RESUMEN

El establecimiento de un sistema de gestión de la calidad es obligatorio para algunas instituciones de salud, entre las que se encuentra el Laboratorio de Biología Molecular, esto queda explícito en la Regulación D 03-21, que plantea la obligatoriedad de su cumplimiento para los laboratorios clínicos que se basan en técnicas de biología molecular. El objetivo del presente texto es exponer los presupuestos teóricos que sustentan un estudio sobre la gestión de la calidad en el mencionado laboratorio. Se realiza una revisión documental a partir de artículos publicados en el período comprendido de 2012-2022. Del total de las fuentes consultadas en las plataformas de acceso a bases de datos de la red de información de la salud cubana Infomed: PubMed, Scopus, SciELo, Redalyc, y Dialnet, se seleccionaron 33 para la revisión al considerar que abordaban el tema con mayor profundidad. La adopción de un sistema de gestión de la calidad garantiza y gestiona el alto nivel de los servicios, aumenta la satisfacción del cliente y el prestigio de la organización.


The establishment of a quality management system is mandatory for some health institutions, among which is the Molecular Biology Laboratory. This is explicit in Regulation D 03-21, which makes compliance mandatory for laboratories. clinical trials that are based on molecular biology techniques. The objective of this text is to present the theoretical assumptions that support a study on quality management in the aforementioned laboratory. A documentary review is carried out based on articles published in the period from 2012-2022. Of the total sources consulted on the database access platforms of the cuban health information network Infomed: PubMed, Scopus, SciELo, Redalyc, and Dialnet, 33 were selected for the review considering that they addressed the topic with greater depth. The adoption of a quality management system guarantees and manages the high level of services, increases customer satisfaction and the prestige of the organization.

Introducción: La infertilidad es una enfermedad del sistema reproductivo que altera el equilibrio biológico, psicológico y social de las parejas. En Cuba existe un programa para el tratamiento de la pareja infértil y el Ministerio de Salud Pública incluye en sus directrices implementar las bases organizativas, sistema de trabajo, indicadores, funciones y control que garanticen el perfeccionamiento del Programa para la óptima atención de parejas infértiles, y dentro de esto se incluye confeccionar bases de datos donde se registren las parejas tratadas por este motivo. Objetivos: Implementar un Sistema de Gestión Hospitalaria para la atención de la pareja infértil, en el Centro Territorial de Holguín. Métodos: Se realizó un estudio de desarrollo tecnológico, se confeccionó sobre el sistema de base de datos relacional postgreSQL, se empleó la herramienta Odoo con el objetivo de englobar en un único software todas las prestaciones que necesita la consulta. Se utilizó el lenguaje de programación Python. Resultados: SCAPI, permite gestionar la información de los pacientes y sus historias clínicas, está compuesto por módulos relacionados: configuración general, permite crear los perfiles de usuarios y los permisos para interactuar con el sistema; SCAPI muestra y gestiona toda la información relacionada con la historia clínica digital, la agenda de turnos, medios diagnósticos (estudios de reserva ovárica, espermogramas, ecografías ováricas, estudios hormonales, técnicas de baja y alta tecnología entre otros). Conclusiones: El Sistema de Gestión Hospitalaria para la atención de la pareja infértil, contribuye al objetivo de una mayor calidad de la atención al paciente pues gestiona la información de estos y sus historias clínicas de manera rápida, oportuna y fiable.


Introduction: Biological infertility is a disease of the reproductive system that alters the psychological and social balance of couples. In Cuba there is a program for the treatment of infertile women and the Ministry of Public Health includes in its guidelines to implement the organizational bases, work system, indicators, functions and control that guarantee the improvement of the Program, including databases where couples treated for this reason are registered. Objective: To implement a Hospital Management System for the infertile couple care in the Holguín Territorial Center. Methods: A study of technological development was carried out using postgre SQL relational database system, Odoo tool, and Python programming language. Results: SCAPI allows to manage the information of patients and their medical records; it is made up of modules that are related to each other: the general configuration module which allows you to create user profiles and permissions to interact with the system; the SCAPI module which shows and manages all the information related to the digital medical history, appointment schedules, and diagnostic means (ovarian reserve studies, spermograms, ovarian ultrasound, hormonal studies, low and high technology techniques, among others). Conclusions: Hospital Management System for the care of infertile couples allows managing fast, timely and reliable information on patients and their medical records during care consultations for infertile couples with the aim of achieving a higher quality of patient care.

Los avances científicos han facilitado la difusión del conocimiento, encontrándose los más recientes hallazgos rápidamente en Internet, esto ha producido la migración de las revistas desde lo impreso a lo digital, pero este proceso no siempre se realiza adecuadamente, por lo que actualmente existen revistas, y en consecuencia sus publicaciones, que no se logran encontrar en los buscadores académicos, lo que se debe al uso de los softwares inadecuados o en su defecto a una mala configuración de los que se han implementado. En esta línea la recomendación es usar Open Journal System, un software diseñado para la publicación científica, pero varias revistas usan gestores de contenido como WordPress, por su facilidad de implementación y personalización aun cuando estos presenten limitaciones en el aspecto editorial. A continuación, se expone un método para la correcta indexación de revistas confeccionadas en WordPress en el buscador Google Scholar.


Scientific advances have facilitated the dissemination of knowledge, and the latest discoveries can be easily found on the Internet. This has produced the migration of journals from print to digital; however, this process is not always done properly since there are journals, and consequently their publications, which are not currently found in academic search engines due to the inappropriate use of software or otherwise to a misconfiguration of those that have been implemented. In this line, the recommendation is to use Open Journal System, a software designed for scientific publication; on the other hand, several journals use content manager systems such as WordPress because of its easy implementation and customization even when they present editorial constrains. The following is a method for the correct indexing of journals created in WordPress in the Google Scholar search engine.

Objectives: Web-based learning is becoming a newly accepted standard in education, more so with COVID-19 pandemic. To use technology to redesign learning experiences, a blend of synchronous and asynchronous approaches was used wherein the routine lectures through MS Teams (synchronous) were integrated with an online learning management system (LMS), ‘Moodle’ (modular object-oriented dynamic learning environment) based, four-quadrant approach (asynchronous). We aimed to determine the student’s engagement, performance and perception of the new online LMS. Materials and Methods: All 170 students of the 1st year MBBS of batch 2020 were enrolled in the course. A module was developed for teaching ‘Physiology of Vision’ with this blended approach. Along with routine online lectures through Microsoft Teams, supplementary materials in the form of multimedia presentations and additional links to useful websites were provided through online LMS. The online activity of students was recorded and assessments were done. A feedback form was filled out by students at the end of the course. Results: A significant positive correlation of examination scores with individual student’s activity logs and significantly better scores in top Moodle users along with significantly higher marks in Moodle-based modules than in other modules confirmed the value of Moodle in improving student performance. Overall feedback from students was clearly in favour of implementing Moodle as a complementary tool to traditional teaching. Conclusion: Moodle improved the quality of learning. It developed interest and motivation among students and has a positive impact on academic outcomes.

Objective: Chemotherapy-induced nausea and vomiting (CINV) can affect a patient’s quality of life and make them resistant to the treatment. We created an electronic patient reported outcome ePRO-linked pharmaceutical management system (PMS) for CINV (CINVePRO) for storing information, such as nausea and vomiting status, food intake, etc., and suggesting the type of anti-nausea medication and dosage changes to the physicians for controlling CINV.Design: At the Gifu University Hospital, the collaborative research institute, inpatients and pharmacists in charge used CINVePRO-PMS, and a questionnaire survey was done to assess the system’s reliability.Methods: The daily entry of data into CINVePRO shows the number and duration of vomiting, degree of nausea, and amount of food consumed and displays a list and graph of these data over time. The PMS enables pharmacists to list the presence or absence of nausea and the number of vomiting for all patients in their charge and record the intervention and display its list.Results: The questionnaire was distributed to 17 inpatients. All patients and pharmacists answered the questionnaire. According to the results of the questionnaire survey of patients, each screen of CINVePRO received a good evaluation that mentioned it was “easy to understand,” “easy to use,” and “especially useful for communicating one’s symptoms.” In addition, the results of a questionnaire survey of the pharmacists revealed that the system was rated as easy to check the patients’ symptoms and practical to use.Conclusion: CINVePRO-PMS was evaluated as a convenient and applicative system. However, linking CINVePRO to the electronic medical record of each hospital is necessary for sharing it among multiple professions.

【Objective】 To establish a blood transfusion adverse event management system and apply it to adverse events management, so as to enhance the ability to identify and process reports as well as prevent adverse events. 【Methods】 According to the errors during the whole process of blood transfusion, the management information system of blood transfusion adverse events was established, and the data of adverse transfusion reactions and adverse transfusion events from 2020 to 2022 were collected according to the system requirements. The system monitoring data and statistical analysis were used to analyze the causes of errors in each link of blood transfusion, sort the incidence of each department, focus on supervising the departments with high frequency of adverse events, and propose effective rectification. 【Results】 The management system counted 51 cases of adverse reactions. The incidence of adverse reactions of plasma (43.1 %, 22/51) was higher than that of red blood cells (39.2 %, 20/51), and the number of allergic reactions was slightly higher than that of febrile non-hemolytic transfusion reaction. Among the 628 cases of adverse transfusion events, blood transport, inbound and storage (TS-A) errors accounted for the highest proportion of 46.2% (290/628), 20.9% (131/628) in post-transfusion disposal and evaluation (TS-G) and 15.8% (99/628) in pre-transfusion evaluation and transfusion application (TS-B). TS-A12 accounted for 63.8 % (185/290) in TS-A errors, which was mainly due to plasma leakage caused by extrusion during transport and the expiration of red blood cells caused by insufficient blood inventory management. TS-B errors mainly focus on the failure to fully assess the bleeding tendency of patients before surgical system surgery, resulting in no or insufficient preoperative blood preparation. Most of the TS-G errors were unqualified medical record. 【Conclusion】 Blood transfusion adverse event management system can help to identify and prevent the occurrence or recurrence of adverse events, formulate targeted rectification and preventive measures, and improve the haemovigilance ability.

【Objective】 To identify the main unqualified items in the external audit of blood station quality management system (referred to as external audit), in order to take necessary measures to continuously improve the quality system. 【Methods】 Unqualified items(data) in the national and Shandong provincial blood safety technical audits (referred to as national and provincial audits) and four blood station blood safety technical joint audits (referred to as inter station mutual audits) from 2017 to 2019 were collected and analyzed by Excel and Pareto curves (graphs). Corresponding corrective and preventive measures were developed and implemented, and then tracked and evaluated by the quality management department three months after the external audit to verify their effectiveness. 【Results】 In a total of 7 external audits of blood station quality management system that our blood station has participated in over the past 3 years (including 2 national audits, 2 provincial audits, and 3 inter station mutual audits), the main unqualified terms were "12 monitoring and continuous improvement" 11.90% (15/126), "13 blood donation services" 11.90% (15/126), "06 equipment" 10.32% (13/126), "11 records" 10.32% (13/126), "03 organization and personnel" 8.73% (11/126), "15 blood preparation" 7.94% (10/126), "08 safety and health" 7.14% (9/126), and "14 blood testing" 7.14% (9/126). Among them, "monitoring and continuous improvement" ranked first in two national audits and two provincial audits, with 16.67% (5/30) and 14.71% (5/34), respectively, and was 8.06% (5/62) in inter station mutual audit, and the difference between the three kinds of audits was not statistically significant (P>0.05). "Records" accounted the highest proportion in inter station mutual review of 19.35% (12/62), while was respectively 0 and 2.94% (1/34) in national and provincial audits, with statistically significant difference between the three kinds of audits (P<0.05). 【Conclusion】 External audit against unqualified items is important for quality improvement. By analyzing the unqualified terms, taking corresponding measures to improve weak links, and evaluating the effectiveness of those measures, it can effectively ensure the effective operation of blood station quality management system.

With the highlighted advantages of 3D printing technology in the field of dental prosthodontics, there is increasing in the numbers of registration applications for additive manufacturing customized dentures. However, there is still a lack of unified analysis in the core elements of process control, the key points of registration and the safety production quality control. Based on the current research status of the industry, the study is intended to clarify confusion and difficulties, deeply analyse the mechanism of the product defects, sort the core elements of process control, then try to establish a systematic evaluation system from product performance research, key process verification, production quality control and the description of registration files, so that it can provide help for practitioners to clarify research direction, establishing quality management system, improving the efficiency of registration and ensuring product quality.

Based on the technical characteristics of molecular diagnostic reagents, the study analyzed the concerned key control points and common problems in the process of design and development from the view of registration quality management system. It aimed at offering technical guidance on design and development process of molecular reagents and registration quality management system to enterprises, thus improving the product development efficiency, optimizing the quality management system, and increasing the efficiency and quality of registration and declaration.

Objective:In the context of China′s increasing standardized management requirements of clinical research, this article aims to explore the management methods of investigator-initiated trials in the new period, to provide possible reference for other medical institutions dedicated to clinical research.Methods:According to the requirements set forth by the"Administrative Measures for Investigator-Initiated Trials in Medical and Health Institutions (Trial)", combined with the hospital management practice, experiences regarding the research management system construction and implementation, management system construction and its implementation effects are summarized and analyzed.Results:By exploring and summarizing the connotation of high-quality clinical research under the New Policy, tailored clinical research management system in our hospital was developed and implemented. And the hospital′s clinical research capability and level have been greatly improved, which enhancing the hospital academic influence, as well as its competence for serving the development of national and regional clinical research.Conclusions:Along with the rapid progress of clinical research, hospitals need to assure the compliance of national laws and regulations, and develop appropriate and applicable institutional management measures to empower the conduct of high quality clinical research.

Objective:This study aims to explore the key points of the operation mode and construction of psychiatric research ward, provide possible reference for the construction of research ward.Methods:In this study, the construction of research ward in a psychiatric hospital was considered and explored through literature review, drawing on the operation mode of other research wards, combined with the characteristics of psychiatric clinical research.Results:This Study put forward the construction orientation and operation management mode of the psychiatric research ward, and analyzed the advantages of existing platform and key points for future development.Conclusions:The construction of psychiatric research ward should be based on clinical needs and hospital practice. Strategies including tailored fine management, optimize platform supporting, efficient organizing and management system, talent team and appropriate incentive mechanism should be implemented to strengthen the construction.

Objective:To establish a standardized information management system (IMS) for preserving, managing, querying, and performing statistics on biospecimens and their clinical data, which is conducive to improving the utilization of biobank.Methods:Under the premise of ensuring operating environment and data security, a database-based data logic relationship model is created and applied to the IMS to manage and analyze biospecimens and their supporting clinical information of patients enrolled in the biobank of our center.Results:To ensure the establishment of the follow-up cohort, biospecimens and clinical information of inpatients and outpatients were continuously collected in the biobank of our center. Since December 2014, more than 270 000 biospecimens from inpatient, outpatient, and scientific research have been preserved. The IMS optimized by this model efficiently completes the basic work of the biobank. At the same time, the data can be queried jointly and in batches, and then converted into a report format for statistical analysis.Conclusions:The IMS of our center is suitable for application and popularization as a construction and management model for the hospital-level biobank, which meets the daily work of the biobank and diverse research needs, and provides a convenient platform and rich resources for the development of precision medicine.

In recent years, the diagnostic radiology, especially CT scanning, has a rapidly increased frequency in our country, becoming the largest artificial radiation source to the average individual doses to the population. In clinical diagnosis and treatment activities, the examined patients and individuals may undergo multiple procedures and multiple frequencies of medical imaging in a short period of time and receive high cumulative radiation doses even exceeding 50 or 100 mSv in a single day, posing a potential risk to their health. Therefore, it is necessary to carry out statistical analysis and management of diagnostic radiation dose information to minimize the probability of excessive dose and associated radiation risk. In this paper, the international cognition of radiation risks in diagnostic radiology, diagnostic equipment and medical imaging frequency, radiation dose and its management status are described. Four countermeasures for radiation dose management are put forward to provide reference for further improving radiation protection in clinical practice.

Objective:To explore application of mixed teaching platform in the clinical practice teaching of the laboratory medicine in Children's hospitals.Methods:We constructed a mixed online and offline teaching platform based on the Laboratory Quality Management System (LQMS) in the Children's Hospital of Chongqing Medical University. The undergraduates from Batch 2016 ( n=15) and Batch 2018 ( n=12) of College of Laboratory Medicine of Chongqing Medical University were taken as control group and experimental group respectively. Traditional teaching method was adopted by the control group, and the mixed teaching method was adopted by the experimental group. The results of two groups' clinical practice assessment, rate of outstanding students (total score ≥ 90) and rate of satisfaction (score ≥ 90) were compared to evaluate the teaching effect. SPSS 17.0 was used to conduct t-test and Chi-square test. Results:The database of teaching platform includes 68 teaching cases, 198 pieces of courseware, 305 clinical cases and 3 036 atlases. The test bank has accumulated 4 657 tests, covering clinical laboratory, immunology, biochemistry, microbiology and blood transfusion. The results of students in experimental group were significantly better than those of the control group [the score of clinical practice assessment: (85.90±5.04) vs. (78.90±6.75)( P<0.05); rate of outstanding students: 33.3% (4/12) vs. 6.7% (1/15), P>0.05; rate of satisfaction: 86.7% (13/15) vs. 100.0% (12/12) ( P>0.05). Conclusion:The mixed online and offline teaching platform based on the LQMS is highly recognized by students and can significantly improve the effect of clinical practice teaching, which can provide typical medical case teaching at any time and make up for limited case type in children's hospital.

A partir de la pandemia del COVID 19, con el conocimiento de la afinidad por los medios digitales de los jóvenes millennials, surge la necesidad de incorporar oficialmente las Tecnologías de la Información y Comunicación para la educación complementaria en salud. Por otra parte, se visibilizaron todas las deficiencias en infraestructura, equipamiento, recursos humanos en nuestras universidades; motivando en las Facultades de Medicina a nivel mundial un cambio de estrategia dentro del proceso enseñanza-aprendizaje. Por todas estas razones se hace imprescindible la implementación de un Sistema de gestión para enseñanza-aprendizaje virtual de Embriología y Genética en la Universidad Mayor de San Andrés, fortaleciendo el vínculo sincrónico y asincrónico con los estudiantes que les permita reforzar los contenidos trabajados en las clases teóricas y prácticas de la asignatura, construyendo material didáctico para que pudieran abordar un contenido determinado de la materia en distintos formatos físicos y digitales, en tiempo real y con la ventaja de acceder a los mismos en cualquier momento. El presente Sistema de gestión para enseñanza- aprendizaje, coadyuvará a fortalecer los diferentes requerimientos actuales de aprendizaje de los universitarios brindándoles conceptos vigentes y útiles sobre Embriología y Genética, contribuyendo en la formación de los futuros Médicos para que respondan a las necesidades de nuestra población.


As of the COVID 19 pandemic, with knowledge of the affinity for digital media of young millennials, the need arises to officially incorporate Information and Communication Technologies for complementary health education. On the other hand, all the deficiencies in infrastructure, equipment, human resources in our universities were made visibilized; motivating in the Faculties of Medicine worldwide a change of strategy within the teaching-learning process. For all these reasons, it is essential to implement a Management System for virtual teaching learning of Embryology and Genetics at the Mayor de San Andrés University, strengthening the synchronous and asynchronous link with the students that allows them to reinforce the contents worked on in the classes. theoretical and practical aspects of the subject, building didactic material so that they could address a specific content of the subject in different physical and digital formats, in real time and with the advantage of accessing them at any time. This teaching-learning management system will help strengthen the different current learning requirements of university students by providing them with current and useful concepts on Embryology and Genetics, contributing to the training of future doctors so that they respond to the needs of our population.

Abstract: Microgrids (MD) is a new technology to improve efficiency, resilience, and reliability in the electricity sector. MD are most likely to have a clean energy generation, but the increase of microgrids with this kind of generation brings new challenges for energy management (EMS), especially concerning load uncertainties and variation of energy generation. In this context, this study has the main objective to propose a method of how to attend this matter, verifying the difference between the day before and real-time. The EMS proposed analyses the MD in real-time, calculating the deviation between dispatched and what was predicted to happen in the operation point in a three-dimensional analysis approach, considering renewable energy generation, battery State of Charge (SOC) and load curve. The system categorized the deviation in three possible quantities (small, medium, or high) and it acts accordingly. For the Next Operation Point predictor are used an artificial neural network (ANN) methodology. For the Decision Support System, it's used a fuzzy logic system to adjust the next operation point, and it uses a mixed-integer linear programming (MILP) approach when the deviation is too high, and the dispatched operation is unfeasible. Simulations with real data and information of a pilot project of MD are carried out to test and validate the proposed method. Results show that the methodology used to attend the matters of uncertainties and variation of energy generation. A reduction of operational cost is observed in the simulations.

RESUMEN

Since the outbreak of a new round of COVID-19 in Xi’an on December 9, 2021, the response capability of the city’s and even the province’s health system to major public health emergencies has been severely tested and challenged. Faced with the rapid spread of the Delta strain and its multiple hidden transmission chains, The First Affiliated Hospital of Xi’an Jiaotong University, as the largest comprehensive tertiary hospital in Northwest China, has successfully applied the 4R crisis management theory to prevent the pandemic spread and continue the clinic work as well. Balancing of epidemic prevention and medical service with scientific, orderly and standardized procedures has been achieved. Hence, we have contributed to the "The First Affiliated Hospital of Xi’an Jiaotong University model" for controlling the epidemic and restoring normal production and life as soon as possible under the guidance and command of the National Health Commission and provincial and municipal health administrative institutions at all levels. The First Affiliated Hospital of Xi’an Jiaotong University has improved the present management system and enhanced its management level amid epidemic prevention and control. This paper aims to provide new epidemic prevention experience under the changeable environment by explaining and summarizing our theoretical basis, operation mode and practical effects of our management system.

RESUMEN

Objective:To explore the critical issues in the construction of management and supporting system of Investigator-Initiated trials (IIT) in pediatrics.Methods:Through summarizing related literature and considering the current status of pediatric clinical research, the critical issues in the construction of management and support system, for instance, the responsibility, training model, and performance evaluation in Europe and U. S. were evaluated, decision-making suggestions were put forward based on domestic pediatric IIT management system.Results:Besides IIT, clinical trials on children′s drugs are also supported by the pediatric clinical research management system in Europe and U. S.. The supporting service covers research consultation, ethical review, research design, trial implementation, patient education, risk control, and investigator training. The organizational structure and management system are relatively mature. Clinical trials are the majority of clinical research in children′s hospitals in China. Main issues identified in the construction of the management and supporting system include ethical review for pediatric clinical research, professional investigator training, multicenter cooperation scheme, performance assessment, and incentive strategies.Conclusions:Taking account into the current status of pediatric IIT in China, it is urgent to accelerate the training of pediatric investigators, set up standard IIT project management team, build the professional project management platform and Electronic Data Capture System, and promote the transformation of research outcomes. Finally, the whole process management of pediatric IIT will be developed to facilitate the development of pediatric medicine.
