RESUMEN

Surgery management is key to surgical quality control. The authors presented the IT system design for surgery anesthesia of the children′s hospital of Fudan University, featuring all-process information support by means of IT development and process reengineering. Such a process comprises perioperative patient handover, medication, surgical safety check, and intraoperative care. This surgery anesthesia system development has interconnected hospital information systems within and beyond operation rooms in terms of informationization. It proves that the system can effectively enhance safety and convenience of surgery related works and supervision, reducing error exposure of surgical operations and ensuring patient safety.

RESUMEN

Objective To investigate the effect of limitation in flow of people on medical care quality in emergency resuscitation room (ERR).Methods This study was retrospectively performed to compare emergency medical quality before and after (October to December,2015 vs.February to April,2016) limitation of flow of people in ERR.Variables included noise level,occupational exposure,adverse event,hospital-acquired infection,length of stay (LOS) in ERR,mortality rate,return of spontaneous circulation (ROSC) rate after cardiopulmonary resuscitation (CPR),the rate of unexpected return to ERR,door to drug and door to balloon time for patients with ST-segment elevated myocardial infarction (STEMI),patients' and emergency personnels' satisfaction level.The data were analyzed with t-test,chi-square test or Poisson Z test where appropriate.Results There were 5 031 and 5 097 patients respectively admitted in ERR before and after limitation of flow of people.Patients' main diagnoses and severity of illness between the two periods had no significantly difference (P>0.05).After the limitation of flow of people,the noise level in ERR was lowered (P<0.01),the numbers of occupational exposure events (14 cases vs.4 cases,Z=2.357,P=0.018) and adverse events (18 cases vs.5 cases,Z=2.711,P=0.007) were decreased,the rate of hospital-acquired infection was reduced (1.1% vs.0.5%,x2=8.111,P=0.004),the LOS in ERR was shortened [(6.3±0.8) h vs.(4.6±0.6) h,t=121.083,P<0.01],the door to balloon time for STEMI patients was also decreased [(91.2±12.8) min vs.(89.3±8.0) min,t=2.486,P=0.013].Moreover,patients' and emergency personnels' satisfaction level were elevated.No significant difference was observed in mortality rate,ROSC rate,rate of unexpected return to ERR and door to drug time for STEMI patients.Conclusions The limitation of flow of people in ERR can lower the noise level,reduce emergency personnels' working pressure,improve their working efficiency,avoid medical errors,elevate patients' and emergency personnels' satisfaction level.

RESUMEN

Se realizó una investigación de evaluación de sistemas y servicios de salud en el Policlínico Docente "José Martí Pérez" de Santiago de Cuba, durante el 2009, a fin de evaluar la calidad de la atención médica (38 médicos y 38 enfermeras de la familia) a las 58 personas que viven con virus de inmunodeficiencia humana/sida en esta área, según las dimensiones científico-técnica, de satisfacción de los usuarios, conocimientos de los proveedores de salud y efectividad. Mediante la creación de un grupo de expertos, que establecieron criterios explícitos, descriptivos y normativos, construyeron indicadores y prefijaron estándares para valorar aspectos seleccionados de la estructura, el proceso y los resultados; se constató que existían deficiencias en la estructura de los consultorios médicos, con limitación de los recursos materiales e insuficientes conocimientos de los prestatarios del sector en aspectos fundamentales de la epidemiología de la infección y del seguimiento a estos afectados, los cuales constituyeron los indicadores de peores resultados y además influyeron en que la evaluación de los indicadores de impacto seleccionados fuera inadecuada. Finalmente, se recomendó llevar a cabo actividades de capacitación sistemáticamente para los proveedores de salud.


An investigation of health systems and services evaluation was carried out in "José Martí Pérez" Teaching Polyclinic in Santiago de Cuba, during the 2009, in order to evaluate the quality of medical care (38 family's physicians and 38 nurses) to the 58 people living with the human immune deficiency virus/aids in this area, according to the scientific-technological dimensions, to the users' satisfaction, to the knowledge of the health providers and effectiveness. By means of a group of experts who established explicit descriptive and normative criteria, they built indicators and they preset standards to evaluate selected aspects of the structure, the process and the results; it was verified that deficiencies existed in the structure of the doctor's offices, with limitation of material resources and scarce knowledge of the sector providers in fundamental aspects of the epidemiology of the infection and of the follow-up to these affected patients, which constituted the indicators of worst results and they also influenced on the inadequate of the evaluation of the selected impact indicators. Finally, it was recommended to carry out systematically training activities for the health providers.

Se realizó una investigación evaluativa de corte transversal con el objetivo de evaluar la calidad de la atención médica en la línea preoperatoria del Centro Oftalmológico "Lara", en el municipio venezolano de Iribarren. Esta metodología tuvo en cuenta las características de la planta física y la disponibilidad de recursos materiales, así como la cobertura, la capacitación y la competencia de los recursos humanos. En el proceso se evaluó el cumplimiento de las principales actividades realizadas por el equipo de salud, mientras que en los resultados se tuvo en cuenta la atención médica a los pacientes que acudieron a la línea preoperatoria en el período analizado. Los criterios, indicadores y estándares se operacionalizaron con la colaboración de varios expertos. Se concluyó que la calidad de la atención médica en la línea preoperatoria en el centro oftalmológico "Lara" no fue satisfactoria. Se recomendó a la dirección del centro oftalmológico discutir los resultados con el colectivo de trabajo para analizar las deficiencias encontradas, y en una segunda etapa la extensión de la evaluación a las otras áreas de atención médica del centro oftalmológico, el salón y el posoperatorio.


A cross-sectional evaluative research was carried out to assess the medical care quality in the preoperative line of "Lara" Ophthalmology Center from the Venezuelan municipality of Iribarren. This methodology took into account the physical plant features and the availability of material resources, as well as the coverage, the recruitment and the competence of human resources. During process authors assessed the fulfillment of main activities performed by health staff, whereas in results we took into account the medical care to patients came to preoperative line in analyzed period. Criteria, indicators and standards were operational with the cooperation of some experts. We conclude that the medical care quality in the preoperative line in above mentioned Ophthalmology center was not satisfactory. We recommended to the management of this center to discuss the results with the working staff to analyze the deficiencies founded and in a second stage, the extent of assessment to other medical care areas of this center, the operating room and the postoperative time.
