RESUMEN

[Objective]To explore the specificity and effectiveness of Medical Records Integration of Palace in Qing Dynasty for the treatment of diseases,and to provide a reference for the modern clinical external therapy.[Methods]Taking 117 fumigating and washing decoctions in Medical Records Integration of Palace in Qing Dynasty as the research object,using Excel software,the formulas,dosage,medicinal properties,and efficacy of Qinggong fumigating and washing decoctions were organized and counted.Combining with the relevant medical cases and the commentaries in the book,the use of the fumigating and washing decoctions in the Qing Palace was systematically organized.[Results]The 117 fumigating and washing decoctions in Medical Records Integration of Palace in Qing Dynasty show many features such as there are many kinds of formula,the quality of the formula is refined;the effect is strength and special focus,formula with modification according to symptoms,flexible usage of medicine,treatment first,independent use of the amount of medicine,good use of the wind-extinguishing medicinal,filling in the poisonous features when needed.It also has other advantages such as a wide range of audiences,a unique approach,sophisticated instruments,and a meticulous process of preparation of the liquid.[Conclusion]The use of the fumigating and washing decoctions in Medical Records Integration of Palace in Qing Dynasty has palace characteristics and advantages,exploring its use can gain unique insights and revelations,which helps to carry forward the characteristics of court medication and promotes the development of external therapeutic methods of traditional Chinese medicine as high research value.

ObjectiveTo explore the medication characteristics and clinical efficacy of the Tenghuang Jiangu tablets in the treatment of knee osteoarthritis (KOA) in the remission stage in the real world,providing references for rational clinical use of this prescription. MethodBased on the "registration system of KOA treated with Tenghuang Jiangu tablets",2 439 KOA cases in the remission stage were analyzed by SPSS 25.0,IBM SPSS Modeler18.0,and Apriori algorithm. To be specific,the age,body mass index (BMI),and course of treatment were described in the form of x̄±s. The information on gender,K-L grade,daily dose,and frequency of drug use was described by frequency analysis. The number of cases,course of treatment,daily dose,and drug use frequency of the single-use group and the combined-use group were described by frequency analysis,and the combination of drugs was described by frequency analysis and Apriori algorithm. Mann-Whitney U test was employed to compare the scores of Visual Analogue Scale (VAS),Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC),pain,stiffness,and joint function between the single-use group and the combined-use group. ResultThe results of clinical treatment showed that 2 439 patients with KOA in the remission stage were treated with Tenghuang Jiangu tablets,with 1 432 (58.71%) in the single-use group and 1 007 (41.29%) in the combined-use group. The average daily dose of Tenghuang Jiangu tablets was (3.90±1.44) g,and the majority of the patients were at grade Ⅱ (54.47%). The daily average daily dose of Tenghuang Jiangu tablets in the single-use group was (3.64±1.35) g,which was lower than that in the combined-use group [(4.26±1.48) g,P<0.05]. In the combined use,the top three western medicines were glucosamine (270 times,14.68%),sodium hyaluronate (126 times,6.85%),and imrecoxib (116 times,6.31%),and the top three Chinese medicines were Huoxuezhitong capsules/tablets/ointments (31 times,1.69%),Biqi capsules (25 times,1.36%),and Maizhiling (23 times,1.25%). As for the overall clinical efficacy,the VAS score was (5.13±0.93) score before treatment and (2.22±1.18) score after treatment (P<0.05),with an overall average decrease of (2.91±1.14) score, and the average decrease in the single-use group was (2.76±1.43) score, which was lower than that in the combined-use group [(3.12±1.36) score,(P<0.01)]. The WOMAC score was (31.05±11.84) score before treatment and (13.55±9.91) score after treatment (P<0.05). The overall average decrease was (17.50±11.79) score, and the average decrease in the single-use group and combined-use group was (16.39±11.14) score and (19.08±12.50) score,respectively (P<0.01). The patients with KOA>grade Ⅱ accounted for 91.34%(1 308/1 432) and 93.55%(942/1 007) in the single-use group and combined-use group,respectively (χ2=80.026,P<0.05). A total of 43.37%(621/1 432) of the patients in the single-use group had other complications,lower than that in the combined-use group [54.92%(553/1 432),(χ2=20.087,P<0.01)]. ConclusionMore than half of the patients with KOA in the remission stage are treated with Tenghuang Jiangu tablets alone,and the combination therapy is mainly applied in patients with severe conditions or other complications. In relieving knee joint pain and improving joint stiffness and joint function,both the Tenghuang Jiangu tablets alone and the combination therapy are effective.

[Objective]To review and summarize the academic experience of Professor LAO Changhui in the treatment of postinfective cough.[Methods]By clinical shadowing,recording medical cases and reviewing literature,the clinical experience of Professor LAO in the treatment of postinfective cough was summarized from three aspects:etiology and pathogenesis,clinical treatment,feeding and nursing.Two medical records were attached to support it.[Results]Professor LAO believes that the postinfective cough belongs to the category of cough in traditional Chinese medicine and the key of differentiation lies in external and internal injury.The treatment should be carried out according to the physiological characteristics of lung"preferring moist,hating dryness and not resistant to cold",and prescription should be based on pathological products and functions of Zang-Fu organs.Xuanfei Huatan Decoction and Shegan Mahuang Decoction were the main prescriptions for patients whose postinfective cough was caused by external factors;while Liujunzi Decoction was the main prescription for patients whose postinfective cough caused by internal injury.This article also discussed dietary therapy for cough after infection.In the first case cited,Professor LAO identified it as phlegmatic hygrosis syndrome,and used Shegan Mahuang Decoction.By warming the lung and evaporating fluid,the lung can restore the function of dispersiveness.In the second case,Professor LAO identified it as liver depression and Qi stagnation,and used Banxia Xiexin Decoction.By soothing the liver and regulating Qi,balancing cold and heat,making the Qi movement normal,all symptoms were relieved.[Conclusion]Professor LAO focuses on the differentiation of external and internal injuries in the treatment of postinfective cough,emphasizes the combination of disease and syndrome differentiation,adds or subtracts drugs according to the pathological products of patients and the function of Zang-Fu organs,pays attention to diet care,and has achieved stable clinical efficacy,which is convenient for clinical application and promotion.

To investigate the clinical characteristics and the rule of administration of Ciwujia Injection in patients with cardiovascular disease by correlation analysis and frequency analysis. The information of 5 904 patients who used Ciwujia Injection to treat cardiovascular disease for at least three times in the hospital information system(HIS) of 19 comprehensive tertiary hospitals nationwide. The frequency analysis method was used to analyze the higher frequency variables, and the algorithm of Apriori correlation analysis method was used to analyze the clinical characteristics and medication laws of Ciwujia Injection in treating patients with cardiovascular disease. Among the 5 904 patients, the median age of the patients was 70 years, and the number of patients between 65-75 years old was up to 2 096(35.5%). There were more women than men in terms of diagnosis and treatment, and the women at age of 45-65 and 65-75 years old were more than men. The top three diagnoses by Western medicine were coronary heart disease in 8 104 cases(65.66%), dyslipidemia in 2 515 cases(20.38%), and cardiac function grade Ⅱ-Ⅲ in 581 cases(4.71%), while the largest number of syndromes in traditional Chinese medicine(TCM) at discharge was 1 109(21.37%) in other type, followed by 739 cases(14.24%) of Qi deficiency and blood stasis syndrome, and 698(13.46%) of liver and kidney Yin deficiency syndrome. The most frequently used Western medicine in combined use was Nifedipine in 2 567 cases(7.21%), and most frequently used TCM was Compound Xianzhuli Liquid leachate in 766 cases(3.53%). From the results of pharmacological analysis, the frequency of using Ciwujia Injection + stasi-seliminating agent + calcium channel blocking drugs was highest when using 1 Chinese medicine combined with 1 or 2 Western medicines. In the use of 2 Chinese medicines combined with 1 Western medicine, Ciwujia Injection + heat-clearing agent + stasis-eliminating agent + calcium channel blocker was the most common. This study demonstrated that Ciwujia Injection was more common in middle-aged and elderly patients and more in women than men. Treatment should be based on different cardiovascular disease treatment guidelines to reduce complications caused by underlying diseases, and attention should be paid to the prevention and treatment of cardio-vascular disease risk factors such as hypertension, diabetes and abnormal lipid metabolism. According to the analysis of frequency distribution and association rules, it was concluded that Ciwujia Injection was mainly used in combination with calcium channel blockers and stasis-eliminating agents in the treatment of cardiovascular disease. In the treatment of cardiovascular diseases by Ciwujia Injection in combination with other Chinese and Western medicines, in order to avoid pharmacological changes and avoid affecting the efficacy of the drug, it is necessary to closely observe whether adverse reactions occur and ensure that the medication is safe and effective. This study provides a good reference for the follow-up clinical guidance of Ciwujia Injection.

In this study, the clinical characteristics and drug combination rules of Danshen Chuanxiongqin Injection in the treatment of patients with cerebral infarction were analyzed. The inpatient information of 2 857 patients of cerebral infarction treated with Danshen Chuanxiongqin Injection in HIS database of 20 grade Ⅲ class A hospitals in China was collected, and a model was established by description analysis and Apriori algorithm, in order to explore the clinical characteristics and drug combination rules of Danshen Chuan-xiongqin Injection in the treatment of cerebral infarction. The results showed that among patients of cerebral infarction treated with Danshen Chuanxiongqin Injection, 1 727 patients were older than 65 years old, accounting for 69.61%, and 1 610 were males, accounting for 63.59%. Commonly used drugs included lipid-lowering agents, anticoagulant thrombolytic agents, antiplatelet agents, stimulants of brain metabolism, vasodilators and other Western drugs, as well as traditional Chinese medicines, such as blood-activating agents, heat-clearing agents and expectorant agents. The Western medicine with the highest use frequency in combination with Danshen Chuan-xiongqin Injection was aspirin enteric-coated tablets(1 528 cases, 53.48%). The traditional Chinese medicine with the highest use frequency in combination with Danshen Chuanxiongqin Injection was Xingnaojing Injection, with a total of 378 cases, accounting for 13.23%. Among them, the most commonly used Western drugs combined with Danshen Chuanxiongqin Injection were anticoagulant thrombolytic and antiplatelet drugs, with a usage rate as high as 83.48%. In order to further explore the drug combination rules of Danshen Chuanxiongqin Injection, the association analysis of drug combination in patients of cerebral infarction treated with Danshen Chuanxiongqin Injection was carried out. In clinical combination of two Western drugs, Atorvastatin Calcium Capsules+Cerebral Proteolytic Injection were the most common combination, with a support of 27.10%. In clinical combination with 3 Western drugs, Clopidogrel Bisulfate Tablets+Atorvastatin Calcium Capsules+Cerebral Proteolytic Injection were most commonly used, with a support of 15.90%. The results showed that the patients of cerebral infarction treated with Danshen Chuanxiongqin Injection were mainly elderly males, and often complicated with hypertension, coronary heart disease, diabetes and other basic diseases. The clinical application of Danshen Chuanxiongqin Injection was principally in line with the guidelines. In the treatment of cerebral infarction, it was often combined with Western medicine anticoagulant thrombolysis, antiplatelet drugs, traditional Chinese medicine blood-activating and stasis-dissolving prescription and other drugs with similar pharmacological effects, with an auxiliary therapeutic effect on patients of cerebral infarction complicated with other diseases, and can provide guidance for clinical medication.

In order to explore the characteristics of traditional Chinese medicine(TCM) syndromes and medications in patients with colonic malignant tumors in China, the clinical data of patients with colonic malignant tumors from the information system of 33 Grade-A tertiary hospitals in China from 2001 to 2016 were extracted and a descriptive analysis was conducted on their disease characteristics, syndrome distribution, drug characteristics and treatment status. A total of 22 691 patients with colonic malignancies nationwide were included in the analysis, with male to female ratio of 1.59∶1, median age of 62 years, susceptible age of 50-79 years. Postoperative patients with colonic malignant tumors accounted for 47.10%, and patients with liver metastases reached 36.02%. Among the 520 patients with colonic malignant tumors, the top three TCM syndromes were: spleen deficiency syndrome(21%), Qi and Yin deficiency syndrome(17%) and liver and stomach disharmony syndrome(15%). The most frequently used Western medicine for patients with colonic malignant tumors was anti-metabolite cytotoxicity(61.23%), most frequently used in combination with platinum-based cytotoxic drugs(support degree 42.40%).The top three Western medicines were oxaliplatin(42.26%), leucovorin injection(38.58%) and dexamethasone(34.67%). The most frequently used traditional Chinese medicine type was heat-clearing and detoxifying drugs(39.59%), most frequently used in combination with Qi replenishing and body resistance strengthening drugs(support degree 12.72%). The top three traditional Chinese medicines were compound Kushen Injection(14.62%), Shenqi Fuzheng Injection(10.18%) and Aidi Injection(6.48%). This study shows that spleen and Qi deficiency may be the dominant syndrome of colonic malignant tumor, and chemotherapy is one of the main treatment methods. Traditional Chinese medicine has shown unique advantages in alleviating the toxic and side effects of chemotherapy and preventing recurrence and metastasis, so integrated TCM and Western medicine can significantly improve the clinical efficacy.

Through the traditional Chinese medicine inheritance platform system, with the help of medical records, Ye Tianshi and Wu Jutong's medication characteristics for summer heat sickness were analyzed, the laws of the two people's medication were summarized, and the similarities and differences between the two were explored to explore the relationship. As a result, it was found that both of them recognized the relationship between summer heat and wetness, and Wu Jutong believed that "wind" was also an important pathogenic factor. Both of the patients were treated with cold medicine and warm medicine. They used mostly bitter, sweet, pungent taste and lungs, spleen, stomach, and heart meridian are the main components; two are commonly used Armeniacae Semen Amarum, Talcum, Rehmanniae Radix, Ophiopogonis Radix, Pinelliae Rhizoma and other drugs, Ye Tianshi use Scrophulariae Radix, Tetrapanacis Medulla, Coicis Semen and other drugs more, Wu Jutong use Gypsum Fibrosum, Sojae Semen Praeparatum, Menthae Haplocalycis Herba and other drugs more; at the same time, a combination of two high-frequency medicines used by two people has been excavated, and a new prescription has been deduced to provide a reference for further understanding and treatment of summer diseases.

Objective To investigate the entry points for clinical work of intensive care unit (ICU) pharmacists.Methods Through combination with daily work and referring the domestic and foreign literature,the characteristics of ICU medications were discussed to find out the entry point for clinical work of ICU pharmacists.Results ICU patients particularly need individualized pharmaceutical care because of the special pathophysiological characteristics and medicine use.Conclusion ICU pharmacists should provide pharmaceutical care based on Pharmacokinetics/pharmacodynamics knowledge and focus on the drug dosage adjustment,drug interactions and adverse event prevention.

Objective]To research and explore the characteristic of medicine in Treatise on Febrile Diseases. [Methods]Combine with the doctors of past dynasties, relevant annotations of the treatment of cough notes in Treatise on Febrile Diseases,and my own learning experience,analyze the characteristic of medicine of Zhongjing treating cough in Treatise on Febrile Diseases.[Results]Its characteristics are summed up in three points.Pay attention to the thought of the simultaneous dispersion and descending, and conform to lung qi.Pay attention to the drugs of the lung channel,especially the application of Ephedra and its compatibility with other drugs.Pay attention to“reinforcing earth to generate metal”and protect stomach qi. [Conclusion]The research of this paper is beneficial to broaden the clinical treatment of cough ideas,expand the clinical application of the classical prescription,deepen the understanding of Zhang Zhongjing's academic thought.
