RESUMEN

A profession is characterized by advanced theoretical and systematic knowledge, which can provide that profession with autonomy and authority. This paper examines the factors affecting the realization of complete professional autonomy such as the market and capital, patients, and the state. The primary factor of weak autonomy is due to the undifferentiated interests of professionalism from the influence of capital. The second factor is the ineffective system of self-regulation over physician behavior. The third factor is the underdevelopment of medical values, which could override the current conflicts between physicians and the state.

RESUMEN

The state-running medical institutions which had been instituted in the earlier period of the Chosun dynasty substantially downsized during the reconstructing process after the major wars with Japan and Qing dynasty. The downsizing was mainly due to the malfunctioning public financial system; but it was also due to the growth of the private medical market. The growth of the private medical market reoriented the focus of the public health system of the Chosun dynasty from providing treatment for every minor disease to providing the more efficient policy against epidemic. Hwal-in Seo (a temporary local public health center established for epidemic) became a new core of the dynasty's health policy under the phrase of "Ae Rye (saving the rituals)." As the changes of the dynasty's public health policy, the growing private medical market had been admitted into the public domain. Chosun government once had declared Sa Yak Gye (a private mutual-aid group for medicine) illegal and prohibited the private groups to be organized. Instead, with the policy change mentioned above, the government tried to support the private mutual-aid group for medicine while forbidding sales of fake medicine, restraining rise of price of medicine. Especially the Do go merchants often caused the sudden rise of price of medicine by bulk purchasing. Medical practice was reassessed as the period when it was considered as one of the lowest professions had been over. Although the Yangban class still refused to be a professional medical practitioner themselves, they also well understood the value of medicine as a field of study to save human and dismissed negative perception on medicine. Medicine as a field of study and medical practice, which had been underestimated under the ruling system influenced by the Song Confucianism and the status system of the Chosun dynasty, faced a new era. The whole society guaranteed more free practices of the medical practitioners and they were recognized for their works. With the change of social environment, the government officials gradually realized needs to discuss how they could educate and recruit medical practitioners to provide advanced medical treatment and what provisions they had to legislate to ensure the stable supply of the medicine. It is certain that the transformation developed in the medical environment and the changes of the public health policy up to 18th century Chosun dynasty accompanied the emergence of the commercial society. However, the overall social urge was still not enough to induce the actual law-making process. The change of the public health policy and the growth of the private medical market were surely the evidence of the transforming Chosun society; at the same time, they also revealed the immaturity of the medical environment which was not able to lead new health policies.

RESUMEN

By analyzing the situation of market competition by modern hospitals, the paper identifies the core competitiveness of the hospital as the key force to be nurtured and promoted by the hospital in its competition in the medical services market; the basic force that spaces out service levels and profits of hospitals at the same level; the unique cultural force shaped by the hospital in a long period of time; the dynamic force rising constantly to mans needs for medical and health care; the main force driving the sustainable development of the hospital in market competition. The paper also proposes five major core competitive capacities to be especially nurtured and promoted by the hospital, viz. the integrative capacity in management, the influencing capacity of specialized fields, the fighting capacity of the team, the cohesive capacity of culture, and the scheming capacity for marketing.

Because of the medical industry valla, the medical market is in a state of oligopoly.Hospitals and patients are in a state of asymmetric information in the medical market, which could lead to moral hazard, adverse selection, and low social and medical efficiency.Based on the gaming model,we analyze the gaming process and equilibrium between hospitals and patients under the circumstances of asymmetric information. The analyzing result suggests that asymmetric information in the medical market is harmful for patient's benefit and the development of medical market as well. Therefore,it's necessary to strengthen the entire social medicare, set up an opening system of medical information and strengthen the national health education so as to relieve the asymmetric information between hospitals and patients, and improve the efficiency of medical market.

RESUMEN

Objective To assess the efficiency of central urban and rural hospitals in the 7 cities, counties and districts under the jurisdiction of Hangzhou City and the status of the medical markets so as to provide theoretical basis for the promotion of decentralized medical markets. Methods On the basis of defining the scope of a medical market, the HHI values of the medical markets in places where 37 hospitals in Hangzhou were located was calculated, the average efficiency of the various hospitals was measured by means of DEA, and an analysis was made on the correlation between the average efficiency and the HHI values. Results The central urban and rural medical markets in the various places surveyed, whose HHI values were all greater than 1 800, belonged to markets of monopolistic competition. Of the 37 hospitals, there were 11 whose relative efficiency reached 100% and another 11 whose relative efficiency was less than 60%. The greater the monopoly of a medical market, the lower the average efficiency of its hospitals. Conclusion Establishing competitive markets with low market concentration is an effective way of enhancing overall hospital efficiency.

The hospital also established a “chain management” framework characterized by “compliance with market laws, customers highest and delivery of quality service”. Setting on itself such high demands as “highest speed, best quality, lowest cost and optimal service”, the hospital did its best to create a strong marketing chain for medical service products, thus providing theoretical basis and practical reform experience for the marketing of medical service products.

RESUMEN

The author first argues that the significance of sectionalizing the medical market is that it is conducive to the discovery of market opportunities; it is conducive to the possession of a greater market share; and it provides a basis for the hospital to identify its target market. Then he sets forth the requirements for sectionalizing the medical market, viz. measurability, accessibility, relative stability, and availability of benefit. Finally he expounds the classification methods when sectionalizing the medical market and the principles for a hospitals selection of a target medical market. These include an appropriate scale and market, a certain purchasing power and endurance, a market not yet completely monopolized by competitors, a market the hospital is capable of managing, a relatively steady social environment, and observance of the procedure for selecting a target medical market.
