foot (DF) and 7 patients of thromboangiitis obliterans (TAO). Observation indicators include target vascular patency rate, amputation rate, vascular reintervention rate and mortality rate. The measurement data were expressed as mean ± standard deviation ( ± s), one-way analysis of variance was used for inter-group comparison, and paired sample t-test was used for intra-group comparison. The Chi-square test was used for comparison between count data. Results:The success rate of operation was 100% in 44 patients. The rate of major amputation in ASO group was 10.3%, while the other two groups had a major amputation rate of 0. The minor amputation rates of the three groups were 6.9%, 25.0% and 28.6%, respectively. The vascular reintervention rate was 10.3% in ASO group, 12.5% in DF group and 0 in TAO group. The 1-year mortality rate in the ASO group was 10.3%, while the other two groups had a mortality rate of 0. The 2-year mortality rate of the three group were 31.0%, 12.5% and 0, respectively.Conclusion:For the treatment of lower extremity arterial ischemic diseases, ELA is safe and effective, but the curative effect need to further clarify by large sample and long-term clinical follow-up observation.

RESUMEN

A 19-year-old man had an accidental fall from the 2nd floor and sustained multiple injuries. On radiological evaluation, the patient had symmetrical quadruple limb involvement with bilateral humerus shaft, bilateral olecranon, bilateral femur shaft, and bilateral patella fractures. The patient was actively managed using damage control orthopaedics, and a sequence of skeletal fixation was planned based on the hemodynamic stability and periodical serum lactate assessment. Symmetrical quadruple limb fractures are very rare, which could be associated with higher mortality. A meticulous clinical evaluation, periodical blood parameter assessment and strict adherence to the principles of damage control orthopaedics can be conducive to prevent life-endangering complications.

ObjectiveTo explore the effect of anodal transcranial direct current stimulation (atDCS) on premotor cortex (PMC) on upper limb motor function in patients with severe stroke. MethodsFrom June, 2021 to December, 2022, 60 patients with severe stroke in Zhejiang Provincial People's Hospital were randomly divided into control group (n = 20), primary motor cortex (M1) group (n = 20) and PMC group (n = 20). All the groups accepted routine rehabilitation treatment, while the control group accepted atDCS pseudo stimulation to the focus side PMC, M1 group accepted atDCS to the focus side M1, and PMC group accepted atDCS to the focus side PMC, for six weeks. Fugl-Meyer Assessment-Upper Extremities (FMA-UE), Wolf Motor Function Test (WMFT), the surface electromyography root mean square (RMS) ratio of affected/unaffected side of triceps and extensor carpus were used to evaluate the efficacy before and after treatment. ResultsAfter treatment, the scores of FMA-UE and WMFT, and RMS ratio of triceps and extensor carpus significantly increased in all the groups (|t| > 2.458, P < 0.05), and were better in PMC group than in the other two groups (F > 4.084, P < 0.05). ConclusionatDCS on PMC could improve the overall function of upper limb in patients with severe stroke.

Sports fatigue of the lower limbs is one of the important factors affecting sports performance. How to improve the anti-fatigue ability of the lower limbs during endurance exercise is the focus of the research field of human sports biomechanics. This study systematically reviewed the relevant literature on transcranial direct current stimulation (tDCS) intervention on lower limb endurance performance, summarized the effect of tDCS on lower limb endurance performance, and analyzed the influencing factors and potential mechanisms. The results showed that: tDCS intervention has a significant effect on the endurance performance of the whole lower limbs, but there is no unified conclusion on the effect on the endurance performance of the knee joint. The researchers deem that tDCS can increase the excitability of the primary motor cortex and reduce the activation of the supplementary motor area and the premotor area to producing a lower rating of perceived exertion, but cannot affect the perception of exercise-induced pain, and stimulation protocols varied across studies, which may be partly responsible. This study can provide a theoretical basis for exploring the central mechanism of tDCS to improve endurance performance, formulating rehabilitation and sports training programsfor different groups of people, and developing new stimulation equipment to enhance the human body’s anti fatigue ability.

ABSTRACT Introduction The explosive power of the lower limbs is an indispensable index in soccer. Having a good level of explosive power establishes a solid foundation for the performance and the competitive level of soccer players. Objective Promote the conditions to improve the explosive level of soccer players and improve their performance and competitive level. Methods This paper presents the influence of compound strength training on the explosive power of the lower limbs of soccer athletes. Based on 27 young 9-year-old soccer players from Zai Ming Football Club of a sports college as experimental material, the composite training was carried out. Results The growth rates of these rates in the experimental group were 3.3%, 2.3%, 5.0%, 3.6%, 9.2%, 12.1%, 20.5%, 5.7%, 11.9%, 16.9% and 19.5% respectively, while those in the control group were 1.2%, 0.7%, 4.2%, 3.1%, 6.8%, 7.8%, 5.1%, 4.2%, 4.8%, 12.5% and 19.2% respectively. The growth rate of the experimental group was higher than that of the control group and the blank control group. Conclusion Compared with the control group and the neutral control group, the experimental group with compound training has a better training effect on improving the lower limb explosiveness of youth soccer players. RESUMO Introdução A potência explosiva dos membros inferiores é um índice indispensável no futebol. Ter um bom nível de potência explosiva estabelece uma base sólida para o desempenho e o nível competitivo dos futebolistas. Objetivo Promover as condições para melhorar o nível explosivo dos jogadores de futebol e aperfeiçoar seu desempenho e nível competitivo. Métodos Este documento apresenta a influência do treinamento de força composta no poder explosivo dos membros inferiores dos atletas de futebol. Com base em 27 jovens jogadores de futebol com 9 anos de idade do Zai Ming Football Club de uma faculdade esportiva como material experimental, foi realizado o treinamento composto. Resultados As taxas de crescimento destes índices no grupo experimental foram de 3,3%, 2,3%, 5,0%, 3,6%, 9,2%, 12,1%, 20,5%, 5,7%, 11,9%, 16,9% e 19,5% respectivamente, enquanto os do grupo de controle foram de 1,2%, 0,7%, 4,2%, 3,1%, 6,8%, 7,8%, 5,1%, 4,2%, 4,8%, 12,5% e 19,2% respectivamente. A taxa de crescimento do grupo experimental foi maior do que a do grupo de controle e do grupo de controle em branco. Conclusão Em comparação com o grupo de controle e o grupo de controle neutro, o grupo experimental com treinamento composto tem um melhor efeito de treinamento para melhorar a explosividade dos membros inferiores dos jogadores de futebol juvenil. RESUMEN Introducción La potencia explosiva de los miembros inferiores es un índice indispensable en el fútbol. Tener un buen nivel de potencia explosiva establece una base sólida para el rendimiento y el nivel competitivo de los futbolistas. Objetivo Promover las condiciones para mejorar el nivel explosivo de los futbolistas y mejorar su rendimiento y nivel competitivo. Métodos Este trabajo presenta la influencia del entrenamiento de fuerza compuesta en la potencia explosiva de los miembros inferiores de los atletas de fútbol. Se llevó a cabo el entrenamiento compuesto con 27 jóvenes jugadores de fútbol de 9 años del Club de Fútbol Zai Ming de una escuela deportiva como material experimental. Resultados Las tasas de crecimiento de estos índices en el grupo experimental fueron del 3,3%, 2,3%, 5,0%, 3,6%, 9,2%, 12,1%, 20,5%, 5,7%, 11,9%, 16,9% y 19,5% respectivamente, mientras que las del grupo de control fueron del 1,2%, 0,7%, 4,2%, 3,1%, 6,8%, 7,8%, 5,1%, 4,2%, 4,8%, 12,5% y 19,2% respectivamente. La tasa de crecimiento del grupo experimental fue mayor que la del grupo de control y del grupo de control en blanco. ABSTRACT Introduction: Exercise is the most effective way to improve physical fitness. One can achieve the effect of wellness and fitness through scientific exercise. Running is a relatively common method of physical exercise. It plays a significant role in improving physical fitness. Objective: This study aimed to investigate the characteristics of lower extremity movements during running. The results of this study may provide better exercise planning for runners. Methods: This paper selects several runners as the research subject. The subjects started running after attaching a motion detector sensor patch to their body. Then, this paper collected kinematic data. The kinematic data includes the joint angles and range of motion (ROM) of the hip, knee, and ankle joints. Results: There was no significant difference in the distribution of peak tibial acceleration, plantar pressure, and maximum pressure of athletes under different track materials (P>0.05). There was a significant age difference between the hip and knee joints of the athletes in the overhead stage (P<0.05). Conclusion: There may not be a necessary connection between ground and lower limb impact in running athletes. Through its adjustment, the human body can dampen the load effect of the foot contact surface. Level of evidence II; Therapeutic studies - investigation of treatment outcomes.


RESUMO Introdução: O exercício físico é a forma mais eficaz de melhorar a aptidão física. Pode-se alcançar o efeito do bem-estar e da boa forma física por meio da prática de exercícios físicos. A corrida é um método relativamente comum de exercício físico. Ele desempenha um papel significativo na melhora da aptidão física. Objetivo: Este estudo teve como objetivo investigar as características dos movimentos das extremidades inferiores durante a corrida. Os resultados deste estudo podem proporcionar melhores planejamentos de exercícios para os corredores. Métodos: Este trabalho seleciona vários corredores como o objeto de pesquisa. Os sujeitos começaram a correr após anexar um adesivo com sensor detector de movimento em seu corpo. Em seguida, este trabalho coletou dados cinemáticos. Os dados cinemáticos incluem os ângulos articulares e a amplitude de movimento (ROM) das articulações do quadril, joelho e tornozelo. Resultados: Não houve diferença significativa na distribuição do pico de aceleração tibial, pressão plantar e pressão máxima dos atletas sob diferentes materiais de pista (P>0,05). Houve uma diferença de idade significativa entre as articulações do quadril e do joelho dos atletas na etapa aérea (P<0,05). Conclusão: Pode não haver uma conexão necessária entre o solo e o impacto dos membros inferiores em atletas de corrida. RESUMEN Introducción: El ejercicio físico es la forma más eficaz de mejorar la forma física. Se puede conseguir el efecto de bienestar y de puesta en forma a través del ejercicio físico. Correr es un método de ejercicio físico relativamente común. Desempeña un papel importante en la mejora de la condición física. Objetivo: Este estudio tiene como objetivo investigar las características de los movimientos de las extremidades inferiores durante la carrera. Los resultados de este estudio pueden proporcionar una mejor planificación del ejercicio para los corredores. Métodos: Este trabajo selecciona a varios corredores como objeto de investigación. Los sujetos empezaron a correr tras colocarse un parche sensor de movimiento en el cuerpo. A continuación, este trabajo recogió datos cinemáticos. Los datos cinemáticos incluyen los ángulos articulares y la amplitud de movimiento (ROM) de las articulaciones de la cadera, la rodilla y el tobillo. Resultados: No hubo diferencias significativas en la distribución de la aceleración tibial máxima, la presión plantar y la presión máxima de los atletas con diferentes materiales de pista (P>0,05). Hubo una diferencia de edad significativa entre las articulaciones de la cadera y la rodilla de los atletas en la etapa de sobrecarga (P<0,05). Conclusión: Puede que no exista una conexión necesaria entre el impacto en el suelo y en las extremidades inferiores en los atletas que corren. ABSTRACT Introduction: Aerobic gymnastics is a sport that involves a sense of beauty, rhythm and dance, with high artistic expressiveness, characteristics that have favored a strong population adherence. It is believed that the specific flexibility exercise can add a greater visual impact to their artistic movements, raising their competitive performance. Objective: Verify the impacts of lower limb flexibility exercise on the performance of aerobics athletes. Methods: Six aerobics athletes were selected to perform an 8-week experiment, adopting the PNF methodology for training. The impacts on lower limb flexibility were measured before and after the experiment, these data were analyzed and studied statistically. Results: The results of the long jump in the physical quality index of the athletes increased by 0.1209 m; the added value in the two-minute, 10-m backstroke test was 1.46. The movement performance indexes showed expressive increases: flexion increased by 2.62; right angle support increased by 0.78; split leg jump increased by 1.95; standing jump with eyes closed and the foot balance quality index was 4.63. Conclusion: Lower limb flexibility exercise can positively impact the performance of aerobic gymnastics athletes. Level of evidence II; Therapeutic studies - investigation of treatment outcomes.


RESUMO Introdução: A ginástica aeróbica é um esporte que envolve senso de beleza, ritmo e dança, com alta expressividade artística, características que favoreceram uma forte adesão populacional. Acredita-se que o exercício específico de flexibilidade possa adicionar um maior impacto visual aos seus movimentos artísticos, elevando o desempenho competitivo. Objetivo: Verificar os impactos do exercício de flexibilidade dos membros inferiores sobre o desempenho dos atletas de aeróbica. Métodos: Seis atletas de aeróbica foram selecionados para realizar uma experiência de 8 semanas, adotando a metodologia PNF para o treinamento. Os impactos sobre a flexibilidade dos membros inferiores foram aferidos antes e depois do experimento, esses dados foram analisados e estudados estatisticamente. Resultados: Os resultados do salto em distância no índice de qualidade física dos atletas aumentaram em 0,1209 m; o valor agregado no teste de dois minutos e 10 m de recuo foi de 1,46. Os índices de desempenho do movimento apresentaram aumentos expressivos: de flexão teve um aumento de 2,62; apoio em ângulo reto aumentou 0,78; salto de pernas divididas aumentou 1,95; salto de pé com os olhos fechados e o índice de qualidade de equilíbrio dos pés foi de 4,63. Conclusão: O exercício de flexibilidade para membros inferiores pode impactar positivamente o desempenho dos atletas de ginástica aeróbica. RESUMEN Introducción: La gimnasia aeróbica es un deporte que envuelve un sentido de belleza, ritmo y danza, con alta expresividad artística, características que favorecieron una fuerte adhesión de la población. Se cree que el ejercicio específico de la flexibilidad puede agregar un mayor impacto visual a sus movimientos artísticos, elevando el desempeño competitivo. Objetivo: Verificar los impactos del ejercicio de flexibilidad de los miembros inferiores en el desempeño de atletas de aeróbica. Métodos: Seis atletas de aeróbic fueron seleccionados para realizar un experimento de 8 semanas, adoptando la metodología PNF para el entrenamiento. Los impactos en la flexibilidad de los miembros inferiores fueron medidos antes y después del experimento, estos datos fueron analizados y estudiados estadísticamente. Resultados: Los resultados del salto de longitud en el índice de la calidad física de los atletas aumentaron en 0,1209 m; el valor añadido en la prueba de dos minutos y 10 m de espalda fue de 1,46. Los índices de rendimiento de movimiento mostraron aumentos expresivos: de flexión tuvo un aumento de 2,62; apoyo en ángulo recto aumentó 0,78; salto de pierna dividida aumentó 1,95; salto de pie con los ojos cerrados y el índice de calidad de equilibrio de los pies fue de 4,63. Conclusión: El ejercicio de flexibilidad de los miembros inferiores puede influir positivamente en el rendimiento de los atletas de gimnasia aeróbica.
