RESUMEN

Abstract@#Medial meniscus root tear (MMRT) is uncommon and is often associated with osteoarthritis during presentation. Whether it is a cause or effect, it is still debatable at this point of time. However, when a combination of injuries occurs in a middle age group patient, a careful examination before offering a treatment is advised. We herein report a case of a middle-aged gentleman suffering from both arthritis and MMRT.

Objective To assess the feasibility and efficiency of pedal-plantar loop technique in the treatment of below-the-ankle arterial occlusion . Methods A retrospective analysis was undertaken on 12 cases ( 14 legs ) of below-the-ankle arterial occlusion.The pedal-plantar loop angioplasty was performed in all the cases .Balloon angioplasty was performed after traversing through the arterial obstruction sites using antegrade or retrograde technique .After 6 months of the surgery , the results of visual analogue scale , dorsal or plantar arterial pulse volume scores , ankle brachial index , TcPO2 , and healing of the ulcer were compared to preoperative measures. Results Technical success was achieved in 9 cases (11 legs).During follow-up, 1 patient died of cerebrovascular events 2 months after the therapy .The other 8 cases (10 legs) were followed up for 6 months and no one had recurrent ischemic symptoms . The results of visual analogue scale , ankle brachial index, and TcPO2 before the surgery were (5.68 ±1.09) points, 0.60 ±0.04, and (25.71 ±5.25) mm Hg, which were improved to (0.44 ±0.27) points, 0.87 ±0.05, and (35.90 ±5.28) mm Hg (P<0.05).Dorsal or plantar arterial pulse volume scores were also enhanced markedly (P=0.004).Complete ulcer healing was observed in 8 limbs.No related complication was observed in other patients after the surgery .One technical unsuccessful patient who suffered infection and severe rest pain was given leg amputation . Conclusion Pedal-plantar loop technique is an effective therapy for below-the-ankle arterial occlusion while long term follow-up data need to be collected .

Objective:To Compare root resorption between T loop technique and sliding technique of orthodontic treatment.Meth-ods:90 patients with Angle class I malocclusion were orthodontically treated by MBT fixed appliances with T loop technique(n =45) and sliding technique(n =45)respectively.Root resorption of the patients were examined by grading evaluation method with orthopan-tomography and periapical films before and after orthodontic treatment.Results:After orthodontic treatment the tooth root resorption rate increased(P 0.05).Root resorption mostly occurred in the incisors,especially in mandibular tooth.Conclusion:There is no difference of the effect of root resorption between T loop technique and sliding technique of orthodontic treatment in patients with Angle class I malocclusion.

OBJECTIVE: To determine the efficacy, safety and primary follow-up results of a guidewire looping technique for the treatment of infrapopliteal arteries. MATERIALS AND METHODS: From October 2006 to May 2008, an intraluminal angioplasty of the infrapopliteal arteries was attempted in 200 consecutive patients. Altogether, 417 infrapopliteal lesions, with lengths varying from 2 cm to 32 cm, were treated as part of this study, including 305 lesions in the anterior tibial arteries, 89 in the posterior tibial arteries, and 23 in the peroneal arteries. The 'U'-shaped guidewire technique was attempted in 393 lesions from 361 limbs. The tip of a hydrophilic 0.035-inch guidewire was formed into a 'U' shape with the aid of a 4-Fr catheter and collateral branch vessel to recanalize the completely occluded long segment lesions. RESULTS: A successful angioplasty with at least one artery recanalized directly to the malleolar or dorsal foot was achieved in 322 limbs (89%). The looping technique had a success rate of 90% (352 of 393 lesions). After the procedure, the rest pain was relieved in 58 of 69 patients, while 207 of 245 limbs (85%) showed improvement for intermittent claudication. Complete wound healing was noted in 21 of 54 patients, while 20 of 54 patients showed an improvement in the wound size or depth. A total of 38 major immediate procedure-related complications were noted, including retroperitoneal hematoma, distal emboli, and vessel rupture. CONCLUSION: The results of this study suggests that the guidewire looping technique is a safe and effective method for the recanalization of the occluded lesions in infrapopliteal vessels.

Because of the large volume of elective sterilizations performed in this country during the past decade, the high rate of subsequent divorce and remarriage, and improved results of new sterilization reversal techniques, surgeons are receiving rapidly increasing numbers of requests for tuboplasty and vasovasostomy. During the past 10 years (1978-1987), a total of 44 corrective procedures were performed for patients with post-vasectomy azoospermias in this department. Herein, we analyze various factors influencing upon results of anastomosis, and compare results of macroscopic and microscopic anastomotic techniques. The results are as follows: 1. Reversal of vasectomy was requested due to change of mind is 25 patients (56.8%), remarriage in 10 patients (22.7%), death of children in 4 patients (9.1 %), psychologic problem in 4 patients (9.1%) and economic change in 1 patient (2.3%) 2. Results of total vasovasostomies revealed that viable sperms were present in 5 of patients (50%) in macrosurgery and in 22 of 24 patients (92%) in microsurgery. Pregnancy occurred in 2 of 10 patients (20%) in macrosurgery and in 16 of 24 patients (67%) in microsurgery. 3. The longer the interval of obstruction, the less anatomical and functional success rates were noted. 4. It is advised that total open loop technique is far more advanced technically with fool-proof measure. 5. Surgeons who perform the procedure regularly can obtain better results.
