RESUMEN

OBJECTIVE@#To compare the clinical effect of electroacupuncture at Siguan points and flunarizine hydrochloride capsule on migraine of liver yang hyperactivity.@*METHODS@#A total of 110 patients with migraine of liver yang hyperactivity were randomly divided into an electroacupuncture group (55 cases, 2 cases dropped off) and a western medication group (55 cases, 2 cases dropped off). In the electroacupuncture group, electroacupuncture was applied at Siguan points (Hegu [LI 4] and Taichong [LR 3]), with disperse-dense wave of 2 Hz/100 Hz in frequency and current intensity of 0.1-1 mA, 30 min each time, once a day, 5 times per week for 4 weeks. Flunarizine hydrochloride capsule was given orally in the western medication group, 10 mg a day for 4 weeks. The visual analogue scale (VAS) score and the migraine attack days were observed before and after treatment, during follow-up of 1, 3 and 6 months, and the migraine symptom score was observed before and after treatment in the two groups.@*RESULTS@#After treatment, during follow-up of 1, 3 and 6 months, the VAS scores and the migraine attack days in the two groups were decreased compared with before treatment (P<0.05), and above indexes in the electroacupuncture group were lower than the western medication group (P<0.05). After treatment, the migraine symptom scores in the two groups were decreased (P<0.05), the change in the electroacupuncture group was greater than the western medication group (P<0.05).@*CONCLUSION@#Electroacupuncture at Siguan points could effectively reduce headache intensity and migraine attack days, relieve migraine symptoms in patients with migraine of liver yang hyperactivity, and the efficacy is superior to oral flunarizine hydrochloride capsules.

RESUMEN

The etiology and pathogenesis of patients with hypertension are lower deficiency and upper excess. The deficiency below the etiology comes first, and then leads to upper excess. Lower deficiency is manifested by the yin deficiency of liver and kidney. The clinical symptoms are lumbar and knee weakness, or low back pain, or lower limb weakness, or systemic fatigue, or dry throat. In fact, it is mostly manifested as excess of liver yang, manifesting Qi inversion symptoms of high blood pressure, dizziness, dry throat, or bitter mouth, or headache, or tinnitus and deafness, or palpitations, or chest pain, or gastroesophageal reflux, or heartburn acid reflux, which are also named like hyperactivity of liver yang, liver yang heat, liver defencing spleen, or spleen deficiency leading to phlegm. Deficiency of liver and kidney leads to yin failing to restrain yang, resulting in hyperactivity of liver yang, spleen deficiency generating phlegm, and phlegm blocking orifices. Hypertension is due to deficiency of liver and kidney, and its sign is hyperactivity of liver yang. Therefore, lower deficiency and upper excess process and guides the treatment and prevention of hypertension. In the treatment, with the principle of lower deficiency and upper excess, we can nourish liver and kidney Yin to defence liver Yang for hypertension.

Objective:To evaluate the clinical efficacy of Tianma Gouteng Decoction combined with amlodipine besylate tabletsin treating patients with hypertension of liver-yang hyperactivity complicated with left ventricular hypertrophy (LVH).Methods:A total of 80 patients with hypertension of liver-yang hyperactivity complicated with LVH, who met the inclusion criteria and were treated in the hospital between June 2019 and January 2021, were divided into two groups, with 40 in each group according to the random number table method. The control group was treated with amlodipine besylate tablets orally, and the observation group was treated with granules of Tianma Gouteng Decoction on the basis of the control group. Both groups were treated for 2 months. Before and after treatment, the TCM syndromes were scored and the office blood pressure was measured with sphygmomanometer, and color Doppler ultrasonic diagnostic apparatus was adopted to detect the interventricular septum thickness (IVST), left ventricular posterior wall thickness (PWT), left ventricular end diastolic diameter (LVDd) and left ventricular mass index (LVMI), and the levels of homocysteine (Hcy) and Cystatin C (Cys-C) were determined by ELISA and the level of uric acid (UA) was determined by fully automatic biochemical analyzer. The clinical efficacy was evaluated.Results:The total effective rate was 95.0% (38/40) in the observation group and that of the control group was 77.5% (31/40) ( χ2=5.17, P=0.023). After treatment, the scores of dizziness, headache, insomnia and dreaminess and vexation and forgetfulness and total score of the observation group were significantly lower than those in the control group ( t=10.06, 14.47, 15.47, 8.74, 14.50, all Ps<0.001), and the systolic blood pressure and diastolic blood pressure were significantly lower than those in the control group ( t=6.30, 8.79, P<0.001). The IVST [(9.36±1.32) mm vs. (11.23±1.07) mm, t=6.96], PWT [(8.89±1.14)mm vs. (10.03±1.02)mm, t=4.71], LVDd[(40.36±4.29) mm vs. (47.62±4.19) mm, t=7.66] and LVMI [(112.39±22.29) g/m 2vs. (148.26± 21.39) g/m 2, t=7.34] were significantly lower compared to the control group ( P<0.01). The levels of Hcy[(12.87±3.11) μmol/L vs. (19.85±3.67) μmol/L, t=9.18], UA [(276.29±19.56) μmol/L vs. (338.52±17.07) μmol/L, t=16.65] and Cys-C [(0.86±0.15) mg/L vs. (1.10±0.17) mg/L, t=6.70] were significantly lower than those in the control group ( P<0.01). Conclusion:The Tianma Gouteng Decoction can improve the clinical symptoms, control the blood pressure, relieve the left ventricular hypertrophy and improve the renal function of patients with hypertension of liver-yang hyperactivity complicated with LVH.

Objective:To observe the clinical efficacy and safety of modified Chaihu Jia Longgu Muli Tang in treating mild to moderate essential hypertension complicated with depression and liver-Yang hyperactivity syndrome.Method:Totally 121 mild to moderate hypertensive patients complicated with depression in line with the inclusive criteria were randomized into treatment group and control group. All of the enrolled patients in treatment group and control group were treated with conventional therapy. In treatment group, patients were given modified Chaihu Jia Longgu Muli Tang, one dose per day. The treatment course lasted for 4 weeks. Blood pressure, patient health questionnaire-9 (PHQ-9) score, score of traditional Chinese medicine syndrome, C-reactive protein (CRP), endothelial-dependent vasodilation, and adverse effect were observed in this study.Result:Both systolic blood pressure and diastolic blood pressure were significantly lowered when compared to control group (P<0.05). PHQ-9 score was significantly improved in treatment group (P<0.05). The score of traditional Chinese medicine syndrome was significantly improved in treatment group compared to control group (P<0.05). CRP was significantly improved in treatment group compared with control group (P<0.05). Endothelial-dependent vasodilation was significantly improved in treatment group compared with control group (P<0.05). No severe adverse effect was observed in this research.Conclusion:Chaihu Jia Longgu Muli Tang has a creation clinical efficacy in the treatment of mild to moderate essential hypertension with depression. In addition to the effect in reducing both systolic and diastolic blood pressure, modified Chaihu Jia Longgu Muli Tang was also effective in improving depression, traditional Chinese medicine syndrome and endothelial-dependent vasodilation, and reducing the level of CRP with little adverse effect.

ABSTRACT OBJECTIVE:To study the effects of the extract of Xiongmatang on 5-HT and CGRP in brain tissue of rats with liver-yang hyperactivity and blood stagnation migraine. METHODS:Fifty male spontaneously hypertension (SHR) rats were randomly divided into model control group,Zhengtian pill positive control group (1.6 g/kg),the extract of Xiongmatang low-dose,medium-dose and high-dose groups(4.5,9.0,18.0 g/kg,by crude drug),with 10 rats in each group. Another 10 normal rats were taken as normal control group. Administration group was given relevant medicine intragastrically,once a day,for consecutive 28 d. Normal control group and model control group were given equal volume normal saline intragastrically. After last administration,except for normal control group,other groups were used to stimulate trigeminal ganglia(10 min)to establish liver-yang hyperactivity and blood stagnation migraine model,and maintained the administration once more after making the model. 30 min later,general behavior and tongue quality of rats were observed,and the blood pressure was measured;the contents of 5-HT and CGRP in cerebral tissue of rats were determined by ELISA. The protein expression of CGRP in cerebral tissue of rats were determined by Western blot method. RESULTS: Compared with the normal control group, the rats in the model control group had behavioral symptoms,such as the color of conjunctiva deepened and reddened, excessive hairdressing,head flicking,and most of the rats had purple tongue;the systolic pressure and the content and protein expression of CGRP were all increased obviously(P<0.05 or P<0.01), while the content of 5-HT in cerebral tissue was decreased obviously(P<0.01). Compared with model control group,general behavior and tongue quality of rats were improved significantly in administration groups. Systolic pressure,the content and the protein expression of CGRP in cerebral tissue of rats were decreased significantly in Zhengtian pill positive control group and the extract of Xiongmatang high-dose group (P<0.05),while the content of 5-HT in cerebral tissue of rats were increased significantly in Zhengtian pill positive control group,the extract of Xiongmatang medium-dose and high-dose groups(P<0.05).CONCLUSIONS:The extract of Xiongmatang has obvious protective effect on liver-yang hyperactivity and blood stagnation migraine model rats,the mechanism of which may be associated with reducing the content of CGRP in cerebral tissue and raising the content of 5-HT in cerebral tissue

The aim of this study was to explore the effects of ZGXF decoction on amphetamine-induced rotation in PD rats with syndrome of liver-yang hyperactivity,and its mechanism involved.Rats received 6-OHDA administration via intra-substantia nigra injection and were intragastrically treated by Fu Zi decoction to establish the PD model with the syndrome of liver-yang hyperactivity.Three doses of ZGXF decoction or selegiline were given by a 28-day intragastric administration.The rats were tested for amphetamine-induced rotation asymmetry.In addition,real-time PCR were adopted to analyze the expressions of Nfe212 and Hmox1 mRNAs,while western blot to analyze the expression of Keap1 protein.As a result,it was found that ZGXF decoction dose-dependently attenuated amphetamine-induced rotation,up-regulated the expressions of Nfe212 and Hmox-1 mRNAs,and down-regulated the expression of Keap1 protein in the substantia nigra in PD rats with syndrome of liver-yang hyperactivity.It was suggested that anti-PD effects of ZGXF decoction be attributed to the up-regulation of Nfe212 and Hmox-1 mRNAs and the down-regulation of Keap1 protein,being associated with oxidative stress,in the substantia nigra of PD rats with syndrome of liver-yang hyperactivity.

Objective To investigate the clinical efficacy of mother-child reinforcing-reducing acupoint combination in treating migraine of liver yang hyperactivity type.Methods Forty patients with migraine of liver yang hyperactivity type were randomly allocated to treatment and control groups, 20 cases each. The treatment group received acupuncture by mother-child reinforcing-reducing acupoint combination and the control group, acupuncture with conventional acupoint combination. Headache symptoms were scored in the two groups before and after treatment. The clinical therapeutic effects were compared between the two groups.Results There was a statistically significant pre-/post-treatment difference in the headache symptom score in the two groups (P<0.05). There was a statistically significant difference in pre-/post-treatment headache symptom score difference value between the treatment and control groups (P<0.05). The total efficacy rate and the cure and marked efficacy rate were 90.0% and 75.0%, respectively, in the treatment group and 85.0% and 35.0%, respectively, in the control group. There was a statistically significant difference in the cure and marked efficacy rate between the two groups (P<0.05).Conclusion Mother-child reinforcing-reducing acupoint combination is an effective way to treat migraine of liver yang hyperactivity type.

Objective]The article summarizes professor Fu Ping clinical evidence-based medicine in the experience of vicarious menstruation. [Methods] From disease name,pathogenesis,therapy and so on,summarize the experience and thoughts of treating,and for a case to analyze treatment characteristics and therapeutic effect. [Results]Fu Ping holds the view that the reason for the occurrence of vicarious menstruation is liver fire invading lung,lung or kidney Yin deficiency,qi stagnation blood stasis or liver-yang hyperactivity and spleen deficienty; The location of disease is in the lung,closely related with liver and kidney.The main therapy is to nourish Yin and moisten lung or clear liver and decrease internal heat. For the syndrome of liver-yang hyperactivity and spleen deficiently,Fu Ping using method of breeding earth and suppressing wood to treat is medicable.[Conclusion]Professor Fu Ping's experience in treating vicarious menstruation is worthy of study.

Objective To investigate clinical anti-hypertension effect,clinical traditional Chinese medicine (TCM) syndrome improvement and endothelium function such as cardio-ankle vascular index (CAVI),flow-mediated dilatation (FMD) and endothelial micro-particle (EMP) protection of Tianma Gouteng Granule in interfering primary hypertension patients with hyperactivity of liver-yang.Methods Sixty six primary hypertension patients with hyperactivity of liver-yang were randomly divided into the control group (standardized treatment,n =35) and the treatment group (Tianma Gouteng Granule + standardized treatment,n =31) according to the random number method.In four weeks' follow-up treatment,the changes in clinical efficiency,TCM syndrome improvement,CAVI,FMD and EMP were observed.Results (1)Total clinical efficiency ratio was 71.43% in control vs.70.97% in treatment without statistical significance (P >0.05).(2) TCM syndrome improvement was better in treatment group than that in control group (93.55% vs 74.29%,x2 =4.39,P < 0.05),especially in improving symptoms such as dissiness,headache,dysphoria in chest,palms and soles,insomia and tinnitus (P < 0.05).(3) In the aspect of endothelium protection,treatment group can greatly improve CAVI compared with control (L-CAVI ∶ t =2.11,P <0.05;R-CAVI∶ t =2.15,P <0.05) ; the levels of FMD(t =-4.65,P <0.01) and EMP (t =4.35,P < 0.01)were significantly improved after treatment (P < 0.05) while no statistical meaning was found between two group.Conclusions Tianma Gouteng Granule can improve TCM syndrome,decrease CAVI level.FMD and EMP were significantly decreased in treatment group.Multi-factor regression analysis showed that EMP level was related to pulse pressure.

Objective Utilization of proteome method,by observing differences on protein expression of hypothalamus in the hyperthyroidism rats with hyperactivity of liver-yang and normal rats,to search the differential protein.Methods Models of animal were prepared by ip L-T4 and Fuzi Decoction.Both the quantitative and qualitative changes of the protein expression were compared between model group and treated group by protein technique.Results Proteins were mainly displayed at range of pI 3~8,Mr(14.4~75)kD.18 protein increased and 24 protein decreased in model group compared with treated group, included Thioredoxin,Ubiquitin carboxyl-terminal hydrolase isozyme L1,Platelet-activating factor acetylhydrolase IB gamma subunit,glutamate dehydrogenase,dihydropyrimidinase-related protein 2, NSFL1 cofactor p47,Tubulin beta 5.Conc(?)sion There were 18 spots high expressed and 24 spots low expressed in the hyperthyroidism with hyperactivity of liver-yang,that maybe related with the develepment of the hyperthyroidism with hyperactivity of liver-yang.

Objective To explore the relationship of plasma norepinephrine (NE) and epinephrine (E) level with TCM syndrome differentiation of attention deficit hyperactivity disorder (ADHD) in children. Methods Plasma NE and E levels in 80 ADHD children (Group A) and 32 normal volunteers (Group B) were measured by the method of high performance liquid chromatography_electrochemical detectio n (HPLC_ECD). Among 80 ADHD children, 49 were differentiated with kidney_yin def iciency and liver_yang hyperactivity syndrome (Group A 1) and 31 with heart_sp leen deficiency syndrome (Group A 2). Results Plasma NE and E levels in Gro up A we re higher than Group B (P 0.05 ). Conclusion Hyperactivity and impulsi veness in ADHD children may be related with the high levels of plasma NE and E. The exc essively sympathetic excitement in ADHD with kidney_yin deficiency and liver_yan g hyperactivity syndrome is uncertain.
