RESUMEN

OBJECTIVE@#Mandibular condyle injury usually results in malocclusion and disharmony of facial growth in growing children. This study aimed to evaluate the long-term effects of autogenous coronoid grafts on the facial growth of children with unilateral temporomandibular joint (TMJ) ankylosis who underwent mandibular condyle reconstruction.@*METHODS@#Ten growing patients with unilateral bony TMJ ankylosis admitted in West China Hospital of Stomatology, Sichuan University between January 1st, 2008 and December 31st, 2016 were followed up and evaluated. These patients include three males and seven females with ages ranging from 5 years to 12 years at the time of surgery. Each patient underwent gap arthroplasty, condyle reconstruction with ipsilateral coronoid, and interposition of the pedicled temporalis fascial flap in a single operation. The postoperative follow-up ranged from 3 years to 8 years with an average of 4.9 years. Postoperative panoramic radiographs determined the growth of the mandibular height and length on the affected side and compared it with those of the healthy side.@*RESULTS@#All patients recovered uneventfully after surgery. At the end of follow-up period, the maximal mouth opening ranged from 32 mm to 41 mm with an average of 35.6 mm. Mandibular height and length continued to grow after the successful treatment of ankylosis using autogenous coronoid grafts for condyle reconstruction. However, growth deficit still existed. The final ramus height and mandibular length of the affected side at the end of follow-up increased by 25.3% (P<0.05) and 26.1% (P<0.05), respectively, compared with the initial values measured immediately after surgery. Growth rates of ramus height and mandibular length of the affected side were 47.1% and 27.2% lower (P<0.05) than those of the healthy side, respectively.@*CONCLUSIONS@#Mandibular height and length continued to grow after the successful treatment of ankylosis by using autogenous coronoid grafts for condyle reconstruction. However, growth deficit still existed. The growth rate of the affected mandible was reduced compared with that of the undisturbed side even after treatment of ankylosis.

Introdução - Este estudo teve como objetivo avaliar o crescimento da base do crânio e do corpo da mandíbula nos estágios de aceleração, transição e desaceleração da maturação das vértebras cervicais. Materiais e Método - A amostra foi composta por 105 radiografias cefalométricas laterais, de 36 indivíduos de ambos os sexos (20 do sexo feminino e 16 do sexo masculino), os quais foram acompanhados radiograficamente por um período médio de cinco anos na Faculdade de Odontologia da Universidade Paulista. Para a avaliação do comprimento total da base do crânio, foi utilizada a medida Ba-Na; para a base do crânio anterior, foi utilizada a medida CC-Na; e para base do crânio posterior, a medida CF-Po. Para a avaliação do crescimento da mandíbula foi utilizado a medida XI-PM e Pg-Go para a mensuração do eixo e do corpo da mandíbula, respectivamente. Resultados - Após a realização dos testes estatísticos, foram encontradas diferenças em todos os estágios de maturação para as medidas da base do crânio total enquanto que a base do crânio anterior e posterior apresentaram diferenças significativas somente entre as fases de aceleração e transição. Enquanto que a mandíbula apresentou medidas significativas em todos os estágios, sendo estas diferenças maiores, principalmente, entre os estágios de transição e desaceleração.Conclusão - Pode-se concluir que a base do crânio e a mandíbula apresentam crescimento durante a puberdade sendo que a época de maior crescimento significativo do corpo da mandíbula não está correlacionado com o crescimento total, anterior e posterior da base do crânio durante a puberdade.


Introduction - The aim of this study is to evaluate the growth of cranial base and body of the mandible in the stages of acceleration, transition and deceleration of cervical vertebrae. Materials and Method - The sample was composed by lateral cephalometric radiographs from 36 subjects (20 females, 16 males), which was followed-up by a period of 5 years in Dentistry School of the Paulista University. For the evaluation of the total length, anterior and posterior regions of the cranial base, was used the measures Ba-Na, CC-Na and CF-Po. For the body of the mandible was used the measures Xi-Pm and Pg-Go. Results - It was found significant growth in all the stages for total cranial base measure, while for anterior and posterior cranial base measures were found significant growth only in between the stages of accelerations and transition. The mandible measures showed a significant growth in all the stages but between the transition and deceleration it had the biggest amount of the growth. Conclusion - The cranial base and the mandible showed a significant growth during the puberty but the timing of the most significative growth of the mandible is different from the cranial base.

Introdução - Este estudo avaliou e comparou o crescimento e desenvolvimento mandibular de indivíduos portadores de Classe II mandibular ao de indivíduos portadores de Classe I esquelética. Materiais e Métodos - Foram analisados 18 indivíduos de cada grupo, brasileiros, de ambos os sexos, os quais não haviam realizado tratamento ortodôntico-ortopédico. Foram analisadas medidas lineares e angulares, e verificado o deslocamento mandibular. Resultados - O crescimento e o deslocamento mandibular foi maior no grupo de Classe I, não sendo estatisticamente significante. O ângulo goníaco apresentou uma diminuição maior nos indivíduos Classe II. Conclusões - Não houve uma diferença estatisticamente significante do crescimento mandibular entre os grupos.


Introduction - This study evaluated and compared the mandibular growth of individuals presenting mandibular skeletal Class II with individuals presenting Class I skeletal pattern. Material and Methods - Were analysed 18 individuals of each group, Brazilians, of both sexes, which none of these individuals had received orthopedic-orthodontic treatment. Were analysed linear and angular measures, and verified the mandibular displacement. Results - The growth and the mandibular displacement were bigger in the group of Class I, not being statistically significant. The goniac angle presented a bigger reduction in the Class II individuals. Conclusions - Didn´t has a statistically significant difference of the mandibular growth between groups.
