RESUMEN

Precision medicine is poised to increasingly improve health outcomes for more people in the near future. In contrast to the more traditional reactive methods of disease treatment, precision medicine is a customizable treatment and disease prevention approach that is tailored for the individual. Artificial intelligence (AI) using sophisticated algorithms and machine learning(ML)tools powers these precision medicine processes. These algorithms analyze big data collected from multiple sources over the past decades to aid physicians to make data-backed critical clinical decisions. However, studies have shown that unintentional biases in the source data and in the process can affect these precision medicine efforts.

Background. The Cryptococcus' colony-forming unit (CFU) count in cerebrospinal fluid (CSF) would be a reliable marker for patient prognosis and a simple and inexpensive tool. Objectives: To evaluate the usefulness of the CFU count of Cryptococcus spp. And to compare it with the variations of Cryptococcus' capsular antigen (CrAg) in CSF.Materials and methods. Clinical records of patients with aids-related meningoencephalitis caused by Cryptococcusassisted in our center between February 2016 and July 2020 were reviewed. CFU count and CrAg values in CSF were evaluated during the evolution of the mycosis. Results and Discussion. Data from 94 clinical episodes of 85 patients with a total of 297 observations of CSF samples were analyzed. The importance of using the CFU count was evidenced as it is a viability and fungal load marker.Low CFU count did not necessarily coexist with low CrAg.Regarding the evolution over time, most of the patients were diagnosed with a high fungal load and its decrease occurred faster than that the one of AgCr. This would reflect the improvement of the patient, allowing behaviors to be taken in this regard

RESUMEN

Background: Epidural anesthesia alloyed with opioids facilitates a dose sparing effect of local anesthetic and superior analgesia. The drugs such as dexmedetomidine and fentanyl are effective adjuvants to ropivacaine to provide efficient post-operative analgesia. Still, the findings are contradictory. Aims and Objectives: The aims of the study were to assess the efficacy of 0.75% ropivacaine with 0.25 ml fentanyl and 0.5 ml dexmedetomidine in the patients undergoing lower abdominal surgeries. Materials and Methods: A total of 80 cases undergoing lower abdominal surgeries above 21 years were recruited. Group RF administered with 18ml of 0.75% ropivacaine + 0.25 ml fentanyl. Group RD administered with 18ml of 0.75% ropivacaine + 0.5 ml dexmedetomidine. During intraoperative period, parameters such as sensory block, motor block by modified Bromage scale, pain score, and hemodynamic parameters were recorded. The sedation score was assessed using Ramsay sedation score. Results: The mean difference of the events of sensory block and motor block between two study groups was statistically significant (P < 0.05). The mean systolic blood pressure (SBP) was comparatively less in Group RD than Group RF during the entire study period. The mean difference of SBP, heart rate, and mean arterial pressure was statistically significant (P < 0.05). Preoperatively, the diastolic blood pressure was higher in Group RD; later stages, the levels were lesser than Group RF. The mean sedation score was less in Group RD till 15 min, later, it was higher in Group RD than Group RF. Bradycardia was commonly associated post-operative complication in both the study groups followed by nausea and vomiting. Conclusion: The fentanyl and dexmedetomidine are effective adjuvants to ropivacaine. However, dexmedetomidine has better efficacy in terms of prolonged duration of sensory block, motor block, and postoperative analgesia, minimal requirement of rescue analgesia and with less post-operative complications.

RESUMEN

@#Cardiovascular diseases (CVDs) are major disease burdens with high mortality worldwide. Early prediction of cardiovascular events can reduce the incidence of acute myocardial infarction and decrease the mortality rates of patients with CVDs. The pathological mechanisms and multiple factors involved in CVDs are complex; thus, traditional data analysis is insufficient and inefficient to manage multidimensional data for the risk prediction of CVDs and heart attacks, medical image interpretations, therapeutic decision-making, and disease prognosis prediction. Meanwhile, traditional Chinese medicine (TCM) has been widely used for treating CVDs. TCM offers unique theoretical and practical applications in the diagnosis and treatment of CVDs. Big data have been generated to investigate the scientific basis of TCM diagnostic methods. TCM formulae contain multiple herbal items. Elucidating the complicated interactions between the active compounds and network modulations requires advanced data-analysis capability. Recent progress in artificial intelligence (AI) technology has allowed these challenges to be resolved, which significantly facilitates the development of integrative diagnostic and therapeutic strategies for CVDs and the understanding of the therapeutic principles of TCM formulae. Herein, we briefly introduce the basic concept and current progress of AI and machine learning (ML) technology, and summarize the applications of advanced AI and ML for the diagnosis and treatment of CVDs. Furthermore, we review the progress of AI and ML technology for investigating the scientific basis of TCM diagnosis and treatment for CVDs. We expect the application of AI and ML technology to promote synergy between western medicine and TCM, which can then boost the development of integrative medicine for the diagnosis and treatment of CVDs.

RESUMEN

As the research in sentiment analysis (SA) has reached the advanced level of maturity, scientific community is exploring possibilities of applying SA in different domains for which huge descriptive data is available. Researcher has developed tools for SA in commercial domains like feedback from customers, review rating of movie, political domains like exit poll prediction from tweets, etc. In medical science, huge data is available which can be used for predictive modelling. Psychomedical diagnosis is an interesting field which deals with exploring behavioural patterns to potential psychological problems. In the domain of medical science, little amount of work has been done in clinical SA. However, the efforts are at an elementary level in the course of research of SA for behavioural psychology. Hence, there is an opportunity of applying SA as the first step towards automatic psychomedical diagnosis of teenagers’ behaviour patterns.

Neuropathic pain is a chronic debilitating symptom characterized by spontaneous pain and mechanical allodynia. It occurs in distinct forms, including brush-evoked dynamic and filament-evoked punctate mechanical allodynia. Potassium channel 2.1 (Kir2.1), which exhibits strong inward rectification, is and regulates the activity of lamina I projection neurons. However, the relationship between Kir2.1 channels and mechanical allodynia is still unclear. In this study, we first found that pretreatment with ML133, a selective Kir2.1 inhibitor, by intrathecal administration, preferentially inhibited dynamic, but not punctate, allodynia in mice with spared nerve injury (SNI). Intrathecal injection of low doses of strychnine, a glycine receptor inhibitor, selectively induced dynamic, but not punctate allodynia, not only in naïve but also in ML133-pretreated mice. In contrast, bicuculline, a GABA receptor antagonist, induced only punctate, but not dynamic, allodynia. These results indicated the involvement of glycinergic transmission in the development of dynamic allodynia. We further found that SNI significantly suppressed the frequency, but not the amplitude, of the glycinergic spontaneous inhibitory postsynaptic currents (gly-sIPSCs) in neurons on the lamina II-III border of the spinal dorsal horn, and pretreatment with ML133 prevented the SNI-induced gly-sIPSC reduction. Furthermore, 5 days after SNI, ML133, either by intrathecal administration or acute bath perfusion, and strychnine sensitively reversed the SNI-induced dynamic, but not punctate, allodynia and the gly-sIPSC reduction in lamina IIi neurons, respectively. In conclusion, our results suggest that blockade of Kir2.1 channels in the spinal dorsal horn selectively inhibits dynamic, but not punctate, mechanical allodynia by enhancing glycinergic inhibitory transmission.

RESUMEN

Increased intracellular levels of Ca²⁺ are generally thought to negatively regulate lipolysis in mature adipocytes, whereas store-operated Ca²⁺ entry was recently reported to facilitate lipolysis and attenuate lipotoxicity by inducing lipophagy. Transient receptor potential mucolipin1 (TRPML1), a Ca²⁺-permeable non-selective cation channel, is mainly expressed on the lysosomal membrane and plays key roles in lysosomal homeostasis and membrane trafficking. However, the roles of TRPML1 in lipolysis remains unclear. In this study, we examined whether the channel function of TRPML1 induces lipolysis in mature adipocytes. We found that treatment of mature adipocytes with ML-SA1, a specific agonist of TRPML1, solely upregulated extracellular glycerol release, but not to the same extent as isoproterenol. In addition, knockdown of TRPML1 in mature adipocytes significantly reduced autophagic flux, regardless of ML-SA1 treatment. Our findings demonstrate that the channel function of TRPML1 partially contributes to lipid metabolism and autophagic membrane trafficking, suggesting that TRPML1, particularly the channel function of TRPML1, is as therapeutic target molecule for treating obesity.

ABSTRACT BACKGROUND AND OBJECTIVES: The I-gel supraglottic airway has a non-inflatable cuff made from a gel-like thermoplastic elastomer. The use of the I-gel during anesthesia for spontaneously breathing patients or intermittent positive pressure ventilation has been reported. But there are a few published reports about the use of the I-gel with pressure-controlled ventilation. CONTENTS AND CONCLUSIONS: In this case report we described the use of the I-gel supraglottic airway along 48 h in intensive care unit for the management of ventilation in a patient needed mechanic ventilation but in whom tracheal intubation could not be performed.

<p><b>INTRODUCTION</b>Children with Down syndrome (DS) are at increased risk of developing distinctive clonal myeloid disorders, including transient abnormal myelopoiesis (TAM) and myeloid leukaemia of DS (ML-DS). TAM connotes a spontaneously resolving congenital myeloproliferative state observed in 10%-20% of DS newborns. Following varying intervals of apparent remission, a proportion of children with TAM progress to develop ML-DS in early childhood. Therefore, TAM and ML-DS represent a biological continuum. Both disorders are characterised by recurring truncating somatic mutations of the GATA1 gene, which are considered key pathogenetic events.</p><p><b>METHODS</b>We herein report, to our knowledge, the first observation on the frequency and nature of GATA1 gene mutations in a cohort of Malaysian children with DS-associated TAM (n = 9) and ML-DS (n = 24) encountered successively over a period of five years at a national referral centre.</p><p><b>RESULTS</b>Of the 29 patients who underwent GATA1 analysis, GATA1 mutations were observed in 15 (51.7%) patients, including 6 (75.0%) out of 8 patients with TAM, and 9 (42.9%) of 21 patients with ML-DS. All identified mutations were located in exon 2 and the majority were sequence-terminating insertions or deletions (66.7%), including several hitherto unreported mutations (12 out of 15).</p><p><b>CONCLUSION</b>The low frequency of GATA1 mutations in ML-DS patients is unusual and potentially indicates distinctive genomic events in our patient cohort.</p>

RESUMEN

Store-operated calcium entry (SOCE), a major mode of extracellular calcium entry, plays roles in a variety of cell activities. Accumulating evidence indicates that the intracellular calcium ion concentration and calcium signaling are critical for the responses induced by oxidative stress. The present study was designed to investigate the potential effect of SOCE inhibition on H₂O₂-induced apoptosis in endothelial progenitor cells (EPCs), which are the predominant cells involved in endothelial repair. The results showed that H₂O₂-induced EPC apoptosis was reversed by SOCE inhibition induced either using the SOCE antagonist ML-9 or via silencing of stromal interaction molecule 1 (STIM1), a component of SOCE. Furthermore, SOCE inhibition repressed the increases in intracellular reactive oxygen species (ROS) levels and endoplasmic reticulum (ER) stress and ameliorated the mitochondrial dysfunction caused by H₂O₂. Our findings provide evidence that SOCE inhibition exerts a protective effect on EPCs in response to oxidative stress induced by H₂O₂ and may serve as a potential therapeutic strategy against vascular endothelial injury.

RESUMEN

Objective: The ratio of monocytes to lymphocytes in peripheral blood could reflect an indi- vidual's immunity to Mycobacterium tuberculosis. The objective of this study was to evaluate the relationship between ratio of monocytes to lymphocytes and clinical status of patients with active tuberculosis. Methods: This was a retrospective review of data collected from the clinical database of The Fifth People's Hospital of Wuxi, Medical College of Jiangnan University. A total of 419 patients who had newly diagnosed active tuberculosis and 108 cases from 419 patients with tuberculosis therapy either near completion or completed were selected. Controls were 327 healthy donors. Results: Median ratio of monocytes to lymphocytes was 0.36 (IQR, 0.22-0.54) in patients before treatment, and 0.16 (IQR, 0.12-0.20) in controls (p < 0.001). Ratio of monocytes to lymphocytes <9% or >25% was significant predictors for active tuberculosis (OR = 114.73, 95% CI, 39.80-330.71; OR = 89.81, 95% CI, 53.18-151.68, respectively). After treatment, the median ratio of monocytes to lymphocytes recovered to be nearly normal. Compared to other patients, patients with extrapulmonary tuberculosis and of age >60 years were more likely to have extreme ratio of monocytes to lymphocytes (AOR = 2.57, 95% CI, 1.08-6.09; AOR = 4.36, 95% CI, 1.43-13.29, respectively). Conclusions: Ratio of monocytes to lymphocytes <9% or >25% is predictive of active tuberculosis. .

The ML15-HA medium allows simulate in vitro morphogenetic events in the life cycle of T. cruzi. It has been demonstrated that the morphological changes of epimastigogenesis in ML15-HA of the T. cruzi clone Dm28c, occur faster than for other isolates. Our purpose was to investigate whether macromolecular changes occur concomitantly with rapid morphological transformation. Trypomastigotes derived from infected cells were incubated for different times in ML15-HA medium at 27°C. Morphological changes were determined by phase contrast microscopy and staining with Giemsa. Peptide and glycopeptide profiles were analyzed by SDS-PAGE and specific staining. Antigenic changes were studied by Western blot using serum anti-trypomastigotes and anti-epimastigotes. The results show: (1) decline in the percentage of trypomastigotes and amastigotes increased in the first 24 hours, epimastigotes gradually increases from 48 hours, (2) three peptide and glycopeptide profiles different: trypomastigotes, epimastigotes from the day 4 and an intermediate profile on days 2 and 3, showing the close correlation between morphological events and protein and glycoprotein changes, (3) antigenic profiles showed close relationship between the morphological changes and the expression of stage-specific antigens. The results show conclusively that the molecular changes are more rapid for clone Dm28c in ML15-HA medium than in other media. These findings open the question, if the epimastigogenesis speed is determined by the genetic constitution of isolated or nutritional wealth differentiation medium.

RESUMEN

Overlapping clinical phenotypes are a diagnostic challenge to the clinician, especially in the cases of mucolipidosis (ML) and mucopolysaccharide disorders (MPS), due to overlapping phenotypes. Present study was carried out in 147 children suspected to have ML or MPS and 100 controls. They were screened for ML II/III by colorimetric method using substrate pNCS. Six children were found screen positive for ML II/III and further confirmatory study showed significantly raised activity in plasma confirming high specificity of the ML screening test. Forty-two (28.5%) children out of remaining 141 children that were screen negative, were found to have various MPS disorders, while rest 99 had normal enzyme activity in plasma and leucocytes. Present study demonstrates prompt and specific chemical method that can be used as a tool for estimating ML II/III, with high specificity.

RESUMEN

The potential of the white rot fungus, Coriolus versicolor ML04 to decolorize the widely used textile dye Blue BB was tested by employing statistical optimization. Response surface methodology (RSM) involving a central composite design (CCD) was applied to evaluate the interactive effects of four significant factors in different ranges i.e.; glucose (0.5 - 2.5 g/L), yeast extract (0.4 -1.2 g/L), dye concentration (100 - 500 ppm) and inoculum size (5 - 20 percent v/v) to decolorize the Blue BB. The results demonstrated the effectiveness of the statistical experimental design and the ability of C. versicolor ML04 for maximum dye decolorization (>96 percent) at the optimum conditions of the significant factors.
